Rapidly progressing hypercorticism was observed in 2 patients with malignant neoplasms, a 72-year-old man with anaplastic small cell carcinoma and a 48-year-old woman with a primary apudoma of the liver. Both patients were treated with chemotherapy in conventional doses. Both patients developed acute dyspnea and sudden death, 7 and 10 days, respectively, after chemotherapy was given. The possibility that patients with ectopic adrenocorticotropic hormone (ACTH) secretion are excessively vulnerable to chemotherapy is raised and judicious management urged.

Substantial amounts of both dopamine and norepinephrine in addition to serotonin were found in a mesenteric metastasis of an ileal carcinoid tumor. Correspondingly, the norepinephrine-synthesizing enzymes were present in the tumor tissue and tyrosine hydroxylase was found in amounts substantially higher than the levels normally present in adrenal medullary cells. These findings confirm that the carcinoid tumors belong to the APUDomas and indicate that catecholamines might play an important role in the pathogenesis of the carcinoid syndrome.

Metaiodobenzylguanidine (MIBG) was developed initially as a tracer for oncological imaging; when labeled with 123 I or 131 I, it may detect APUDomas, such as pheochromocytomas and paragangliomas. In the last years, MIBG has found an important role also in neurology and cardiology, as cardiac innervation tracer. Actually, MIBG cardiac imaging is a universally accepted method to estimate cardiac sympathetic innervations. This review covers the role of MIBG cardiac imaging in Parkinson disease and parkinsonisms, from the pathophysiological premises for cardiac denervation to new emerging data.

An autopsy case of a 54-year-old woman with malignant pheochromocytoma and ectopic ACTH production was reported. Noradrenaline was increased in 24 hour urine and in the blood sample from the left adrenal vein. Hormone assay studies of the tumor tissue and plasma revealed abnormally high levels of ACTH. 37 patients suffering from apudomas of the pancreas are reported (18 insulinomas, 6 isletcell-hyperplasias, 9 Zollinger-Ellison syndroms, 1 glucagonoma, 3 without hormone production). In preoperative localization computerized tomography and angiography were the best with 65% positive findings. Insulinomas were enucleated, all free of recidives. 50% of operated isletcell hyperplasias had a postoperative resisting hyperinsulinism. Either gastrectomy or tumour enucleation was performed in the Zollinger-Ellison syndrome. The five-years survival rate was 43%.

We describe a patient with multicentric small bowel carcinoids, severe hypertension, primary hyperparathyroidism, and multiple parathyroid adenomas. Intense uptake of I-131 metaiodobenzylguanidine (MIBG) occurred in a parathyroid adenoma. There was no biochemical evidence of catecholamine secretion by the tumor but elevated serum levels of parathyroid hormone were demonstrated. We suspect that occasional parathyroid adenomas, like other APUDomas, may give false positive results when MIBG imaging is used to search for pheochromocytomas. Two patients with melanoma and one with apudoma, all three with metastatic disease, received monoclonal antibody infusions with mAb R-24, specific for the disialoganglioside GD3. This marker was shown to be restricted to melanoma cells and a few other tumors of neural crest origin. Following treatment with mAb R-24 both melanoma patients showed inflammatory cutaneous responses around tumor nodules, i.e. blister formation or inflammatory perinodular halos.
