Preparedness of the Brazilian health-care system to provide access to a disease-modifying Alzheimer's disease treatment.

BACKGROUND: While the majority of patients with Alzheimer's disease resides in low and middle-income countries, little is known of their preparedness for emerging disease-modifying treatments. We analyze the preparedness of Brazil, one of the most populous middle-income countries, from a capacity and institutional preparedness perspective. METHODS: Desk research and 12 interviews for background and capacity data. Markov model to estimate wait times for access to treatment. FINDINGS: Brazil has no national dementia strategy or established pathway for evaluation of cognitive concerns, and deme

Effectiveness of Short Message Service Reminders on Timely Pick-up of Antiretroviral Therapy Among Consenting Persons with HIV in Zambézia Province, Mozambique.

A short message service (SMS) intervention was implemented in July 2016 at eight health facilities (HFs) within Zambézia Province, Mozambique. We assessed effectiveness by comparing antiretroviral therapy (ART) pick-up rates among persons with HIV (PWH) eligible for SMS reminders. All adult (≥15 years of age) PWH enrolled in ART services reporting cell phone access were offered SMS. PWH were sent messages 15, 7, and 2 days before scheduled ART pick-up appointments. Using routine data (July 2016-May 2018), mixed-effect logistic regression was used to determine adjusted odds ratios (aORs) of ART

Health disparities investigated in a primary care penicillin allergy removal pathway.

Background: Unconfirmed penicillin allergies over time lead to poor health outcomes and increased health-care cost. Health disparities (HD) can create barriers in optimizing penicillin allergy care. Objective: The objective was to characterize HDs in our primary care-led amoxicillin challenge (PLAC) delabeling pathway within a universal coverage health care system. Methods: In three outpatient clinic sites, 41,104 patients were screened, and 1,749 patients were discovered to have penicillin allergies. Of the 1,749 patients with penicillin allergy, 336 (ages 4 months to 76 years) were determine

Rapid Transition to Telemedicine During the COVID-19 Pandemic: Medical Genetics Experience.

BACKGROUND: The coronavirus SARS-CoV-2 (COVID-19) pandemic interrupted delivery of outpatient health care to minimize risk of exposure. This pandemic threatened to increase longstanding national concerns about access to both initial and follow-up genetics clinics services. The Medical Genetics Clinic at the University of Wisconsin-Madison Waisman Center (WCMGC) rapidly transitioned to offering appointments using telemedicine in March 2020 when the public health emergency for COVID-19 pandemic was declared. METHODS: Datasets were reviewed for the periods April - July 2019 (pre-COVID baseline)

Adherence to behavioral therapy and psychiatry visits in a safety-net setting in Virginia, USA.

Little is known about predictors of adherence to outpatient behavioural therapy and psychiatry visits in those who experience homelessness. Yet, consistent receipt of services in the community is critical to preventing use of acute care psychiatric services, which cause a significant cost burden to the mental health system. This retrospective study examined sociodemographic, housing instability and health-related factors associated with adherence to behavioural therapy and psychiatry appointments among 1711 clients served by an urban healthcare for the homeless centre in Virginia, USA. Modeling Patient No-Show History and Predicting Future Appointment Behavior at the Veterans Administration's Outpatient Mental Health Clinics: NIRMO-2.

INTRODUCTION: No-shows are detrimental to both patients' health and health care systems. Literature documents no-show rates ranging from 10% in primary care clinics to over 60% in mental health clinics. Our model predicts the probability that a mental health clinic outpatient appointment will not be completed and identifies actionable variables associated with lowering the probability of no-show. MATERIALS AND METHODS: We were granted access to de-identified administrative data from the Veterans Administration Corporate Data Warehouse related to appointments at 13 Veterans Administration Medi

Sociodemographic characteristics associated with medical appointment adherence among HIV-seropositive patients seeking treatment in a county outpatient facility.

BACKGROUND: Adherence is a major problem facing HIV-seropositive patients. Low adherence has been associated with faster disease progression as well as development of drug-resistant strains of HIV. Thus it is critical to understand factors associated with treatment compliance. This study examined the independent contributions of disease severity, age, gender, household income, homelessness, ethnicity, and sexual orientation on appointment adherence. METHODS: Participants (n = 671) scheduled for appointments in an outpatient county treatment facility for HIV-related medical care completed demo

A Secret-Shopper Study Evaluating Logistic Barriers to Care Among Young Adults with Acute Lymphoblastic Leukemia.

Adolescents and young adults (AYAs) with acute lymphoblastic leukemia (ALL) experience inferior survival relative to children. As access barriers may increase outcome disparities, we conducted a secret-shopper study to identify logistical burdens when initiating care for AYAs. We developed scripts describing hypothetical AYAs with ALL seeking initial oncology appointments and, from February 2024 to March 2025, used data from population-based cancer registries to call facilities treating AYAs with ALL. Outcomes included number of attempts (up to 3) and transfers, call time, and required documen

The Effect of Telerehabilitation on Missed Appointment Rates.

The purpose of this study was to examine the effect of telerehabilitation on missed appointment rates in a rehabilitation clinic. Clients fail to attend scheduled appointments for a variety of reasons. Unmet appointments represent a loss of financial support as well as diminished efficiency and capacity to provide services. Speech therapy utilizing multiple appointments is most difficult to maintain during a treatment regimen. This may cause individuals to miss appointments and therefore not achieve desired results. For this study, researchers utilized an intense speech therapy technique, the

Challenges to sustained HIV care engagement after ED Re-linkage: The influence of substance use disorders.

Background: Emergency department (ED)-based HIV linkage-to-care programs effectively connect newly diagnosed people living with HIV (PLWH) to care and can also facilitate re-engagement among patients disengaged from HIV care. The linkage-to-care needs of these populations may differ substantially. We sought to identify factors associated with engagement outcomes following ED-based re-linkage to inform service delivery. Methods: This cross-sectional analysis used programmatic data from an ED-based HIV linkage-to-care program that served PLWH identified by automated electronic health record aler

Day surgery in Bristol.

As in the rest of the United Kingdom, day surgery in Bristol is on the increase. In the Bristol Royal Infirmary day surgery unit (DSU) we treat approximately 3,500 patients a year. We have recently opened a second operating theatre. This has shifted the proportion of cases from our general theatres. This has also improved our case mix, less minor surgery under local anaesthetics, to more of the recommended 'basket type' surgery ie hernia repair, varicose vein surgery, excision of breast lumps etc. We have recently introduced an appointment booking system that is progressing. Such changes have
