Transdermal and Powdered Scopolamine-Induced Anisocoria: A Report of Two Cases.

INTRODUCTION: In this case report, we present 2 cases of sudden-onset anisocoria caused by accidental exposure to scopolamine in 2 young female patients. CASE PRESENTATION: Two patients presented with unilateral anisocoria. One patient experienced unilateral mydriasis accompanied by neurological symptoms from a transdermal scopolamine patch, while the other, exposed to powdered scopolamine, presented with unilateral mydriasis without additional neurological symptoms. Both cases showed gradual resolution of symptoms over several days without intervention. Initially, Adie's pupil (tonic pupil)

Anisocoria secondary to antiperspirant wipes in a pediatric population: a case series.

Acute-onset anisocoria or mydriasis in children carries a broad differential diagnosis and includes both benign and life-threatening causes, ranging from systemic or topical drug use to peripheral or central nervous system disease. The topical anticholinergic agent glycopyrronium (approved by the Food and Drug Administration in June 2018) is used to treat hyperhidrosis. We present the first case series of pediatric patients presenting with acute mydriasis due to exposure to glycopyrronium wipes. Six cases (ages 12-16) were identified: 3 presented emergently and 3 to a primary care physician. A

Tournay's description of anisocoria on lateral gaze: reaction, myth, or phenomenon?

The year 1917 witnessed the opening of a great debate in neuro-ophthalmology, with Tournay's description of a phenomenon thought to be universal: anisocoria in lateral gaze, due to mydriasis of the abducting pupil and a minor miosis of the fellow one. Through many publications, this phenomenon was soon assessed and confirmed to be physiologic, while the cause of the pupil reaction in lateral gaze was extensively discussed. Cases of abolition and reversal of the phenomenon were also reported in specific clinical situations, such as neurosyphilis, and its study became part of the neuro-ophthalmo

Peroperative anisocoria after extravasal loss of i.v. fluids in the neck.

A case is presented of a patient who developed unequal pupils peroperatively after extravasal loss of IV fluids in the neck. The reason for this loss was a malpositioned catheter near the internal jugular vein. Causes of peroperative anisocoria are discussed, and a new explanation is proposed for this particular case.

Benign episodic unilateral mydriasis presenting as acute postoperative anisocoria.

Acute anisocoria in the postoperative setting raises immediate concern for life-threatening intracranial or ophthalmic pathology. We report the case of a female patient in her 30s who developed a left-sided fixed dilated pupil following uncomplicated right-sided functional endoscopic sinus surgery. Urgent imaging and neurological evaluation were unremarkable. The patient had a history of migraine and previous self-resolving episodes of unilateral mydriasis associated with headaches, for which extensive investigations had previously been normal. Her postoperative headache resolved with analgesi

Anisocoria after scopolamine transdermal patch contamination: A case report.

RATIONALE: We report a case of anisocoria that occurred after contamination with a scopolamine transdermal patch, and introduce a diagnostic approach for anisocoria patients. PATIENT CONCERNS: A 35-year-old woman with no past ophthalmologic history presented to the ophthalmology department complaining of a dilated pupil in the right eye. Corrected visual acuities was 20/20 in both eyes, and the intraocular pressures were 20 and 18 mm Hg in the right and left eye, respectively. The anterior chambers in both eyes were unremarkable on slit-lamp examination. The pupil size was 5.0 mm in the right

Differentiation of Horner Syndrome and Physiological Anisocoria by Automated Pupillometry.

BACKGROUND: The differentiation of Horner syndrome from physiological anisocoria is important yet clinically challenging. We investigated the diagnostic accuracy of pupillometry to discriminate Horner syndrome from physiological anisocoria compared to pharmacological testing with the alpha-2-agonist apraclonidine, which is considered the current gold standard. METHODS: Forty-four adult patients, mostly referred to our neuro-ophthalmology service for evaluation of anisocoria, were included. Automated binocular pupillometry was performed under standardized light conditions before and >30 minute

Anisocoria associated with the medical treatment of irritable bowel syndrome.

A case of anisocoria associated with oral pharmacologic treatment of irritable bowel syndrome is reported. A 26-year-old woman developed sudden onset of anisocoria and compromised accommodation that lasted 2 days after the use of oral scopolamine methylbromide for treatment of irritable bowel syndrome. The anisocoria and compromised accommodation occurred after contamination of the ocular surface after administration of scopolamine methylbromide and resolved within 1 week without further contamination. Oral preparations used for the pharmacologic treatment of irritable bowel syndrome can cause

Cluster headache: pupillometric patterns as a function of the degree of anisocoria.

Fifty-three patients with cluster headache, mean age 42.6 years, were examined by means of pupillometry. Pharmacological stimulation was carried out by the instillation of eye drops; the sympathomimetic agents hydroxyamphetamine (a norepinephrine releaser) and phenylephrine (an agent acting directly on the postsynaptic receptors) were applied. Pupillary dilatation was measured at set time intervals, comparing the responses of the symptomatic and non-symptomatic sides. The material was divided into groups according to the degree of basal anisocoria. The subgroup with the most pronounced basal m

Transient anisocoria after a traumatic cervical spinal cord injury: A case report.

Context: Autonomic dysreflexia and dysautonomia can be a common complication following spinal cord injury, especially at levels of T6 and above and can lead to complications associated with the pupil. In this case report, we review autonomic dysreflexia, the anatomy and physiology of the sympathetic nervous system of the eye, as well as review the clinical relevance in understanding autonomic and pupillary changes in response to autonomic dysfunction.Findings: We present a patient with an incomplete C4 level injury who was found to have isolated anisocoria on two separate occasions during his

Ocular ultrasound in the evaluation of anisocoria following head injury: a case report.

BACKGROUND: Anisocoria following severe head injury is traditionally interpreted as an impending neurological emergency, often prompting immediate empiric interventions for presumed intracranial hypertension. However, traumatic ocular injuries can present with identical pupillary findings, creating a critical diagnostic dilemma in the unresponsive trauma patient. CASE PRESENTATION: We present the case of a 70-year-old intubated male with a severe traumatic brain injury who presented with a unilateral fixed, dilated pupil. Because urgent transport for head computed tomography (CT) was delayed

The Prevalence of Physiological Anisocoria and its Clinical Significance - A Neurosurgical Perspective.

AIM: We aimed to estimate the prevalence of physiological anisocoria and also evaluate the accuracy of clinical assessment of anisocoria employed in routine bedside examination. MATERIALS AND METHODS: A total of 708 voluntary subjects between the ages of 20-69 years who had no history of ophthalmologic or neurological disease other than refractive error were included in the study. In a closed room with uniform ambient lighting, the subjects' pupils were examined clinically and the presence or absence of anisocoria was recorded. This was followed by photography of the subjects' pupils using a
