Cardiovascular risk factors and vascular neuropathological findings in primary age-related tauopathy versus Alzheimer's disease.

Cardiovascular risk factors (CVRFs) are prevalent and modifiable contributors to dementia risk; however, their contributions to vascular pathologies across dementia etiologies has not been examined extensively. This study assessed differences in the prevalence of CVRFs and vascular neuropathologies between people with primary age-related tauopathy (PART) and Alzheimer's disease neuropathology (ADNP), and whether CVRFs associated differentially with vascular co-neuropathologies and antemortem cognitive status between groups. 5144 individuals from the National Alzheimer's Coordinating Center wit

Brain microRNAs differentially expressed in age-related cerebral pathologies.

Multiple brain pathologies accumulate with age, but their underlying biology remains unclear. We investigated the role of microRNAs (miRNAs) in ten age-related cerebral pathologies. Using miRNA sequencing profiles from the dorsolateral prefrontal cortex of 617 brain donors, we identified miRNAs associated with Alzheimer's disease (AD) pathology, Lewy body pathology, arteriolosclerosis, cerebral amyloid angiopathy, and LATE-NC after adjusting for age, sex, and education. After additionally adjusting for co-existing cerebral pathologies, we found miRNAs specifically associated with AD pathology

Stroke: causes and clinical features.

Stroke is a clinically defined syndrome of acute, focal neurological deficit attributed to vascular injury (infarction, haemorrhage) of the central nervous system. Stroke is the second leading cause of death and disability worldwide. Stroke is not a single disease but can be caused by a wide range of risk factors, disease processes and mechanisms. Hypertension is the most important modifiable risk factor for stroke, although its contribution differs for different subtypes. Most (85%) strokes are ischaemic, predominantly caused by small vessel arteriolosclerosis, cardioembolism and large artery

Association of vascular and degenerative brain pathologies and past medical history from the National Alzheimer's Coordinating Center Database.

The relationship between past medical histories (PMH) and dementia-related neuropathologies is not well understood. Using the National Alzheimer's Coordinating Center (NACC) database, we explored the relationship between patient-reported PMH and various vascular and degenerative neuropathologies. We examined the following PMH: transient ischemic attack (TIA), stroke, traumatic brain injury, seizures, hypertension, cardiovascular events, hypercholesterolemia, B12 deficiency, diabetes mellitus, and thyroid disease. We dichotomized the following neuropathologies: atherosclerosis, arterioloscleros

Association Between Alcohol Consumption, Cognitive Abilities, and Neuropathologic Changes: A Population-Based Autopsy Study.

BACKGROUND AND OBJECTIVES: Heavy alcohol consumption is a major global health concern linked to increased morbidity and mortality. However, the long-term impact of excessive alcohol consumption on cognitive abilities and dementia-related neuropathology is unclear. The aim of this study was to analyze the association between alcohol consumption and age-related neuropathologic outcomes in a population-based autopsy study. METHODS: This cross-sectional study used data from the Biobank for Aging Studies, classifying participants as never, moderate, heavy, or former drinkers. Alzheimer disease pat

Distinct cerebrovascular pathways underlying Alzheimer's disease-related neurodegeneration.

The etiology of cerebrovascular pathology is heterogeneous. Independent or synergistic role of this pathology relative to Alzheimer's disease (AD) pathology is necessary to clarify distinct neurodegenerative pathways. We evaluated the interplay of various cerebrovascular markers postmortem and their in vivo neuroimaging, clinical and neuropathologic correlates using data from the Alzheimer's Disease Neuroimaging Initiative (ADNI). In 109 individuals, postmortem cerebrovascular pathology (atherosclerosis of the circle of Willis, cerebral amyloid angiopathy [CAA], arteriolosclerosis, white matte

ARTS is associated with vascular risk factors, MCI, dementia, and stroke.

INTRODUCTION: ARTS, an in vivo marker of cerebral arteriolosclerosis, may identify older individuals at risk of mild cognitive impairment (MCI), dementia, and stroke due to cerebral small vessel disease, but deeper characterization in a large, diverse sample is needed. METHODS: Associations between ARTS and incident MCI, dementia, and stroke, and with several common vascular risk factors, were examined in 1226 older adults without dementia and within White (n = 707), Black (n = 400), and Latino (n = 110) subgroups. RESULTS: Higher ARTS score was associated (all ps < 0.01) with incident MCI (

[An extremely painful ulcer on the lower leg; Martorell arteriolosclerotic ulcer].

A 63-year-old woman was referred to the dermatology outpatient department with extremely painful ulcers on the right lower leg. Medical history listed hypertension, diabetes mellitus and chronic obstructive pulmonary disease. Intensive analgesia was insufficient. Blood analysis, microbial cultures and venous ultrasound did not reveal a cause. At histopathologic examination of an ulcer, arteriolosclerosis was revealed. The patient was treated for Martorell arteriolosclerotic ulcer with nifedipine, acenocoumarol and split-thickness skin grafts followed by vacuum-assisted closure therapy. Two wee

Significance of vasculopathy in IgA nephropathy patients with regard to Oxford classification and immunostaining findings: a single center experience.

INTRODUCTION: immunoglobulin A nephropathy is the most common primary glomerulonephritis around the world. It remains poorly understood whether vascular pathology plays an important role in the progression of IgAN. OBJECTIVES: Since very few studies regarding vascular disorders in IgAN are published, we aimed to evaluate the role of vascular disease in a group of IgAN patients, especially with regard to Oxford classification, immunostaining findings and various laboratory and clinical data. PATIENTS AND METHODS: The study was conducted from July 2009 to March 2013. All kidney biopsies were p

Cerebral atherosclerosis is associated with cystic infarcts and microinfarcts but not Alzheimer pathologic changes.

BACKGROUND AND PURPOSE: Some studies have reported associations between intracranial atherosclerosis and Alzheimer disease pathology. We aimed to correlate severity of cerebral atherosclerosis, arteriolosclerosis, and cerebral amyloid angiopathy with neurofibrillary tangles, neuritic plaques, and cerebral infarcts. METHODS: This autopsy study (n=163) was drawn from a longitudinal study of subcortical ischemic vascular disease, Alzheimer disease, and normal aging. Multivariable logistic regression models were used to test associations among the 3 forms of cerebrovascular disease and the presen

Early imaging in spontaneous intracranial hemorrhage: Practices and etiological diagnosis.

PurposeSpontaneous intracerebral hemorrhage (ICH) has a poor prognosis. Early imaging <72 hours is essential for etiological and prognosis assessment. We aimed to describe early imaging practices and evaluate their etiological relevance, before adopting standardized protocols.MethodsAcute spontaneous ICH patients from two university centers (2021-2022) were retrospectively included. Data included imaging, lesion, and associated vascular lesions parameters. Etiologies were classified into six categories based on imaging and clinical criteria: arteriolosclerosis, cerebral amyloid angiopathy (CAA

Differences in lobar microbleed topography in cerebral amyloid angiopathy and hypertensive arteriopathy.

Lobar cerebral microbleeds are a characteristic neuroimaging finding in cerebral amyloid angiopathy (CAA) but can also be found in hypertensive arteriolosclerosis. We aimed to investigate whether CAA is more associated with intracortical lobar microbleeds than hypertensive arteriosclerosis. Ninety-one survivors of spontaneous intracerebral hemorrhage with at least one lobar microbleed were included and underwent brain MRI and amyloid PET. We categorized lobar microbleeds as intracortical, juxtacortical, or subcortical. We assessed the associations between the lobar microbleed categories and mi

Idiopathic nodular glomerulosclerosis: three Japanese cases and review of the literature.

Idiopathic nodular glomerulosclerosis (ING) is characterized as diffuse nodular glomerulosclerotic lesions, closely resembling Kimmelstiel-Wilson lesions without diabetic mellitus. We report here three Japanese cases of ING and discuss the previous reports. The patients were 75-, 48- and 84-year-old males with a history of long-term hypertension. Laboratory examination revealed moderate proteinuria and mild renal dysfunction. Diabetes mellitus was excluded by repeated clinical and laboratory investigations. Renal histology revealed nodular glomerulosclerosis, and both afferent and efferent art

[Some pathologico-anatomic substrates of hypertensive lesions].

After some introductory remarks on the definition of the various forms of hypertension, a short description of the changes in arterial vessels associated with hypertension is given. Hypertension is by no means a prerequisite for atherosclerosis and arteriolosclerosis, but hypertension enhances such vascular changes. In malignant hypertension, typical fibrinoid arteriolonecroses occur. Hypertension leads to hypertrophy of the left ventricle through enlargement of individual muscle fibres, but numeric hyperplasia also occurs. At the same time, coronary atherosclerosis may be aggravated by system

Diffusion MRI Identifies a Prolonged Pre-WMH Phase in Cerebral Small Vessel Disease.

BACKGROUND: White matter hyperintensities (WMH) are widely used to assess cerebral small vessel disease but reflect late-stage injury. Diffusion magnetic resonance imaging (MRI) biomarkers have been proposed to capture earlier small vessel disease-related microstructural damage but their temporal progression relative to WMH and risk factors associated with progression remain unexplored. METHODS: We identified 2077 participants from the population-based cohort study of the Mayo Clinic Study of Aging in Olmsted County, Minnesota (aged 50-101 years) collected between 05/2005 and 09/2024 with lon
