Current Practice: Rationale for Screening Children with Hereditary Hemorrhagic Telangiectasia for Brain Vascular Malformations.

BACKGROUND: Hereditary hemorrhagic telangiectasia is an autosomal dominant vascular dysplasia characterized by mucocutaneous telangiectasias, recurrent epistaxis, and organ vascular malformations including in the brain, which occur in about 10% of patients. These brain vascular malformations include high-flow AVMs and AVFs as well as low-flow capillary malformations. High-flow lesions can rupture, causing neurologic morbidity and mortality. STATE OF PRACTICE: International guidelines for the diagnosis and management of hereditary hemorrhagic telangiectasia recommend screening children for bra

An Algorithm for Mining the Living Habits of Elderly People Living Alone Based on AIoT.

With the global aging population on the rise, the health and safety of elderly individuals living alone have become increasingly critical. This study introduces a novel AIoT-based habit mining algorithm designed to enhance activity monitoring in smart home environments. The proposed method integrates a one-dimensional U-Net neural network for accurate behavioral classification and an FP-Growth-based temporal association rule analysis for uncovering meaningful living patterns. By leveraging environmental sensor data, the algorithm first classifies daily activities and then uses timestamps to de

[Psychiatry and brua].

BACKGROUND: Patients from Aruba, Bonaire and Curacao are often inclined to attribute mental problems to brua, the Dutch-Antillean counterpart of voodoo. Because little is known about brua and patients are usually reluctant to talk about it, problems can arise in the communication with biomedically trained health practitioners. AIM: To provide an overview of the literature on brua, and of the ways in which brua may interfere with the diagnosis and treatment of Dutch-Antillean patients with mental health problems. METHOD: We searched the literature via PubMed and Embase (up to October 2012) an

Dead non-native corals (Tubastraea coccinea) and their associated fauna trawled from the North Sea provide evidence for long-distance transport by hull fouling.

Four large skeletons of the non-native Indo-Pacific coral Tubastraea coccinea Lesson, 1830 were trawled by a fishing boat in the Dutch part of the North Sea in October 2024. They had empty shells of a non-indigenous worm snail, probably Eualetes tulipa (Rousseau, 1843), attached to them. The corals had remnants of black ship-hull coating on their flat attachment base and resembled biofouling specimens found on semisubmersible platforms in the oil and gas industry. They were compared to museum specimens of T. coccinea from Aruba (Southern Caribbean), collected in 1955 from a metal substrate in

Surgical Treatment of Unruptured Brain AVMs: Short- and Long-Term Results.

Unruptured brain AVMs (bAVMs) remain a controversial subject for practicing neurosurgeons, especially in the light of ARUBA and other observational studies. This retrospective study aims to analyze our experience with unruptured bAVMs to see whether it is beneficial in the long-term and how it corresponds to large literature trials. The study comprised 160 adult patients with unruptured bAVMs surgically treated in Burdenko NMC (Moscow) in 2009-2017. Mean age: 33.4 ± 10лет. Clinical presentations were: seizures in 99 (61.9%), chronical headaches-49 (30.6%), ischemic symptoms-4 (2.5%), asymptoma

Recent Trends in Neuro-endovascular Treatment for Acute Ischemic Stroke, Cerebral Aneurysms, Carotid Stenosis, and Brain Arteriovenous Malformations.

The efficacy of mechanical thrombectomy with stent retrievers for emergent large vessel occlusion has been proved by randomized trials. Mechanical thrombectomy is increasingly being adopted in Japan since stent retrievers were first approved in 2014. An urgent clinical task is to offer structured systems of care to provide this treatment in a timely fashion to all patients with emergent large vessel occlusion. Treatment with flow-diverting stents is currently a preferred treatment option worldwide for large and giant unruptured aneurysms. Initial studies reported high rates of complete aneurys

Unobtrusive Activity Recognition of Elderly People Living Alone Using Anonymous Binary Sensors and DCNN.

Elderly population (over the age of 60) is predicted to be 1.2 billion by 2025. Most of the elderly people would like to stay alone in their own house due to the high eldercare cost and privacy invasion. Unobtrusive activity recognition is the most preferred solution for monitoring daily activities of the elderly people living alone rather than the camera and wearable devices based systems. Thus, we propose an unobtrusive activity recognition classifier using deep convolutional neural network (DCNN) and anonymous binary sensors that are passive infrared motion sensors and door sensors. We empl

Lionfish envenomation case treated in an emergency unit in Colombia.

The lionfish (Pterois volitans) is an invasive species in the Caribbean Sea that can accidentally inject venom into humans through its spines. Here, we report a case of lionfish envenomation, highlighting a potential emerging threat to public health in Colombia. The case describes a healthy 59-year-old woman who suffered an accidental sting on the fourth finger of her left hand after contact with a lionfish spin while diving in Aruba. On-site treatment included hot compresses, antihistamines, and systemic corticosteroids. However, the patient experienced severe pain, coldness, and paleness in

Overview of the current concepts in the management of arteriovenous malformations of the brain.

BACKGROUND: There is a lack of consensus in the management of arteriovenous malformations (AVMs) of the brain since ARUBA (A Randomised trial of Unruptured Brain Arteriovenous malformations) trial showed that medical management is superior to interventional therapy in patients with unruptured brain AVMs. The treatment of brain AVM is associated with significant morbidity. OBJECTIVES AND METHODS: A review was done to determine the behaviour of brain AVMs and analyse the risks and benefits of the available treatment options. A search was done in the literature for studies on brain AVMs. Descrip

Interventions for treating brain arteriovenous malformations in adults.

BACKGROUND: Brain arteriovenous malformations (AVMs) are the single most common cause of intracerebral haemorrhage in young adults. Brain AVMs also cause seizure(s) and focal neurological deficits (in the absence of haemorrhage, migraine or an epileptic seizure); approximately one-fifth are incidental discoveries. Various interventions are used in an attempt to eradicate brain AVMs: neurosurgical excision, stereotactic radiosurgery, endovascular embolization, and staged combinations of these interventions. This is an update of a Cochrane Review first published in 2006, and last updated in 2009

Management of brain arteriovenous malformations.

PURPOSE OF REVIEW: This review highlights recent data regarding factors associated with brain arteriovenous malformation hemorrhage and different treatment options. RECENT FINDINGS: More risk factors were identified in association with intracranial hemorrhage, including age at initial diagnosis of arteriovenous malformation, co-existing extranidal aneurysms and genetic factors. Patients with unruptured arteriovenous malformations were found to be more susceptible to worsening in neurological function after microsurgery compared with those presenting with hemorrhagic arteriovenous malformation

Comprehensive neurosurgical treatment of right occipital arteriovenous malformation with advanced angiographic techniques.

/Summary. A 39-year-old female with a notable medical history of smoking and a familial predisposition to unruptured aneurysms presented with clinical symptoms of intermittent right-sided headaches, flashes of light, and pulsatile tinnitus in the right ear. Diagnostic evaluations, including advanced angiographic techniques, identified a right occipital arteriovenous malformation (AVM). The angiogram revealed significant venous flow voids, emphasizing the need for a comprehensive treatment approach. The Spetzler-Martin grading system classified the AVM as Grade 2, indicating a moderate risk pro

When time itself is the comparator: Methodological problems and a way forward for trials of surgery versus conservative management in neurosurgery.

BACKGROUND: Randomised trials that compare a surgical intervention to medical or conservative management occupy a peculiar space in clinical research. The two arms are not symmetric in time: surgery imposes a concentrated, acute risk in exchange for a long-term benefit, whereas conservative management distributes a smaller hazard across the patient's remaining life. In neurosurgery, where the conditions under study are frequently rare and natural histories are measured in decades, this asymmetry exposes weaknesses in conventional trial methodology that have repeatedly led to inconclusive, non-

Zoonotic influenza preparedness: Dutch medical labs efficiently detect animal influenza A viruses - External Quality Assessment, 2023.

BACKGROUND: Since 2022, highly pathogenic H5N1 influenza A virus clade 2.3.4.4b has caused global outbreaks among wild birds and poultry, with increasing mammalian and sporadic human infections. This elevates concerns about zoonotic transmission and pandemic risk, highlighting the need for accurate detection and identification of animal influenza A viruses by human clinical diagnostic laboratories (hCDL). METHODS: To evaluate routine diagnostic performance, an External Quality Assessment (EQA) panel containing inactivated influenza A viruses of avian (three subtype H5, one H7), swine (two H1,

Risk Reduction of Cerebral Stroke After Stereotactic Radiosurgery for Small Unruptured Brain Arteriovenous Malformations.

BACKGROUND AND PURPOSE: A Randomized Trial of Unruptured Brain Arteriovenous Malformations (ARUBA) indicated the superiority of medical management in reducing the risks for strokes and other neurological deficits over observation alone. The aim of our study was to verify the rationale for stereotactic radiosurgery (SRS) for small unruptured arteriovenous malformation. METHODS: A retrospective review was performed for 292 patients with unruptured arteriovenous malformations referred for SRS. The risks for cerebral hemorrhages were statistically compared before and after SRS. RESULTS: Of the 2

DSTGAT: A Spatio-Temporal Dynamic Graph Attention Network for User Activity Recognition.

Human Activity Recognition (HAR) in smart home environments is essential for Ambient Assisted Living (AAL). However, existing methods utilizing Graph Neural Networks (GNNs) typically rely on static sensor topologies and fail to adequately capture the dynamic spatio-temporal dependencies and heterogeneous nature of multi-sensor data. To address these challenges, this paper proposes a Dynamic Spatio-Temporal Graph Attention Network (DSTGAT) for robust HAR in smart homes. First, a multi-modal feature embedding mechanism is introduced to unify temporal, spatial, sensor type, and observation data i

Uptake and radiological findings of screening cerebral magnetic resonance scans in patients with hereditary haemorrhagic telangiectasia.

Hereditary haemorrhagic telangiectasia (HHT) results in arteriovenous malformations (AVMs), most commonly in the lungs, liver and brain. Discussion of cerebral vascular malformations is an important element of patient management. The current study objectives were to examine uptake and results of screening cerebral magnetic resonance (MR) scans, excluding symptomatic patients requiring neurological investigations. The remaining non-symptomatic individuals received formal pretest counselling that differed according to family history. For the 603 patients with no neurological symptoms of concern,

Non-Intrusive Monitoring and Detection of Mobility Loss in Older Adults Using Binary Sensors.

(1) Background and objective: Mobility is crucial for healthy aging, and its loss significantly impacts the quality of life, healthcare costs, and mortality among older adults. Clinical mobility assessment methods, though precise, are resource-intensive and economically impractical, and most of the existing solutions for automatic detection of mobility anomalies are either obtrusive or improper for long time monitoring. This study explores the feasibility of using non-intrusive, low-cost binary sensors for continuous, remote detection of mobility anomalies in older adults, aiming to identify b

Management of perisylvian arteriovenous malformations: a retrospective institutional case series and review of the literature.

OBJECT: Sylvian arteriovenous malformations (sAVMs) are challenging lesions of the central nervous system. The natural history of these unique lesions as well as clinical outcomes following treatment of sAVMs has been limited to case series owing to the rarity of these lesions. The authors present their experience with sAVMs and review the literature. METHODS: In accordance with the Henry Ford Institutional Review Board, medical records of patients with sAVMs treated from 2000 to 2012 were reviewed. Clinical data were retrospectively collected to calculate pre- and posttreatment modified Rank
