Results of arthroscopic meniscectomy.

The results of 100 arthroscopic partial meniscectomies are reported. Hospitalisation was on average 1.5 days; return to work 3 weeks and to sports activities 7 weeks. This is particularly rewarding for the patient. The results were excellent or good in 87% of cases. In 10 of the 13 unsatisfactory results there was associated chronic anterior laxity. Its prior evaluation by dynamic tests is therefore particularly important. The duration of symptoms and site and type of lesion had no bearing on the final result. Less satisfactory results were recorded in subjects with quadriceps wasting exceedin

Arthroscopic chondroplasty of the patella.

Following failure of conservative treatment, arthroscopic chondroplasty was performed on 41 knees in 40 patients with chondromalacia of the patella as the only lesion. Within a follow-up period averaging 40 months, results were excellent in 10%, good in 39%, fair in 44% and poor in 7%. Seventy-eight percent of patients were subjectively satisfied with their surgery. Age or sex of the patients, grade of the lesion, and duration of the symptoms prior to surgery did not correlate with the final result. The method is considered safe since the few complications recorded were minor and did not affec

Arthroscopic knee surgery.

Endoscopic knee surgery, like other endoscopic procedures in other specialties, takes considerable time and patience to learn. But once the technique is learned, the noninvasive, nondestructive nature of endoscopy lends itself well to many types of knee operations, and we are beginning to see results at 4 and 5 years that are at least equal to those achieved by the same operation done with an arthrotomy, and with a marked reduction in complications and morbidity. Patient acceptance of these endoscopic knee procedures is very high because of the small scars, minimal postoperative discomfort, ra

An unusual use for the arthroscope.

In recent years, arthroscopy has gained importance in the diagnosis and treatment of patients with a painful knee (Ireland et al., 1980; Dandy, 1981). An important application of the arthroscope is in the removal of loose bodies from the knee, and endoscopy has also proved to be more accurate in diagnosis than more conventional methods (Dandy and O'Carroll, 1982). The identification and removal of foreign bodies from soft tissues and joints can be difficult. In addition to conventional radiography, other more specialized techniques including stereotaxis (McFadden, 1972) have been applied to th

Arthroscopic Tibiotalar Arthrodesis Using an Arthroscopic Autologous Tissue Collector: A Technique Guide.

Tibiotalar arthrodesis is a common procedure performed in patients with ankle arthritis to relieve pain, restore function, and improve the quality of life. Obtaining proper bone-to-bone apposition is crucial to obtaining a solid fusion. Therefore, adequate joint preparation is critical. Open-joint preparation is the standard of care, but arthroscopic arthrodesis is becoming increasingly popular, as it can decrease tissue trauma, postoperative pain, hospital stay, and complications. The addition of bone matrix allograft and autograft also improves healing rates. Use of an arthroscopic autologou

Arthroscopic brostrom technique.

UNLABELLED: Surgical strategy regarding chronic lateral ankle instability is undergoing an evolution from traditional open procedures to minimally invasive and arthroscopic techniques. The development of arthroscopic techniques for the ankle mirrors the processes witnessed for the shoulder and knee over the last 30 years. The arthroscopic Brostrom is a novel technique that allows the surgeon to use an arthroscope to perform a lateral ankle ligament reconstruction that was previously thought possible only through open surgical technique. Indications and contraindications for the arthroscopic te

Arthroscopic synovectomy of the wrist.

Arthroscopic synovectomy is safe and reliable, with mild postoperative morbidity. The rationale of a surgical synovectomy is to excise inflamed synovium and thereby, remove as much effusion and inflammatory substrate as possible. In most cases, arthroscopic synovectomy is performed as an outpatient procedure. The technique has also been used for other diagnoses causing wrist arthritis, but very few results have been reported and the indications remain to be defined. In rheumatoid arthritis (RA), juvenile chronic arthritis (JCA), systemic lupus erythematosus (SLE), and postinfectious monoarthri

Arthroscopic surgery in children.

Arthroscopic surgery for children and adolescents is developing fast, thanks to technical progress in adult arthroscopy, improved knowledge of child joint disorders, and instrument miniaturization. The specificity of arthroscopy in children lies, on the one hand, in the small joint size and, on the other, in the presence of neighboring growth plates. There also exist specific pathologies as well as differences in indications in pathologies common to children and adults. These specificities need to be known, and adapted techniques and equipment require to be used, given which arthroscopy is fea

The arthroscopic deepening trochleoplasty.

Since a dysplastic trochlea has been identified as the main pathomorphology in recurrent patellofemoral instability, trochleoplasty became a surgical solution in some of these cases. However, in comparison to other surgical procedures stabilizing the patellofemoral joint, trochleoplasty is a major operation with an arthrotomy, and associated typical risks of open surgery such as arthrofibrosis. Therefore, we developed a technique to perform an arthroscopic deepening trochleoplasty via suprapatellar portals using shaver burrs. Comparable to the open procedure, a cartilage flake is released and

Arthroscopic ankle arthrodesis.

BACKGROUND: Arthroscopic arthrodesis has been used mainly for in situ fusion of arthritic ankles without deformity. This paper presents the application of arthroscopic arthrodesis of ankles with marked deformity. METHODS: The results of 78 consecutive cases of arthroscopic ankle arthrodeses, performed in 74 patients, were retrospectively evaluated. Forty-eight ankles had minor deformity (group A), whereas 30 ankles had a varus or valgus deformity of more than 15 degrees (maximum 45 degrees) (group B). The average hospital stay was 3.8 and 3.4 days in groups A and B, respectively (p = 0.74). P

[Arthroscopic stabilization of the shoulder].

PURPOSE OF THE STUDY: Minimally invasive technique have recently gained importance because of their apparent advantages. One of them is arthroscopic stabilization of the shoulder used for treatment of traumatic anterior glenohumeral dislocation with subsequent instability. In this study we describe the results of and experience with this technique. MATERIAL: Conventional treatment of glenohumeral dislocation, which includes reduction and subsequent immobilization for 4 weeks, has a high risk of recurrent dislocation particularly in young patients. Therefore surgical treatment lowering this ris

Arthroscopic psoas tenotomy.

Tenotomy may be indicated for psoas tendinitis or painful snapping if conservative treatment remains unsuccessful. Because of significant complications with open techniques, endoscopic operations have been developed. We present a new arthroscopic technique to access and release the psoas tendon from the hip joint. This procedure can be performed in addition to other arthroscopic procedures of the hip joint or alone. To exclude additional hip disease, a diagnostic round of the joint should be completed. After hip arthroscopy of the central compartment has been performed, traction is released an

Arthroscopic ankle arthrodesis.

We reviewed 116 patients who underwent 118 arthroscopic ankle arthrodeses. The mean age at operation was 57 years, 2 months (20 to 86 years). The indication for operation was post-traumatic osteoarthritis in 67, primary osteoarthritis in 36, inflammatory arthropathy in 13 and avascular necrosis in two. The mean follow-up was 65 months (18 to 144). Nine patients (10 ankles) died before final review and three were lost to follow-up, leaving 104 patients (105 ankles) who were assessed by a standard telephone interview. The preoperative talocrural deformity was between 22 degrees valgus and 28 deg

[Arthroscopic meniscus surgery].

Surgery on the meniscus of the knee joint is one of the most frequently performed operations. The arthroscopic technique currently represents the general treatment standard and except in a few cases has completely superseded the open approach. Arthroscopy facilitates precise diagnosis and classification of all forms of meniscus lesions. In many cases even today partial resection on irreparably damaged meniscus tissue cannot be avoided. The goal is to preserve as much healthy meniscus tissue as possible. Types of tears amenable to reconstruction always require arthroscopic refixation. Various t

Arthroscopic biceps tenodesis.

The proximal biceps tendon is a significant source of shoulder pain that may be treated with biceps tenotomy or tenodesis. Biceps tenodesis has suggested advantages over tenotomy that include maintenance of the length-tension relationship, prevention of muscle atrophy, maintenance of elbow flexion and supination strength, avoidance of cramping pain, and avoidance of cosmetic deformity. The recent advancement of all arthroscopic tenodesis techniques has provided sufficient fixation strength while easing technical demands and minimizing neurovascular injury risk. With our newer techniques and be

Arthroscopic biceps tenodesis.

Surgical treatment of symptomatic pathology of the long head of the biceps tendon generally consists of either biceps tenotomy or tenodesis. Biceps tenodesis is generally recommended for younger patients and has been well described using open techniques. With advancements in arthroscopic ability and equipment, new arthroscopic techniques have recently been reported. These techniques can be especially useful when used in conjunction with other arthroscopic procedures such as distal clavicle resection, rotator cuff repair, and subacromial decompression. We present a modification of the technique

Arthroscopic subacromial decompression.

In this study, we analyzed the results of two series of patients treated for impingement syndrome by undergoing arthroscopic subacromial decompression (ASD). Patients had not responded to nonoperative treatment. Group 1 included 112 consecutive patients (average age, 41 years) with 96 (77%) patients available for 2-year follow-up. Group 2 (28 patients, 29 shoulders; average age, 43 years; range, 22 to 72) had ASD and the subacromial space digitally palpated to determine if adequate decompression was performed. Twenty-two (85%) of 26 shoulders were available for follow-up. At follow-up, pain, f
