Isotope localization of infarcts in aphasia.

Radionucleide localization of infarcts producing aphasia was undertaken in 65 patients with a scan-test and onset-test interval of one months or less. The scans were traced on anatomical templates without knowledge of the aphasias. A phasics were classified by their test scores according to taxonomic criteria, independently from localization. Scans belonging to each clinically distinct group were overlapped "blindly". The results showed distinct areas for Broca's conduction and Wernicke's aphasics along the parasylvian axis of the lateral templates. Lesions of global aphasics covered all these

Structural Characteristic of the Arcuate Fasciculus in Patients with Fluent Aphasia Following Intracranial Hemorrhage: A Diffusion Tensor Tractography Study.

This study investigated the relationship between the structural characteristics of the left arcuate fasciculus (AF) reconstructed using diffusion tensor image (DTI) and the type of fluent aphasia according to hemorrhage lesions in patients with fluent aphasia following intracranial hemorrhage (ICH). Five patients with fluent aphasia following ICH (three males, two females; mean age 55.0 years; range 47 to 60 years) and with sixteen age-matched heathy control subjects were involved in this study. The ICHs of patients 1 and 2 were located in the left parietal lobe and the left basal ganglia. ICH

Error variability and the differentiation between apraxia of speech and aphasia with phonemic paraphasia.

PURPOSE: This study was conducted to evaluate the clinical utility of error variability for differentiating between apraxia of speech (AOS) and aphasia with phonemic paraphasia. METHOD: Participants were 32 individuals with aphasia after left cerebral injury. Diagnostic groups were formed on the basis of operationalized measures of recognized articulatory and prosodic characteristics of AOS and phonemic paraphasia. Sequential repetitions of multisyllabic words were elicited as part of a motor speech evaluation and transcribed phonetically. Four metrics of variability at the syllable and word

[A suddenly nonsensical speaking man].

A 61-year-old man presented to the emergency department with acute fluent but nonsensical speech and intact comprehension. Imaging showed a contrast-enhancing lesion near Wernicke's area, suspect for an intracranial neoplasm, without signs of ischemia or hemorrhage. Suspecting focal epilepsy with conduction aphasia, symptoms resolved after anti-convulsive therapy.

Crossed aphasia with left spatial neglect and visual imperception: a case report.

A 64-year-old right-handed woman with no left-handers in the family developed aphasia associated with moderate left hemiparesis and dense left homonymous hemianopia following rupture of a right middle cerebral artery aneurysm and subsequent selective surgery confined to the right hemisphere. Severe left spatial neglect and constructional apraxia were also present. The patient was an achondroplasic dwarf whose previous medical and neurological history was otherwise unremarkable. Computed tomography of the brain showed a large right temporo-insulofrontoparietal lesion. Language and nonverbal cog

Lexical stress contrast marking in fluent and non-fluent aphasia in Spanish: The relationship between acoustic cues and compensatory strategies.

This study sought to investigate stress production in Spanish by patients with Broca's (BA) and conduction aphasia (CA) as compared to controls. Our objectives were to assess whether: a) there were many abnormal acoustic correlates of stress as produced by patients, b) these abnormalities had a phonetic component and c) ability for articulatory compensation for stress marking was preserved. The results showed abnormal acoustic values in both BA and CA's productions, affecting not only duration but also F0 and intensity cues, and an interaction effect of stress pattern and duration on intensity

[CHANGES IN THE WAY WE OBSERVE, ASSESS AND TREAT APHASIA: FROM THE TRADITIONAL APPROACH TO THE PSYCHOLINGUISTIC AND SOCIAL APPROACHES].

Aphasia is an acquired language impairment, initially identified and described before the 19th century. According to traditional models (Wernicke-Lichtheim-Geschwind) the aphasic symptoms can be clustered into particular syndromes, such as Broca's, Wernicke's, Conduction aphasias, and more. Each syndrome is allegedly associated with a specific anatomical site. The major motivation for this model was to use the behavioral symptoms to learn about language and brain relationships. However, current advanced imaging techniques identify more precisely the loci of the deficit. Moreover, the model fre

Revisiting the public awareness of aphasia in Exeter: 16 years on.

Background: Surveys of awareness of aphasia have been conducted worldwide. There has been no survey of change in awareness in one place over time. A survey in Exeter, UK in 2001 found awareness of aphasia was strikingly low. The aim of this study was to conduct a repeat survey using the same methods in the same city 16 years later to examine changes in awareness and knowledge. Method: We surveyed 167 shoppers in Exeter examining awareness and knowledge of aphasia. Awareness of stroke, stuttering, dyslexia and autism were examined for comparison. Demographic information was collected. Result: T

Negotiation of identity in group therapy for aphasia: the Aphasia Café.

BACKGROUND: There has been increasing interest in ensuring that aphasia intervention includes attention to the negotiation of a robust identity after the life-altering changes that often accompany the onset of aphasia. But how does one go about simultaneously improving communication and positive identity development within aphasia therapy? Socially oriented group therapy for aphasia has been touted as one means of addressing both psychosocial and communicative goals in aphasia. AIMS: This article describes the results of a sociolinguistic analysis of group therapy for aphasia in which positiv

Arcuate fasciculus tractography integrated into Gamma Knife surgery.

OBJECT: To prevent speech disturbances after Gamma Knife surgery (GKS), the authors integrated arcuate fasciculus (AF) tractography based on diffusion tensor (DT) MR imaging into treatment planning for GKS. METHODS: Arcuate fasciculus tractography was retrospectively integrated into planning that had been previously performed by neurosurgeons and radiation oncologists. This technique was retrospectively applied to 12 patients with arteriovenous malformations adjacent to the AF. Diffusion tensor images were acquired before the frame was affixed to the patient's head and DT tractography images

Difficulty in reaching objects and body parts: a sensory motor disconnexion syndrome.

A 19-year-old woman is described who, following an acute episode of eclampsia during pregnancy, displayed a complex neuropsychological syndrome. Along with a mild pattern of conduction aphasia, she showed a severe apraxia, total agraphia, marked constructional disturbances and was unable to reach targets outside and on her body and to perform accurate motor tasks under visual, tactile and proprioceptive control. These symptoms are discussed as the results of a disconnexion syndrome and related to the C.T. scan findings.

Intraventricular, cystic, atypical meningioma.

A 37-year-old male presented with a very rare cystic meningioma in the trigone of the left lateral ventricle. Neurological examination revealed mild conduction aphasia and right hemisensory disturbance. Computed tomography and magnetic resonance imaging showed a solid, enhancing tumor in the left trigone which had multiple cystic components located anteriorly and superiorly. The tumor was totally resected via a parietal, transventricular approach. Histological examination revealed an atypical meningioma with cellular pleomorphism and prominent nucleoli. Both the solid component and the cyst wa

Restoring one's language edifice: A case study of long-term effects of intensive aphasia therapy employing cognitive modifiability strategies.

NG is an architect who suffered a left occipital-parietal hemorrhage cerebral vascular accident (CVA) in 2000, resulting in aphasia of Wernicke and conduction types. He was characterized with fluent paraphasic speech, decreased repetition, and impaired object naming. Comprehension was relatively preserved but reading and writing were severely compromised, as well as his auditory working memory. Despite a grim prognosis he underwent intensive aphasia therapy, lasting from 2001 to 2010, at the Center for Cognitive Rehabilitation of the Brain Injured at the Feuerstein Institute. Prosodic disturbance in aphasia: vowel length in Thai.

The aim of this study is to determine to what extent a phonologically contrastive function of the prosodic feature of length is resistant to impairment in aphasia. The language chosen for investigation is Thai, a language which contrasts short and long vowels. Subjects included two Broca aphasics, one transcortical motor aphasic, one Wernicke aphasic, one conduction aphasic, one nonaphasic right-brain-damaged patient, one nonaphasic cerebellar dysarthric patient, and five normal controls. The subjects read a list of words containing short and long vowels. Vowel durations were measured from spe

A new life with aphasia: everyday activities and social support.

People who develop aphasia must adjust their lifestyles and learn to cope with the activity limitations that may follow from their disability. The purpose of this study was to describe aphasic individuals' experiences of everyday activities and social support in daily life. Interviews were conducted with 20 people with aphasia, and analysed with qualitative content analysis. The results show that everyday activities changed considerably with the onset of aphasia, and the participants were hindered from participating in activities by communication problems or physical disabilities. Aphasia led

Neurobiology of learning disabilities.

Learning disabilities are the product of neurological damage which occurs during fetal development. The malformations produced are unique to each individual and do not resemble acquired neurological damage. Consequently, models of developmental disorders must be developed independently and cannot be based on acquired adult aphasia or dyslexia literature. Neurological conditions of adults with developmental disorders are difficult to interpret since aberrations that occur early during fetal development can have a cascading effect, disrupting neural organization in other brain regions. Diffusion tensor imaging magnetic resonance imaging (DTI-MRI) helps to tailor speech therapy: A case report with a short narrative review.

BACKGROUND: Stroke has functional sequelae, including motor weakness, spasticity, dysphagia, and neurogenic bladder deteriorating activities of daily living. Speech therapy is more often an essential part of the rehabilitation program. Studies in aphasia have primarily focused on two major pathways: the arcuate fasciculus and the superior longitudinal fasciculus. The arcuate fasciculus is a major fiber bundle connecting Broca's area (associated with language production) and Wernicke's area (associated with language comprehension). Damage to this pathway can result in different types of aphasia

[Language and aphasias].

INTRODUCTION AND OBJECTIVE: Approximately 400,000 years ago men started to use language. Initially it was probably poor with few phonemes. With social evolution it became more complex, with the appearance of new phonemes and a more complete grammatical structure. The current concept of the processing of language dates, with little change, from the nineteenth century. DEVELOPMENT: With the birth of phrenology language began to be studied. This lead to the hypothesis of Wernicke, with two main areas joined by the fasciculo arcuato, which is still held to be valid with modifications by Gerchwind
