Bilateral Vestibulopathy: Vestibular Function, Dynamic Visual Acuity and Functional Impact.

Introduction: Patients suffering from bilateral vestibular hypofunction (BVH) often experience ataxia as well as visual instability. Even though progress has been made in vestibular testing, insights regarding vestibular deficit in BVH remain incomplete since no method allows evaluation of frequency ranges of vestibular sensors in a continuous way. The aim of our study was to give a detailed description of the level of vestibular deficit in different ranges of vestibular stimulation and an exhaustive evaluation of the functional impact including dynamic visual acuity (DVA) in a cohort of BVH p

Bilateral vestibulopathy with dissociated deficits in the superior and inferior vestibular systems.

OBJECTIVES: The aim of this study was to reveal the clinical characteristics of dissociated bilateral vestibulopathy (BV) affecting the superior vestibular nerve system on one side and the inferior vestibular nerve system on the other side. It is probable that BV does not always show bilateral deficits of the same vestibular nerve system. METHODS: In a retrospective study of 2,752 consecutive new patients, 1,560 patients had undergone both caloric testing and cervical vestibular evoked myogenic potential testing. All 1,560 patients had also undergone a battery of tests including standardized

Patterns of Vestibular Impairment in Bilateral Vestibulopathy and Its Relation to Etiology.

OBJECTIVE: This study aimed to investigate (1) the patterns of vestibular impairment in bilateral vestibulopathy (BVP) and subsequently, the implications regarding patient eligibility for vestibular implantation, and (2) whether this pattern and severity of vestibular impairment is etiology dependent. METHODS: A total of one hundred and seventy-three subjects from three tertiary referral centers in Europe were diagnosed with BVP according to the Bárány Society diagnostic criteria. The subjects underwent vestibular testing such as the caloric test, torsion swing test, video Head Impulse Test (

Impact of Bilateral Vestibulopathy on Spatial and Nonspatial Cognition: A Systematic Review.

OBJECTIVES: Hearing loss is considered an independent risk factor for dementia. Growing evidence in animal and human studies suggest that not only hearing loss but also vestibular loss might result in cognitive deficits. The objective of this study is to evaluate the presence of spatial and nonspatial cognitive deficits in patients with bilateral vestibulopathy. As different causes of bilateral vestibulopathy are associated with hearing loss, the objective is to evaluate if these cognitive deficits are due to the vestibular loss of the patients with bilateral vestibulopathy, or to their hearin

Disease-specific sparing of the anterior semicircular canals in bilateral vestibulopathy.

OBJECTIVE: Bilateral vestibular loss (BVL) is often diagnosed with great delay and an underlying cause is only identified in 50-80%. We measured horizontal and vertical semicircular canal function using the video-head-impulse test (vHIT) and hypothesized that specific vHIT-patterns may be linked to certain etiologies. METHODS: We retrospectively analyzed 109 BVL-patients linked to aminoglycoside vestibulotoxicity (n=16), Menière's disease (n=10), infectious inner-ear disorders (n=11), sensorineural hearing-loss (n=11), cerebellar-ataxia-neuropathy-vestibular-areflexia-syndrome (CANVAS, n=5),

An unusual case of bilateral vestibulopathy, chronic subjective dizziness and spondyloarthropathy.

OBJECTIVE: Vertigo and dizziness are two common symptoms seen in everyday practice. However, in some cases, making a diagnosis can be challenging. This case report shows the relevance of a careful psychiatric history, which led to the diagnosis of chronic subjective dizziness associated with bilateral peripheral vestibulopathy. METHODS: A case of a 33-year-old lady with multiple episodes of vertigo attack after a minor head injury is reported. RESULTS: A comprehensive audiometric and vestibular evaluation reviewed bilateral peripheral vestibulopathy. The frequency of the vertigo attacks decr

Noisy galvanic vestibular stimulation: an emerging treatment option for bilateral vestibulopathy.

Patients with bilateral vestibulopathy (BVP) suffer from persistent imbalance during standing and walking as well as an impaired gaze stabilization during head movements. Disabilities associated with BVP severely compromise patients' daily activities and are often linked to an increased risk of falls. Currently, the only established treatment option in BVP is physical therapy. However, treatment effects of physical therapy in BVP are most often limited and many patients do not adequately recover performance. Therefore, a number of technical therapeutic approaches are being explored that either

Bilateral Vestibulopathy Documented by Video Head Impulse Tests in Superficial Siderosis.

OBJECTIVE: Superficial siderosis (SS) of the CNS is associated with cerebellar ataxia, sensorineural hearing loss, and pyramidal symptoms, which result from iron depositions on CNS surfaces. SS can produce bilateral vestibulopathy as the vestibulo-cochlear nerve is particularly vulnerable. To our knowledge, however, vestibular dysfunction in SS has not been reported thoroughly in the literature. Here, we describe a case of bilateral vestibulopathy, documented quantitatively by the video head impulse test (vHIT), in a patient with SS. PATIENT: A 60-year-old man presented with slowly progressin

Serum antibodies against membranous labyrinth in patients with "idiopathic" bilateral vestibulopathy.

To investigate the possibility of an autoimmune mechanism in idiopathic bilateral vestibulopathy (IBV), we screened patients' sera for antibodies against inner ear structures. IgG antibodies against membranous labyrinth (ampulla, semicircular canals, saccule and utricle) were detected in 8 of 12 patients by immunofluorescence on rat inner ear cryosections. All but one serum of 22 healthy controls and the sera of 6 patients with known autoimmune disorders showed only background staining. Low-titre anti-nuclear IgM antibodies were present in three control sera and one IBV serum. High-titre anti-

Dynamic visual acuity in bilateral vestibulopathy and healthy age-sex-matched participants.

OBJECTIVES: Dynamic visual acuity (DVA) can be assessed on a treadmill while walking at different speeds and is used to assess people with bilateral vestibulopathy (BVP). However, the effects and interactions of age, BVP, and walking speed on DVA loss and assessment dropout are unclear. Our objective was to investigate the effects of BVP, age, and walking speed on DVA loss and assessment dropout in participants with BVP and healthy age-sex-matched participants. METHODS: 41 participants with BVP and 41 age-sex-matched healthy participants completed a treadmill-based DVA assessment, including a

Multi-frequency VEMPs improve detection of present otolith responses in bilateral vestibulopathy.

OBJECTIVE: To investigate whether multi-frequency Vestibular Evoked Myogenic Potential (VEMP) testing at 500, 750, 1,000, and 2,000 Hz, would improve the detection of present dynamic otolith responses in patients with bilateral vestibulopathy (BV). METHODS: Prospective study in a tertiary referral center. BV patients underwent multi-frequency VEMP testing. Cervical VEMPs and ocular VEMPs were recorded with the Neuro-Audio system (v2010, Neurosoft, Ivanovo, Russia). The stimuli included air-conducted tone bursts of 500, 750, 1,000, and 2,000 Hz, at a stimulation rate of 13 Hz. Outcome measures

Hierarchical Cluster Analysis of Semicircular Canal and Otolith Deficits in Bilateral Vestibulopathy.

BACKGROUND: Gait imbalance and oscillopsia are frequent complaints of bilateral vestibular loss (BLV). Video-head-impulse testing (vHIT) of all six semicircular canals (SCCs) has demonstrated varying involvement of the different canals. Sparing of anterior-canal function has been linked to aminoglycoside-related vestibulopathy and Menière's disease. We hypothesized that utricular and saccular impairment [assessed by vestibular-evoked myogenic potentials (VEMPs)] may be disease-specific also, possibly facilitating the differential diagnosis. METHODS: We searched our vHIT database (n = 3,271) f

Noisy vestibular stimulation increases gait speed in normals and in bilateral vestibulopathy.

BACKGROUND: Galvanic vestibular stimulation delivered as zero-mean current noise (noisy GVS) has been shown to improve static and dynamic postural stability probably by enhancing vestibular information. OBJECTIVE: /Hypothesis: To examine the effect of an imperceptible level of noisy GVS on dynamic locomotion in normal subjects as well as in patients with bilateral vestibulopathy. METHODS: Walking performance of 19 healthy subjects and 12 patients with bilateral vestibulopathy at their preferred speed was examined during application of noisy GVS with an amplitude ranging from 0 to 1000 μA. Bilateral vestibulopathy causes selective deficits in recombining novel routes in real space.

The differential impact of complete and incomplete bilateral vestibulopathy (BVP) on spatial orientation, visual exploration, and navigation-induced brain network activations is still under debate. In this study, 14 BVP patients (6 complete, 8 incomplete) and 14 age-matched healthy controls performed a navigation task requiring them to retrace familiar routes and recombine novel routes to find five items in real space. [18F]-fluorodeoxyglucose-PET was used to determine navigation-induced brain activations. Participants wore a gaze-controlled, head-fixed camera that recorded their visual explor

Mitochondrial 12S rRNA susceptibility mutations in aminoglycoside-associated and idiopathic bilateral vestibulopathy.

The mitochondrial 12S rRNA is considered a hotspot for mutations associated with nonsyndromic (NSHL) and aminoglycoside-induced hearing loss (AIHL). Although aminoglycoside ototoxicity is the most common cause of bilateral vestibular dysfunction, the conceivable role of 12S rRNA mutations has never been systematically investigated. We sequenced the 12S rRNA of 66 patients with bilateral vestibulopathy (BV) with (n=15) or without (n=51) prior exposure to aminoglycosides, as well as 155 healthy controls with intact vestibular function (sport pilots), and compared these to 2704 published sequence

Etiologies and hearing status in bilateral vestibulopathy: a retrospective study of 315 patients.

IMPORTANCE: The development of a vestibular implant has reached milestones and seems to be a promising therapeutic tool for bilateral vestibulopathy (BV). Given the former lack of therapeutic options for BV, the disease has received scant attention in the previous research literature. It is therefore of major importance to gain more insight into the underlying pathology of BV. Furthermore, as some research groups specifically use a combined vestibulo-cochlear implant, the size of the group of BV patients with associated hearing loss is of special interest. OBJECTIVES: The study aimed to deter

Multimodal MRI of the reorganization of multisensory and sensorimotor networks in chronic bilateral vestibulopathy.

Chronic bilateral vestibulopathy (BVP) occurs preferably in elderly patients presenting with postural imbalance and head movement induced oscillopsia. The condition is often incomplete with residual functions in both ears. Beyond the vestibular reflexive deficits, an impairment of spatial orientation and navigation has been described associated with an atrophy of the hippocampal formation. However, this finding was inconsistent in various studies on rodents and humans. In the current MRI study on 15 BVP patients and 15 healthy controls (HC) we combined analyses of whole brain voxel-based morph

Association of Bilateral Vestibulopathy With and Without Hearing Loss With Cognitive-Motor Interference.

IMPORTANCE: The past years, evidence suggested that the primary symptoms traditionally associated with bilateral vestibulopathy (BV) do not represent the full picture of this patient population. Recent literature also demonstrated cognitive impairment. However, although multitasking and dual-tasking are widely present in everyday activities, most of these studies assessed cognitive function only in single-task conditions. OBJECTIVE: To uncover the association of BV with and without hearing loss with cognitive and motor performance and cognitive-motor interference. DESIGN, SETTING, AND PARTIC
