Comparative quantitative electrophysiologic effects of adenosine triphosphate on the sinus node and atrioventricular node.

A pharmacologic approach was used to characterize the sinus node (SN) and atrioventricular (AV) node responses to intravenous adenosine triphosphate (ATP) in 21 patients during electrophysiologic testing. ATP produced dose-dependent prolongation of spontaneous sinus cycle length. The mean maximal increase in sinus cycle length was 294 +/- 60 ms (n = 10). The mean dose of ATP required to produce this maximal sinus cycle length prolongation was 3.5 +/- 1.0 mg. The dose-response curve for sinus cycle length prolongation was sigmoid in shape with an EC50 of 0.39 mg derived from a median effect plo

Cystic tumor of the atrioventricular node: rare antemortem diagnosis.

BACKGROUND: The cystic tumor of the atrioventricular node (TAV) is a rare, congenital cardiac tumor, typically located at the base of the atrial septum. Histologically benign, this multicystic mass is a tumor of the conduction system and is considered the smallest tumor capable of causing sudden and unexpected death. TAV has shown a predilection for women with a mean age at presentation of 38 years. The majority of cases are diagnosed incidentally at autopsy, while antemortem surgical excision is rare, with ours being the fifth and sixth reported cases in the medical literature. METHODS: We p

Catheter ablation of the atrioventricular node using radiofrequency energy.

BACKGROUND: Catheter ablation of the atrioventricular (AV) junction using stored direct current (DC) energy from a standard DC Cardioverter defibrillator was first reported in 1982. Since then many patients have been treated using this procedure for refractory supraventricular arrhythmias, usually atrial fibrillation and flutter. Undesirable thermal effects such as barotrauma and arcing are largely responsible for complications associated with the use of DC energy. This report details our experience of catheter ablation of the AV junction using radiofrequency (RF) energy in a series of 30 cons

Transcatheter modification of the atrioventricular node using radiofrequency energy.

120 consecutive patients with symptomatic atrioventricular nodal reentrant tachycardia (AVNRT) underwent catheter ablation using radiofrequency energy. Fast pathway ablation was attempted in the first 16 consecutive patients by application of radiofrequency current in the anterior and superior aspect of the tricuspid annulus. Successful results were accomplished in 13 patients, complete AV block occurred in three. The other 104 patients initially underwent ablation of the slow pathway in the posterior and inferior aspects of the tricuspid annulus which was successful in 98 patients. The remain

[Congenital atrioventricular block and maternal lupus erythematosus. Histologic discovery of tumor of the atrioventricular node].

A case of congenital atrioventricular block in a newborn whose mother presented with systemic lupus erythematosus (SLE) is reported. Despite intensive care the child died a few hours after birth. Serial sections of the heart could be examined. Histology provided information on the appearance and distribution of the lesions. In particular, the sinus node was small for the child's age, and its supplying artery was found to have a hyperplastic media with adventitial sclerosis; the interatrial and interventricular septa showed subendocardial fibrosis invading the adjacent myocardium. Owing to the

Impact of Coronary Dominance on Atrioventricular Node Ablation Approach.

Persistent atrial fibrillation often requires atrioventricular node (AVN) ablation when conventional rhythm - or rate-control strategies are unsucessful, but procedural outcomes may be influenced by anatomic variability. We hypothesized that coronary artery dominance affects AVN positioning and the preferred ablation approach. In this retrospective study, 132 patients who underwent AVN ablation and coronary CTA were analyzed. Right coronary dominance was present in 78.8% of patients, left in 13.6% and co-dominant in 7.6%. Patients with left or co-dominant circulation had significantly higher o

Transient atrioventricular block following catheter radiofrequency sinus node modification.

Complete atrioventricular block, second-degree Mobitz type II and first-degree atrioventricular block with right bundle branch block were observed consecutively following successful radiofrequency ablation in close proximity to the sinus node. This resulted in the modification and disappearance of the inappropriate sinus tachycardia that had previously been present. Neither tachycardia nor conduction disturbances have been recorded in the 9-year follow-up, implying that ventricular pacing standby should also be considered in high atrial ablation cases. (Cardiol J 2007; 14: 504-507).

Multicenter Outcomes of Catheter Ablation for Atrioventricular Reciprocating Tachycardia Mediated by Twin Atrioventricular Nodes.

OBJECTIVES: This study sought to describe the electrophysiologic properties and catheter ablation outcomes for atrioventricular reciprocating tacchycardia via twin atrioventricular nodes (T-AVRT). BACKGROUND: Although catheter ablation for T-AVRT is an established entity, there are few data on the electrophysiological properties and outcomes of this procedure. METHODS: An international, multicenter study was conducted to collect retrospective procedural and outcomes data for catheter ablation of T-AVRT. RESULTS: Fifty-nine patients with T-AVRT were identified (median age at procedure, 8 yea

Twin atrioventricular node associated with interruption of the inferior vena cava and atrioventricular nodal reentrant tachycardia.

We report the case of a patient exhibiting symptomatic junctional bigeminy associated with twin atrioventricular (AV) node and an anomaly in the inferior vena cava. The patient evidenced twin AV node and complete interruption of the inferior vena cava, with azygos continuation. The catheters for mapping of the AV junctional area and ablation were accessed via jugular and subclavian venous approaches, and azygos venous approach via the femoral vein. Twin AV node was diagnosed by (1) the existence of 2 discrete non-preexcited QRS morphologies of junctional bigeminy, (2) decremental anterograde a

Sinus node-atrioventricular node isolation: long-term results with the "corridor" operation for atrial fibrillation.

The "corridor" operation is designed to restore sinus rhythm to patients with atrial fibrillation by electrically isolating the sinus node, a band of atrial tissue and the atrioventricular (AV) node from the remaining atrial tissue. Nine patients with drug-refractory atrial fibrillation underwent this operation; four patients had chronic atrial fibrillation and five had paroxysmal atrial fibrillation; the mean duration of symptoms was 12 +/- 8 years. Patient ages ranged from 25 to 68 years (mean 48 +/- 12). At preoperative electrophysiologic study, no patient had evidence of an accessory AV pa

Intrinsic Sinus Node/Atrioventricular Node Dysfunction Requiring Pacemaker Implantation: Role of Former Professional Sport Activity.

BACKGROUND: Sinus bradycardia and first degree or second degree Mobitz type I atrioventricular (AV) block in an athlete are considered adaptive and reversible phenomena; however, some evidence suggests that they may persist after detraining and become pathological. The aim of the study was to investigate the characteristics of a group of former professional athletes who required pacemaker (PM) implantation for intrinsic (idiopathic) sinus node (SN) dysfunction or AV block in comparison to control groups of sedentary individuals. METHODS: We included all patients who underwent PM implantation

Uncovering the mystery of the atrioventricular node dual-pathway electrophysiology.

With the discovery and validation of a novel index of dual-pathway electrophysiology, known as the His electrogram alternans, major advances have been made in understanding the electrical propagation through the atrioventricular (AV) node. At slow rates, AV conduction starts at the interatrial septal boundary of the node (superior nodal domain) and spreads toward the tricuspid annulus side (inferior nodal domain) in a direction perpendicular to the AV axis and across fiber orientation. Such activation results in an early, superior input into the superior His bundle. This is the fast pathway (F

Anatomic assessment of variations in myocardial approaches to the atrioventricular node.

INTRODUCTION: The tissues in the posteroinferior atrioventricular (AV) junction around the AV node are important in procedures for ablating and manipulation of catheters in and around the coronary sinus (CS). However, information with regard to the histological arrangement of perinodal myocardium relative to the CS is lacking. METHODS AND RESULTS: We examined 21 postmortem human hearts without any abnormalities (9 women; mean age 68.8 ± 14.3 years). After making measurements, the posteroinferior AV junction was removed and processed for histology. Sections were cut parallel to the septum. We

[Ablation of the atrioventricular node using radiofrequency in a cirrhosis patient].

The ablation of cardiac arrhythmias by radiofrequency has changed radically the treatment of arrhythmias. We present the case of a woman with cirrhosis and no structural heart disease, who was severely limited in her activities because of drug-refractory paroxysmal supraventricular tachyarrhythmias. She underwent an atrio-ventricular node ablation by radiofrequency. She was discharged with no antiarrhythmic drugs and on a permanent cardiac pacing. She showed obvious improvement in her quality of life.

Fatal Lyme carditis and endodermal heterotopia of the atrioventricular node.

A fatal case of Lyme carditis occurring in a Suffolk farmworker is reported. Post-mortem examination of the heart showed pericarditis, focal myocarditis and prominent endocardial and interstitial fibrosis. The additional finding of endodermal heterotopia ('mesothelioma') of the atrioventricular node raises the possibility that this could also be related to Lyme infection and account for the relatively frequent occurrence of atrioventricular block in this condition. Lyme disease should always be considered in a case of atrioventricular block, particularly in a young patient from a rural area. T

Topography and blood supply of atrioventricular node of the human heart.

Characteristic features of the topography and blood supply of the atrioventricular (A-V) node were studied on 80 autopsy heart specimens of adults deceased of other than cardiovascular disease. The authors used injection methods, angioroentgenography, macro- and microscopy. They refined nodal projection and established relationships between its location and the fossa ovalis, coronary sinus orifice, the base of the septal leaflet of the tricuspid valve and cardiac parameters. New information was obtained on the structural variability of the main source of A-V node nutrition - the arteria septi
