Risk Factors for Smoking in Rural Women: The Role of Gender-Based Sexual and Intimate Partner Violence.

BACKGROUND: Women living in Ohio Appalachia experience cervical cancer at disproportionately high rates. Intimate partner and sexual gender-based violence (GBV) and smoking are independent risk factors for cervical cancer and interact to heighten risk. Appalachian women smoke at higher rates than other Ohio women, but little is known about GBV exposure in the region. The purpose of this study was to establish prevalence of women's exposure to GBV in Ohio Appalachia and examine the association between GBV and smoking among women in the region. METHODS: A two-phase address-based random sampling

Exploring Patterns of the Use of Electronic Nicotine Delivery Systems among Adolescents in High-Risk Appalachian (U.S.A) Communities.

Background: Electronic nicotine delivery systems (ENDS) use among adolescents in the United States (U.S.) has surpassed conventional tobacco products (CTPs), including cigarettes. Increasingly, ENDS are used concurrently with CTPs and substances such as cannabis. However, few studies involve Central Appalachia, a region with historically high rates of tobacco and other substance use. Objective:  To examine prevalence of concurrent use of ENDS and cannabis among school-going adolescents in Appalachian Tennessee and delineate associations between ENDS use and substance-related risk behavior (can

Caring for Appalachians With Intravenous Drug Use-Associated Infective Endocarditis at End of Life: A Phenomenological Secondary Data Analysis.

The purpose of this study was to describe the experiences of nurses caring for patients with intravenous drug use-associated infective endocarditis at the end of life in Appalachia. This study was a secondary analysis of 9 phenomenological unstructured interviews conducted by the author with nurses as part of a study exploring their lived experiences caring for this population. Data were analyzed using van Manen's 6-step approach. The analysis revealed 5 themes: (1) care at the end of life as a resolve to care for lost causes, (2) care at the end of life as an act of courageous communication,

Hepatitis C and HIV Screening, Testing, and Risk Perceptions among South-Central Appalachian County Health Department Staff.

OBJECTIVES: Hepatitis C virus (HCV) and human immunodeficiency virus (HIV) are significant public health concerns, especially given the risks for disease interactions. Rates of HCV and HIV are increasing, especially in rural areas. Local health departments (LHDs) play an important role in rural health care, offering screening, testing, and treatment for HCV and HIV. Gaps persist in LHD resources for meeting these demands, especially in Appalachia and the US South. METHODS: To explore HCV/HIV screening, testing, and treatment approaches and perspectives in south-central Appalachian North Carol

Discerning rural Appalachian stakeholder attitudes toward memory screening.

OBJECTIVE: The aim of this descriptive study was to examine Appalachian stakeholder attitudes toward routine memory screening, and to compare and contrast results from a similar study conducted in an ethnically diverse rural Florida cohort. Determining perceptions about memory screening is essential prior to developing culturally relevant programs for increasing early dementia detection and management among rural underserved older adults at risk of cognitive impairment. Benefits of early detection include ruling out other causes of illness and treating accordingly, delaying onset of dementia s

Comparative Outcomes Following Randomization to a Pilot Facebook-Based HIV Prevention Intervention Among Appalachian Women Involved in the Criminal Legal System.

INTRODUCTION: Rural Appalachian women who use drugs and are involved in the criminal legal system are at increased risk for health consequences (such as HIV/Hepatitis C). Service barriers throughout rural communities have prompted a need to examine the effectiveness of novel intervention delivery methods (e.g., social media). PURPOSE: This study aims to determine if enhancing an existing HIV prevention intervention with additional modules delivered via Facebook improves service access by examining short-term outcomes among Appalachian women returning to the community following jail release.

Community-level Factors and their Associations with Changing Opioid Overdose Fatality Rates in Kentucky, 2019-2021.

INTRODUCTION: Kentucky has one of the highest opioid overdose fatality rates in the United States, which has increased significantly from 2019 to 2020. The COVID-19 pandemic has caused lasting effects on mental health and health care, which have been linked with increased opioid overdose. These effects are exacerbated in Appalachian regions, where there is a lack of sufficient access to community pharmacies and adequate health care. PURPOSE: In this study, we characterize changes in opioid overdose fatality rates in Kentucky from 2019 to 2021, with a specific focus on changes in Appalachian v

Creating a community-based protocol to prevent unintended pregnancy in homeless women in north central Appalachia.

Homeless women are pregnant about twice the rate of other U.S. women of reproductive age. It is estimated that at the time of conception, approximately three out of four pregnancies among homeless women are unintended. In addition, children born into homelessness have about 12 times the chance of other children being placed into foster care. This rapid-cycle improvement project focused on developing a community-based protocol to address unintended pregnancy in homeless women in north central Appalachia. Phone interviews with four homeless women and a focus group of ten community stakeholders w

Severe floods increase long-term opioid overdose mortality.

The relationship between climate disasters and substance use-related harms has been understudied. This study employed a mixed methods research design to assess the impact of severe floods on opioid overdose mortality and identify mechanisms driving this relationship. Drawing on opioid overdose death data from NCHS, presidential disaster declarations from FEMA, and severe storm event data from NOAA, we found severe floods in rural Appalachia led to a significant increase in county-level overdose mortality that persisted for a decade postflood. We then collected retrospective qualitative data fr

"No rest for me tonight": A social-ecological exploration of insomnia in rural Appalachian women.

OBJECTIVES: Insomnia disproportionally affects women and is prevalent among rural Appalachian adults at higher rates than in the general US population. Given the strong, bi-directional relationship between sleep and health, a better understanding of insomnia in this health-disparate population is critical. The present study focused on the sex (females), gender (women), and age group (45+) at highest insomnia risk and explores the social determinants of sleep that contributed to insomnia. METHODS: Semistructured telephone interviews were conducted to understand factors associated with insomnia

Evaluation of a community approach to address substance abuse in Appalachia.

The purpose of this study was to evaluate a conference based program designed to enable 26 Appalachian teams to create intervention plans to address substance abuse in their communities. In March 2006, 26 Appalachian teams attended a conference to address substance abuse in their region. Teams of four to seven stakeholders generated community plans. The teams were evaluated 8 months later using a framework based on an adaptation of the Health Belief Model. Team leaders participated in structured telephone interviews. Results of this study found that the most common benefit to promulgating plan

Buprenorphine dispensing in an epicenter of the U.S. opioid epidemic: A case study of the rural risk environment in Appalachian Kentucky.

BACKGROUND: Buprenorphine is a cornerstone to curbing opioid epidemics, but emerging data suggest that rural pharmacists in the US sometimes refuse to dispense this medication. We conducted a case study to explore buprenorphine dispensing practices in 12 rural Appalachian Kentucky counties, and analyze whether and how they were shaped by features of the rural risk environment. METHODS: In this case study, we conducted one-on-one semi-structured interviews with 14 pharmacists operating 15 pharmacies in these counties to explore buprenorphine dispensing practices and perceived influences on the
