[Best's disease. Overview of pathology and its causes].

Best's disease is a hereditary affection with reduced penetrance and juvenile onset. The fundus may be unremarkable or present various stages up to scarring of the macula. Histopathology of advanced stages discloses deposits of lipofuscin on Bruch's membrane, the innermost layer of which is the basal membrane of the retinal pigment epithelium (RPE). The deposits correlate with lipofuscin and melanofuscin granulae in the RPE. Photoreceptors correlating with the lesions have lost their outer segments and the RPE as well as the photoreceptors appear edematous. Loss of photoreceptor function does

Exploring the homolog of a novel proinflammatory microfilarial sheath protein (MfP) of Wuchereria bancrofti in the adult-stage bovine filarial parasite Setaria cervi.

A novel microfilarial sheath protein (MfP) of the human filarial parasite Wuchereria bancrofti and its proinflammatory activity on host macrophages were identified recently. MfP is a homolog of the nematode bestrophin-9 superfamily that acts as a ligand of macrophage Toll-like receptor 4 (TLR4) to induce inflammation through NF-κB activation. Therefore, the presence and functional implication of this novel protein in adult-stage parasites were open questions to answer. In this study, the bovine filarial parasite Setaria cervi was used to simulate adult W. bancrofti. We detected the presence of

Expression of three distinct families of calcium-activated chloride channel genes in the mouse dorsal root ganglion.

Objective A calcium-activated chloride current (IClCa) has been observed in medium-sized sensory neurons of the dorsal root ganglion (DRG). Axotomy of the sciatic nerve induces a similar current in the majority of medium and large diameter neurons. Our aim is to identify the molecule(s) underlying this current. Methods Using conventional and quantitative RT-PCR, we examined the expression in DRG of members of three families of genes, which have been shown to have IClCa current inducing properties. Results We showed the detection of transcripts representing several members of these families, i.

The short wavelength electro-oculogram (SW-EOG) in best disease: preliminary results.

PURPOSE: The clinical electrooculogram (EOG) is used for the diagnosis of bestrophinopathies to evaluate the function of the retinal pigment epithelium. The current ISCEV test protocol uses a broad band white light with luminance of 100 cd/m2. An alternative monochromatic 448 nm short wavelength (SW-EOG) has been proposed but to date has not been evaluated in clinical cases where the standard EOG is abnormal. METHODS: To evaluate the clinical potential of the SW-EOG four genetically confirmed cases of Best Vitelliform Macular Dystrophy were tested using the standard white (100 cd/m2) and SW-E

Review of the clinical electrooculogram - Part 2: the bestrophinopathies and modified protocols.

The light-rise of the electro-oculogram (EOG) is used as a clinical marker for a collection of disorders known as the 'bestrophinopathies.' This review provides an overview of these conditions including Best Vitelliform Macular Dystrophy (BVMD, Autosomal Recessive Bestrophinopathy (ARB), Adult Onset Vitelliform Macular Dystrophy (AVMD) and Autosomal Dominant Vitreoretinalchoriodopathy (ADVIRC) and potential future therapies. One drawback of the EOG is the time to administer the test and shortened protocols that have been developed to improve the clinical testing of the EOG which include incorp

Bilateral consecutive choroidal neovascularization in Best vitelliform macular dystrophy.

Best vitelliform macular dystrophy (BVMD) is a slowly progressive macular disease caused by a pathogenic variant of the Bestrophin (BEST1) gene. Examination coupled with multimodal imaging and genetic testing are used to guide diagnosis and treatment. A 12-year-old girl was examined for decreased vision in the left eye and showed bilateral "egg-yolk"-like macular lesions with choroidal neovascularization (CNV) in the left eye. Six months later, she experienced decreased vision with appearance of CNV on optical coherence tomography angiography in the right eye. Injections of anti-vascular endot

Detection of Novel BEST1 Variations in Autosomal Recessive Bestrophinopathy Using Third-generation Sequencing.

OBJECTIVE: Autosomal recessive bestrophinopathy (ARB), a retinal degenerative disease, is characterized by central visual loss, yellowish multifocal diffuse subretinal deposits, and a dramatic decrease in the light peak on electrooculogram. The potential pathogenic mechanism involves mutations in the BEST1 gene, which encodes Ca2+-activated Cl- channels in the retinal pigment epithelium (RPE), resulting in degeneration of RPE and photoreceptor. In this study, the complete clinical characteristics of two Chinese ARB families were summarized. METHODS: Pacific Biosciences (PacBio) single-molecul

Genetic treatment for autosomal dominant inherited retinal dystrophies: approaches, challenges and targeted genotypes.

Inherited retinal diseases (IRDs) have been in the front line of gene therapy development for the last decade, providing a useful platform to test novel therapeutic approaches. More than 40 clinical trials have been completed or are ongoing, tackling autosomal recessive and X-linked conditions, mostly through adeno-associated viral vector delivery of a normal copy of the disease-causing gene. However, only recently has autosomal dominant (ad) disease been targeted, with the commencement of a trial for rhodopsin (RHO)-associated retinitis pigmentosa (RP), implementing antisense oligonucleotide

Gene therapy induces pigmentation in induced pluripotent stem cell-derived retinal pigment epithelium of OCA1A patients and albino rat eye.

Oculocutaneous albinism type 1A (OCA1A), the most severe and prevalent form of albinism, is caused by recessive mutations in the TYROSINASE gene. We provide proof-of-principle of an effective gene therapy-based strategy for future human application in OCA1A patients. Here, we show that an AAV-based TYROSINASE gene delivery under the control of human BESTROPHIN promoter VMD2 can successfully induce pigmentation in human retinal pigment epithelium (RPE) monolayer tissue, derived from OCA1A patients, via induced pluripotent stem cells. Pigmentation was induced without significantly compromising t

Ginsenoside Rb1, a novel activator of the TMEM16A chloride channel, augments the contraction of guinea pig ileum.

Calcium-activated chloride channels (CaCCs) play important roles in many physiological processes, and the molecular basis of CaCCs has been identified as TMEM16A in many cell types. It is well established that TMEM16A is a drug target in many diseases, including cystic fibrosis, hypertension, asthma, and various tumors. Therefore, identifying potent and specific modulators of the TMEM16A channel is crucial. In this study, we identified the first natural activator of TMEM16A from traditional Chinese medicine and explored its mechanism. Our data showed that Ginsenoside Rb1 (GRb1) can activate TM

Inhibition of Ca2+-activated chloride channels by the NSAID meclofenamate for anti-diarrhea.

Dysfunctional calcium-activated chloride channels (CaCCs) are implicated in many pathological phenotypes and diseases. The CaCC ANO1/TMEM16A robustly expressed in epithelial cells plays an essential role in regulation of Cl- secretion and intestinal motility. In this study, we investigated the effects of a nonsteroidal anti-inflammatory drug (NSAID) meclofenamate on ANO1 channel and dextran sulfate sodium (DSS)-induced diarrhea in mice. Meclofenamate inhibits CaCC ANO1 channel in a concentration-dependent manner with an IC50 of 16.2 ± 2.7 μM. Meclofenamate also reduces single-channel open prob

Astrocytes Coordinate Neurovascular Coupling During Prolonged Sensory Demand via AQP4/TRPA1/BEST1/D-Serine Signaling.

Astrocytes are recognized as key components of neurovascular coupling (NVC), yet whether and how astrocytic engagement in NVC depends on sensory stimulus duration remains unclear. Here, we investigated a duration-dependent astrocytic mechanism underlying cerebral blood volume (CBV) modulation by comparing neurovascular responses to brief (5 s) and prolonged (20 s) sensory stimulation, using in vivo intrinsic optical signal (IOS) imaging and two-photon microscopy combined with electrophysiology and vascular cannulation. We show that prolonged stimulation selectively recruits an integrated astro

Gain of function in BEST1: photoreceptor changes and myopia in autosomal dominant vitreoretinochoroidopathy.

PURPOSE: Autosomal dominant vitreoretinochoroidopathy (ADVIRC) is associated with gain-of-function in bestrophin-1 instead of loss-of-function or dominant-negative mechanisms in other bestrophinopathies. We aimed to characterize photoreceptor (PR) changes in ADVIRC compared to Best vitelliform macular dystrophy (BVMD) and controls. METHODS: A retrospective case series of 8 ADVIRC, 10 BVMD, and 10 control eyes was conducted. Outer segment and PR layer (OPL/ONL junction to RPE) thicknesses were measured on horizontal optical-coherence-tomography line scans at -2.5 (nasal) to +2.5 (temporal) mm

Generation of the induced pluripotent stem cell line SJTUGHi004-A derived from a Best's disease patient with c.763C > T mutation in BEST1 gene.

Best's disease (BD), referred to as Best vitelliform macular dystrophy, is an autosomal dominant macular degeneration caused by mutations in the BESTROPHIN1 (BEST1) gene. Here, we generated an induced pluripotent stem cell (iPSC) line by the non-integrative Sendai-virus delivery system derived from peripheral blood mononuclear cells (PBMCs) of a patient with c.763C > T mutation in the BEST1 gene. The iPSC line was characterized and validated for the karyotype stability, pluripotency markers, and differentiation potential in vitro, which may serve as a powerful model for exploring the pathologi

Photoreceptor function and structure in retinal degenerations caused by biallelic BEST1 mutations.

The only approved retinal gene therapy is for biallelic RPE65 mutations which cause a recessive retinopathy with a primary molecular defect located at the retinal pigment epithelium (RPE). For a distinct recessive RPE disease caused by biallelic BEST1 mutations, a pre-clinical proof-of-concept for gene therapy has been demonstrated in canine eyes. The current study was undertaken to consider potential outcome measures for a BEST1 clinical trial in patients demonstrating a classic autosomal recessive bestrophinopathy (ARB) phenotype. Spatial distribution of retinal structure showed a wide expan
