Arteriovenous malformations of the brain (AMB) can cause stroke when they rupture. Epidemiological and imaging research has found that about 50% of patients with AMB present with haemorrhage, and the other 50% either present with non-focal symptoms, such as headache, seizure, or focal neurological deficit, or have no symptoms and the lesion is found during unrelated investigations. Treatment for arteriovenous malformations aims to prevent and resolve haemorrhage and is a growing interdisciplinary challenge. Although treatment uses enormous resources, there have been few studies on the risk-ben…

Cowden syndrome (OMIM No 158350) is a pleomorphic, autosomal dominant syndrome characterised by hamartomas in tissues derived from the endod … ous haemangiomas are occasionally noted. Malignancies often arise in the affected tissues. Visceral arteriovenous malformations are a recognised component of the Bannayan-Riley-Ruvalcaba syndrome but have been reported rarely in Cowden syndrome. A family is described with a clinical diagnosis of Cowden syndrome, a familial frameshift mutation in the PTEN gene, and large visc …

BACKGROUND: A rare case of a left temporal arteriovenous malformation (AVM) with bleeding 10 days after removal of another hemorrhagic AVM in the frontal lobe is reported.
METHOD: A 47-year-old man had an attack of headache and vomiting and was admitted to our hospital. On admission, a CT scan showed subcortical hemorrhage in the left frontal lobe. Left carotid angiography revealed four AVMs and a vertebro-basilar angiogram demonstrated a vascular malformation.
FINDINGS: First, a hemorrhagic AVM was removed with a hematoma. Following the operation, left carotid angiograms continued to show,

Cerebral arteriovenous malformations (AVMs) are classified angiographically into two types: the arteriovenous fistula (AVF) type and the plexiform type. However, the differences in vascular structure of these two types have not been clarified. The purpose of the present study is to elucidate the vascular structure of plexiform AVMs and to discuss the clinical significance of this classification of AVMs. Specimens of AVMs resected in 8 cases and identified by cerebral angiography as plexiform AVMs were examined. Immediately after their removal, microdissection of the terminal arterial feeder, t…

We review the presentation, imaging findings and outcome in 18 children with cerebellar arteriovenous malformations (AVM). This group is of particular interest because of the reported poor outcome despite modern imaging and neurosurgical techniques. All children had CT and 15 underwent catheter angiography at presentation. Several of the children in the latter part of the study had MRI. Of the 18 children, 17 presented with a ruptured AVM producing intracranial haemorrhage. The remaining child presented with temporal lobe epilepsy and was shown to have temporal, vermian and cerebellar hemisphe…

The purpose of this report is to determine the role of various imaging modalities, and the outcome of early intervention in neonates with high flow pial arteriovenous malformations (AVM). This report describes 4 neonates with pial AVM associated with congestive cardiac failure, and reviews previous reports. Among the 4 infants in our series, 3 underwent early surgical resection of the AVM, complete in 2 and partial in 1. One infant suffered hemorrhagic infarction postoperatively and has a mild deficit on clinical follow-up; the other 2 patients had no worsening of mild neurologic deficits afte…

A patient with polysplenia syndrome, dextrocardia, left atrial isomerism, normal great vessel relationships, and no intracardiac shunts developed progressive cyanosis and clubbing. Pulmonary arteriovenous malformations (PAVMs) were diagnosed by angiography and confirmed by lung biopsy. Superior mesenteric arteriogram revealed hypoplasia of the intrahepatic portal vein branches and a portosystemic shunt. The possible etiologies of PAVMs are discussed.

We have treated 21 patients (13 female, 8 male) with pulmonary arteriovenous malformations (PAVMs). Mean age at diagnosis was 37.5 years (range, 15 to 72 years). Presenting symptoms included dyspnea on exertion (67%), hereditary hemorrhagic telangiectasia (57%), and major neurologic events (33%). In our early experience, 8 patients had no specific treatment; their case histories illustrate the major neurologic complications of untreated PAVMs. Nine patients (8 primarily, 1 after recurrence) underwent conservative surgical excision; 4 had lobectomy, and 5 had segmentectomy or subsegmental excis…

The authors describe a case of supratentorial bilateral arteriovenous malformations in a patient who suffered an intracerebral hemorrhage. It was radiologically demonstrated that the arteriovenous malformations were separate and bilateral, located in the temporal lobes. They were removed in two operations with preoperative embolization of one. The patient did well with no neurological damage.

Four children with arteriovenous malformation were studied during a two year period (1989-1991). The main localization of these arteriovenous malformations were: sigmoid rectum, ascending colon, terminal ileum and apoendix. The diagnosis was made with colonoscopy and arteriography. The surgical resection was the procedure of choice.

24 cases of AVMs posterior cranial fossa are reported. All these cases were confirmed by pathological examination or vertebral angiogram. There are 17 cases located in cerebellar hemisphere, 4 in vermis, 2 spread to brain stem, and 1 in fourth ventricle. Most of these patients are young adults, the mean age below 40 years. The most clinical manifestations were acute subarachnoid hemorrhage, increased intracranial pressure, ataxia, and cranial nerves palsy. 22 cases underwent surgical intervention, with 17 total, 3 subtotal, 2 partial resection of AVMs. After operation, 16 cases were good recov

This is the first report in the American literature elaborating on the clinical and angiographic characteristics of an iris arteriovenous malformation. Iris vascular abnormalities that do not leak fluorescein have previously been described only in association with underlying disorders. Iris arteriovenous malformations must be differentiated from neovascularization and other diseases affecting iris vasculature in which treatment is necessary in order to prevent further sequelae.

Concerning infratentorial AVMs in three representative localizations, their surgical approaches, technical standards and technical difficulties were described and discussed. At the beginning, our opinion emphasized that feeding arteries should be divided into parent feeders and proper feeders. Parent arteries are anatomically normal ones even if dilated by the presence of peripheral shunt. Proper feeders, however, supply only AVMs without perfusing any normal brain tissue. Draining veins were also divided into two, parent drainers and proper drainers. Vascular connections should be separated a

Pulmonary arteriovenous malformations are a life-threatening, but curable, cause of persistent cyanosis in children. Abnormal communications may be single or multiple and are usually in the lower lobes. Three patients with pulmonary arteriovenous malformations have recently been managed at this hospital. Ranging in age from 2 to 4 yr, all were female, all were from families with Rendu-Osler-Weber syndrome, and all underwent pulmonary angiography with confirmation of diagnosis. All have had uneventful resection, one of the left lower lobe, one the right upper lobe, and the other the right middl…

Arterial venous malformations (AVM) of the uterus are uncommon entities and should be considered in patients who present with profuse genital bleeding. There are two types of uterine AVM: acquired and congenital. Acquired uterine AVMs are conformed by communications between the uterine arteries and the myometrial veins, and are caused by an iatrogenic event or a pathological condition. Congenital AVMs are the result of abnormal development of primitive vessels that result in connections between pelvic arteries and veins in the uterus without an interconnecting capillary bed. Ultrasonography is

Intracranial arteriovenous malformations (AVMs) are congenital lesions that can cause serious neurological deficits or even death. They can manifest as intracranial hemorrhage, epileptic seizure, or other symptoms such as headache or tinnitus. They are detected by computed tomography or magnetic resonance imaging. Recently there have been significant developments in the management of AVMs. In this paper, the authors represent an overview of the epidemiology of AVMs and the existing treatment strategies. AVMs are ideally excised by standard microsurgical techniques. The grading scale which was …

PURPOSE OF REVIEW: This review highlights recent data regarding factors associated with brain arteriovenous malformation hemorrhage and different treatment options.
RECENT FINDINGS: More risk factors were identified in association with intracranial hemorrhage, including age at initial diagnosis of arteriovenous malformation, co-existing extranidal aneurysms and genetic factors. Patients with unruptured arteriovenous malformations were found to be more susceptible to worsening in neurological function after microsurgery compared with those presenting with hemorrhagic arteriovenous malformation…
