Asthma and chronic obstructive pulmonary disease (COPD) represent two major public health problems. However, there is a significant proportion of patients with a mixed asthma-COPD phenotype. This condition is defined as asthma-COPD overlap syndrome (ACOS). Since there are no internationally accepted criteria for the diagnosis of that syndrome, its management remains difficult. Given the fact that patients with ACOS have an increased risk of exacerbation and hospitalization, there is a pressing need for a more targeted approach and better management. We propose that fractional exhaled nitric ox

BACKGROUND: Asthma COPD overlap (ACO) is a consensus-based phenotype having characteristics of both COPD and asthma. Distinguishing ACO from other diseases is even more important as it is related to low health-related quality of life, augmented exacerbation rate and hospital admission, a rapid deterioration in lung function, and increased morbidity and mortality. But it cannot be diagnosed explicitly based on spirometry tests, patient demographics, radiology, or by-sputum cytology. There is an unmet need to develop biomarkers.
OBJECTIVES: To assess the role of sputum neutrophil gelatinase-ass

Obstructive sleep apnoea (OSA) syndrome, the most frequent sleep-disordered breathing, is a comorbidity of asthma, whose prevalence covers about 49.5% of asthmatic adult patients. A 61-year-old female patient, affected by severe allergic asthma and obesity, started treatment with omalizumab and underwent polysomnography showing a severe OSA pattern (apnoea/hypopnoea index (AHI): 72.7). After six months, she showed functional improvement and good asthma symptoms control and underwent a new polygraphy for the persistence of the night symptoms which showed an ameliorated, despite still severe, OS

The patient was a 66-year-old man who developed exacerbation of respiratory and heart failure that necessitated tracheal intubation. The exacerbation was considered to be largely caused by asthma-chronic obstructive pulmonary disease overlap syndrome and type 3 pulmonary hypertension. However, left diaphragmatic eventration and tracheal stenosis were also found. We hypothesized that diaphragmatic eventration and tracheal stenosis surgeries would improve the patient's pulmonary function, pulmonary hypertension, and cardiac function. Postoperatively, he recovered well and was discharged home on

BACKGROUND: There is no consensus on how to define patients with symptoms of asthma and chronic obstructive pulmonary disease (COPD). A diagnosis of asthma-COPD overlap (ACO) syndrome has been proposed, but its value is debated. This study (GSK Study 201703 [NCT02302417]) investigated the ability of statistical modeling approaches to define distinct disease groups in patients with obstructive lung disease (OLD) using medical history and spirometric data.
METHODS: Patients aged ≥18 years with diagnoses of asthma and/or COPD were categorized into three groups: 1) asthma (nonobstructive; reversi

Asthma and chronic obstructive pulmonary disease (COPD) are common lung diseases characterized by chronic airway inflammation and airway obstruction. Among patient with COPD and asthma; there is a group of patients with an overlap between clinical, functional characteristics and airway inflammation patterns, named "Asthma-COPD Overlap Syndrome" (ACOS). ACOS is a syndrome characterized by reversible but persistant airflow limitation (postbronchodilator FEV1/FVC < 70%) which has some features of both asthma and COPD. ACOS should be suspected in a patient > 40 years, with smoking history, previou

The management of chronic obstructive lung diseases, particularly severe asthma, chronic obstructive pulmonary disease (COPD) and non-cystic fibrosis bronchiectasis, is complicated by frequent overlap syndromes such as asthma-bronchiectasis overlap and bronchiectasis-COPD overlap syndrome. These overlapping phenotypes are characterized by severe symptoms, frequent exacerbations, accelerated lung function decline and increased mortality, driven by a common, destructive endotype: persistent, neutrophil-dominant airway inflammation. This inflammation is fuelled by the overactivity of neutrophil s

Previous studies have explored the connection between the dietary inflammation index (DII) and asthma-COPD Overlap (ACO); however, the relationship between diet and the risk of ACO from an antioxidant perspective remains unexamined. We hypothesized that a proinflammatory diet may increase the risk of ACO by promoting inflammation, whereas an antioxidant diet may reduce the risk by mitigating inflammation. To test this hypothesis, we conducted a cross-sectional study utilizing data from the National Health and Nutrition Examination Survey (NHANES) in 2003-2018, including 23,050 participants. Th

Objective Fungal sensitization is known to be prevalent in chronic obstructive pulmonary disease (COPD) and it is recognized as a risk factor for severe asthma. However, its relationship to the development of asthmatic features in COPD remains unclear. Because the diagnostic criteria for asthma-COPD overlap (ACO) are not standardized, we focused on the specific asthmatic features that form the diagnostic basis of ACO, namely, blood eosinophilia and recurrent wheezing, to clarify how fungal sensitization contributes to their manifestation in COPD. Methods We retrospectively analyzed data from 7

PURPOSE: This study aimed to evaluate the relationship between lung volume and apnea-hypopnea index (AHI) in patients with overlap syndrome (OVS).
METHODS: Chronic obstructive pulmonary disease (COPD) patients diagnosed by spirometry were recruited from Renmin Hospital of Wuhan University. They were asked to take an overnight polysomnography (PSG). Multiple regression analyses were used to assess the relationships between lung volume measures and AHI in OVS patients.
RESULTS: Finally, 354 OVS patients and 229 patients with only COPD met the study's entry criteria. After adjusting for age, se

PURPOSE: Asthma and obstructive sleep apnea (OSA) are two respiratory diseases that often may coexist, resulting in Alternative Overlap Syndrome (aOVS), which is still underestimated and underdiagnosed.
OBJECTIVES: This state-of-art review aims to describe the current evidence on aOVS, including its pathophysiology, clinical, functional and therapeutic implications. A secondary objective is to assess whether aOVS can be identified as a distinct endophenotype needing personalized diagnostic and therapeutic strategies.
RESULTS: Asthma and OSA share several common risk factors, including obesit

Asthma-chronic obstructive pulmonary disease (COPD) overlap (ACO) is a severe clinical syndrome characterized to describe patients with both asthma and COPD clinical characteristics, which has posed a serious threat to patients' quality of life and life safety. However, there are many difficulties and uncertainties in its diagnosis and treatment in clinic; especially, its animal model has not been fully and thoroughly established, and the evaluation of therapeutic drugs is still in its infancy. Here, we used ovalbumin (OVA), lipopolysaccharide (LPS), and smoke costimulation to establish an ACO

Background/Objectives: Obstructive sleep apnea (OSA) and obstructive lung diseases (OLD) are common and interdependent respiratory disorders, where one condition may contribute to the development and worsening of the other (OLDOSA syndrome). The term OLDOSA syndrome includes two different conditions: Overlap syndrome (OVS: OSA + chronic obstructive pulmonary disease, COPD) and Alternative Overlap syndrome (aOVS: OSA + Asthma). Data on the interactions between lung function and respiratory monitoring during sleep in OLDOSA patients are few and controversial. Our study aims to evaluate the impac

Approximately one in four patients with chronic obstructive pulmonary disease (COPD) have asthmatic features consisting of wheezing, airway hyper-responsiveness or atopy. The Global initiative for Asthma/Globalinitiative for chronic Obstructive Lung Disease committee recently labelled these patients as having asthma-COPD overlap syndrome or ACOS. ACOS also encompasses patients with asthma, ≥40 years of age, who have been cigarette smokers (more than 5-10 pack years) or have had significant biomass exposure, and demonstrate persistent airflow limitation defined as a post-bronchodilator forced e

BACKGROUND: Chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA) are associated with mild cognitive impairment (MCI). However, this association is unclear. This study aimed to assess the prevalence of MCI in patients with overlap syndrome, determine whether OSA increases the risk of MCI in patients with COPD, and investigate the potential mechanisms for this association.
METHODS: Participants with stable Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage 2-4 COPD and complaints of snoring in 2016-2018 were enrolled in this cross-sectional observation

INTRODUCTION: Asthma and chronic obstructive pulmonary disease (COPD) overlap (ACO) is a term used to describe a patient with coexisting clinical features of asthma and COPD. We have previously reported that epithelial to mesenchymal transition (EMT) is active in the lungs of patients with COPD however, EMT in ACO remains an unexplored area. We hypothesize that EMT is an active process in ACO.
METHODS: In this cross-sectional study, large airway endobronchial biopsy (EBB) tissues from patients with asthma (14), COPD (22), current (CS) and ex-smokers (ES), and ACO (12) were immunohistochemical

BACKGROUND: Asthma, chronic obstructive pulmonary disease (COPD) and asthma-COPD Overlap (ACO) are inflammatory diseases characterized by specific T helper (Th) phenotypes. CD4 T follicular helper (Tfh) cells constitute a heterogeneous subset of T cells (Tfh1, Tfh2, Tfh17, Tfh17.1 and Tfr) essential for B cell differentiation and immunoglobulin production. Although alterations in their distribution and/or function have been associated with several pathologies, little is known about their relevance in these airway diseases.
METHODS: We sought to evaluate, by flow cytometry, the pattern of bloo

BACKGROUND: An accurate distinction between asthma, chronic obstructive pulmonary disease (COPD), and asthma-COPD overlap (ACO) in patients with fixed airflow obstruction (FAO) is crucial for optimizing treatment strategies and improving clinical outcomes. The role of bronchodilator response in differentiating between these diseases is unclear. We aimed to identify factors that could aid in differential diagnosis.
METHODS: This study employed a single-center, retrospective cohort design at a tertiary referral hospital. Patients with fixed airway obstruction, characterized by a post-bronchodil

Overlap of asthma and COPD has attracted attention recently. We aimed to clarify physiological and morphological differences of the airways between COPD and asthma-COPD overlap (ACO). Respiratory resistance and reactance and three-dimensional computed tomography data were evaluated in 167 patients with COPD. Among them, 43 patients who fulfilled the diagnosis of asthma were defined as having ACO. Among 124 patients with COPD without ACO, 86 with a comparable smoking history and airflow limitation as those with ACO were selected using propensity score matching (matched COPD).
