Carbonic anhydrase inhibition/activation: trip of a scientist around the world in the search of novel chemotypes and drug targets.

Carbonic anhydrases (CAs, EC 4.2.1.1) are metalloenzymes which catalyze CO(2) hydration to bicarbonate and protons. Five genetically distinct classes are known, which represent an excellent example of convergent evolution. Inhibition of α-CAs from vertebrates, including humans, with sulfonamides was exploited clinically for decades for various classes of diuretics and systemically acting antiglaucoma agents, whereas newer inhibitors are used as topically acting antiglaucoma drugs, anticonvulsants, antiobesity, antipain and antitumor agents/diagnostic tools. Recently, novel interesting chemotyp

Ocular hypotensive actions of serotonin antagonist-ketanserin analogs.

A series of ketanserin analogs were studied for their effects on intraocular pressure (IOP) recovery curve of rabbits infused with hypertonic saline. Eight out of 31 compounds tested showed a marked delay in IOP recovery indicating that they are potential antiglaucoma agents. Five out of eight compounds showed equal suppression of the IOP recovery curves of treated and contralateral eyes indicating that these compounds could be absorbed systemically to affect the IOP of the contralateral eye. There were three compounds (GC 679, GC 646, and GC 526) which showed more suppression of the IOP recov

Nasolacrimal occlusion improves the therapeutic index of antiglaucoma medications.

The purpose of this study was to assess whether nasolacrimal occlusion improves the therapeutic index of various antiglaucoma medications. Use of nasolacrimal occlusion resulted in better efficacy and safer usage. Nasolacrimal occlusion used with 2% pilocarpine, 1.5% carbachol, 0.25% timolol maleate, or 1% epinephrine every 12 hours gave the maximal response for these drugs. Applying 0.25% timolol and 1.5% carbachol every 12 hours gave the maximal response for this combination. Nasolacrimal occlusion did not alter the effect of 0.1% dipivefrin, and since the corneal penetration of this prodrug

[Use of small-dose Cyclosporine HEXAL in the treatment of postoperative inflammation during intraocular surgical reinterventions].

The effectiveness of small doses of the selective immunosuppressive agent Cyclosporine HEXAL (2-2.5 mg/kg) was studied in 112 patients with early inflammatory reactions developing after various surgical reinterventions (cataract phacoemulsification with intraocular lens implantation after antiglaucoma surgery, repeated antiglaucoma, reconstructive operations, etc.). The drug was given as a short course for 5-10 days. As compared to the conventional treatment with corticosteroids and nonsteroidal anti-inflammatory drugs (NSAID), Cyclosporine HEXAL exerted a rapider therapeutic effect, reducing

Carbonic anhydrase inhibitors: inhibition of isozymes I, II and IV with N-hydroxysulfonamides--a novel class of intraocular pressure lowering agents.

A series of N-hydroxy sulfonamides has been prepared by reaction of alkyl-, arylalkyl- and arylsulfonyl halides or sulfonic acid anhydrides with hydroxylamine. Structurally related inhibitors were also obtained from acyl chlorides and hydroxylamine, as well as by reaction of tosyl isocyanate with hydroxylamine, sulfamic acid and sulfamide. Inhibition of three carbonic anhydrase (CA) isozymes, hCA I, hCA II and bCA IV (h = human; b = bovine) with the prepared compounds has been investigated. Good inhibitors, as well as compounds with moderate activity against these isozymes were detected, depen

Applications of Carbonic Anhydrase Inhibitors in Arthritis, Neuropathic Pain, Acute Mountain Sickness, and Cerebral Ischemia.

Carbonic anhydrase inhibitors (CAIs) have long been used as diuretics, topically acting antiglaucoma agents, anticonvulsants, and for the treatment of altitude sickness. Lately, they have shown promising results as anti-obesity, antimicrobial, and mainly antitumor agents. Recent studies have reported the efficacy of CAIs in treating inflammatory conditions such as arthritis and neuropathic pain. Moreover, a polypharmacological profile has been demonstrated for sulfonamide COX-2 inhibitors such as celecoxib, which also effectively inhibit CAs. Additionally, emerging research showed the effectiv

Minimally Invasive Bleb Surgery for Glaucoma: A Health Technology Assessment.

BACKGROUND: Glaucoma is the term for a group of eye disorders that causes progressive damage to the optic nerve, which can lead to visual impairment and, potentially, irreversible blindness. Minimally invasive bleb surgery (MIBS) reduces eye pressure through the implantation of a device that creates a new subconjunctival outflow pathway for eye fluid drainage. MIBS is a less invasive alternative to conventional/incisional glaucoma surgery (e.g., trabeculectomy). We conducted a health technology assessment of MIBS for people with glaucoma, which included an evaluation of effectiveness, safety,

[Cell cultures - model object for experimental studies in ophthalmology].

The article presents results obtained on several types of model cultures. Primary cultures of corneal epithelial cells and fibroblast-like cells from the limbus were obtained from the tissues of the anterior eye segment. Tests were performed on these cultures to determine the cytotoxicity of antiglaucoma drugs and to trial a protection method. In addition, a comparative analysis of the regenerative potential of various blood derivatives was carried out. An in vitro culture of endotheliocytes showed a correlation between the degree of damaging effects of antiglaucoma drugs of various groups and

Dendritic Oligoethylenimine Decorated Liposome with Augmented Corneal Retention and Permeation for Efficient Topical Delivery of Antiglaucoma Drugs.

The topically administered glaucoma medications usually encounter serious precorneal drug loss and low corneal penetration, leading to a low bioavailability. In addition, due to the complexity of glaucoma etiology, a single medication is often insufficient. In this work, we report a novel dendritic oligoethylenimine decorated liposome for codelivery of two antiglaucoma drugs, latanoprost and timolol. The liposome showed a uniform nanoscopic particle size, positive surface charge, and excellent dual-drug loading capacity. A prolonged precorneal retention is observed by using this liposomal deli

Extraocular, periocular, and intraocular routes for sustained drug delivery for glaucoma.

Although once daily anti-glaucoma drug therapy is a current clinical reality, most therapies require multiple dosing and there is an unmet need to develop convenient, safe, and effective sustained release drug delivery systems for long-term treatment to improve patient adherence and outcomes. One of the first sustained release drug delivery systems was approved for the reduction of intraocular pressure in glaucoma patients. It is a polymeric reservoir-type insert delivery system, Ocusert™, placed under the eyelid and on the ocular surface for zero-order drug release over one week. The insert,

Carbonic anhydrase inhibitors: the X-ray crystal structure of ethoxzolamide complexed to human isoform II reveals the importance of thr200 and gln92 for obtaining tight-binding inhibitors.

Ethoxzolamide, an almost forgotten inhibitor of the metalloenzyme carbonic anhydrase (CA, EC 4.2.1.1), is the only classical inhibitor whose structure in adduct with any isoform was not reported yet. We report here the inhibition data of this molecule with the 12 catalytically active mammalian isozymes (CA I-CA XIV) and the X-ray crystal structure with the cytosolic, ubiquitous isoform CA II. These data are presumably useful for the design of novel CA inhibitors, targeting various CA isozymes, considering that ethoxzolamide was already the lead molecule to obtain the second generation inhibito

Carbonic anhydrase inhibitors: topically acting antiglaucoma sulfonamides incorporating phthaloyl and phthalimido moieties.

Sulfonamides incorporating 2-carboxy-benzenecarboxamido (phthaloyl) moieties in their molecules were prepared by reaction of phthalic anhydride with aromatic/heterocyclic sulfonamides in mild conditions. Another closely related series of derivatives was prepared by reaction of the same reagents in more energetic conditions, when the corresponding phthalimides were obtained. Some of these compounds showed very good in vitro carbonic anhydrase (CA) isozymes I, II and IV inhibitory properties, with affinities for the enzymes in the low nanomolar range for the best inhibitors. Some of the potent C
