Factors associated with death in bedridden patients in China: A longitudinal study.

BACKGROUND: Immobility is common and associated with adverse outcomes in hospitalized patients, especially older people. However, the factors contributing to mortality in bedridden patients are not well known. This study aimed to estimate short-term mortality and analyze risk factors that affect the prognosis of bedridden patients. METHODS: This was a multicenter study in China involving 23,738 patients admitted to 25 hospitals between November 2015 and June 2016. All-cause mortality was recorded for 90 days after enrollment regardless of whether death occurred before or after discharge. Soci

Case report of a woman with anti amphiphysin positive stiff person syndrome.

Stiff person syndrome is a rare neuroimmunological disease, characterized by severe, involuntary stiffness with superimposed painful muscle spasms, which are worsened by external stimuli. The classical form is associated with high levels of antibodies against glutamic acid decarboxylase. One of the variant forms is associated with antibodies against amphiphysin. This entity is a paraneoplastic syndrome, caused primarily by breast cancer, secondarily by lung cancer. Symptomatic therapy of anti amphiphysin positive stiff person syndrome includes treatment with benzodiazepines and baclofen (inclu

Clinical Expectations for Better Influenza Virus Vaccines-Perspectives from the Young Investigators' Point of View.

The influenza virus is one of a few viruses that is capable of rendering an otherwise healthy person acutly bedridden for several days. This impressive knock-out effect, without prodromal symptoms, challenges our immune system. The influenza virus undergoes continuous mutations, escaping our pre-existing immunity and causing epidemics, and its segmented genome is subject to reassortment, resulting in novel viruses with pandemic potential. The personal and socieoeconomic burden from influenza is high. Vaccination is the most cost-effective countermeasure, with several vaccines that are availabl

Hypercoagulable states and venous thrombosis.

Many factors predispose patients to thromboembolic disease. A young person presenting with idiopathic deep venous thrombosis may never have its etiology elucidated, despite exhaustive testing. On the other hand, hypercoagulability is no mystery in an obese, bedridden, postoperative patient with a malignancy. Invasive or noninvasive testing should be performed in all suspicious cases. Patients with positive results should be treated promptly; those with negative findings should not be subjected to anticoagulation. The length of anticoagulation depends on the length of time the patient remains a

Incidence and Predictors of Mortality Among Adults on Antiretroviral Therapy in Ethiopia: A Systematic Review and Meta-Analysis.

BACKGROUND: Despite the United Nations' plan to end Human Immunodeficiency Virus (HIV) as a public health concern by 2030, it remains a significant issue in low-income countries, including Ethiopia. Currently, there are no pooled estimates of mortality rate among adults on antiretroviral therapy in Ethiopia. This meta-analysis was conducted to fill this information gap, focusing on the incidence and predictors of mortality among adults receiving antiretroviral therapy in Ethiopia. METHODS: This systematic review followed the PRISMA guidelines, and relevant studies were obtained from the PubMe

Surgical treatment and strategy in patients with pressure sores: A single-surgeon experience.

With aging, pressure ulcers become a common health problem causing significant morbidity and mortality for physically limited or bedridden elderly persons. Here, we present our strategy for such patients. Between August 2010 and March 2019, 117 patients were enrolled. Patient age, etiology, defect size and location, flap reconstruction, outcome, and follow-up period were reviewed. Of these patients, 64 were female and 53 were male, with an age range of 21 to 96 years (mean 75.6). The mean area of defect was 61.5 cm. The most common etiology was dementia (33.3%), and ulcers were most frequently

Preliminary Detection of Acute Exacerbation of Lobar Pneumonia and Heart Failure Using an Anomaly-Detection System Based on a Circadian Rhythm Model Constructed from Non-contact Vital Data.

Many frail older patients with multiple comorbidities are hospitalized in long-term care hospitals and nursing facilities. Due to pre-existing conditions and immunosuppressive states, there are significant individual differences, such as weakness, sluggishness, and asymptomatic status. These differences make it challenging to find a patient's exacerbation using a conventional threshold of vital signs. We developed a circadian rhythm anomaly-detection system designed for each patient, which compares each patient's past 2 weeks average respiratory rate circadian rhythm with that of the last 24 h

Incidence and predictors of Tuberculosis among patients enrolled in Anti-Retroviral Therapy after universal test and treat program, Addis Ababa, Ethiopia. A retrospective follow -up study.

INTRODUCTION: Tuberculosis (TB) is the leading killer of people living with HIV (PLHIV) and almost one-third of deaths in the world are attributed to it and many of these deaths occur in developing countries. Despite these evidences, after the implementation of universal test and treat (UTT) strategy, information regarding the incidence and predictors of tuberculosis among PLHIV is limited in Ethiopia. Therefore, this study aimed to assess the incidence and predictors of tuberculosis among patients enrolled in Anti-Retroviral Therapy (ART) after universal test and treat program at St. Peter ho

Association of serum albumin level, feeding route, and mobility status with pressure ulcer presence in bedridden adults receiving long-term care.

BACKGROUND: Pressure ulcers are a largely preventable complication and an important patient-safety concern in community and home-care settings. Evidence from primary care–led home-care cohorts remains limited, particularly regarding how routine functional and laboratory markers relate to pressure ulcer presence. We examined associations between mobility, nutritional indicators, feeding route, and inflammatory markers—including serum albumin and the C-reactive protein (CRP)–to–albumin ratio (CAR)—and pressure ulcer presence in adults receiving home-care services. METHODS: This retrospective, cr
