Language treatment effects on communicative abilities and working memory in Korean-speaking agrammatic Broca's aphasia caused by moyamoya disease: Phase II evidence from a case study.

The present study reports on the language treatment outcomes from sentence- and story-level linguistic facilitation and its generalization effect on communicative abilities, working memory, and sentence processing in the case of an adult with Moyamoya Disease (MMD). After treatment, the patient's overall performance, including the Aphasia Quotient, and sentence processing ability as measured by language testing, were improved. Furthermore, the treatment effects were generalizable to working memory abilities. Our case study conveys clinically meaningful implications since it is the first report

Effects of Combining Modified Response Elaboration Training, Semantic Feature Analysis, and Arm Ability Training on Cognitive, Language, and Motor Recovery in Poststroke Broca's Aphasia: A Randomized Controlled Trial.

PURPOSE: Stroke survivors often face problems in language, motor, and cognitive skills because the neural networks for these functions overlap. Consequently, the recovery is complex and requires a multimodal approach. This study examined the interactions among linguistic, motor, and cognitive processes. We also evaluated how integrating speech and language therapy (SLT) with arm ability training (AAT) affects recovery in patients with Broca's aphasia. METHOD: This was a prospective, assessor-blinded, three-arm, parallel-group, randomized controlled trial with an open-label design. Forty-five

Patient Tan revisited: a case of atypical global aphasia?

Broca's first patient presented in support of a relationship between a lesion of the frontal lobe and aphasia was patient Tan. Although Pierre Marie refers to this case as "indisputably aphasia of Broca," the clinical diagnosis of Tan's aphasia has not been re-examined in light of current clinical criteria. Superficially, the patient's extremely limited verbal output and intact comprehension appear to fit with the diagnosis of Broca's aphasia, but a more thorough examination of the onset, evolution and nature of the patient's speech symptoms suggests alternate interpretations. Broca's biases in modern aphasiology.

Broca's description of aphemia signals the emergence of aphasiology as a discipline. It also marks the beginning of a long tradition of studies of right-handed patients ("Nous parlons avec l'hémisphère gauche") with lesions in the inferior frontal gyrus (Broca's area) displaying production deficits (Broca's aphasia) in their single, mainly Romance or, as years went by, Germanic language. In modern-day aphasiology, language crosses hemispheric boundaries, production is no longer confined to the left inferior frontal gyrus, and lesions in Broca's area do not necessarily entail Broca's aphasia, w

Primary Progressive Aphasia and Stroke Aphasia.

PURPOSE OF REVIEW: This article summarizes the clinical and anatomic features of the three named variants of primary progressive aphasia (PPA): semantic variant PPA, nonfluent/agrammatic variant PPA, and logopenic variant PPA. Three stroke aphasia syndromes that resemble the PPA variants (Broca aphasia, Wernicke aphasia, and conduction aphasia) are also presented. RECENT FINDINGS: Semantic variant PPA and Wernicke aphasia are characterized by fluent speech with naming and comprehension difficulty; these syndromes are associated with disease in different portions of the left temporal lobe. Aphasia disorders outcome after stroke.

INTRODUCTION: Aphasia is considered to be the most difficult disorders of speech-language communication, and is often companion by all forms of cerebrovascular disease. GOAL: To determine the outcome of aphasia disorder a year after a stroke and stroke type influence on the outcome of aphasia disorders. MATERIAL AND METHODS: We analyzed one-year outcome of aphasia disorders in patients who had a first stroke. Patients were tested by a speech pathologist with the International test for aphasia, immediately after admission and one year after the stroke. All patients that were hospitalized duri

Cerebellar Tracts Microstructural Characteristics and Severity of Language Impairment in Post-stroke Patients with Aphasia: A DTI Study.

Aphasia is a language impairment resulting from stroke. While numerous studies have explored the association between supratentorial white matter damage and post-stroke aphasia using diffusion tensor imaging (DTI), the role of infratentorial structures, particularly the cerebellum, remains underinvestigated. Evidence suggests that the cerebellum may play a supportive role in language processing, but its contribution across different aphasia types is not well understood. This study aimed to explore cerebellar white matter involvement in individuals with Broca's, conduction, and anomic aphasia us

Crossed aphasia in a dextral: a clinicopathological study.

Severe Broca aphasia and left hemiplegia without right limb apraxia occurred suddenly in a right-handed man with no personal or family history of left-handedness. Postmortem examination showed infarction of the right hemisphere, limited almost entirely to the precentral gyrus. In this patient, cerebral dominance for speech lay in the right hemisphere, but dominance for limb praxis lay in the left. This case provides evidence that cerebral dominance for speech and handedness in dextrals may be dissociated. It also suggests that lesions of the precentral gyrus are of major importance in producin

Recovery of injured Broca's portion of arcuate fasciculus in the dominant hemisphere in a patient with traumatic brain injury.

RATIONALE: Recovery of injured AF in patients with traumatic brain injury (TBI) has not been reported. In this study, we report on a patient with TBI who recovered from an injury to Broca's portion of AF in the dominant hemisphere, diagnosed by diffusion tensor tractography (DTT). PATIENT CONCERNS: A 28-year-old right-handed male patient suffered head trauma resulting from sliding while riding a motorcycle. DIAGNOSES: He was diagnosed with a traumatic contusional hemorrhage in the left frontal lobe, subarachnoid hemorrhage, and subdural hemorrhage in the left fronto-temporal lobe. INTERVENT

[Cerebrovascular disorder and the language areas].

Recent progress of imaging techniques has achieved precise descriptions of aphasic syndromes associated with cerebrovascular disorders. Infarction of the left anterior cerebral artery brings about transcortical motor aphasia. Occlusion of each branches of the left middle cerebral artery produces characteristic language symptoms. Recently, cases with infarction of the areas of prefrontal and precentral arteries were reported to manifest fluent aphasia indistinguishable from the classical transcortical sensory aphasia. Rare cases with lesions restricted to the Broca's area (territory of the prec

Broca's faculté du langage articulé: Language or Praxis?

De Oliveira-Souza, Moll, and Tovar-Moll (this issue) historically reevaluate that Paul Broca's aphemia should be considered as a kind of apraxia rather than aphasia. I argue that such a claim is unwarranted, given the interpretation of the faculty of speech Broca derived from his predecessors, Jean-Baptiste Bouillaud and Franz Joseph Gall, and also with a view on the then generally held opinion that the terms aphémie and aphasie were synonyms. I will discuss evidence that patients such as Leborgne, producing only very few words or syllables, suffer from a global aphasia, affecting all modaliti

Transcranial direct stimulation over left inferior frontal gyrus improves language production and comprehension in post-stroke aphasia: A double-blind randomized controlled study.

Transcranial direct current stimulation (tDCS) targeting Broca's area has shown promise for augmenting language production in post-stroke aphasia (PSA). However, previous research has been limited by small sample sizes and inconsistent outcomes. This study employed a double-blind, parallel, randomized, controlled design to evaluate the efficacy of anodal Broca's tDCS, paired with 20-minute speech and language therapy (SLT) focused primarily on expressive language, across 5 daily sessions in 45 chronic PSA patients. Utilizing the Western Aphasia Battery-Revised, which assesses a spectrum of lin

[Case report--Crossed aphasia].

A case study of crossed aphasia is presented. A 60-year-old right-handed individual suffered stroke in the right hemisphere leaving him with Broca's aphasia and severe verbal apraxia. A CT scan 3 days after hospitalization showed a new frontotemporal infarct in the right hemisphere, insula and frontal portion of the superior and middle temporal gyrus. MRI 2 weeks later showed more diverse changes involving the parietal lobe. In addition to the Broca's aphasia and verbal apraxia AA had prosodic difficulties involving intonation, stress and conversational vocal variations. Interesting phonologic

A critical appraisal of neuropsychological correlates of Japanese ideogram (kanji) and phonogram (kana) reading.

Owing to the Japanese language's unique writing system, which consists of phonograms and ideograms, reading impairments of Japanese brain-damaged patients have attracted the interest of many researchers. Past case reports as well as some widely accepted handbooks and textbooks have concluded that a specific aphasia type or lesion site is associated with a particular impairment pattern of phonograms and ideograms in reading. However, the methodology and analytical procedures in previous studies were inadequate for reliable generalizations to be made. First, the test materials were unspecified o

Picture-naming in aphasia.

The distribution of picture-naming errors for Broca's aphasics (n = 9), Wernicke's aphasics (n = 9), conduction aphasics (n = 9), frontal anomics (n = 7), and posterior anomics (n = 9) was examined to determine the diagnostic power of error types in picture-naming. Negated responses were associated with Broca's aphasia, whole-part errors ("hose" for nozzle) were associated with frontal anomia, and poor phonemic cuing was associated with Wernicke's aphasia. In addition, the relative distribution of the three most prominent naming errors-phonemic errors, semantic errors, and multiword circumlocu

Selective impairment of phonation: a case study.

A 47-year-old right-handed man underwent craniotomy for clipping of an aneurism at the trifurcation of the left middle cerebral artery. Subsequently, he suffered a left hemisphere CVA after which his speech and language resembled that of Broca's aphasia with accompanying apraxia of speech. Medical, behavioral, and acoustical data amassed over a period of several months indicated numerous contraindications to traditional diagnoses of Broca's aphasia, apraxia of speech, and dysarthria. Ultimately, it was determined that the patient had a selective impairement of phonation or laryngeal apraxia. T

Language-related brain function during word repetition in post-stroke aphasics.

We compared fMRI findings (using SPM99) obtained with repetition task in normal subjects with those of two patients with Broca's and Wernicke's aphasia who received speech therapy and showed complete recovery. Both aphasic patients with left hemisphere damage who showed complete recovery exhibited activation of only the compensatory area in the right hemisphere during the repetition task. Recovery from Broca's aphasia involves reorganization and neuromodulation between the external temporopolar area and the anterior superior temporal area of the superior temporal gyrus, putamen and the inferio
