Subtalar Arthrodesis in Patients With Prior Tibiotalar Arthrodesis for Posttraumatic Osteoarthritis.

BACKGROUND: The tibiotalar arthrodesis for end-stage ankle osteoarthritis is a surgical procedure that leads to a modification of the kinematics of the adjacent joints and may result in the development of secondary osteoarthritic degeneration of the subtalar joint. It has previously been observed that subtalar arthrodesis in this context shows a lower fusion rate than isolated subtalar arthrodesis. This retrospective study reports the results of subtalar joint arthrodesis with previous ipsilateral tibiotalar arthrodesis and suggests some factors that may compromise the fusion of the joint. ME…

AO-wrist arthrodesis: with and without arthrodesis of the third carpometacarpal joint.

The incorporation of the third carpometacarpal joint (CMCJ-3) during wrist arthrodesis is controversial. This retrospective study of 146 consecutive wrist arthrodeses with AO plate fixation specifically addresses this question. In 79 wrist arthrodeses the CMCJ-3 was also arthrodesed, and in 67 the CMCJ-3 was simply bridged. Problems relating specifically to the CMCJ-3 could not be analyzed clearly with the plate in situ. Therefore after plate removal only 81 wrists were evaluated with respect to the CMCJ-3. Of 47 wrists that had CMCJ-3 arthrodesis and plate removal, 20 developed a nonunion. El…

Arthroscopic Ankle Arthrodesis.

Despite the increasing popularity of total ankle replacement, ankle arthrodesis remains the gold standard for the treatment of end-stage ankle arthritis. Historically, open techniques have been utilized for ankle arthrodesis. There have been many variations and techniques described, including transfibular, anterior, medial, and miniarthrotomy. Inherent disadvantages to these open techniques include postoperative pain, delayed or nonunion, wound complications, shortening, prolonged healing times, and prolonged hospital stays. Arthroscopic ankle arthrodesis provides the foot and ankle surgeon wi…

Open Ankle Arthrodesis.

First described in 1879, ankle arthrodesis is a procedure that has undergone significant advancements not only in technique but also in technology and fixation. Surgeon preference has often dictated those changes with regard to incisional approaches, fixation methods, and use of bone graft and biologics but one constant has always remained: open ankle arthrodesis is a predictable, time-tested procedure with consistent results when performed in appropriate patients. This article highlights the changes that have occurred since the introduction of this procedure and provides a brief overview of t…

Arthroscopic Ankle Arthrodesis.

Arthroscopic ankle arthrodesis is a cost-effective option for many patients with posttraumatic arthritis of the ankle joint. Rehabilitation is generally quicker than conventional open techniques, and rates of fusion are comparable or better than traditional open techniques. Unless the arthroscopic surgeon has considerable experience, the best results are seen in patients with very little deformity in the ankle joint.

[Arthroscopic tibiotalocalcaneal arthrodesis].

Tibiotalocalcaneal arthrodesis (TTCA) is indicated for a variety of disorders, including end-stage osteoarthritis, severe deformities and complications after operative interventions on the upper and lower ankle joints. Due to the biomechanical advantages, TTCA is predominantly performed with curved retrograde intramedullary nails allowing compression before locking. Hindfoot arthrodesis is most commonly performed by extensive open surgical approaches. Despite a patient satisfaction rate greater than 80 %, current reviews have reported mean complication rates of more than 50 % with a pronounced…

Isolated subtalar arthrodesis.

The aim of this study is to review the results of isolated subtalar arthrodesis in adults and to make a comparative analysis of the clinical outcomes between the patients with posttraumatic subtalar arthritis and the ones with other etiologic factors, and to evaluate the effects of grafting. This study included 19 men and 12 women. The mean postoperative follow-up was 36.8 months. The mean AOFAS hindfoot score improved from a mean of 46 preoperatively to a mean of 77.3 postoperatively. Thirty-one of 33 arthrodeses achieved bony union at a mean time of 15.7 weeks. The mean increase in the taloc…

Distraction subtalar arthrodesis.

There is a high potential for disability following calcaneal fracture. This potential exists whether a patient is treated with conservative or operative management. Subfibular impingement and irritation of the peroneal tendon and sural nerve may also be present. Posttraumatic arthritis of the subtalar joint can occur. In patients with symptomatic calcaneal malunion, systematic evaluation is required to determine the source of pain. Nonsurgical treatment may be effective. One surgical treatment option is subtalar distraction arthrodesis. High rates of successful arthrodesis and patient satisfac…

Arthroscopic ankle arthrodesis.

Arthroscopic ankle arthrodesis provides the foot and ankle surgeon with an alternative to traditional open techniques. Advancements in arthroscopic techniques and instrumentation have made the procedure easier to perform. Advantages include faster rates of union, decreased complications, reduced postoperative pain, shorter hospital stays, minimal loss of length of the lower limb, and minimal clinical deformity or shapechanges to the ankle. This article reviews the literature on the subject and discusses the procedure's indications and contraindications, and surgical technique. It concludes tha…

First metatarsophalangeal arthrodesis.

Arthrodesis of the first metatarsophalangeal joint is a highly successful treatment for patients with symptomatic hallux rigidus who have failed conservative management. Before arthrodesis, the importance of host factors, such as use of nicotine, local blood supply, medical comorbidites, and use of systemic immunosuppressive agents, must be considered. Arthrodesis is currently considered the gold standard treatment for end-stage arthritis of the metatarsophalangeal joint. Careful attention to surgical detail is critical to achieving optimal outcomes.

Arthroscopic Lapidus arthrodesis.

Lapidus arthrodesis is the surgical treatment of choice for symptomatic hypermobility of the medial cuneiform metatarsal joint that is not responding to conservative treatment. The open procedure was criticized for its prolonged healing, high nonunion rate, and the tendency for dorsal angulation of the first metatarsal. We describe an arthroscopic approach that has the advantage of more thorough preparation of the fusion site with minimal bone removal and better control of the arthrodesis position and with less chance of malunion.

Isolated subtalar arthrodesis.

BACKGROUND: The purposes of this retrospective study were to review the results of isolated subtalar arthrodesis in adults and to identify factors influencing the union rate. The hypotheses were that (1) the overall outcome is acceptable but is not as favorable as previously reported, (2) complication rates, especially the nonunion rate, are higher than previously reported, and (3) factors contributing to a less favorable union rate can be identified. METHODS: Between January 1988 and July 1995, 184 consecutive isolated subtalar arthrodeses were performed in 174 adults (115 men and fifty-nine…

Isolated Subtalar Arthrodesis.

Isolated subtalar arthrodesis is a common surgical procedure used to treat subtalar joint pathologies including primary and post-traumatic arthritis, calcaneal fractures, tarsal coalitions, and posterior tibial tendon dysfunction. Its goals are pain relief, rearfoot realignment, and improved stability while preserving more motion than triple arthrodesis. This review covers the relevant anatomy of the subtalar joint and its facets, clinical examination findings, and imaging modalities. It details surgical techniques for native fusions, neglected calcaneal fractures, and rigid valgus deformities…

Percutaneous Plate Arthrodesis.

Arthrodesis is an elective surgical procedure that aims at eliminating pain and dysfunction by promoting deliberate osseous fusion of the involved joint(s). Percutaneous plating can be used to perform carpal and tarsal arthrodeses in dogs and cats. After cartilage debridement is performed, the plate is introduced through separate plate insertion incisions made remote to the arthrodesis site and advanced along an epiperiosteal tunnel, and screws are inserted through the 3 existing skin incisions. The primary advantage of this technique is a decreased risk of soft-tissue complications, including…

Arthroscopic ankle arthrodesis.

BACKGROUND: Arthroscopic arthrodesis has been used mainly for in situ fusion of arthritic ankles without deformity. This paper presents the application of arthroscopic arthrodesis of ankles with marked deformity. METHODS: The results of 78 consecutive cases of arthroscopic ankle arthrodeses, performed in 74 patients, were retrospectively evaluated. Forty-eight ankles had minor deformity (group A), whereas 30 ankles had a varus or valgus deformity of more than 15 degrees (maximum 45 degrees) (group B). The average hospital stay was 3.8 and 3.4 days in groups A and B, respectively (p = 0.74). P…
