Astronaut omics and the impact of space on the human body at scale.

Future multi-year crewed planetary missions will motivate advances in aerospace nutrition and telehealth. On Earth, the Human Cell Atlas project aims to spatially map all cell types in the human body. Here, we propose that a parallel Human Cell Space Atlas could serve as an openly available, global resource for space life science research. As humanity becomes increasingly spacefaring, high-resolution omics on orbit could permit an advent of precision spaceflight healthcare. Alongside the scientific potential, we consider the complex ethical, cultural, and legal challenges intrinsic to the huma

Neurostimulation as a technology countermeasure for dry eye syndrome in astronauts.

Dry eye syndrome (DES) poses a significant challenge for astronauts during space missions, with reports indicating up to 30% of International Space Station (ISS) crew members. The microgravity environment of space alters fluid dynamics, affecting distribution of fluids on the surface of the eye as well as inducing cephalad fluid shifts that can alter tear drainage. Chronic and persistent DES not only impairs visual function, but also compromises the removal of debris, a heightened risk for corneal abrasions in the microgravity environment. Despite the availability of artificial tears on the IS

Predicting Spaceflight-Associated Neuro-Ocular Syndrome in International Space Station Astronauts.

IMPORTANCE: Spaceflight-associated neuro-ocular syndrome (SANS) could severely impact astronaut performance during long-duration spaceflight (LDSF). The pathophysiology of SANS remains elusive, and no predictors of SANS have been discovered; reliable prediction may offer novel insight into individual susceptibility and interperson variability for SANS and permit prelaunch preparations for in-flight treatment. OBJECTIVE: To conduct a preliminary analysis of SANS flight data to determine mission-to-mission trends in SANS diagnosis and optic disc edema (ODE) severity. DESIGN, SETTING, AND PARTI

Optic Disc Edema in Astronauts from a Choroidal Point of View.

INTRODUCTION: Optic disc edema has been well documented in astronauts both during and after long-duration spaceflight and is hypothesized to largely result from increased pressure within the orbital subarachnoid space brought about by a generalized rise in intracranial pressure or from sequestration of cerebrospinal fluid within the orbital subarachnoid space with locally elevated optic nerve sheath pressure. In addition, a recent prospective study documented substantial spaceflight-associated peripapillary choroidal thickening, which may be a contributing factor in spaceflight-associated neur

Hemodynamic effects of midodrine after spaceflight in astronauts without orthostatic hypotension.

INTRODUCTION: Orthostatic hypotension and presyncope are common and potentially serious risks for astronauts returning from space. Susceptible subjects fail to generate an adequate adrenergic response to upright posture. The alpha-1 adrenergic agonist, midodrine, may be an effective countermeasure. We tested the hypothesis that midodrine would have no negative hemodynamic effect on healthy astronauts returning from space. METHODS: Five male astronauts participated in preflight and post-flight tilt testing on a control flight as well as on the test flights, where midodrine (10 mg, orally) was

Chromosome aberrations of clonal origin are present in astronauts' blood lymphocytes.

BACKGROUND AND OBJECTIVES: The cause of hyperbilirubinemia cannot be found in about 45% of cases of neonatal jaundice. Gilbert syndrome (GS) is the most common congenital disease associated with bilirubin metabolism in the liver. Since the screening value of genetic tests cannot be fully determined until accurate data on the prevalence and penetrance of the GS genotype are known, we sought to estimate whether the prevalence of GS is higher in the parents of neonates with severe unexplained indirect hyperbilirubinemia. DESIGN AND SETTING: Case-control study of parents of neonates with severe u

Individual differences in susceptibility to motion sickness among six Skylab astronauts.

One of the Skylab experiments dealt with motion sickness, comparing susceptibility in the workshop aloft with susceptibility preflight and postflight. Tests were conducted on and after mission-day 8 (MD 8) by which time the astronauts were adapted to working conditions. Stressful accelerations were generated by requiring the astronauts, with eyes covered, to execute standardized head movements (front, back, left, and right) while in a chair that could be rotated at angular velocities up to 30 rpm. The selected endpoint was either 150 discrete head movements or a very mild level of motion sickn

Hearing Assessments and Tympanometry in NASA Astronauts on the International Space Station.

INTRODUCTION: Exposure to prolonged microgravity is associated with cephalad fluid shifts that potentially could affect tympanomastoid ventilation and auditory mechanisms. We conducted a retrospective review of NASA astronaut on-orbit hearing assessments (OOHAs) and middle-ear pressure measurements obtained during International Space Station missions to search for evidence of spaceflight-related hearing changes and disturbances of middle-ear aeration. METHODS: Nine NASA astronauts conducted on-orbit hearing assessments and tympanometry at least twice while on space station missions. Kuduwave

Pilots, Astronauts, and the Aerospace Microbiota: A Narrative Review of Occupational Impact.

The human microbiota plays a crucial role in maintaining health and preventing disease; however, the effects of occupational exposure on the microbiota of aircrew and astronauts are not fully understood. This narrative review aims to synthesize the current knowledge on microbiota alterations in aerospace medicine, assess the potential of probiotics as a countermeasure, and identify key gaps that warrant further research. The references were identified through searching PubMed for English articles published between 2010 and 2024, using keywords related to microbiota, probiotics, aviation, space

A bespoke health risk assessment methodology for the radiation protection of astronauts.

An alternative approach that is particularly suitable for the radiation health risk assessment (HRA) of astronauts is presented. The quantity, Radiation Attributed Decrease of Survival (RADS), representing the cumulative decrease in the unknown survival curve at a certain attained age, due to the radiation exposure at an earlier age, forms the basis for this alternative approach. Results are provided for all solid cancer plus leukemia incidence RADS from estimated doses from theoretical radiation exposures accumulated during long-term missions to the Moon or Mars. For example, it is shown that

The escape of retrobulbar cerebrospinal fluid in the astronaut's eye: mission impossible?

Ophthalmic abnormalities including unilateral and bilateral optic disc edema, optic nerve sheath distention, globe flattening, choroidal folds, and hyperopic shifts have been observed in astronauts during and after long-duration spaceflight. An increased understanding of factors contributing to this syndrome, termed spaceflight-associated neuro-ocular syndrome, is currently a top priority for the ESA and NASA, especially since this medical obstacle could impact the visual health of astronauts as well as the success of future missions, including continued trips to the International Space Statio

Dose estimation for astronauts using adult mesh-type reference computational phantoms.

Astronaut radiation exposure in low Earth orbit is strongly affected by orbital radiation environment, shielding condition, and radiation quality. In this study, a Geant4-based Monte Carlo model coupled with the adult male mesh-type reference computational phantom of the International Commission on Radiological Protection (ICRP) was developed to estimate organ doses for astronauts in the Chinese Space Station (CSS) orbit. Proton and electron spectra from the Earth's radiation belts (ERB) were calculated using AP9/AE9, and solar-minimum galactic cosmic ray (GCR) spectra were generated using CRE

Postural adaptation to microgravity underlies fine motor impairment in astronauts' speech.

Understanding the role of anti-gravity behaviour in fine motor control is crucial to achieving a unified theory of motor control. We compare speech from astronauts before and immediately after microgravity exposure to evaluate the role of anti-gravity posture during fine motor skills. Here we show a generalized lowering of vowel space after space travel, which suggests a generalized postural shift of the articulators. Biomechanical modelling of gravitational effects on the vocal tract supports this analysis-the jaw and tongue are pulled down in 1g, but movement trajectories of the tongue are o

Radioprotection for Astronauts' Missions: Numerical Results on the Nomex Shielding Effectiveness.

Space missions with humans expose the crews to ionizing radiation, mainly due to the galactic cosmic radiation (GCR). All radiation protection programs in space aim to minimize crews' exposure to radiation. The radiation protection of astronauts can be achieved through the use of shields. The shields could serve as a suit to reduce GCR exposure and, in an emergency, as a radiation shelter to perform necessary interventions outside the space habitat in case of a solar proton event (SPE). A space radiation shielding that is suitable for exploration during space missions requires particular featu

Longitudinal Analysis of Quantitative Brain MRI in Astronauts Following Microgravity Exposure.

BACKGROUND AND PURPOSE: Assessment of the effects of microgravity on astronauts' brains using microstructural measures by utilizing quantitative MRI, before and after spaceflight would help understand the structural changes. METHODS: Quantitative MRI data sets in 19 astronauts were acquired before and after space missions. Both diffusion tensor metrics and volumetric measures were analyzed in the brain regions involved in the visual function. RESULTS: The fractional anisotropy was reduced in the right posterior thalamic radiations (P = .0009) and remained significant after a false discovery

Temazepam, but not zolpidem, causes orthostatic hypotension in astronauts after spaceflight.

Insomnia is a common symptom, not only in the adult population but also in many astronauts. Hypnotics, such as temazepam (a benzodiazepine) and zolpidem (an imidazopyridine), are often taken to relieve insomnia. Temazepam has been shown clinically to have hemodynamic side effects, particularly in the elderly; however, the mechanism is not clear. Zolpidem does not cause hemodynamic side effects. The purpose of this study was to determine whether the use of different hypnotics during spaceflight might contribute significantly to the high incidence of postflight orthostatic hypotension, and to co

Melatonin and cortisol assessment of circadian shifts in astronauts before flight.

Melatonin and cortisol were measured in saliva and urine samples to assess the effectiveness of a 7-day protocol combining bright-light exposure with sleep shifting in eliciting a 12-hr phase-shift delay in eight U.S. Space Shuttle astronauts before launch. Baseline acrophases for 15 control subjects with normal sleep-wake cycles were as follows: cortisol (saliva) at 0700 (0730 in urine); melatonin (saliva) at 0130 (6-hydroxymelatonin sulfate at 0230 in urine). Acrophases of the astronaut group fell within 2.5 hr of these values before the treatment protocols were begun. During the bright-ligh

Biopsychosocial Health Considerations for Astronauts in Long-Duration Spaceflight: A Narrative Review.

Long-duration spaceflights beyond low-Earth orbit, including missions to the Moon and Mars, pose significant health risks. Although biomedical approaches commonly appear in the literature, considering psychological and social factors alongside physiologic health offers a more holistic approach to astronaut care. Integrating the biopsychosocial (BPS) framework into medical planning addresses complex spaceflight challenges and aids in developing mitigation strategies. This review examined health risks associated with long-duration spaceflight within a BPS framework. Sources included governmental

Cognitive performance in ISS astronauts on 6-month low earth orbit missions.

INTRODUCTION: Current and future astronauts will endure prolonged exposure to spaceflight hazards and environmental stressors that could compromise cognitive functioning, yet cognitive performance in current missions to the International Space Station remains critically under-characterized. We systematically assessed cognitive performance across 10 cognitive domains in astronauts on 6-month missions to the ISS. METHODS: Twenty-five professional astronauts were administered the Cognition Battery as part of National Aeronautics and Space Administration (NASA) Human Research Program Standard Mea
