The vertical-apical suture Bankart lesion repair for anteroinferior glenohumeral instability.

We report the results of the vertical-apical suture Bankart lesion repair in 59 patients with traumatic anteroinferior glenohumeral instability. According to the system of Rowe et al, at a mean follow-up of 42 months (minimum, 2 years), 94.9% (56 patients) had a rating of good or excellent. Three patients had a recurrent dislocation due to further trauma. The mean Rowe score was 94.6, the mean Walch-Duplay score was 94.3, the mean Constant score was 90.4, and the mean rating on a numerical satisfaction scale was 8.7. The mean loss of external rotation with the arm at the side was 2.4 degrees,

Arthroscopic versus open reconstruction of the shoulder in patients with isolated Bankart lesions.

We compared open and arthroscopic stabilizations of true Bankart lesions in patients with traumatic, unidirectional anterior glenohumeral dislocations. The 27 patients were men (age range, 18 to 56 years) who were involved in recreational sports. One group (15 patients) had elected an arthroscopic Bankart repair; the other group (12 patients) had chosen open stabilization with a standard deltopectoral approach. Patients were followed up 17 to 42 months after surgery by examination, radiographs, and interviews. In the open repair group, 1 of the 12 patients experienced a subluxation in the foll…

Grading system for Hill-Sachs lesion and its association with Bankart lesion: a cross-sectional study.

BACKGROUND: Bankart lesions and Hill-Sachs lesions can be detected in up to the majority of recurrent dislocations. This study aims to develop a grading system for Hill-Sachs lesions and assess its association with Bankart lesions. The proposed grading system, based on the dimensions of Hill-Sachs lesions, offers a standardized approach for classifying these defects, thereby facilitating the comparison of research findings across studies. Additionally, this study tends to estimate the highly significant association between Hill-Sachs lesions and the presence of Bankart lesions compared to prev…

Perthes lesion (a variant of the Bankart lesion): MR imaging and MR arthrographic findings with surgical correlation.

OBJECTIVE: The aim of this study was to evaluate the use of MR imaging in the characterization of the Perthes lesion by correlating MR findings with findings at arthroscopy. CONCLUSION: The use of a combination of axial and abduction-external rotation position sequences on MR images can be helpful in the diagnosis of a Perthes lesion. A fluid-filled joint with capsular distension, caused by either a large amount of effusion or MR arthrography, was found to be helpful in outlining Perthes lesions. Adding the abduction-external rotation position to the protocol in patients in whom Perthes lesio

Comparison between arthroscopic Bankart repair versus arthroscopic Bankart/SLAP lesion repair in limited-contact athletes with type V SLAP lesion. A prospective cohort study.

PURPOSE: The aim of this study was to compare the functional outcomes, recurrence rate, range of motion (ROM) and return to sports activities between arthroscopic Bankart repair (ABR) versus arthroscopic Bankart/SLAP repair (ABR/S) in limited contact-athletes with a type V SLAP lesion in the scenario of recurrent anterior shoulder instability (RASI). Our hypothesis was that there is no difference between the two treatments. METHODS: Two groups of 45 limited-contact athletes with type V SLAP lesion were created. Group 1 underwent an arthroscopic Bankart repair, while group 2 had an arthroscopi

Outcomes following open repair of Bankart lesions for recurrent, traumatic anterior glenohumeral dislocations.

Though open stabilization using the Bankart procedure is still considered the "gold standard," few studies have examined the long-term outcomes. Between 1988 and 2001, 64 patients met inclusion criteria and had an open stabilization performed. Forty-one patients were available for follow-up at an average of 6.5 years. Forty (98%) patients reported having a stable shoulder. Average loss of external rotation was 4 degrees and average Rowe score was 96. This study further supports the trustworthiness of an open anatomic subcapsulolabral reconstruction for recurrent anterior instability and provid

Quantitative assessment of classic anteroinferior bony Bankart lesions by radiography and computed tomography.

BACKGROUND: An anteroinferior osseous defect of the glenoid rim is sometimes encountered in patients with recurrent anterior dislocations of the shoulder. A defect of more than 21% of the glenoid length is reported to cause instability after Bankart repair. HYPOTHESIS: We can estimate the critical size of glenoid defects by using radiography or computed tomography. STUDY DESIGN: A controlled laboratory study. METHODS: Osseous defects of 0%, 9%, 21%, 34%, and 46% of the glenoid length were created stepwise in 12 cadaveric scapulae, and plain radiographs simulating the axillary and West Point

Clinical Outcomes of Adolescents With Anterior Shoulder Instability and Glenolabral Articular Disruption Lesions Compared With Isolated Bankart Lesions.

BACKGROUND: Glenolabral articular disruption (GLAD) lesions may occur in adolescents with anterior shoulder instability, resulting in articular cartilage loss and reduced functional glenoid surface area. PURPOSE/HYPOTHESIS: To compare failure rates and patient-reported outcomes (PROs) between adolescents with versus without GLAD lesions who were treated for anterior shoulder instability with arthroscopic stabilization. It was hypothesized that the comparison would yield no significant differences. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Patients aged ≤18 years who were tr

[The role of capsulolabral complex in Bankart lesion repair using Mitek anchors].

PURPOSE OF THE STUDY: The reconstructions of Bankart lesions, carried out according to the procedure recommended by the Mitek company, showed a high rate of recurrent dislocation. Therefore, based on operative experience, anatomical studies and reconstructive surgery on cadaver shoulders, we developed a new surgical technique that provided good shoulder stability and better operative outcomes. The results are reported here. MATERIAL: Between 1997 and 2005, arthroscopic Bankart repair with use of Mitek anchors was performed on 368 shoulders at the Department of Orthopedics, Faculty of Medicine…

Isolated arthroscopic Bankart repair vs. Bankart repair with "remplissage" for anterior shoulder instability with engaging Hill-Sachs lesion: A meta-analysis.

INTRODUCTION: Arthroscopic "remplissage" of a Hill-Sachs lesion (HSL) associated with a Bankart repair (BR) has been recently introduced as a surgical option to treat chronic anterior shoulder instability. The purpose of this study was to analyze the current literature comparing results of isolated BR versus BR+remplissage in the treatment of anterior shoulder instability with engaging HSL. HYPOTHESIS: BR+remplissage are superior to isolated BR in the management of anterior shoulder instability with engaging HSL. MATERIAL AND METHODS: Four electronic databases were searched for original, Eng

The Influence of Arthroscopic Remplissage for Engaging Hill-Sachs Lesions Combined with Bankart Repair on Redislocation and Shoulder Function Compared with Bankart Repair Alone.

BACKGROUND: Recurrence of glenohumeral dislocation after arthroscopic Bankart repair can be associated with a large osseous defect in the posterosuperior part of the humeral head. Our hypothesis is that remplissage is more effective to prevent recurrence of glenohumeral instability without a severe motion deficit. METHODS: Engaging Hill-Sachs lesions were observed in 48 of 737 patients (6.5%). Twenty-four patients underwent arthroscopic Bankart repair combined with remplissage (group I) and the other 24 patients underwent arthroscopic Bankart repair alone (group II). Clinical outcomes were pr

Chondral Damage After Arthroscopic Repair Techniques for Acute Bony Bankart Lesions: A Biomechanical Study.

BACKGROUND: Bony Bankart lesions can be encountered during treatment of shoulder instability. Current arthroscopic bony Bankart repair techniques involve intra-articular suture placement, but the effect of these repair techniques on the integrity of the humeral head articular surface warrants further investigation. PURPOSE: To quantify the degree of humeral head articular cartilage damage secondary to current arthroscopic bony Bankart repair techniques in a cadaveric model. STUDY DESIGN: Controlled laboratory study. METHODS: Testing was performed in 13 matched pairs of cadaveric glenoids wi…

Pull-out strength of suture anchors for rotator cuff and Bankart lesion repairs.

Surgical reconstructions of anterior-inferior shoulder instabilities and rotator cuff injuries require secure fixation of soft tissue to bon … h a metallic suture anchor. Our results indicate no significant differences in fixation strength of Bankart lesions or rotator cuff repairs using sutures only, or using wedge or rod polyacetal suture anchors (P = 0.70). Pull-out force did not differ significantly (P = 0.37) between the two polyacetal anchors. Polyacetal anchors exhibited higher pull-out forces th …

Arthroscopic repair with transosseous sling-suture technique for acute and chronic bony Bankart lesions.

BACKGROUND: Failure to fix the fractured fragment can result in bony fragment resorption and consequent glenoid bone loss. Current arthroscopic repair techniques might lead to insecure fixation and refracture. The purpose of this study was to evaluate the effectiveness of the transosseous sling-suture technique for bony Bankart lesions, and to compare the clinical outcomes for acute and chronic bony Bankart lesions treated with this technique. METHODS: A retrospective case series consisting of 46 patients with bony fracture of the glenoid rim following traumatic injury was identified from May…

Concomitant Coracoid Process Fracture with Bony Bankart Lesion Treated with the Latarjet Procedure.

Bony lesions of the glenoid and Hill-Sachs lesions are the most common injuries after a first-time traumatic shoulder dislocation. However, fracture of the coracoid process after traumatic shoulder dislocation is rare. A single, open surgical procedure could be performed by a Latarjet procedure using a fractured fragment of the coracoid process. If a fracture of the coracoid process is associated with a traumatic anterior shoulder dislocation, the Latarjet procedure may be the most appropriate surgical option.

Isolated HAGL lesion after arthroscopic Bankart repair in a professional soccer player.

Post-traumatic anterior shoulder instability commonly occurs following an avulsion of capsulolabral complex from glenoid (Bankart lesion) or rarely after humeral avulsion of the glenohumeral ligaments (HAGL lesion). Arthroscopic Bankart repair offers high success rates of healing. However, trauma following the treatment may cause implant failure or re-avulsion of the treated tissue. We aim to present the diagnosis and treatment of an isolated HAGL lesion in a professional soccer player who had previously undergone arthroscopic Bankart repair.
