Cerebrospinal fluid concentrations of laudanosine after administration of atracurium.

The concentration of laudanosine in cerebrospinal fluid (CSF) was measured in four patients undergoing brain electrode placement after the administration of atracurium. CSF: plasma laudanosine concentration ratios ranged from less than 1 to 14%, with a range of CSF laudanosine concentrations of less than 2-14 ng ml-1. One patient had no detectable laudanosine in CSF, but sampling in this patient was possible for only 30 min. There was no atracurium detectable in the CSF of any patient. We conclude that laudanosine crosses the blood-brain barrier and further study of its central nervous system

[Anesthesia with propofol and atracurium in a case of porphyria cutanea tarda].

The conduction of anaesthesia in a patient with a diagnosis of porphyria cutanea tarda is described. The aim of this paper is to provide further evidence for safety of TIVA (Total Intra Venous Anaesthesia) paying particular attention to propofol and atracurium (there are not sufficient data about it in the literature) in patients with porphyria cutanea tarda and high levels of uroporphyrin. The study of the anaesthetic management of a patient suffering from Porphyria Cutanea Tarda (PCT), the most common form of porphyria, can offer useful indications. Due to the fact that PCT is less severe th

Effect of age, sex and anaesthetic technique on the pharmacokinetics of atracurium.

We have defined the pharmacokinetics of atracurium besylate 0.25 mg kg-1 in 41 patients anaesthetized with 0.9% isoflurane end-tidal, 0.5% halothane end-tidal or midazolam 3-10 mg as a supplement to 66% nitrous oxide in oxygen. The pharmacokinetic profile was affected by age, sex and anaesthetic technique. Advancing age was associated with a reduced clearance and a longer elimination half-time; clearance was greater and elimination half-time was shorter in males than in females. Clearance was also greater in patients anaesthetized with isoflurane than with the two other techniques. Age, sex an

Electrophysiological effects of atracurium and vecuronium on normal and denervated hearts.

Atracurium and vecuronium offer some advantages with respect to other nondepolarizing muscle relaxants. However, their administration has been associated with episodes of bradycardia and asystole, the mechanism of which has not yet been clarified. In an effort to analyze the effects of these drugs on automaticity, refractoriness, and conduction, 30 dogs underwent heterotopic heart transplantation to establish a model in which a normal heart (recipient) and a denervated heart (donor) could coexist. The electrophysiological studies included the determination of cycle length (CL), sinoatrial cond

The response of a patient with von Recklinghausen's disease to succinylcholine and atracurium.

The anesthetic management of a 17-year-old female with von Recklinghausen's disease (multiple neurofibromatosis) is described. Multiple neurofibromatosis is a rare syndrome characterized by abnormal cutaneous pigmentation and numerous skin tumors. In addition to the usual stigmata of the syndrome, patients with neurofibromatosis have been reported to be either sensitive or resistant to succinylcholine. Further, all reports indicate that response to nondepolarizing muscle relaxants is exaggerated. In this patient, the nerve conduction studies were within limit of normal values. During surgery,

Comparison of atracurium and d-tubocurarine for prevention of succinylcholine myalgia.

We compared the incidence of postoperative myalgia (POM) and fasciculations when atracurium (ATR) or d-tubocurarine (DTC) was given prior to succinylcholine (SDC) for tracheal intubation in 44 ASA class I or II outpatient females undergoing laparoscopy. The subjects were assigned to one of three groups: group 1 received 0.025 mg/kg ATR; group 2 received 0.05 mg/kg DTC; and group 3 received saline (NS), all in a double-blind manner. Thiopental was administered 1 min and 45 sec after pretreatment in doses adequate to allow control of ventilation. Three minutes after pretreatment, SDC 1.5 mg/kg w

Atracurium and vecuronium block nicotine-induced carotid body chemoreceptor responses.

BACKGROUND: Vecuronium depresses carotid body chemosensitivity during hypoxia. We hypothesized that this is caused by inhibition of cholinergic transmission of the carotid body. METHODS: The carotid body with its sinus nerve was removed en bloc from thiopentone-anaesthetized adult male New Zealand rabbits and perfused in vitro with modified Tyrodes buffer solution at constant perfusion pressure, temperature, a buffer pH of 7.4 and normocapnia. Chemoreceptor discharge and spike frequencies (fx) were recorded from the whole sinus nerve after administration of 500 microg nicotine, given as dupli

Intraoperative latex anaphylaxis compounded by atracurium sensitivity: a case report.

This report describes a life-threatening anaphylactic reaction in an 8-year-old male patient that occurred shortly after intra-abdominal contact with latex surgical gloves. Follow-up skin testing as an outpatient revealed a positive reaction to both latex and atracurium while the other anesthetic agents tested proved negative. For a subsequent operation in which this patient received a kidney transplant, an uneventful anesthetic course was achieved by avoiding the use of latex products and atracurium. Careful planning for that successful procedure included use of vinyl gloves by the surgical t

Atracurium Versus Cisatracurium in the Treatment of Acute Respiratory Distress Syndrome.

Background: Neuromuscular blocking agents are one of the few medication classes that have demonstrated a clinical benefit in patients with severe acute respiratory distress syndrome (ARDS). However, most literature utilized cisatracurium, and utilization of atracurium is limited to 1 small study. Objective: The purpose of this study was to provide further evidence comparing the safety and efficacy of atracurium versus cisatracurium for the treatment of ARDS. Methods: This multicenter, retrospective, observational cohort noninferiority study was conducted at 3 hospitals within a tertiary health

[Influence of different local anesthetics on atracurium neuromuscular blockade on rats].

INTRODUCTION: The association between local anesthetics (LA) and neuromuscular blocking (NMB) drugs in clinical practice, and the possibility of interaction between these drugs has been investigated. LAs act on neuromuscular transmission in a dose-dependent manner and may potentiate the effects of NMB drugs. OBJECTIVE: The aim of this study was to evaluate, in an experimental model, the effect of lidocaine and racemic bupivacaine on neuromuscular transmission and the influence on neuromuscular blockade produced by atracurium. METHODS: Male Wistar rats, weighing from 250  to 300 g were used.

Prolonged paralysis with atracurium use in a patient with Rubinstein-Taybi syndrome.

Rubinstein-Taybi syndrome (RTS) is a rare autosomal dominant disorder that occurs due to a microdeletion of chromosome 16p13. The craniofacial abnormalities in these patients may pose a challenge for anaesthetist performing tracheal intubation. However, there are no known reported cases of drug interaction with non-depolarising muscle relaxant in patients with RTS. This young patient with RTS presented with an unexpected prolonged atracurium effect during the course of anaesthesia. After ruling out other possible causes, we have come to a conclusion that RTS itself could have played a role in

The distribution of laudanosine in tissues after death from atracurium injection.

A case is presented involving an acute fatality resulting from self-administration of atracurium, a muscle relaxant by a 45-year-old nurse. In the body, atracurium undergoes a spontaneous non-enzymatic degradation to laudanosine and an acrylate moiety. Laudanosine was quantified using gas chromatography coupled to mass spectrometry after extraction with chloroform-isopropanol-n-heptane (50:17:33 v/v) at pH 9.5 and separation on a HP5-MS capillary column. Laudanosine was subject to postmortem redistribution due to release from drug-rich tissues such as the lung and heart. The heart blood (917 n

A study of different doses of atracurium to prevent suxamethonium-induced fasciculations.

Pretreatment with 3 different doses of atracurium (0.035 mg/kg, 0.05 mg/kg, 0.07 mg/kg) was investigated in order to determine the optimal dose that would prevent suxamethonium-induced fasciculations with minimal side effects to the patient and without significant antagonism of suxamethonium block. A total of 80 patients was studied. There were 20 patients in each of three atracurium groups while a fourth group of 20 patients served as control. Atracurium reduced the incidence of fasciculations from 100% observed in the control to between 0 and 40% in the atracurium groups. The optimal dose of

[The use of atracurium besylate (Tracrium) in patients undergoing kidney transplantation].

According to literature data Atracurium besylate is a nondepolarizing muscle relaxant akin to the "ideal", insofar as it is governed by the "dose-relaxation time" principle. To secure normal clinical effect, doses in the range 0.4-0.5 mg/kg body mass are recommended, with the maximal manifestation of neuromuscular block occurring within 4.6-7.7 min, and lasting for 35.1 to 39.2 minutes. Maintenance dose: 0.05-0.1 mg/kg body mass. Special emphasis is laid on the fact that at normal temperature and pH the dose/relaxation time ratio is strictly fixed which in turn renders relaxation readily contr

Evaluation of succinylcholine-induced fasciculations and myalgias with or without atracurium pretreatment.

Pretreatment regimens that decrease the incidence of fasciculations and postoperative myalgias have been the focus of many research studies. The subject of pretreatment remains controversial. An experimental double blind study was conducted of 50 patients, men and women, aged 18 to 65 years who were having elective minor orthopedic surgery. Group A participants (n = 24) received normal saline, and group B participants (n = 26) received atracurium 0.05 mg/kg, followed by succinylcholine 1.5 mg/kg. Data that were collected included age, ASA physical status, weight, height, anesthesia and postane

Comparative pharmacology of cisatracurium (51W89), atracurium, and five isomers in cats.

BACKGROUND: Atracurium has four chiral centers and the marketed product is a mixture of ten optical and geometric isomers. Six of the isomers were prepared and evaluated for neuromuscular blocking activity and autonomic effects in anesthetized cats. This study reports the comparative pharmacology of the six isomers and atracurium that led to the selection of one isomer, cisatracurium (Nimbex, 51W89) for clinical development. METHODS: Purpose bred cats, anesthetized with alpha-chloralose (80 mg/kg) and pentobarbital sodium (7 mg/kg) administered intraperitoneally, were used in this study. Neur

[Malignant hyperthermia in swine: a study of atracurium in MH-susceptible swine].

Since muscle relaxants have been implicated in triggering malignant hyperthermia (MH) in MH-susceptible humans and animals, the potential of new muscle relaxants for triggering MH needs to be assessed in vivo in MH-susceptible pigs. The triggering potential of atracurium was evaluated in six MH-susceptible pigs during one hour infusion of a 90% blocking dose (0.4 mg/kg/h) of atracurium. The mean recovery time (25% to 75%) was 10.2 min and a slight increase in heart rate (142 to 170 beats per minute) was observed. Rectal temperature decreased slightly (36.4 to 35.6 degrees C) and end tidal CO2
