Airway immunopathology of asthma with exercise-induced bronchoconstriction.

BACKGROUND: Exercise-induced bronchoconstriction (EIB) is a common cause of symptoms in a subgroup of asthmatic subjects. The pathobiology that makes this group of asthmatic subjects susceptible to bronchoconstriction after a brief period of exercise remains poorly understood. OBJECTIVE: We sought to determine whether there are differences in lower airway inflammation and production of cytokines and eicosanoids between asthmatic subjects with and without EIB. METHODS: Two distinct groups of asthmatic subjects based on a priori definitions were identified, one with moderate-to-severe EIB and

Circulating catecholamines in exercise and hyperventilation induced asthma.

Plasma noradrenaline, adrenaline, and cyclic 3'5' AMP (cAMP) were measured in seven asthmatic patients with known exercise-induced bronchospasm and six matched non-atopic control subjects during a standard treadmill exercise test and then during matched isocapnic hyperventilation. Normal subjects showed a 5.5 fold rise in noradrenaline and a 3.2 fold rise in adrenaline during exercise compared with a 2.1 fold rise in noradrenaline and no significant rise in adrenaline in asthmatics who all developed bronchoconstriction after exercise (mean fall in peak flow rate 28.4 +/- 5.8%). Plasma cAMP ros

Exercise-induced asthma--clinical, physiological, and therapeutic implications.

Exercise provokes acute airways obstruction, maximum shortly after stopping, in virtually all asthmatic patients. The severity of this exercise-induced asthma (EIA) depends upon the type of exercise, with running being the most asthmogenic, swimming and walking the least, and cycling intermediate even with the same metabolic stress. The duration and severity of the exercise also affect the amount of EIA, the maximum amount of being obtained after 6 to 8 min of running hard enough to raise the heart rate to 180 beats per minute (bpm) in children or 140 bpm in adults. EIA is not the result of hy

Effects of azelastine on allergen- and exercise-induced asthma.

The effects of the new anti-allergic drug, azelastine, on allergen- and exercise-induced asthma were studied. In six allergen inhalation tests for five asymptomatic asthmatic patients, the maximum percentage fall in FEV1.0 immediately after inhalation of allergen extract was 37.2 +/- 6.4 per cent (mean +/- SEM). As compared with a placebo, the maximum percentage fall in FEV1.0 with azelastine after inhalation of allergen extract in the same manner as with the placebo was 17.3 +/- 6.9 per cent. The difference was statistically significant (p less than 0.05). The percentage fall in FEV1.0 with p

Choices of therapy for exercise-induced asthma in children.

Cough and wheezing interferes with sport and other forms of physical activity in half of asthmatic children. Airway obstruction can be induced by a standard exercise test in over 70% of children with asthma. A beta-adrenergic agonist or cromone taken by inhalation beforehand will usually inhibit bronchoconstriction provoked by a free running exercise test. The duration of protective effect with salbutamol, terbutaline and cromones is less than 4 h. The long acting beta-adrenergic agonists formoterol and salmeterol give protection against exercise-induced airway obstruction for up to 12 h, whic

Comparison of isocapnic hyperventilation and treadmill exercise in children with exercise-induced asthma.

The degree of post-exercise airway obstruction (Exercise-Induced Asthma (EIA] in 14 children was compared to the degree of airway obstruction following isocapnic hyperventilation. EIA was provoked by 6 min of treadmill running. Isocapnic hyperventilation was performed sitting during 6 min. The total ventilation (Vtot) during the two provocations was identical. The temperature of the inspired air was also identical during the two provocations, and the relative humidity was 40% during treadmill-running and 15% during hyperventilation. The decrease in peak expiratory flow after treadmill-running

Preventing bronchoconstriction in exercise-induced asthma with inhaled heparin.

BACKGROUND: We have previously reported that inhaled heparin prevents allergic bronchoconstriction in sheep and inhibits the anti-IgE-mediated release of histamine from mast cells in vitro. Since the release of such mediators has been implicated in exercise-induced asthma, we investigated whether inhaled heparin could also attenuate the bronchoconstrictor response in this disease. METHODS: On five days we studied 12 subjects with a history of exercise-induced asthma. On day 1 they underwent a standardized exercise challenge on a treadmill to document the presence of exercise-induced asthma. M

Felodipine, a new calcium antagonist, modifies exercise-induced asthma.

Recently, calcium antagonists have been reported to have a clinically beneficial effect in patients with asthma. Felodipine is a new calcium antagonist of the dihydropyridine group with a high selectivity for arteriolar smooth muscle; it is under clinical investigation for the treatment of hypertension. In this double-blind, randomized crossover trial in 9 patients, the effect of 10 mg felodipine in oral solution on exercise-induced asthma was compared with a placebo on separate days. The FEV1 was at least 80% of the predicted normal value, with variation between study days of less than 10%. H

Neutrophil chemotactic factor in exercise- and hyperventilation-induced asthma.

Elevated levels of the mast-cell-associated serum neutrophil chemotactic factor (NCF) and an increase in blood basophil counts were observed in 6 atopic asthmatics during exercise-induced asthma (EIA). These changes were not found when the same degree of airways obstruction was elicited in the same subjects by isocapnic hyperventilation (ISH) with cold air. The NCF was unlikely to be related to the basophilia alone, because asthmatics without EIA who underwent the same exercise task, produced a similar basophilia but significantly less NCF. These findings suggest that mast-cell-associated (as

[Dysfunction of the vocal cords simulating exercise-induced asthma].

Vocal cord dysfunction is a respiratory condition characterized by anomalous adduction of the vocal cords during inspiration, causing significant air flow limitation in the larynx. Few such cases have been described in which dysfunction is triggered by exercise. We report the case of a young women with severe dyspnea appearing as a result of physical activity. We first deal with issues of differential diagnosis in relation to several other diseases, particularly exercise-induced asthma and then discuss therapeutic approaches.

Comparison of refractoriness after exercise- and hyperventilation-induced asthma.

To study the relationship of bronchoconstriction and refractoriness we performed pairs of exercise and hyperventilation tests in 15 patients (mean [SEM] age 28.5 [2.8] years) with a history of exercise-induced asthma. Mean (SEM) maximum specific airway resistance (SRaw) increased during the first exercise test to 33.9 (4.5) and during the second exercise test to 29.8 (5.2) cmH2O x s (n.s.). Mean (SEM) maximum specific airway resistance (SRaw) increased during the first hyperventilation test to 44.0 (5.9) and during the second hyperventilation test to 27.4 (3.3) cmH2O x s (p less than 0.01). Exercise intensity determines and climatic conditions modify the severity of exercise-induced asthma.

Recent studies have shown some evidence that exercise-induced asthma (EIA) may not be entirely explained by respiratory heat loss (RHL). We investigated the interrelationship between heat exchange, exercise intensity and EIA. In order to differentiate between the effects of RHL and exercise intensity, we arranged for tests to be performed with the same RHL, but with different intensities of exercise and inspired air conditions. Each of 8 asthmatic children exercised twice in random order for 6 min on a cycle ergometer. One test consisted of exercise performed at a greater level of effort while

Vitamins C and E for asthma and exercise-induced bronchoconstriction.

BACKGROUND: The association between dietary antioxidants and asthma or exercise-induced bronchoconstriction (EIB) is not fully understood. Vitamin C and vitamin E are natural antioxidants that are predominantly present in fruits and vegetables; inadequate vitamin E intake is associated with airway inflammation. It has been postulated that the combination may be more beneficial than either single antioxidant for people with asthma and exercise-induced bronchoconstriction. OBJECTIVES: To assess the effects of supplementation of vitamins C and E versus placebo (or no vitamin C and E supplementat

Laser acupuncture in children and adolescents with exercise induced asthma.

BACKGROUND: Laser acupuncture, a painless technique, is a widely used alternative treatment method for childhood asthma, although its efficacy has not been proved in controlled clinical studies. METHODS: A double blind, placebo controlled, crossover study was performed to investigate the possible protective effect of a single laser acupuncture treatment on cold dry air hyperventilation induced bronchoconstriction in 44 children and adolescents of mean age 11.9 years (range 7.5-16.7) with exercise induced asthma. Laser acupuncture was performed on real and placebo points in random order on two

The effect of body posture on exercise- and hyperventilation-induced asthma.

Recent studies have shown that swimming is of relatively low asthmogenicity, even under conditions of high respiratory heat (and/or water) loss (RHL). It has been suggested that the horizontal body position may contribute to swimming's low asthmogenicity. We studied the effects of upright and prone body postures on pulmonary function following exercise (EIA) and after nonexercise hyperventilation (HIA). Twelve asthmatic boys (aged 12 to 16 years) underwent two 8-min exercise sessions of shoulder flexion-extension and two 8-min isocapnic hyperventilation treatments, in a counterbalanced order,

Exercise-induced dyspnea in children and adolescents: if not asthma then what?

BACKGROUND: Exercise-induced dyspnea (EID) in children and adolescents is a common manifestation of asthma and is therefore commonly attributed to exercise-induced asthma (EIA) when present in otherwise healthy children. OBJECTIVE: To report the outcome of evaluations for EID when other symptoms and signs of asthma were absent or if there was no response to previous use of an inhaled beta2-agonist. METHODS: We reviewed the results of all exercise tests performed in otherwise healthy patients with EID during 1996 to 2003. Physiologic measures included preexercise and postexercise spirometry w

Inhaled frusemide and exercise-induced bronchoconstriction in children with asthma.

BACKGROUND: Nebulised frusemide has been shown to be protective against bronchoconstricting stimuli in adult asthmatic subjects and against cold air challenge in children. Animal studies suggest that inhaled frusemide may be more effective in the young. METHODS: A double blind placebo, controlled, crossover study on the effect on exercise of pretreatment with frusemide (20 mg) from a metered dose inhaler via a large volume spacer (Volumatic) was performed in 12 asthmatic children. Exercise testing consisted of eight minutes of running on a treadmill in an environmentally controlled laboratory

The functional conversion hypothesis: a contributor to exercise-induced asthma?

Studies on receptor stability suggest that functional conversion of adrenoceptors between alpha and beta can occur in mammalian myocardium due to variations in the environment such as temperature changes, pH or hormonal changes. If adrenoceptors of the respiratory system behave similarly, heat and water loss of airways noted during hyperventilation could lead to functional loss of inhibitory beta and expression of excitatory alpha adrenoceptors. This would have the effect of counter-action of adrenergic homeostatic mechanisms which may be of particular importance in asthmatic subjects. The hyp

Prevention with and without the use of medications for exercise-induced asthma.

Exercise-induced asthma or bronchospasm occurs in approximately 12% of the population. It can be prevented with proper techniques of exercise and environmental exposures and with medication. Exercise techniques include timing of exercise to less than 5-min interludes and continued repeat exercise every hour. Exercising to less than full tolerance may avoid the onset of exercise-induced asthma. In some individuals exercise in warm, humidified air is less asthmogenic than in dry, cold air. Medications that are effective and safe include bronchodilators such as theophylline and beta-agonists and
