When a tooth with an apical lesion is identified, an apicoectomy is frequently performed when nonsurgical treatment is considered unfeasible or has previously failed. However, the treatment is usually difficult in molars. This article describes a minimally invasive procedure for removing a gutta-percha point under the maxillary sinus mucosa using an ultrathin arthroscope and a visualization approach in apicoectomy using an intranasal endoscope. These surgical techniques using endoscopes are minimally invasive and reliable procedures that provide limited incision and bone removal and respect th

PURPOSE: This study evaluated the treatment outcome of intentional replantation of molars.
PATIENTS AND METHODS: Twenty-nine patients were treated with intentional replantation because conventional apicoectomy was not possible. The success rate was judged by clinical and radiographic parameters.
RESULTS: One molar (3%) had to be removed because of pain and mobility 4 weeks postsurgery, and three molars (11%) had to be removed during the first year because of periodontal problems. Four molars (14%) showed periodontal problems or root resorption, but are still in function and causing no obviou

BACKGROUND: Radicular cyst is a lesion of odontogenic origin that arises from epithelial remains due to periapical periodontitis caused by inflammatory reactions generated at the apex of affected teeth with infected or necrotic pulps. The therapeutic management of radicular cysts is controversial. There is only one case report of enucleation of a radicular cyst managed with microsurgery and apicoectomy, but without the use of the guided tissue regeneration (GTR) technique in the same surgical procedure. The present clinical case describes the management of a radicular cyst with microsurgical a

Dens in dente is the result of an invagination of the enamel organ into the developing dental papilla. The full pathologic potential of this lesion is often not fully appreciated. A case is reported in which a cyst resulting from a dens in dente obliterated the maxillary sinus and necessitated root canal therapy and apicoectomies in four adjacent teeth.

BACKGROUND: Teeth with deep gingival recessions (GRs) associated with apex exposure present a challenging clinical scenario, often requiring a multidisciplinary approach to improve tooth prognosis. This case study presents seven RT1/RT2 deep GR (≥5 mm) associated with root apex exposure, treated with endodontic treatment, apicoectomy, and root coverage procedures.
METHODS: For this retrospective study, all surgical procedures were performed using either the laterally positioned flap technique or the modified double papilla flap combined with a connective tissue graft (CTG).
RESULTS: After a

OBJECTIVE: To explore an effective orthodontic method for treating impacted dilacerated maxillary central incisors.
METHODS: 12 impacted teeth of 11 cases were chosen. The position of the impacted teeth and the relations with neighbour tissues were assessed by X-ray images, oral examination and plaster model, etc. The impacted teeth were induced to erupt with the closed-eruption technique and fixed orthodontic appliances, root canal therapy, apicoectomy and prosthetic treatment.
RESULTS: 12 impacted teeth were moved into arches, and the occlusal relationship was normal. There were no root re

A case of external inflammatory root resorption and labial fenestration in a maxillary central incisor is presented. The root canal was dressed with pure calcium hydroxide mixed with normal saline for 1 month before it was obturated with gutta percha and apicoectomy surgery undertaken to attempt primary closure of the fenestration. The PDL and the fenestration healed uneventfully.

The present communication describes the endodontic retreatment of a rare four-rooted maxillary second molar. A 25-year-old patient was referred to our dental practice requesting an apicoectomy because of continuous and permanent pain reaction six months after the first endodontic treatment. The sent radiograph demonstrated three filled root canals (one mesial, two distal) and four radiographically superimposing roots (two mesial, two distal). Due to the diagnosed chronic apical periodontitis and based on the visible untreated root canal, we decided to endodontically re-treat the quadrangular t

A case is reported of a patient who requested treatment for chronic pain of 7 yr duration. After several unsuccessful endodontic procedures, a cotton pellet was discovered under the soft tissue near the apex of the left maxillary cuspid. It was theorized that the cotton was left under the tissue flap during a previous apicoectomy procedure. After the cotton was removed the patient subsequentially reported that the pain was relieved.

INTRODUCTION: This study investigated the differentiation of radicular cysts from granulomas. Cone beam computed tomography (CBCT) imaging was compared with the existing standard, biopsy and histopathology.
METHODS: Forty-five patients scheduled for an apicoectomy received a CBCT scan of the involved arch. Two oral and maxillofacial radiologists, working independently and using the same criteria, categorized the CBCT images as one of the following: cyst, likely cyst, likely granuloma, granuloma, or other. After apicoectomies, two oral pathologists, working independently and using the same cri

We describe the enucleation of large radicular cysts to the maximum extent, and their treatment based on the concept of marsupialization and drainage after apicoectomy. Marsupialization requires a long period for healing, imposing a burden on the patient with regard to postoperative management. Considering this, together with the difficulty involved in the clinical diagnosis of radicular cysts, curettage of the cyst wall and drainage may be more effective for facilitating the healing process than use of marsupialization alone.

A large maxillary cyst was treated for 3 months with marsupialization and decompression, followed by surgical endodontic therapy of the affected teeth and cystectomy. Although small cystic lesions will typically heal with nonsurgical endodontic therapy, larger lesions may need additional treatment. Surgical enucleation of a large cystic lesion may lead to damage of other teeth or anatomic structures. Therefore, treatment should begin with the more conservative approach of decompression, to reduce the size of the lesion, followed by apicoectomy and cystectomy.

"Lumpy jaw" of artiodactyls and macropods, although often considered a manifestation of actinomycosis, is actually an osteomyelitis, perhaps commencing with a dental root abscess or trauma to the jaw. Anaerobes may be cultured from the lesions, and vegetable matter may be impacted in them. It is a chronic disease and difficult to treat. Successful treatment may include draining the abscess, cleaning the cavity with saline and antibiotics, flushing with a combination of hydrogen peroxide-sodium hypochlorite and Betadine, and apicoectomy and endodontic filling when active inflammation has subsid

The most frequent cystic lesions affecting the jaws are radicular cysts, which present radiographically as an oval or pear-shaped unilocular radiolucency in the periapical region. Radicular cysts are usually asymptomatic. This case study describes the surgical treatment of a radicular cyst involving two maxillary anterior teeth. The diagnosis of radicular cyst was confirmed through a clinical and radiographic evaluation. An apicoectomy was performed, and Mineral Trioxide Aggregate (MTA) was placed as the apical barrier. This case report illustrates the effective surgical management of an exten

Dental implantation has become an important procedure for both dental treatment and head and neck reconstructive surgery. However, this useful technique sometimes results in peri-implantitis. We describe a rare complication of peri-implantitis in the maxilla which extended to the soft tissue and caused an extra-oral fistula above the alar region. The patient underwent the placement of dental implants in the maxilla 8 years earlier. Radiography showed osteolysis of the maxilla and implant exposure. After the implants were removed, the patient was fitted with a conventional fixed partial denture

This paper describes the application of ropivacaine, local, amid, long-acting anesthetic in oral surgery. For infiltrative anesthesia, 0.75% of Naropin (ropivacaine) was used in eight patients undergoing various operations (maxillary sinus with oro-antral communication, extraction of upper and lower impacted wisdom teeth, cystectomy, apicoectomy and tooth extraction). The achieved anesthesia in all patients enabled analgesia in the course of the operation, and the expected intraoperative and postoperative bleeding, whereas postoperative analgesia lasted long enough (up to 380 minutes) to preve

A right maxillary central incisor and an adjacent lateral incisor with a dens invaginatus were identified as requiring endodontic treatment. Both teeth were treated by nonsurgical means. Thirteen months postoperatively the central incisor showed evidence of periapical resolution, whereas the lateral incisor did not. The lateral incisor was then treated with apical curettage, an apicoectomy, and retrograde amalgam filling. One year after the surgical procedure the periapical tissues of both the lateral and central incisors exhibited satisfactory resolution. This case demonstrates that certain c

INTRODUCTION: A new technique was performed for the removal of an amalgam tattoo (AT), consisting in mucoabrasion combined with a free connective tissue graft (FCTG).
CASE PRESENTATION: A 59-year-old female presented with a large AT involving the alveolar mucosa and gingiva between teeth #7 and #8, where endodontic treatment and apicoectomy was performed 21 years ago. To avoid compromising esthetics, the lesion was managed with a new technique including mucoabrasion and an FCTG. At the 53-month follow-up, the patient exhibited a naturally appearing soft tissue with no evidence of the AT.
