Hematochezia due to Angiodysplasia of the Appendix.

Common causes of lower gastrointestinal bleeding include diverticular disease, vascular disease, inflammatory bowel disease, neoplasms, and hemorrhoids. Lower gastrointestinal bleeding of appendiceal origin is extremely rare. We report a case of lower gastrointestinal bleeding due to angiodysplasia of the appendix. A 72-year-old man presented with hematochezia. Colonoscopy showed active bleeding from the orifice of the appendix. We performed a laparoscopic appendectomy. Microscopically, dilated veins were found at the submucosal layer of the appendix. The patient was discharged uneventfully. A

Diagnosis and management of gastrointestinal angiodysplasia.

Eighteen patients with gastrointestinal angiodysplasia were seen in a single surgical unit over a period of 8 years. The mean age at onset of symptoms was 33 years. The average duration of symptoms was 54 months (range 2 days-16 years). Three patients had gastric angiodysplasia, two had colonic angiodysplasia, both diagnosed endoscopically preoperatively. The remaining patients required further investigation, which included small bowel enema (SBE), erythrocyte tagged scan (ETC), selective visceral angiography and intraoperative enteroscopy (IOE). SBE was useful but not diagnostic in 3, ETC in

An unusual case of colonic angiodysplasia.

An unusual case of a colonic vascular anomaly resembling angiodysplasia associated with right sided diverticular disease is presented. The patient, a 74 year old man, presented with a four day history of rectal bleeding and subsequently underwent hemicolectomy. The resected specimen was flushed out with heparin-saline solution and injected with a barium-gelatine mixture. Preoperative barium enema revealed right sided diverticula, whereas post-resection angioradiography revealed the "coral reef" vascular anomaly consistent with angiodysplasia. Histology confirmed the presence of both diverticul

[Clinical significance of colonic angiodysplasias].

Among 9343 colonoscopic examinations (1975-1983) colonic angiodysplasias (AD) were diagnosed in 78 patients (0.8%). The mean age of patients with AD was 68 (17-86) years which was significantly higher than that of patients without AD (p less than 0.0001). 76% of the AD were localized in cecum or colon ascendens, 13% in colon transversum, 4% in colon descendens and 6% in sigmoid colon. 69 patients (88.5%) had 1-4 AD; 4 (5.1%) 5-10 AD and 5 (6%) more than 10 AD. A high incidence of associated diseases leading to vascular complications (i.e. hypertension, diabetes mellitus) was noted. 14 of the 7

[A case of bleeding angiodysplasia of the colon].

Report on a patient with bleeding angiodysplasia of the ascending colon. The clinical course was characteristic for this kind of lesions: long-term, chronic, recurrent intestinal hemorrhages with a gradual increase of anemia. Diagnosis was complicated by lack of success in finding the source of the hemorrhage and by evidence of various pathological findings. Histologically, there were vessel variations typical for angiodysplasia and, furthermore, cholesterol emboli of the submucosal colic arteries. The latter's pathogenetic role as trigger for the hemorrhage is discussed.

[Micromorphological and clinical aspects of angiodysplasias].

As is known, 1900 Klippel and Trenaunay summarized naevus teleangiectatisuc lateralis, vein ectasias and isolated gigantism to a triad. Seven years later Weber described the same syndrom adding the symptom of arterio-venous fistulas, that he clinically diagnosed. Some authors deduce from this symptomatology two syndroms ; on one hand the Klippel and Trenaunay syndrom and on the other the P. F. Weber-syndrom. As it is shown, in spite of an immense variability of symptoms, arterio-venous short circuits can regularly be demonstrated-clinically, arteriographically or histomorphologically. In addit

Von Willebrand factor, angiodysplasia and angiogenesis.

The large multimeric glycoprotein Von Willebrand factor (VWF) is best known for its role in haemostasis; however in recent years other functions of VWF have been identified, indicating that this protein is involved in multiple vascular processes. We recently described a new role for VWF in controlling angiogenesis, which may have significant clinical implications for patients with Von Willebrand disease (VWD), a genetic or acquired condition caused by the deficiency or dysfunction of VWF. VWD can be associated with angiodysplasia, a condition of degenerative blood vessels often present in the

Gastrointestinal angiodysplasia in three Saudi children.

Angiodysplasia is a term used to describe distinct gastrointestinal mucosal ectasias that are not associated with cutaneous lesions, systemic vascular disease or a familial syndrome. Seventy-seven percent of angiodysplasia are located in the cecum and/or ascending colon. Fifteen percent are located in the jejunum and/or ileum and the remainder are distributed throughout the alimentary tract. Most commonly, the angiodysplastic lesions are typically seen in elderly patients of both genders, although gastric and duodenal lesions have been reported occasionally in subjects within the third decade

Gastrointestinal angiodysplasia in chronic renal failure.

Gastrointestinal (GI) hemorrhage is a frequent and sometimes life-threatening complication of end-stage renal failure. Angiodysplasia (AD), vascular malformation, is the most common cause of recurrent lower-intestinal hemorrhage in patients with renal failure. We report four chronic hemodialysis patients with AD. All patients presented with severe anemia requiring transfusion. GI hemorrhage ceased spontaneously in three cases and after treatment with argon plasma coagulation in another. Diagnosis of AD is usually challenging, since its cause is still unknown, and its clinical presentation is v

[Pulmonary angiodysplasia with fatal pulmonary hemorrhage].

For 6 weeks a 52-year-old woman had complained of increasing fatigue, blood-streaked vomitus, retrosternal burning and pain between the shoulder blades. Physical examination showed no abnormalities. Blood sedimentation rate was increased to 15/40 mm and the iron concentration was slightly reduced. Computed tomography demonstrated densities in the left upper lobe of the lung and both lower lobes. Scintigraphy revealed a perfusion defect in the left apex of the lung while bronchoscopy demonstrated acute bronchitis in the left upper lobe. Further haemoptysis occurred 3 months later, but several b

Angiodysplasia in patients with renal insufficiency.

Sixty-five patients with angiodysplasia were studied: 42 (64.6%) had normal renal function and 23 (35.4%) had renal insufficiency, defined as serum creatinine consistently greater than 1.5 mg/dl. Comparison of results in the two groups showed that there were no significant differences in the mode of presentation or anatomic distribution of angiodysplasia. Systemic hypertension, congestive cardiac failure, coronary artery disease, and diabetes mellitus were significantly more common in patients with renal failure. Recurrent bleeding from upper alimentary angiodysplasias occurred significantly m

Angiodysplasia of the upper gastrointestinal tract.

I have reviewed the English language literature on angiodysplasia of the upper gastrointestinal tract. Angiodysplasia is a distinct mucosal vascular lesion associated with acute or chronic gastrointestinal bleeding. Its etiology is unknown, but theories of its pathogenesis have evolved from its similarity to colonic angiodysplasia and an association with renal failure. Despite recurrent gastrointestinal bleeding, the endoscopic diagnosis is difficult because the lesions are so small and so similar to fresh blood. Endoscopic ablation by electrocautery or laser photocoagulation can reduce bleedi

Small intestinal angiodysplasia in the elderly.

The predominant site of bleeding intestinal angiodysplasia in elderly patients will be the cecum or ascending colon, but recent experience in the Yale-Affiliated Gastroenterology Program in 1 year indicates that elderly patients may have bleeding acquired angiodysplasia (AD) confined to the small intestine only. A review of the literature confirms that symptomatic small intestinal AD is infrequent and occurs at an average age of 32 years in some series. Five patients with symptomatic small intestinal AD diagnosed during 1981 at Yale were older, with an average of 62 years. Three of the five ca

Aortic stenosis--angiodysplasia of the caecum.

A 73-year-old female patient developed beside aortic stenosis angiodysplasia in the caecum, which gave rise to serious, repeated intestinal bleedings. As a result, the patient was admitted to hospital 32 times. Over a period of 8 years she was given 178 litres of blood. The vascular disorder which was the source of bleeding, was detected by mesenterial angiography. After resection of the colonic section concerned, the bleeding stopped. The diagnosis of angiodysplasia of the caecum was confirmed by pathological examination.
