PURPOSE: To compare the relative effectiveness and side effect profiles of antimetabolite drugs in the treatment of noninfectious ocular inflammation.
DESIGN: Retrospective cohort study.
PARTICIPANTS: A total of 257 patients with inflammatory eye disease seen in a single-center, academic practice and treated with an antimetabolite as a first-line immunosuppressive agent from 1984 to 2006.
METHODS: Data recorded included demographics, antimetabolite and prednisone doses, use of other immunosuppressive drugs, response to therapy, and side effects associated with drug use.

A 71-year-old white woman with primary open angle glaucoma received trabeculectomy with antimetabolite. After laser suture lysis, intraocular pressure has remained at 1-2 mmHg for 23 months and 12 months in the right and left eye, respectively. However, visual acuity has remained at the preoperative levels of 6/6 and 6/7.5 in the right and left eye, respectively, without hypotony maculopathy in either eye.

Contact hypersensitivity is one of the most common skin diseases and its pharmacological control is an important clinical issue. We investigated the control of contact hypersensitivity by immunosuppressive drugs administered during sensitization or challenge. PURPOSE: To evaluate childhood filtration surgery when using releasable sutures, antimetabolites, and bleb-needling with 5-fluorouracil (5FU).
METHODS: Retrospective case note review of 30 patients (38 eyes) with childhood glaucoma presenting between 1999 and 2004 to a tertiary pediatric ophthalmology center. Either trabeculectomy or combined trabeculotomy-trabeculectomy using antimetabolites, releasable sutures, and bleb modification was performed. We have developed a novel liposome for sustained drug delivery to the eye of the antimetabolite 5-fluorouridine-5'-monophosphate (FUMP), a potent metabolite of 5-fluorouracil. The in vitro half-life of FUMP incorporated within these liposomes was 585 hours. Following subconjunctival administration of 1 mg of FUMP to 13 New Zealand white rabbits, the tissue drug level at the injection site was significantly greater in the group treated with liposomes than it was in the group not treated with liposomes (p = .02). The survival in culture of adult female Brugia pahangi, Acanthocheilonema viteae, and Onchocerca volvulus and adult male Onchocerca gibsoni was assessed by measuring parasite motility. Survival of all species was maximal in a nutritionally complex medium (RPMI-1640).
