Improved transdural exposure and temporary occlusion of the petrous internal carotid artery for cavernous sinus surgery. Technical note.

Safe surgery for vascular and neoplastic lesions involving the cavernous sinus requires adequate proximal control of the cavernous internal carotid artery (ICA). Classically, control of the cavernous ICA has necessitated dissection and isolation of the cervical vessels. The authors describe an alternative method for obtaining vascular control via transdural exposure of the petrous ICA and the use of the Fogarty balloon embolectomy catheter inserted extra-arterially into the carotid canal to provide temporary occlusion. This method is particularly well suited for proximal ICA occlusion during s

[Abdominal Aortic Bifurcation Occlusion Caused by a Left Atrial Myxoma:Report of a Case].

A 30-year-old male patient who reported a medical history of bronchial asthma and diabetes mellitus presented with bilateral leg pain and weakness during exercise. He experienced leg numbness for the previous six months. Imaging detected a thromboembolic occlusion in the abdominal aorta, along with a left atrial mass. He underwent an emergency embolectomy with a Fogarty balloon catheter;however, persistent embolism at the aortic bifurcation required surgical embolus removal. The embolus consisted of both fresh thrombus and mucous-like tumor tissue, which was determined as a myxoma. Further lef

Balloon-assisted tracking technique to overcome intracranial stenosis during thrombectomy for stroke.

Anatomical vessel obstacles such as an intracranial stenosis in stroke are challenging and may lead to delayed clot access for thrombectomy. We describe a simple and effective technique to overcome the step-off between the intermediate catheter and an intracranial vessel stenosis during thrombectomy. The patient presented with acute embolic left middle cerebral artery occlusion and a favorable penumbral pattern. Clot access was made challenging by focal stenosis in the ipsilateral cavernous segment. The balloon-assisted tracking technique was effective in traversing the step-off to enable TICI

Use of Amplatzer duct occluder to close severely hypertensive ducts: utility of transient balloon occlusion.

BACKGROUND: The dividing line between a patent ductus arteriosus with severe but reversible pulmonary artery hypertension and an Eisenmenger duct remains elusive. Increasing experience with the Amplatzer duct occluder has made non-surgical closure of even large patent ductus arteriosus simple and safe. However, data on the use of this device in severly hypertensive ducti is sparse. METHODS AND RESULTS: There were six patients (2 males, 4 females), wherein transient ductal occlusion was done with an embolectomy balloon catheter. The post-occlusion hemodynamics along with overall clinical and h

Elective or emergent interventional radiology through introducer sheath inserted during emergent vascular surgery for critical ischemia.

During performance of embolectomy or thrombectomy one often encounters arterial stenoses. These may be treated by adjunctive intraoperative angioplasty during the operation or delayed percutaneous angioplasty (PTA) may be performed in the interventional radiology department. Delayed PTA may have to wait until the wound has healed, and during this time there is a risk of re-occlusion. It may often be desirable to perform the radiologic intervention at a later time than the thrombo-embolectomy for logistic reasons. The vascular surgeon and interventional radiologist may perform best when working

Thrombectomy approach for access maintenance in the end stage renal disease population: a narrative review.

OBJECTIVE: This article reviews current practices and outcomes in endovascular thrombectomy techniques for the treatment of thrombosed arteriovenous grafts (AVGs) and fistulas (AVFs). BACKGROUND: Arteriovenous (AV) access allows patients with end-stage renal disease (ESRD) to receive hemodialysis. Thrombosis of AV access can lead to delay in hemodialysis or abandonment of access requiring dialysis catheter placement. Endovascular approach has become the preferred treatment option for thrombosed access over surgery. Interventions include removal of thrombus from the AV circuit and treatment of

Simple strategy of anesthesia for the neonate with tracheoesophageal fistula: a case report.

A 3-day-old neonate, given a diagnosis of esophageal atresia (EA) with tracheoesophageal fistula (TEF), which is large and just above the carina, was scheduled for TEF repair. Routine anesthetic management focuses on adequate ventilation and avoidance of gastric distension during positive pressure ventilation. Using a balloon-tipped embolectomy catheter or a Fogarty catheter to block the fistula under the guidance of fiberoptic scope has been described. In most of the medical centers, however, the pediatric fiberoptic scope may not be available. We present a case of a newborn undergoing type C

[Update in endovascular therapy of ischemic cerebrovascular disease].

Endovascular therapy of ischemic cerebrovascular disease has developed rapidly in the past few years. The big breakthrough in acute ischemic stroke treatment is mechanical embolectomy,of which new devices, technologies, concepts and trials are bringing great benefits to more patients.Evidence becomes more substantial and reliable for endovascular stenting of carotid artery stenosis.New stents and embolic protection devices might further lower its peri-procedure risk of brain ischemia and improve its efficacy of stroke prevention. For patients with intracranial stenosis,stented-assisted angioplasty

Use of a Fogarty balloon catheter for management of distal nasolacrimal duct atresia in a standing sedated horse.

A 4-year-old Arabian cross paint mare was referred for evaluation of chronic mucopurulent discharge and epiphora of the left eye. Ophthalmic and radiographic evaluations confirmed distal nasolacrimal duct atresia with absence of the nasolacrimal orifice. Initial minimally invasive treatments, including guidewire-assisted catheterization and diode laser photoablation, were unsuccessful. Definitive treatment was achieved using a Fogarty arterial embolectomy catheter under standing sedation. Controlled balloon inflation and deflation enabled progressive dilation of the obstructed duct, and a surg

Mechanical thrombectomy in peripheral arterial interventions.

Acute arterial closure either from thrombosis or embolization often is a serious and potentially life-threatening clinical problem. Traditional surgical treatment options, such as bypass or balloon embolectomy, are efficacious but their overall clinical success is limited by the significant morbidity and mortality associated with the procedures. The use of less invasive stand-alone thrombolysis is limited by bleeding risk and the inherent delay in restoration of blood flow. Mechanical devices have been developed to help speed percutaneous treatment of arterial occlusion due to clot. Mechanical

Management of primary acute arterial occlusion.

The management of primary acute arterial occlusion depends upon distinguishing between embolism and thrombosis. Emboli are sudden in onset, have a demonstrable source and lodge most frequently at the common femoral bifurcation. Embolectomy is the treatment of choice. Acute thrombosis is usually preceded by prolonged, progressive, ischemia, is less abrupt in onset and occurs most commonly in the superficial femoral artery. The initial treatment is anticoagulation with heparin, followed by artery repair if indicated. Emergency surgical reconstruction is necessary if the condition of the limb det

A case of superior mesenteric artery embolism successfully treated by combined thrombus retrieval therapy using the CAPTIVE technique.

Acute superior mesenteric artery (SMA) embolism is potentially fatal if intestinal necrosis develops, and prompt revascularization is essential. Although thrombolytic therapy and aspiration thrombectomy are commonly employed, thrombolysis is contraindicated in patients with hemorrhagic lesions. We report the application of the continuous aspiration prior to intracranial vascular embolectomy (CAPTIVE) technique in an elderly woman with acute SMA embolism complicated by hemorrhagic stroke. Mechanical thrombectomy using a React 71 aspiration catheter and Solitaire stent retriever was performed un

Subclavian artery intervention with a balloon-tipped occlusion catheter via the ipsilateral brachial artery without an introducer sheath.

To protect against a distal embolism in the vertebral artery, subclavian artery stenting can be achieved by positioning a filter device or balloon in the ipsilateral vertebral artery. However, treatment with these devices is not easy, and it is also an inaccurate method for cerebral protection. We developed a balloon-tipped occlusion catheter (Optimo(®) occlusion catheter) without an introducer sheath, which allowed us to perform the minimally invasive endovascular therapy (EVT). Herein, we report two EVT cases for subclavian artery disease treated with an Optimo(®) occlusion catheter via the

Prevention of intimal hyperplasia; Initial results with low-dose beta-radiation applied by an external vascular polymer wrap.

OBJECTIVE: Endovascular brachytherapy, delivered by a variety of catheter-based devices, has proven clinically effective for the inhibition of neointimal hyperplasia (NIH) after coronary and peripheral balloon/stent angioplasty. No platform, however, has been developed to deliver low-dose radiation in concert with vascular surgical operations. The purpose of this study was to evaluate the vascular response following balloon injury to the rabbit carotid artery, with and without topical low-dose 45Ca, applied by an external vascular "wrap". METHODS: Twelve rabbit carotid arteries were subjected

Percutaneous intraarterial thrombolysis in the treatment of thrombosis of lower extremity arterial reconstructions.

Vascular grafts may be salvaged with thrombolytic therapy after acute occlusion as an alternative to balloon catheter thrombectomy. From October 1987 to May 1990, 15 arterial bypasses to the lower extremity (infrainguinal saphenous vein [n = 7] or expanded polytetrafluoroethylene [n = 6], and Dacron aortofemoral bifurcation graft limbs [n = 2]) were treated for 30 occulsions with intraarterial urokinase (390,000 IU to 5,808,000 IU) infused from 3 to 40 hours. The origins of 15 graft occlusions were morphologic defects (intimal hyperplasia with anastomotic or conduit stricture), pseudoaneurysm,

Embolic lower limb ischemia as a rare complication of ulcerative colitis: A case report and literature review.

Thromboembolic events in chronic inflammatory bowel diseases are rare and predominated by venous damage. Arterial thromboembolic events are extremely uncommon. We are reporting the case of a 50-year-old male patient with an 8-year history of ulcerative colitis, who presented a recurring severe lower limb ischemia during hospital stay for exacerbation of his inflammatory bowel disease. During the first visit, the patient underwent a balloon thromboembolectomy via femoral approach, with fasciotomy of the leg. Because of recurrence of symptoms, he underwent a second and a third thromboembolectomy

Clot-trapper device for transjugular thrombectomy from the inferior vena cava.

The clot-trapper device for transjugular thrombectomy was used in 10 dogs with acute thrombosis and two dogs with chronic thrombosis. In eight of these animals, the clots were trapped with the clot-trapper device, then a mechanical thrombectomy device was used to destroy the clots. In 10 animals, including eight with in vitro thrombi and two with in situ thrombi, thrombi were efficiently trapped with the clot-trapper device. In two animals, pulmonary embolism occurred because of a discrepancy between the diameter of the inferior vena cava and the diameter of the bag opening. After increasing t

The effects of embolectomy-thrombectomy catheters on vascular architecture.

The purpose of this report is to describe the evolution of an embolectomy-thrombectomy catheter (ETC) injury over a six week period. Carotid arteries and jugular veins in six adult dogs were subjected to ETC withdrawals at a standard velocity and balloon size. Vascular segments were excised as early as one hour and as late as six weeks. The specimens were prepared for light, scanning electron and transmission electron microscopic examination. In early specimens, arteries and veins showed endothelial denudation followed by regeneration. In later specimens, the arteries showed progressive disrup

Acute embolic occlusion of the superior mesenteric artery: a case report and discussion of management.

INTRODUCTION: We present a case of acute embolus to the superior mesenteric artery (SMA). CLINICAL PICTURE: A 70-year-old gentleman with atrial fibrillation complained of colicky abdominal pain with clinical signs of tenderness and mild guarding. TREATMENT AND OUTCOME: Laparotomy revealed extensive bowel ischaemia but no overt infarction. The SMA was occluded by an embolus at the root of the mesentery and balloon catheter embolectomy was carried out at once. Bowel resection was deferred in order to allow clear demarcation of gangrene to avoid sacrificing unnecessary length. At second look la
