In 1970s, Heroin-associated Nephropathy (HAN), one form of focal and segmental glomerulosclerosis (FSGS), was a predominant cause of End-stage Kidney Disease (ESKD) in African-Americans (AAs). In 1980s, with the surge of Acquired Immune Deficiency Syndrome (AIDS) in AAs, HAN more or less disappeared, and the incidence of Human Immunodeficiency Virus associated Nephropathy (HIVAN) markedly increased. Recent studies in AAs have identified APOL1 variants (Vs) as a major risk factor for the development and progression of non-diabetic kidney diseases including idiopathic FSGS and hypertension-attri

PURPOSE OF REVIEW: To summarize current understanding of the epidemiology, pathogenesis and management of HIV-Associated Nephropathy (HIVAN) and other forms of Apolipoprotein L1 (APOL1)-mediated chronic kidney disease (CKD) in people of African ancestry with HIV.
RECENT FINDINGS: The G1 and G2 variants of the APOL1 gene, which have emerged in trypanosomiasis-endemic areas of Africa, predispose to HIV-associated nephropathy (HIVAN) and other forms of CKD. The risk of APOL1-mediated CKD is attenuated with HIV control and carriage of the p.N264K genetic variant. Several treatments targeting the

Despite intensive antihypertensive therapy there was a high incidence of renal end points in participants of the African American Study of Kidney Disease and Hypertension (AASK) cohort. To better understand this, coding variants in the apolipoprotein L1 (APOL1) and the nonmuscle myosin heavy chain 9 (MYH9) genes were evaluated for an association with hypertension-attributed nephropathy and clinical outcomes in a case-control study. Clinical data and DNA were available for 675 AASK participant cases and 618 African American non-nephropathy control individuals. APOL1 G1 and G2, and MYH9 E1 varia

A case is presented which demonstrates the perils of opioid inefficacy and how pharmacogenomic testing may have prevented nonsteroidal anti-inflammatory drug (NSAID)-induced nephrotoxicity and progression to chronic kidney disease (CKD). A 62 year-old female with back pain was treated with tramadol and hydrocodone; however, neither proved effective. Consequently, to control her pain, she resorted to cocaine, marijuana, and high dose nonsteroidal anti-inflammatory drugs (NSAIDs). She eventually developed CKD. To identify CKD contributors, she underwent genotyping for Apolipoprotein L1 (APOL1),

KEY POINTS: Patients with nondiabetic apolipoprotein L1-mediated kidney disease progressed to dialysis and transplant faster than patients with other forms of nondiabetic CKD. Although patients with apolipoprotein L1-mediated kidney disease were younger with fewer comorbidities, kidney decline was accelerated underscoring the aggressive nature of apolipoprotein L1-mediated kidney disease. These findings highlight the need of targeted therapies for apolipoprotein L1-mediated kidney disease and earlier identification through genetic testing to improve patient outcomes.

Kidney injury is a well-known complication in people with coronavirus disease 2019 (COVID-19). In kidney transplant recipients with COVID-19, presentation with nephrotic syndrome has not been well described. We report on a 49-year-old black female kidney transplant recipient who presented 25 years after transplant with clinical features of nephrotic syndrome following a diagnosis of COVID-19. Histologic examination showed acute tubular injury with unremarkable glomeruli on light microscopy and diffuse foot process effacement of podocytes on electron microscopy, consistent with minimal change-l

BACKGROUND: Systemic lupus erythematosus (SLE) is characterized by a prominent interferon signature that upregulates apolipoprotein L1 (APOL1) expression. High-risk APOL1 genotypes have been associated with end-stage renal disease (ESRD) as a complication of lupus nephritis (LN); however, the strength and consistency of this association remain heterogeneous across studies.
METHODS: We conducted a systematic review and meta-analysis of observational studies to evaluate the association between APOL1 genotypes and the odds of LN-associated kidney failure among patients with SLE. A systematic sea

The Apolipoprotein L1 (APOL1) innate immunity gene product represents the sole member of the APOL gene family in humans capable of secretion into circulation, thereby mediating the trypanolysis of T. brucei brucei. Gain-of-function variants of the APOL1 gene originated and spread among human population groups to extend APOL1's protective capacity to include also serum-resistant subspecies, such as T. brucei gambiense (S342G known as APOL1-G1) and T. brucei rhodesiense (N388_Y389del known as APOL1-G2). The biochemical pathways underlying the lytic activity of these evolutionary favored mutation

INTRODUCTION: Beverage intake is an important yet understudied contributor to blood pressure (BP) and proteinuria. This is particularly relevant in sub-Saharan Africa, where rapid shifts toward sugar-sweetened beverages (SSBs) and ultra-processed beverages, driven by affordability and aggressive marketing, are occurring alongside a high burden of Chronic Kidney Disease (CKD) within resource-limited health systems. Additionally, there are cultural differences within African populations that make beverage patterns in sub-Saharan Africa differ across populations and from Western cultures.

BACKGROUND AND AIMS: The associations of dietary patterns with blood pressure (BP) and Chronic Kidney Disease (CKD) in sub-Saharan Africa remains uncharacterized. We examined population-specific dietary patterns and their associations with BP and proteinuria in West Africans with CKD.
METHODS AND RESULTS: In a cross-sectional analysis of the Diet, CKD and APOL1 (DCA) study, dietary data from 713 participants were analyzed. Five dietary patterns were identified using principal component analysis: Processed Cereals and Soups & Sauces; Poultry and Vegetable & Seed Oils; Processed Meat and Season

Two coding variants of APOL1 account for much of the excess risk of focal segmental glomerulosclerosis in people of recent West African ancestry. There is an unmet need of treatment for apolipoprotein L1 kidney disease. In this issue, Sula Karreci et al. reported that lisinopril reduced proteinuria and glomerulosclerosis in a mouse model of apolipoprotein L1-induced focal segmental glomerulosclerosis, in a genotype-specific manner. By contrast, hydralazine and dapagliflozin had no effect. This study highlights the potential therapeutic utility of angiotensin-converting enzyme inhibitor in apol

Collapsing glomerulopathy (CG) is a pattern of kidney injury characterized by segmental or global collapse of the glomerular tuft associated with overlying epithelial cell hyperplasia. Although CG may be idiopathic, a wide range of etiologies have been identified that can lead to this pattern of injury. Recent advances have highlighted the role of inflammatory and interferon signaling pathways and upregulation of apolipoprotein L1 (APOL1) within podocytes in those carrying a high-risk APOL1 genotype. In this review, we describe the etiology, pathogenesis, pathology, and clinical course of CG,

In coronavirus disease-19 biopsies, detection of severe acute respiratory syndrome coronavirus 2 was rare with no evidence of viral replication, whereas autopsy tissue failed quality control. In patients with FSGS, apolipoprotein L1 expression differed by degree of immune cell infiltrates, with some podocytes exhibiting up to 18-fold higher expression. In COVAN, the predicted high induction of apolipoprotein L1 expression occurs in a pattern consistent with the stochastic nature of FSGS pathology.

Apolipoprotein L1 risk variants contribute to high rates of kidney disease among individuals of African ancestry. These gain-of-function variants cause toxicity to kidney cells, but the mechanism of injury is not clear. Many theories have been proposed, yet the field has not arrived at a consensus. Kim et al. make a compelling argument for a peroxisomal pathway of injury specifically activated under hypoxic conditions. Introducing cellular context reframes the debate about apolipoprotein L1 mechanism of disease.

Chronic kidney disease (CKD) is highly prevalent World-Wide and it is an important cause of morbidity and mortality. In 2010, variants in apolipoprotein type 1 (APOL1) gene was identified as a major risk factor for higher prevalence of CKD in individuals of African ancestry. The mechanisms by which toxic gain of function APOL1 variants cause disease are the subjects of current investigations, and there is currently no effective targeted therapy for APOL1 associated kidney disease. Egbuna and others recently reported that an orally administered small molecule compound inaxaplin (VX-147) that bi

The recent and exclusively in humans and a few other higher primates expressed APOL1 (apolipoprotein L1) gene is linked to African human trypanosomiasis (also known as African sleeping sickness) as well as to different forms of kidney diseases. Whereas APOL1's role as a trypanolytic factor is well established, pathobiological mechanisms explaining its cytotoxicity in renal cells remain unclear. In this study, we compared the APOL family members using a combination of evolutionary studies and cell biological experiments to detect unique features causal for APOL1 nephrotoxic effects. We investig

African trypanosomes (the prototype of which is Trypanosoma brucei brucei) are protozoan parasites that infect a wide range of mammals. Human blood, unlike the blood of other mammals, has efficient trypanolytic activity, and this needs to be counteracted by these parasites. Resistance to this activity has arisen in two subspecies of Trypanosoma brucei - Trypanosoma brucei rhodesiense and Trypanosoma brucei gambiense - allowing these parasites to infect humans, and this results in sleeping sickness in East Africa and West Africa, respectively. Study of the mechanism by which T. b. rhodesiense e

BACKGROUND: FSGS is a heterogeneous diagnosis with a guarded prognosis. Polymorphisms in the apolipoprotein L1 ( APOL1 ) gene are associated with developing FSGS and faster progression to kidney failure in affected patients. Better understanding the natural history of patients with FSGS and APOL1 risk alleles is essential to improve patient care and support the design and interpretation of interventional studies. The objective of this study was to evaluate the quantitative association between APOL1 and kidney disease progression and the interaction with other clinical and laboratory factors.
