Temporal Trends and Outcomes of Percutaneous Coronary Atherectomy in the United States.

BACKGROUND: There is paucity of data regarding the temporal trends and outcomes of coronary atherectomy in the United States. METHODS: We queried the National Inpatient Sample database (2011-2016) for hospitalizations of patients undergoing coronary atherectomy procedures. We also compared outcomes of non-orbital vs orbital coronary atherectomy in a more contemporary cohort. RESULTS: Our analysis included 2,990,223 hospitalizations with PCI, of which 114,462 (3.8%) included an atherectomy procedure. A significant increase in coronary atherectomy procedures was observed over time (0.66% in 20

IL-8 is an angiogenic factor in human coronary atherectomy tissue.

BACKGROUND: Interleukin-8 (IL-8), a CXC chemokine that induces the migration and proliferation of endothelial cells and smooth muscle cells, is a potent angiogenic factor that may play a role in atherosclerosis. Previously, IL-8 has been reported in atherosclerotic lesions and circulating macrophages from patients with atherosclerosis. Therefore, we sought to determine whether IL-8 plays a role in mediating angiogenic activity in atherosclerosis. METHODS AND RESULTS: Homogenates from 16 patients undergoing directional coronary atherectomy (DCA) and control samples from the internal mammary ar

Clinical Outcomes of Atherectomy Prior to Percutaneous Coronary Intervention: A Comparative Assessment of Atherectomy in Patients With Obesity (COAP-PCI Subanalysis).

OBJECTIVES: The aim of this study was to investigate the safety and efficacy of atherectomy devices in obese patients with coronary artery calcification (CAC). BACKGROUND: Atherectomy is an important tool for lesion preparation in patients with CAC undergoing percutaneous coronary intervention (PCI). There have been no studies that compared the outcomes of orbital atherectomy (OA) and rotational atherectomy (RA) in obese patients. METHODS: A total of 35,590 patients from five tertiary-care hospitals who underwent PCI between January 2011 to April 2016 were identified. All adult patients with

Subclavian atherectomy and angioplasty for coronary subclavian steal syndrome post CABG.

Coronary subclavian steal syndrome is an uncommon complication occurring in patients with coronary artery bypass graft (CABG). We describe a case of a 69-year-old male with a remote history of CABG who presented with exertional left arm pain and angina. Computed Tomographic Angiography of the chest demonstrated a severe left proximal subclavian artery stenosis. The case demonstrates successful application of subclavian atherectomy with use of embolic protective device, alleviating the need of stent, for treatment of Coronary subclavian steal syndrome in patient with remote history of CABG.

Emergency bail-out stenting for major coronary dissection complicating rotational atherectomy.

The problems of restenosis and abrupt occlusion following percutaneous transluminal coronary angioplasty (PTCA) remain a challenge. Restenosis occurs in 30-50% of lesions treated and substantially erodes the potential advantage of percutaneous treatment of obstructive coronary disease over alternative revascularisation strategies. Abrupt closure occurs in 5-7% of cases with significant associated morbidity and mortality. Up to 35% of patients require emergency bypass surgery for treatment of abrupt closure and more than half sustain a perioperative myocardial infarction. By debulking atheroscl

Clinical, angiographic and histopathological predictors of restenosis after directional coronary atherectomy.

Out of 127 directional coronary atherectomy (DCA) procedures done in our laboratory, there were 81 patients who had completed a minimum of six months of follow-up. To study the factors predisposing restenosis after successful DCA, we analysed the clinical and angiographic profile of the patients and the histopathological findings of the excised tissue in 44 patients whose complete follow-up data was available with us. The indication of DCA was an extremely eccentric significant stenosis located in the proximal or midsegment of a large vessel (> or = 3 mm size) in 97 percent of the cases. Angio

Overview of in-hospital outcomes in patients undergoing percutaneous coronary intervention with the revived directional coronary atherectomy.

OBJECTIVES: We sought to provide clinical insights on the usage rate, indications, and in-hospital outcomes of the revived directional coronary atherectomy (DCA) catheter (Atherocut™) in a Japanese nationwide percutaneous coronary intervention (PCI) registry. BACKGROUND: Debulking devices such as the revived DCA catheter have become increasingly important in the era of complex PCI. However, little is known about PCI outcomes using a novel DCA catheter in contemporary real-world practice. METHODS: We analyzed 188,324 patients who underwent PCI in 1112 hospitals from January to December 2018.

Coronary artery atherectomy reduces plaque shear strains: an endovascular elastography imaging study.

Mechanical response and properties of the arterial wall can be used to identify the biomechanical instability of plaques and predict their vulnerability to rupture. Shear strain elastography (SSE) is proposed to identify vulnerable plaque features attributed to mechanical structural heterogeneities. The aims of this study were: 1) to report on the potential of SSE to identify atherosclerotic plaques; and 2) to use SSE maps to highlight biomechanical changes in lesion characteristics after directional coronary atherectomy (DCA) interventions. For this purpose, SSE was imaged using in vivo intra

Increased plasminogen activator inhibitor-1 and apolipoprotein (a) in coronary atherectomy specimens in acute coronary syndromes.

BACKGROUND: Although increased tissue factor expression is known in vulnerable plaques, there is no reported study to compare plaque fibrinolysis in stable and unstable plaques. This study investigates the extent of plasminogen activator inhibitor-1 (PAI-1) and apolipoprotein (a) [apo(a)] in the plaques of different types of coronary artery disease as well as the correlation between these molecules and infiltration of macrophages to plaques. METHODS: Using immunohistochemical staining, we examined PAI-1 expression and apo(a) deposition in coronary atherosclerotic specimens obtained by directi

Excimer laser coronary atherectomy prior to paclitaxel-coated balloon angioplasty for de novo coronary artery lesions.

This study aimed to evaluate the efficacy and safety of excimer laser coronary atherectomy (ELCA) prior to paclitaxel-coated balloon angioplasty for de novo coronary artery lesions. This retrospective observational study analyzed 118 eligible patients with de novo coronary artery disease whose only percutaneous coronary intervention was a drug-coated balloon angioplasty (i.e., no subsequent stent placement). Data related to our primary outcomes of interest-incidence of major adverse cardiovascular and cerebral events (MACCE), and incidence of procedural complications (bailout stenting and mino

Non-invasive follow-up of a coronary artery pseudoaneurysm after percutaneous transluminal coronary rotational atherectomy (PTCRA).

Percutaneous transluminal coronary rotational atherectomy (PTCRA) is an established treatment forvery calcified and/or undilatable coronary lesions. In contrast to data from directional coronary atherectomy (DCA) procedures, coronary artery aneurysms (CAA) or pseudoaneurysms have not routinely been described as a complication after PTCRA. We present a case of a 74-year-old patient, with a major CAA, found on a routine control coronary angiogram 4 months after a PTCRA of LAD complicated by a coronary perforation. A conservative management guided by coronary CT angiography is proposed.

Recurrent unstable angina after directional coronary atherectomy is related to the extent of initial coronary plaque inflammation.

OBJECTIVES: This study was performed to evaluate the relationship between plaque inflammation of the initial culprit lesion and the incidence of recurrent angina for one year after directional coronary atherectomy (DCA). BACKGROUND: A positive correlation between coronary plaque inflammation and angiographic restenosis has been reported. METHODS: A total of 110 patients underwent DCA. Cryostat sections were immunohistochemically stained with monoclonal antibodies CD68 (macrophages), CD-3 (T lymphocytes) and alpha-actin (smooth muscle cells [SMCs]). The SMC and macrophage contents were planim

Combined atherectomy and intravascular lithotripsy in calcified coronary lesions: a meta-analysis.

OBJECTIVES: Studies exploring the feasibility and safety of combined atherectomy and intravascular lithotripsy (IVL) for treatment of severely calcified coronary lesions are limited. The authors aimed to study the pooled evidence for the safety and efficacy of this combined approach in severely calcified coronary lesions and evaluate its clinical and procedural outcomes. METHODS: Online databases were searched for studies using combined atherectomy (rotational, orbital, or excimer laser) and IVL during PCI for calcified coronary lesions. The outcomes of interest were acute procedural success,

Incidence and Mechanisms of Coronary Perforations during Rotational Atherectomy in Modern Practice.

OBJECTIVE: Heavy calcifications remain formidable challenges to PCI, even for well-experienced operators. However, rotational atherectomy (RA)-induced coronary perforations (CPs) still could not be obviated. This study was to explore incidence and mechanisms of RA-induced CP in real-world practice. Knowing why CPs occur in RA should help operators avert such mishaps. METHOD: Patients who received coronary RA from April 2010 to December 2019 with keywords related to perforations were retrieved from database. The procedure details, angiography, and clinical information were reviewed in detail.

Effect of Rotablator atherectomy and adjunctive balloon angioplasty on coronary blood flow.

BACKGROUND: The purpose of this study was to assess serial changes in coronary blood flow velocity before and after Rotablator atherectomy and after adjunctive percutaneous transluminal coronary angioplasty (PTCA). Since Rotablator atherectomy results in luminal enlargement by plaque pulverization and distal embolization, improvement in coronary blood flow could be attenuated despite luminal enlargement. METHODS AND RESULTS: Intracoronary Doppler blood flow velocity measurements were obtained with a Doppler Flowire. Basal average peak velocity (bAPV), hyperemic APV (hAPV), diastolic/systolic

Guidewire Traction Retrieval of a Dislodged Classic Crown During Coronary Orbital Atherectomy.

BACKGROUND Heavily calcified coronary lesions often lead to stent underexpansion and procedural complications. Treatment comprises plaque modification techniques, such as atherectomy. Orbital atherectomy (OA) is effective for preparing severely calcified lesions prior to stenting. Device-related complications, including crown detachment, are rare but potentially serious. CASE REPORT A 70-year-old man with stable angina and 90% proximal right coronary artery stenosis displayed concentric, heavy calcification on intravascular ultrasound. OA was performed using the Diamondback 360 Orbital Atherec
