[Barrett's esophagus in children].

Barrett's esophagus (BE) is a condition of esophageal dysplasia in which the tubular esophagus is lined with columnar instead of squamous mucosa--not with just any type of columnar mucosa, but with a specialized type with goblet cells. It is considered to be an acquired phenomenon secondary to acid exposure from gastro-esophageal reflux (GER). This report shows a review of BE of children and our data about BE from the study of 19 handicapped children with GER. 3 had intestinal dysplasia with goblet cells (BE). The % time of pH under 4 on 24-hour pH monitoring was significantly lower in the pat

Barrett's esophagus: an update.

Barrett's esophagus (BE) is a complication observed in a small subset of patients with chronic gastroesophageal reflux. It is characterized by the presence of intestinal-type goblet cells in biopsies from the lower esophagus. The prevalence of BE increases with age, affecting 1% of the population older than 60 years. A genetic predisposition to reflux disease has been proposed. In a twin study of reflux disease, concordance was greater in monozygotic than dizygotic twins. An association between BE and esophageal adenocarcinoma, the incidence of which has increased remarkably in the last few de

[Barrett's esophagus: overview].

Barrett's oesophagus (BE) is a segment of columnar lined epithelium in the distal oesophagus, above the gastrooesophageal junction. So it is important to localize this junction endoscopically; the proximal margin of the gastric folds is the anatomic landmark. Another important feature of BE is the specialized intestinal metaplasia. In the aetiology of this condition, acid reflux is a primary event but there is evidence that other factors are causal. As BE is a well known premalignant condition, detection of dysplastic epithelium and its severity is the crucial element. Practical guidelines are

Barrett's esophagus and achalasia.

Two unusual cases of achalasia with endoscopic and histologic documentation of Barrett's esophagus are presented. One patient had Barrett's esophagus at the time of initial endoscopy for achalasia, before any treatment. The other patient developed specialized columnar epithelia in the esophagus after treatment with pneumatic dilation. Each patient had evidence of low-grade dysplasia. Including these two patients, 30 cases of Barrett's esophagus in patients with achalasia have been reported in the literature. In 73% (22 of 30) of the cases, Barrett's esophagus was detected after esophagomyotomy

Karyometry in Barrett's esophagus.

OBJECTIVE: To derive a progression curve for lesions in Barrett's esophagus based on karyometric features. STUDY DESIGN: High-resolution imagery of 900 nuclei from normal gastric tissue, Barrett's metaplasia, Barrett's high grade dysplasia and adenocarcinoma of the esophagus was recorded. Karyometric features were computed, and nuclear signatures and lesion signatures for these lesions were derived. A progression curve was defined. RESULTS: Esophageal lesions were distinctly different from the normal gastric fundus tissue, with nuclei from Barrett's metaplasia deviating from normal almost as

[Barrett's esophagus associated to caustic stenosis of the esophagus].

BACKGROUND: The esophageal stenosis secondary to the ingestion of caustic products is frequent in Brazil, mainly due to an attempt suicide. The Barrett's esophagus is consequence of the chronic gastroesophageal reflux. The literature consulted showed that are a rare association. CASUISTIC AND METHODS: From 1981 to 2000 were admitted and treated in the "Gastrocentro" State University of Campinas, SP, Brazil, 120 patients presenting caustic stenosis of the esophagus, and during the follow-up were found nine cases with Barrett's esophagus associated (7,5%). The time of caustic ingestion varied f

Current management of Barrett esophagus and esophageal adenocarcinoma.

Barrett esophagus is found in 5% to 15% of patients with gastroesophageal reflux disease and is a precursor of esophageal adenocarcinoma, yet the condition often goes undiagnosed. Patients with reflux disease who are male, over age 50, or white, and who smoke or have central obesity or a family history of Barrett esophagus or esophageal adenocarcinoma, should undergo initial screening endoscopy and, if no dysplasia is noted, surveillance endoscopy every 3 to 5 years. Dysplasia is treated with endoscopic eradication by ablation, resection, or both. Chemoprotective agents are being studied to pr

[Barrett esophagus--esophageal carcinoma: conservative therapy and observation].

Chronic acid reflux is strongly associated with cancer of the esophagogastric junction and the main reason for the development of specialised intestinalised metaplasia in the esophagus (Barrett's esophagus). Endoscopic surveillance, therefore, is mandatory for long-segment Barrett's esophagus as well as short segment Barrett's. Videoendoscopy with four quadrant random biopsies are standard and new diagnostic tools like chromoendoscopy with methylene blue, magnifying endoscopy or fluorescence detection may turn out to be helpful. Differential surveillance strategies according to the recommendat

[Reflux disease and Barrett esophagus--monitoring and therapy].

Heartburn and epigastric pain are the leading symptoms of reflux disease. Next to other symptoms like pharyngeal burning, regurgitation and retrosternal pain, chronic hoarseness and coughing as well as angina pectoris symptoms may point towards a pathological reflux. In endoscopically verified reflux esophagitis proton pump inhibitors are the treatment of first choice. Aim of therapy is loss of symptoms, healing of epithelial defects and prevention of Barrett's esophagus. If a columnar epithelium-lined esophagus is seen, surveillance is recommended in one- or two-year intervals.

[Barrett++ esophagus in children. Presentation of 12 cases].

This paper describes 12 cases of Barrett's oesophagus selected out of a population of 364 refluxing children studied from 1986 to 1990. Their mean age was 14, seven were cared for in special institutions for brain damaged and 2 had been operated upon for oesophageal atresia at birth. Excessive acid exposure was documented in 8/10 patient studied and simultaneous gastric and oesophageal pH-monitoring demonstrated massive duodeno-gastric alkaline reflux in 3. Histological studies revealed that columnar-lined epithelium was of fundic type in 2 cases, of junctional type in 7 and of specialized typ

[A radio-endoscopic case: Barrett esophagus in children].

The authors report a case of Barrett's oesophagus in a child. This observation is characterized by the extent of the lesion which is of three types of metaplasic epithelium usually seen in this disease (fundic, cardial and intestinal tissue); the difficulties in differential diagnosis from uncomplicated hiatal hernia justify radiological and endoscopical examinations with multiple biopsies. The discussion centers on the risk of malignant transformation with its therapeutical consequences and the long term follow-up of such patients.

[Organisms of the Campylobacter type in Barrett esophagus].

It is clear that CP is present in a higher or lower degree in different gastric-duodenum pathologies, especially in active superficial chronic gastritis, gastric ulcer and duodenum ulcer with gastric metaplasia. It is also found in atrophic chronic gastritis and, to a lesser extent, if it has intestinal metaplasia, as well as in some normal stomachs. It is not found in a histologically normal duodenum, nor in the oesophagus. As the fact that there was no publication on BE drew our attention, we set to make a retrospective research of CP of spinal metaplasia of distal oesophagus. Its presence p

Adenocarcinoma of the esophagus: relationship to Barrett mucosa.

Primary adenocarcinoma of the esophagus is thought to be a rare lesion. However, pathologic records at our institution from 1979 to 1982 show that adenocarcinoma accounted for 17 of 89 (19%) of all primary malignant tumors in the esophagus. All 17 cases arose in Barrett mucosa. In eight cases, there was surgically proved involvement of the gastric cardia or fundus. In six of these cases, there was evidence of esophageal dysplasia and/or carcinoma in situ adjacent to or remote from the proximal margin of the tumor. The pathologic findings therefore strongly suggest an esophageal origin of these

Recent advances in Barrett's esophagus: short-segment Barrett's esophagus and cardia intestinal metaplasia.

The recent rapid increase in the incidence of adenocarcinoma of the distal esophagus and the gastric cardia has generated significant interest in the premalignant lesion, Barrett's esophagus. The traditional definition of Barrett's esophagus included the presence of 3 cm or greater of columnar mucosa in the distal esophagus. Studies have clarified that intestinal metaplasia was not only the most common and distinctive type of epithelium detected within the columnar mucosa, but also the one with greatest malignant potential; therefore, Barrett's esophagus has come to be defined by the histologi

Does this patient have Barrett's esophagus? The utility of predicting Barrett's esophagus at the index endoscopy.

OBJECTIVES: Few studies have evaluated the ability of the endoscopist to predict the presence of Barrett's esophagus (BE) at index endoscopy. The goals of this study were to determine the operating characteristics of endoscopy in diagnosing BE, and to determine the clinical and endoscopic predictors of BE in suspected BE patients at the index endoscopy. METHODS: From September 1993 to October 1997, endoscopic reports were examined to identify patients with suspected BE. All esophageal pathology reports during the same period were evaluated for the presence of specialized intestinal metaplasia

Advances in the Diagnosis of Barrett's Esophagus.

Barrett's esophagus (BE), the intestinal metaplasia arising from chronic gastroesophageal reflux disease, is the principal identifiable precursor of esophageal adenocarcinoma (EAC), whose incidence rose from 0.4 to 2.8/100,000 person-years between 1975 and 2017 and whose prognosis, once symptomatic, remains poor. Because outcomes depend on intercepting the metaplasia-dysplasia-carcinoma sequence, diagnostic accuracy is decisive. White-light endoscopy with Seattle-protocol biopsy remains the reference standard, yet it is constrained by the following three interrelated weaknesses: sampling error

Endoscopic Screening of Barrett's Esophagus.

Endoscopic Barrett's esophagus (BE) screening detects premalignant changes, enabling the detection of BE and endoscopic surveillance to detect dysplasia/esophageal adenocarcinoma (EAC). Endoscopic therapy for BE dysplasia reduces EAC risk and can treat early stage EAC effectively. Screening is advised for those with multiple risk factors: chronic reflux, age over 50 years, male sex, Caucasian race, smoking, central obesity, hiatal hernia, or family history. Risk prediction tools are evolving. High-definition endoscopy with biopsies enables detection; precise landmark identification is key.

Barrett's Esophagus: An Updated Review.

Barrett’s esophagus (BE) is a change in the distal esophageal mucosal lining, whereby metaplastic columnar epithelium replaces squamous epithelium of the esophagus. This change represents a pre-malignant mucosal transformation which has a known association with the development of esophageal adenocarcinoma. Gastroesophageal reflux disease is a risk factor for BE, other risk factors include patients who are Caucasian, age > 50 years, central obesity, tobacco use, history of peptic stricture and erosive gastritis. Screening for BE remains selective based on risk factors, a screening program in th

Endoscopic diagnosis of Barrett's esophagus.

The Prague C and M Criteria have been developed for the objective endoscopic diagnosis of Barrett's esophagus (BE). BE arises between the squamocolumnar junction and the gastroesophageal junction at the proximal margin of the gastric folds. In this study, we reported that 43.0% of the subjects examined were diagnosed with BE based on the Prague C and M Criteria. Previous criticism by John Dent proposed that our data should be considered invalid because the prevalence of BE reported in our study was extraordinarily high and discordant with previous studies. Dent predicted that the position of t

Screening and surveillance of Barrett's esophagus.

Esophageal adenocarcinoma (EAC) is the most rapidly increasing cancer in the Western world and Barrett's esophagus (BE) is the only known precursor lesion for this lethal cancer. Long-term survival may be improved if EAC is diagnosed early, providing an opportunity for early intervention. Screening for BE in patients with gastroesophageal reflux disease is not routinely recommended; however, if diagnosed, enrollment into a surveillance program may be beneficial. Surveillance of all patients with known BE is probably not cost-effective and factors predictive of BE progression to dysplasia/EAC a
