Inflammatory abdominal aortic aneurysm (IAAA) is a distinct clinicopathological entity, characterized by: (1) clinical presentation, such as back pain, weight loss, and increased ESR, (2) patchy and/or diffuse lymphoplasmacytic infiltration, and (3) marked periaortic fibrosis resulting in thickening of the aneurysmal wall and occasional retroperitoneal fibrosis. Its pathogenesis is unknown, but some authors support the theory that IAAA is a subtype of atherosclerotic abdominal aortic aneurysm because of close relationship between IAAA and atherosclerotic change. In this article, we describe cl

Sonography is a highly accurate method for assessing the abdominal aorta and the iliac arteries for aneurysms and their complications. Proper technique is required, however, to ensure diagnostic accuracy. With respect to abdominal aneurysms, the principal duplex applications are diagnosis, measurement, and follow-up after surgery. Assessment of contained aortic rupture is best accomplished with CT, rather than ultrasound. Aortic dissection may be diagnosed with duplex sonography, but the extent of dissection and the circulatory ramifications are best evaluated arteriographically. This review a

A family is described in which nine members over two generations had an aortic dissecting aneurysm or aortic or arterial dilation at a young age. The family has been followed up since 1977 after the death of a second teenager from a kindred of 11. None of the patients had the Marfan syndrome or a history of systemic hypertension. Three members died of ruptured aortic dissecting aneurysm and acute hemopericardium at 14, 18 and 24 years of age, respectively; a fourth member died suddenly at age 48 years, a few years after aortic repair for aneurysmal dilation. One member underwent surgical repai

348 cases of abdominal aortic aneurysm were reviewed for typical features of inflammatory aneurysm (IAAA) (marked thickening of aneurysm wall, retroperitoneal fibrosis and rigid adherence of adjacent structures). IAAA was present in 15 cases (14 male, 1 female). When compared with patients who had ordinary aneurysms, significantly more patients complained of back or abdominal pain (p less than 0.01). Erythrocyte sedimentation rate was highly elevated. Diagnosis was established in 7 of 10 computed tomographies. 2 patients underwent emergency repair for ruptured aneurysm. Unilateral ureteral obs

PURPOSE: Abdominal aortic aneurysm (AAA) with concomitant Horseshoe kidney (HSK) is rare. When open surgery is not feasible, preserving the renal isthmus artery (RIA) during endovascular treatment presents a challenge.
CASE REPORT: A 70-year-old male presented with a 57.8 mm × 54.3 mm AAA and type V HSK perfused by a 4.4 mm RIA from the inferior mesenteric artery and a 4.6 mm RIA from the aortic bifurcation. This case report describes an endovascular aortic aneurysm repair (EVAR) performed on an AAA coexisting with a type V HSK. One-year follow-up: the AAA had regressed, and the HSK remained

OBJECTIVE: Previous studies suggest partly different risk factor profiles of thoracic aortic aneurysms (TAAs) and abdominal aortic aneurysms (AAAs), but prospective data are scarce. The purpose of this prospective population-based case-control study was to investigate differences in risk factor profile between TAAs and AAAs.
METHODS: Participants in two prospective population-based studies, the Västerbotten Intervention Project (VIP) and the Monitoring of Trends and Determinants in Cardiovascular Disease (MONICA) study, between 1986 and 2010, underwent cardiovascular risk assessments, includi

INTRODUCTION: An Infectious Aortic Aneurysm (IAA), also known as a Mycotic Aortic Aneurysm (MAA), is a rare lesion of the aorta resulting from an infection of its wall.
PRESENTATION: A male patient in his 70s presented to our emergency department with fever, intense abdominal pain, and a pulsatile mass in the middle of the abdomen. A Computed Tomography (CT) angiography scan was done two weeks earlier and it showed a 6.6 cm subrenal aortic aneurysm. A new CT angiography scan revealed a 3.4 cm growth (10*10*9.3) with periaortic fluids. The diagnosis of MAA was considered, and emergency surgery

BACKGROUND: Replacement of the aortic valve and ascending aorta with a composite graft is the most common surgical treatment for aortic root aneurysms with or without aortic regurgitation (AR). In the early 90's reconstructive procedures of the aortic root have been described with encouraging results. This paper presents our experience with this technique.
METHODS: Between January 2001 and May 2003, 28 patients (25 males, 3 females, mean age 60 years) with aortic root aneurysm were treated with reimplantation of the aortic valve. Twenty-two patients had AR > 2+, 5 had Marfan syndrome, 5 had a

PURPOSE: The purpose of this study was to determine whether the morbidity and mortality of surgery for thoracoabdominal aortic aneurysm (TAAA) in patients with prior aortic surgery are increased.
METHODS: The results for all patients undergoing operation for TAAA at a single institution were reviewed.
RESULTS: Over a 10-year interval, 279 patients (136 women and 143 men) underwent aortic replacement for TAAA. The mean patient age was 68 years (range, 34-90). The extent of aortic replacement was relatively evenly distributed: type I (91), type II (54), type III (78), or type IV (56). Of these

The object of this report is to review the clinical outcomes of aortic valve-sparing operations in patients with aortic root aneurysm. From May 1988 to June 2000, 120 patients with aortic root aneurysm with or without aortic insufficiency underwent aortic valve-sparing operations. There were 83 men and 37 women whose mean age was 46 years (range 16 to 72 years). Forty-eight patients had the Marfan syndrome, and 22 had either acute or chronic type A aortic dissections. Reconstruction of the aortic root was performed using the technique of reimplanation of the aortic valve in 64 patients and the

We reviewed 22 consecutive patients undergoing surgery during 13 years from 1968 to 1981 for ascending aortic aneurysms associated with aortic regurgitation due primarily to idiopathic medionecrosis resulting in annuloaortic ectasia. Chronic aortic dissection was accompanied in 45% of the patients. The operative period was divided into 2 stages, the first from 1968 to 1976, and the second from 1977 to 1981. In the earlier period, the operative mortality was 42%, and it improved significantly to 0% (p less than 0.05) in the later period. The reasons for improved surgical results are 1) the empl

The aim of this study is to give an overview of current knowledge regarding abdominal aortic aneurysm (AAA) growth after endovascular aortic aneurysm repair (EVAR) that could potentially lead to aortic rupture. A search on Pubmed was performed. A total of 705 articles were found after initial search, of which 49 were included in the final selection. Reports on the incidence of aneurysm enlargement after EVAR vary between 0.2% and 41%. Continuous growth could lead to rupture of the aneurysm sac. There are several supposed risk factors for growth after EVAR. Endoleaks remain a hot topic as these

BACKGROUND AND PURPOSE: The high prevalence of intracranial aneurysms (IAs) in patients with a bicuspid aortic valve or coarctation of the aorta suggests a link between IA and aortic pathology. However, studies reporting this link do not sufficiently address the heterogeneity of IAs arising from different anatomic locations. This study aimed to explore whether a location-specific relationship exists between the 2 kinds of aneurysms.
METHODS: Retrospective institutional analysis of patients aged ≥18 years with both IA and an aortic aneurysm (AA) was performed from 2005 to 2014. IAs were catego

BACKGROUND: Coronary artery aneurysms (CAA) are seen in 1-5% of angiograms. Aneurysmal coronary disease has been thought to be a variant of atherosclerotic coronary artery disease (CAD) in most patients, but this has not been systematically studied.
METHODS: To better understand the pathophysiology of CAA, we reviewed the cardiac catheterization films of 403 patients with ascending thoracic aortic aneurysms and 74 patients with abdominal aortic aneurysms (AAA) who underwent surgery for their aortic aneurysms at our institution. Coronary aneurysms had diameters 1.5-fold that of a reference seg

The aorta is a conduit from the left ventricle that delivers pulsatile blood distally in either a compliant or stiffened vessel to organs and tissue beds. Only recently, since the advent of transesophageal echocardiographic imaging, did its presence and associated pathologies become more profound and more prominent for the intensivist. Angiography, the "gold standard" for diagnostic imaging, now seems to be in question since the advent of ultrasound (transesophageal echocardiography), improvements in magnetic resonance imaging, and particularly the advancement to 64-slice computed tomography.
