Freezing negatively impacts the interlamellar mechanical properties of the degenerated human annulus fibrosus.

Mechanical testing of biological tissues often utilizes previously frozen samples but there are conflicting reports about the impact of freezing on mechanical properties. There are no current studies examining the impact of freezing on the interlamellar matrix in the annulus fibrosus of intervertebral disc. We quantified the freezing-induced changes in the interlamellar mechanical properties of degenerated human and non-degenerated bovine annulus fibrosus tissues. Fresh human annulus fibrosus samples were obtained from surgical cases involving disc degeneration (loss of disc height with no def

Fibrotic alterations in human annulus fibrosus correlate with progression of intervertebral disc herniation.

BACKGROUND: Intervertebral disc (IVD) herniation is characterized by annulus fibrosus failure (AF) in containing the nucleus pulposus (NP). IVD herniation involves cellular and extracellular matrix (ECM) alterations that have been associated with tissue fibrosis, although still poorly investigated. METHODS: Here, fibrotic alterations in human AF were evaluated, by characterizing the herniated ECM. Human AF samples (herniated lumbar IVD (n = 39, age 24-83) and scoliosis controls (n = 6, age 15-21)) were processed for transmission electron microscopy and histological/immunohistochemical analysi

Neonatal annulus fibrosus regeneration occurs via recruitment and proliferation of Scleraxis-lineage cells.

Intervertebral disc (IVD) injuries are a cause of degenerative changes in adults which can lead to back pain, a leading cause of disability. We developed a model of neonatal IVD regeneration with full functional restoration and investigate the cellular dynamics underlying this unique healing response. We employed genetic lineage tracing in mice using Scleraxis (Scx) and Sonic hedgehog (Shh) to fate-map annulus fibrosus (AF) and nucleus pulposus (NP) cells, respectively. Results indicate functional AF regeneration after severe herniation injury occurs in neonates and not adults. AF regeneration

Application of Endogenous Stem Cells in the Repair of Annulus Fibrosus Injury of Intervertebral Discs.

Intervertebral disc degeneration (IVDD), a major contributor to chronic low back pain (LBP), involves progressive extracellular matrix (ECM) degradation and limited self-repair. Current therapies alleviate symptoms but fail to halt degeneration, driving interest in endogenous stem cell-based regeneration. Endogenous stem/progenitor cells within disc niches exhibit regenerative potential through ECM synthesis, anti-inflammatory signaling, and exosomal miRNA-mediated repair. Preclinical studies highlight mesenchymal stem cell (MSC) transplantation and reprogramed induced pluripotent stem cells (

Injury of the annulus fibrosus and disc protrusions. An in vitro investigation on human lumbar discs.

Discs of 25 specimens of human lumbar motion segments were subjected to an internal division of the annulus fibrosus, sparing only a peripheral layer 1 mm thick. Thus an attempt was made to simulate an internal disruption of the annulus caused by a traumatic episode or a degenerative process. The disc bulge that developed at the site of the injury was observed under axial loads in the physiologic load range, after axial compression fracture and after intradiscal injection. Under 1,000-N load, the bulge amounted to less than 0.5 mm; typically it increased to less than 1.0 mm after fracture. An

Biochemical changes of intervertebral discs in patients with spondylolisthesis or with tears of the posterior annulus fibrosus.

Surgically removed discs from patients with either spondylolisthesis or tears of the posterior annulus fibrosus were analysed for water, proteoglycan, and collagen content and compared with post-mortem control material. Discs from patients with spondylolisthesis had a reduced proteoglycan content in all section sampled and less collagen in the outer annular layers. In contrast discs containing tears in the posterior annulus were unaltered biochemically, although extended studies on 2 patients indicated that there may be localised biochemical changes in the region of the tear itself. Collagen t

Mechanical and Biological Treatments for Annulus Fibrosus Repair and Closure: A Review of Clinical Studies.

To address disc reherniation, several annular repair strategies have been developed, including implantable devices, suture-based techniques, and biomaterials. This review evaluates the clinical evidence supporting these strategies. A systematic search of PubMed, Cochrane, and Embase and MEDLINE via Ovid was performed from database inception to July 2025. A combination of keywords was used in the search string. Publications were included if they were clinical studies and described closing the annulus or blocking the annulus defect with biologics, sutures, or physical devices. Exclusion criteria

Mechanical and structural changes to the annulus fibrosus in response to Sub-failure cyclic loading.

This study aimed to quantify how repetitive tensile loading alters the mechanical and structural properties of the annulus fibrosus (AF). Mechanical changes were evaluated through a three-step protocol involving pre-damage characterization of dynamic and viscoelastic properties, damage induction using predetermined loading cycles (n=400, 1600, 6400, 12,800) to a specified strain magnitude (11 %, 20 %, 28 %, 44 %), and post-damage characterization of the same properties. Structural changes were assessed by subjecting tissue to damage cycles and staining with hematoxylin and eosin or fluorescing

The effect of intervertebral disc damage on the mechanical strength of the annulus fibrosus in the adjacent segment.

BACKGROUND CONTEXT: A herniated intervertebral disc (IVD) is a common injury in the human population. Despite the injury being isolated to a singular IVD in the spine, it is important to look at the biomechanical effects that a damaged IVD has on the entire spine, specifically the IVD adjacent to the injury. PURPOSE: This study examined the effects of a damaged IVD on the mechanical properties of the annulus fibrosus (AF) in the adjacent cranial IVD. STUDY DESIGN: Basic science study using an in-vitro porcine model. METHODS: Sixteen porcine cervical spines were used; specifically spinal lev

Effect of overload on changes in mechanical and structural properties of the annulus fibrosus of the intervertebral disc.

The research focussed on analysing structural and mechanical properties in the intervertebral disc (IVD), caused by long-term cyclic loading. Spinal motion segments were divided into two groups: the control (C), and the group in which it was analysed the impact of posterior column in the load-bearing system of the spine-specimens with intact posterior column (IPC) and without posterior column (WPC). To evaluate the structural and mechanical changes, the specimens were tested with simulation of 100,000 compression-flexion load cycles after which it was performed macroscopic analysis. Mechanical

Alk3/Bmpr1a receptor is required for development of the atrioventricular canal into valves and annulus fibrosus.

Endocardial cushions are precursors of mature atrioventricular (AV) valves. Their formation is induced by signaling molecules originating from the AV myocardium, including bone morphogenetic proteins (BMPs). Here, we hypothesized that BMP signaling plays an important role in the AV myocardium during the maturation of AV valves from the cushions. To test our hypothesis, we used a unique Cre/lox system to target the deletion of a floxed Alk3 allele, the type IA receptor for BMPs, to cardiac myocytes of the AV canal (AVC). Lineage analysis indicated that cardiac myocytes of the AVC contributed to

A Suture Technique for Ruptured Annulus Fibrosus Following Decompression Under Percutaneous Transforaminal Endoscopic Discectomy.

The suture technique for a ruptured annulus fibrosus (AF) under full-endoscopy remains challenging. Direct suturing of a ruptured annular tear after full decompression has been shown to decrease the recurrence rate of lumbar disc herniation during endoscopic surgery. Traditional suture operations under endoscopy involve only simple suturing of the ruptured AF. Due to the weak and poor quality of the AF tissue around the tear portal, using this area as needle insertion points during suturing may lead to insufficient tension and a low success rate of AF closure. Currently, there is no detailed t

An annulus fibrosus anchored nanovector reprograms osteopontin driven macrophages in preclinical disc degeneration models.

Intervertebral disc degeneration is a major cause of low back pain and is associated with inflammatory macrophages that promote annulus fibrosus degeneration via osteopontin signaling. Here we report an annulus fibrosus anchored nanovector carrying small interfering RNA targeting osteopontin. The nanovector first binds annulus fibrosus cells via a targeting peptide and is subsequently activated by reactive oxygen species in inflammatory microenvironments to release macrophage targeted hybrid nanovesicles. This sequential design enables local retention in disc tissue and subsequent delivery to

Curcumin-loaded dynamic crosslinked injectable hydrogel for annulus fibrosus repair and disc degeneration prevention.

OBJECTIVE: Annulus fibrosus (AF) defects following discectomy often induce biomechanical instability and inflammatory microenvironment deterioration, key drivers of recurrent herniation and accelerated degeneration. Given the limited self-repair capacity of the AF, this study aimed to develop a novel composite hydrogel integrating immediate mechanical support, tissue adhesion, and sustained anti-inflammatory release to promote functional AF repair. METHODS: A composite hydrogel was prepared by incorporating curcumin-loaded liposomes (LIP@CUR) into a Schiff-base-crosslinked network formed betw

Growth Plate Morphogens as Potential Modulators of Cellular Tissue Remodeling by Annulus Fibrosus Cells.

BACKGROUND: Adolescent Idiopathic Scoliosis (AIS) is a spinal deformity of unknown etiology that begins in the intervertebral disc and emerges during pubertal growth. In the vertebral growth plate, adjacent to the disc, powerful growth plate morphogens (GPMs) modulate this growth. We evaluated whether GPMs can also alter the capacity of annulus fibrosus (AF) disc cells to proliferate and/or remodel their extracellular matrix (ECM). METHODS: AF cell responsiveness to seven GPMs was screened in monolayer culture by RT-qPCR for downstream signaling markers. GPMs and concentrations eliciting sign

Fetal Supraventricular Tachycardia: Histologic Evidence of Accessory Pathways Due to Incomplete Annulus Fibrosus Formation.

BACKGROUND: Atrioventricular (AV) reentrant tachycardia is a common type of supraventricular tachycardia (SVT) that occurs in the fetus and neonate. Although many tachycardias resolve within several weeks of birth or respond to medical management, disruptions in the cardiac annulus fibrosus and development of additional accessory pathways may lead to refractory dysrhythmia resulting in fetal hydrops and ultimately, fetal death. OBJECTIVES: While accessory pathways have been well documented anatomically in adult and childhood tachyarrhythmias, there are no reports of the histology of these pat

Melatonin-loaded self-healing hydrogel targets mitochondrial energy metabolism and promotes annulus fibrosus regeneration.

Intervertebral disc (IVD) herniation is a major cause of chronic low back pain and disability. The current nucleus pulposus (NP) discectomy effectively relieves pain symptoms, but the annulus fibrosus (AF) defects are left unrepaired. Tissue engineering approaches show promise in treating AF injury and IVD degeneration; however, the presence of an inflammatory milieu at the injury site hinders the mitochondrial energy metabolism of AF cells, resulting in a lack of AF regeneration. In this study, we fabricated a dynamic self-healing hydrogel loaded with melatonin (an endocrine hormone well-know

Design and Ex Vivo Evaluation of a PCLA Degradable Device To Improve Annulus Fibrosus Repair.

With a prevalence of over 90% in people over 50, intervertebral disc degeneration (IVDD) is a major health concern. This weakening of the intervertebral discs can lead to herniation, where the nucleus pulpus (NP) leaks through the surrounding Annulus Fibrosus (AF). Considering the limited self-healing capacity of AF tissue, an implant is needed to restore its architecture and function. Here, we developed a biomimetic electrospun nanofibrous biodegradable scaffold that could be potentially used to repair AF defects. To that aim, we synthesized copolymers and blends of ε-caprolactone and lactide

Standardisation of basal medium for reproducible culture of human annulus fibrosus and nucleus pulposus cells.

BACKGROUND: The lifetime prevalence of degenerative disc disease is dramatically high. Numerous investigations on disc degeneration have been performed on cells from annulus fibrosus (AF) and nucleus pulposus (NP) of the intervertebral disc (IVD) in cell culture experiments utilising a broad variety of basal culture media. Although the basal media differ in nutrient formulation, it is not known whether the choice of the basal media itself has an impact on the cell's behaviour in vitro. In this study, we evaluated the most common media used for monolayer expansion of AF and NP cells to set stan

Ultrastructural organization of elastic fibres in the partition boundaries of the annulus fibrosus within the intervertebral disc.

UNLABELLED: The relationship between elastic fibre disorders and disc degeneration, aging and progression of spine deformity have been discussed in a small number of studies. However, the clinical relevance of elastic fibres in the annulus fibrosus (AF) of the disc is poorly understood. Ultrastructural visualization of elastic fibres is an important step towards understanding their structure-function relationship. In our previous studies, a novel technique for visualization of elastic fibres across the AF was presented and their ultrastructural organization in intra- and inter-lamellar regions
