Management of the patella during revision total knee replacement arthroplasty.

Managing the patella and balancing the patellofemoral joint space is one of the most difficult aspects of performing a primary total knee replacement (TKR). The situation is compounded in the revision situation. Unfortunately, an otherwise well-performed TKR will fail because of problems with the patella or the extensor mechanism. This article discusses various aspects of the management of the patella and extensor mechanism during revision TKR with an objective of minimizing complications and maximizing functional outcomes. The topics covered include exposure of the patella and extensor mechan

Replacement arthroplasty using the Charnley prosthesis in old tuberculosis of the hip.

This report is based upon a retrospective study of a consecutive series of twenty patients who had tuberculosis of the hip in quiescent state. Two patients had only a short history but were included in this study. An interesting feature at the time of operation was that in many cases the bone showed old features of tuberculous disease despite the long history of quiescence. A modification of the Charnley technique is discussed. In addition, details of radiographic assessment before operation, antituberculous drug therapy and the use of Kanamycin in bone cement are given. The complications and

Robotic Arthroplasty in Context: Balancing Evidence and Innovation in Joint Replacement.

➢ This review examines the contemporary evidence surrounding robotic-assisted joint replacement, including its effects on surgical precision, clinical outcomes, and implant survivorship, and the broader implications for innovation in arthroplasty.➢ Current evidence consistently demonstrates that robotic-assisted arthroplasty improves the precision and reproducibility of component positioning. However, systematic reviews, meta-analyses, randomized trials, and national registry data have not demonstrated consistent improvements in patient-reported outcomes, complication rates, or implant survivo

Treatment of osteonecrosis of the humeral head with cementless surface replacement arthroplasty.

BACKGROUND: Osteonecrosis of the humeral head is a disabling condition that can lead to joint destruction with persistent pain and dysfunction. The aim of the present study was to examine cementless humeral surface replacement arthroplasty as a treatment option for posttraumatic and nontraumatic osteonecrosis. METHODS: Between 2002 and 2005, seventeen shoulders with posttraumatic (n = 8) or nontraumatic (n = 9) osteonecrosis of the humeral head were treated with cementless humeral replacement arthroplasty. The mean proportion of the humeral head that was occupied by necrotic bone was 18.6% (r

Cementless surface replacement arthroplasty of the shoulder with biologic resurfacing of the glenoid.

BACKGROUND: We report a retrospective review of surface replacement hemiarthroplasty of the shoulder with biologic resurfacing of the glenoid, for relatively young patients suffering from advanced glenohumeral arthritis. It was hoped that the resurfacing fascia would prevent glenoid erosion for a period of time. METHODS: This surgery was performed by two surgeons between 1996 and 2005. The indications for the arthroplasty were severe pain and limitation of function, in relatively young patients with advanced glenohumeral arthritis but an intact rotator cuff. RESULTS: Eighteen shoulders, with

A conservative femoral replacement for total hip arthroplasty. A prospective study.

Between 1985 and 1993, 146 patients (162 hips) had total hip replacement (THR) using a conservative uncemented femoral component. The mean age of the patients was 50.8 years and the mean follow-up was 6.2 years (2 to 13). One patient was lost to follow-up, one died within two years of surgery and one had a revision procedure after a fracture sustained in a road-traffic accident. For the remaining 159, Kaplan-Meier survival analysis was calculated for the incidence of revision because of mechanical loosening or osteolysis. Survival without mechanical loosening at both five and ten years was 98.

Total ankle replacement in patients with haemophilic arthropathy: primary arthroplasty and conversion of painful ankle arthrodesis to arthroplasty.

INTRODUCTION: There are two general surgical approaches for operative treatment of end-stage haemophilic ankle arthropathy: ankle arthrodesis and total ankle replacement (TAR). AIM: The aim of this study was to determine intraoperative and postoperative complications and evaluate the mid-term clinical and radiographic outcomes of TAR in patients with haemophilic arthropathy. METHODS: Fourteen patients with a mean age of 51.4 ± 10.2 years (range = 32.9-63.7) were treated for end-stage haemophilic ankle arthropathy. Nine procedures were primary arthroplasties, five procedures were conversions

Modular total knee-replacement arthroplasty. A review of eighty-nine patients.

Fifty-eight osteoarthritic and thirty-one rheumatoid patients underwent modular total knee-replacement arthroplasty. The major indication for the operation was relief of pain. Contraindications to this resurfacing arthroplasty included varus-valgus instability of over 20 degrees, combined varus-valgus instability with flexion contracture of over 40 degrees, marked recurvatum, and predominant patellofemoral symptoms. In 59 per cent of the osteoarthritic and 58 per cent of the rheumatoid patients, complete relief of pain was evident when they were evaluated twenty-four months after surgery, whil

Distal femur replacement after catastrophic failure of a total knee arthroplasty.

Catastrophic implant failure is a rare complication after total knee arthroplasty (TKA) that can lead to metallosis and pseudotumour formation requiring complex revision surgery. This case report describes a man in his 60s who presented with progressive knee pain, a squeaking implant and ligamentous instability 20 years post-primary TKA. Imaging demonstrated polyethylene failure, tibial baseplate breakage, extensive osteolysis and metallosis. The patient underwent a revision procedure with a distal femur replacement (DFR). Postoperatively he had no complications, re-admissions or revision surg

Replacement arthroplasty versus internal fixation for extracapsular hip fractures in adults.

BACKGROUND: Internal fixation, commonly used for extracapsular hip fractures, may fail particularly in unstable fractures. Replacement of the hip using arthroplasty, often used for intracapsular fractures, has been used as an alternative. OBJECTIVES: To compare replacement arthroplasty with internal fixation for the treatment of extracapsular hip fractures in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (December 2005), the Cochrane Central Register of Controlled Trials (The Cochrane Library Issue 4, 2005), MEDLINE, EMBASE, the UK

Distal femur replacement is useful in complex total knee arthroplasty revisions.

UNLABELLED: Revision total knee arthroplasty can be considerably more difficult in cases involving severe bone loss, complete absence of collateral ligaments, and persistent periprosthetic supracondylar femoral fractures. Modular segmental endoprosthetic distal femur replacement is a limb salvage option when other surgical options are unfeasible. The clinical performance of rotating hinge knee prostheses has greatly improved with the evolution of second and third generation designs. The increased freedom of rotation decreases the prosthetic bone stresses and the longevity. While designs have i

Fatal fat embolism following replacement arthroplasty for transcervical fractures of femur.

Seven cases are reported of death in old people associated with transcervical fractures of the femur and occurring shortly after insertion of a Thompson prosthesis. They are compared with six fatalities following similar injury but not surgically treated and with three which followed internal fixation. Positive controls for massive fat embolism were selected from fatal cases of multiple and severe skeletal fractures due to road accidents. It seems likely that patients undergoing this operation are peculiarly susceptible to lethal fat embolism. The clinical features, diagnosis, pathological aspe

Convertible glenoid replacement in the anatomical total shoulder arthroplasty: medium-term results.

INTRODUCTION: The older generation of high thickness metal-backed glenoid prostheses had a high failure rate. The goal of our study was to analyze the medium-term clinical and radiological results of a technically modified generation of metal backed glenoids with 35% decrease in thickness. METHODS: Thirty-nine patients (43 shoulders) with a mean age of 66.5 years at surgery were examined from two to ten years (mean 71.2 months) using the Constant-Score, Subjective Shoulder Value and X-ray in a.p. and axial view. Indication for surgery included patients with glenoid types B1, B2, or B3 and/or

Which knee replacement do the patients forget? Unicondylar or total knee arthroplasty.

OBJECTIVE: The aim of this study was to determine which type of knee arthroplasty is easier to forget by comparing levels of joint awareness evaluated with the Forgotten Joint Score (FJS-12) after unicondylar versus total knee arthroplasty. METHODS: Patients who underwent either unicondylar or total knee arthroplasty due to primary gonarthrosis were retrospectively identified and then divided into 2 groups: the TKA group (218 patients; mean age = 68.93 ± 7.14 years) and the UKA group (131 patients; mean age = 60.39 ± 7.03 years). The status of joint awareness after knee replacement surgery wa

Outcomes after shoulder replacement: comparison between reverse and anatomic total shoulder arthroplasty.

BACKGROUND: Anatomic total shoulder arthroplasty (TSA) and reverse total shoulder arthroplasty (RTSA) are increasingly common procedures employed to treat arthritic conditions. Although TSA is a widely accepted procedure for glenohumeral arthritis with intact rotator cuff, concerns about RTSA persist because of variable complication rates and outcomes. METHODS: This is a prospective, case-control study comparing outcomes and complications after TSA and RTSA. The study included 47 patients undergoing TSA for glenohumeral arthritis and 53 patients undergoing RTSA for rotator cuff tear arthropat

Role of total hip replacement arthroplasty between transplantation and acute kidney injury.

Osteonecrosis of the femoral head is a severely disabling complication of steroid immunosuppression in renal transplant patients. The increased number of patients undergoing transplantation has increased the number of transplant recipients undergoing total hip replacement arthroplasty (THRA). In this study, we retrospectively assessed patients who underwent THRA from May 2004 to February 2014, and evaluated their demographic and clinical characteristics, the results of peri-operative laboratory tests, the amounts of fluids transfused during surgery, and anesthesia time. Our results found that

A case report of total finger joint replacement arthroplasty after traumatic amputation.

INTRODUCTION: Joint reconstruction following trauma at the proximal interphalangeal or metacarpophalangeal levels remain a difficult problem in hand surgery. Function of the injured finger depends on the mobility and stability at these joints. In fingers amputated at these levels with joint destruction, the hand surgeon can perform either an arthrodesis or a replacement arthroplasty (either as an emergency or as a secondary procedure). Arthrodesis will give a stable pain-free joint, but at the cost of sacrificing mobility and even cosmesis. CLINICAL PICTURE: We report a case of traumatic ampu

Total articular replacement arthroplasty. A three- to ten-year case-controlled study.

One hundred eighty-four cemented primary total articular replacement arthroplasty (TARA) resurfacing total hip procedures were performed by a single surgeon from 1981 to 1985. One hundred seventy-four hips had a mean follow-up period of eight years (range, 3.5-10.4 years). Osteoarthrosis was the predominant diagnosis (79%). Failure was defined as a hip needing additional surgery because of an implant failure and occurred in 13.2% of the patients. No statistically significant difference was found between preoperative etiology, patient gender, or the side of the hip involved between the revised

Excision arthroplasty with delayed wound closure for the infected total hip replacement.

Methods of management of infections in total hip replacement vary from the use of antibiotics with reimplantation to a complete removal of the prosthetic unit. This is a series of 10 consecutive patients having documented sepsis following total hip replacement characterized by gram-negative organisms which was managed with the method of excision (Girdlestone) arthroplasty with delayed wound closure. Parenteral antibiotics were used while the wound remained open. Once wound healing had been accomplished, the patients were placed on appropriate oral antibiotics and continued on same for approxim
