Anticholinergic Burden in Individuals with Down Syndrome: An Examination of Risk and Clinical Implications.

Background/objective: Down syndrome (DS) is characterized by premature aging, with comorbidities observed in the older population without DS, a high prevalence of multimorbidity and cognitive decline. The use of multiple medications further increases clinical complexity. The objective of this paper is to evaluate anticholinergic use and the anticholinergic burden (ACB) in a sample of persons with DS. Methods: cross-sectional retrospective study involving community-dwelling persons with DS, afferent to the DS clinics of the Fondazione Policlinico Universitario "A. Gemelli" (Rome, Italy). Indivi

Anticholinergic syndrome due to mydriatic drops intoxication in a child: A case report.

Anticholinergic syndrome consists of neurological and systemic symptoms, including agitation, mydriasis and dryness that can be an adverse reaction to a number of medications, some of them as seemly harmless as cycloplegic mydriatic drops. We present the case of a 7-year-old previously healthy female child who presented to the emergency room with impaired neurological status, inability to recognize family members and incoherent speech, as well as facial flushing, mucosal dryness and bilateral mydriasis after having mistakenly received a high dose of mydriatic drops. She made a complete recover

[Central anticholinergic intoxication syndrome. A contribution to the differential diagnosis of exogenous psychoses].

A withdrawal syndrome in a 50-year-old alcoholic subsided within 5 days in response to treatment with doxepin (150 mg/d). But 2 days later he developed auditory hallucinations which were interpreted as alcohol hallucinations, for which he was additionally given haloperidol, 15 mg/d. He then developed early dyskinesia which was treated with 5 mg biperiden i.v. followed by twice 2 mg/d by mouth, while doxepin and haloperidol were continued. 5 days after detoxification there occurred, under this combination of drugs which included two with marked anticholinergic action, an anticholinergic intoxic

Central anticholinergic syndrome after ofloxacin overdose and therapeutic doses of diphenhydramine and chlormezanone.

A 14-year-old female with no history of psychiatric disease ingested an unknown amount of ofloxacin, diphenhydramine and chlormezanone after an argument with her patients. Approximately 12 hours after ingestion, the patient was admitted to the hospital in a delirious state with extreme mydriasis and warm and dry skin. Analytical data on admission were consistent with ofloxacin overdose and ingestion of therapeutic doses of diphenhydramine and chlormezanone. The patient received activated charcoal and forced diuresis was instituted. Psychosis and anticholinergic symptoms lasted in the next 2 da

Central anticholinergic syndrome vs. idiosyncratic reaction triggered by a small IV dose of atropine.

A 58-year-old male was scheduled to undergo radical gastrectomy for cancer under general anesthesia. The patient developed agitation and irregular breathing after receiving a single dose of atropine (0.5 mg) to treat bradycardia immediately prior to induction of anesthesia. Within 5 min after the atropine injection, the patient became unresponsive with facial flushing and diaphoresis. When a drop in oxygen saturation was observed, a laryngeal mask airway was inserted after administering a small bolus dose of propofol (80 mg) and the patient was ventilated with 100% oxygen. Physostigmine was no

Controlled clinical trial of sedative-anticholinergic drugs in patients with the irritable bowel syndrome.

We studied 16 patients with long-standing irritable bowel syndrome of moderate severity using a controlled, double-blind crossover method. Five sedative-anticholinergic drug combinations and a placebo were tested. The subjective response was assessed with four subjective methods to include an increasing number of response variables. The patients preferred 30 mg phenobarbital plus 8 mg belladonna (P & B) to placebo (P = 0.02). Five of ten patients were helped "some" or "a lot" with placebo, while ten of 15 were helped "some" or "a lot" with P & B (P = 0.07). The ten prominent-symptoms method re

Atropine-Induced Anticholinergic Syndrome Following Off-Label Sublingual Administration for Refractory Sialorrhea: A Case Report.

A sublingual atropine ophthalmic solution is increasingly used as an off-label therapy for the symptomatic management of refractory sialorrhea because of its rapid onset of action, ease of administration, and generally favourable safety profile when used at recommended doses. Although generally well-tolerated at therapeutic doses, systemic anticholinergic toxicity may occur following medication administration errors or unintentional overdose. We report the case of a 76-year-old man with recurrent head and neck squamous cell carcinoma receiving palliative systemic therapy who presented with acu

Oral anticholinergic drugs versus placebo or no treatment for managing overactive bladder syndrome in adults.

BACKGROUND: Around 16% of adults have symptoms of overactive bladder (OAB; urgency with frequency and/or urge incontinence), with prevalence increasing with age. Anticholinergic drugs are commonly used to treat this condition. This is an update of a Cochrane Review first published in 2002 and last updated in 2006. OBJECTIVES: To assess the effects of anticholinergic drugs compared with placebo or no treatment for treating overactive bladder syndrome in adults. SEARCH METHODS: We searched the Cochrane Incontinence Specialised Register, which contains trials identified from the Cochrane Centra

Acute Anticholinergic Toxic Syndrome Due to Scopolamine Exposure: A Case Report with Possible Drug-Facilitated Administration.

INTRODUCTION: Scopolamine is a tropane alkaloid and non-selective muscarinic receptor antagonist capable of inducing a central anticholinergic toxidrome with pronounced neuropsychiatric and autonomic manifestations. Despite its well-characterized pharmacology, diagnosis remains clinically challenging due to variable presentation and often absent or unreliable exposure history, particularly in non-medical contexts. METHODS: We report a case of a 34-year-old female presenting with an acute onset of altered mental status following a social event. Clinical findings included delirium, hallucinatio

[Central anticholinergic, neuroleptic malignant and serotonin syndromes : Important differential diagnoses in postoperative impairment of consciousness].

Impaired consciousness is a frequent phenomenon after general anesthesia. In addition to the classical causes (e.g., overhang of sedatives), an impairment of consciousness can also be an adverse side effect of drugs. Many drugs used in anesthesia can trigger these symptoms. Alkaloids, such as atropine can trigger a central anticholinergic syndrome, opioids can promote the occurrence of serotonin syndrome and the administration of a neuroleptic can lead to neuroleptic malignant syndrome. These three syndromes are difficult to diagnose due to the individually very heterogeneous symptoms. Mutual

Risk of dementia in patients treated with anticholinergics for overactive bladder syndrome: a systematic review and meta-analysis.

BACKGROUND: Urinary anticholinergic (AC) medications are commonly prescribed for Overactive Bladder (OAB) syndrome. Although recent studies suggest a potential link between their use and an increased risk of dementia, this association remains debated. METHODS: We conducted a systematic review and meta-analysis to evaluate the risk of dementia in patients receiving AC treatment for OAB syndrome. A comprehensive search of PubMed, Embase, and Cochrane databases was performed. Outcomes of interest included dementia risk and its association with age, sex distribution, treatment duration, and follo

Ogilvie Syndrome Secondary Due To Underlying Hypokalaemia And Anticholinergics: Case Report And Brief Review Of The Literature.

Ogilvie's syndrome is a rare but potentially life-threatening condition characterised by massive dilation of the colon without a mechanical obstruction. It typically affects older adults and those with underlying medical conditions, such as neurological or cardiovascular diseases, and may result in severe complications such as perforation or sepsis. Diagnosis is based on clinical presentation and radiological studies, and treatment involves a combination of conservative measures, such as bowel rest and pharmacological agents, and interventional procedures, such as endoscopic decompression or s
