OBJECTIVE: To evaluate the use of the unreamed femoral nail with spiral blade (UFN-SB) in the treatment of subtrochanteric femoral fractures.
DESIGN: A retrospective review of a consecutive series of 55 fractures. Fourteen patients had metastatic disease (four had prophylactic nailing).
RESULTS: In five fractures, the UFN-SB failed: there was migration in three cases and breakage of the spiral blade in two cases, with breakage of the nail in two cases. Revision surgery was necessary in four cases. Five out of seven complications related to the spiral blade were seen in patients with a Seinsh

Trochanteric (intertrochanteric) hip fractures are among the most common fragility fractures in older adults. Cephalomedullary nailing (CMN) has become the standard treatment strategy for unstable fracture patterns (AO/OTA 31-A2/A3), largely replacing the sliding hip screw (dynamic hip screw, DHS) for this indication. Two proximal fixation designs are in widespread use within CMN systems: the traditional lag screw, which relies on thread engagement in cancellous bone, and the helical blade, which is inserted by impaction, compacting surrounding cancellous bone rather than removing it. This nar

INTRODUCTION: Compressive trabecular bone plays a vital role in load transmission within the proximal femur, and regional variations in cancellous bone distribution have been shown to affect implant stability. However, the biomechanical influence of the spatial relationship between compressive trabecular bone and cephalocervical implants on postoperative fracture stability in intertrochanteric femoral fractures remains unclear.
METHODS: In this study, we conducted a retrospective analysis of 64 patients treated with proximal femoral nail antirotation (PFNA) Lever arm parameters reflecting the

Conventional osteosynthesis of proximal femur fractures is still affected by serious complication rates between 4-18%, even though advanced implant modifications and surgical techniques are common practice. In terms of increasing age and co-morbidity of patients this complication ratio is expected to increase even further in the immediate future. One major reason for implant failure is the decreasing stability potential of the implant due to a loss in mechanical properties of cancellous bone. Therefore, efforts in new intramedulary techniques specifically focus on the load bearing characterist

OBJECTIVES: The angled blade plate has been the historical standard in fixed-angle extramedullary subtrochanteric femur fracture fixation, but it requires an extensile lateral approach to the femur. Little formal evaluation exists for specifically designed percutaneous extramedullary implants. The purpose of this study was to compare 3 locked plating constructs, all with percutaneous insertion capability, with the standard 95-degree angled blade plate to determine whether specifically designed fixed-angle extramedullary implants for subtrochanteric femur fractures were biomechanically comparab

The avian scapula is a long bone located dorsally on the thorax. The cranial part that articulates with the upper limb is derived from the somatopleure of the forelimb field, while the caudal part, the scapula blade, originates from the dermomyotomes of brachial and thoracic somites. In previous studies, we have shown that scapula blade formation is intrinsically controlled by segment-specific information as well as extrinsically by ectoderm-derived signals. Here, we addressed the role of signals derived from the lateral plate mesoderm on scapula development. Chick-quail chimera experiments re

BACKGROUND: The blade-plate is the earliest of the contemporary internal fixation devices introduced for distal femoral fractures. The recent development of dedicated, fixation devices has considerably limited its use. The objective of this study was to evaluate outcomes after blade-plate fixation and after fixation using other devices.
HYPOTHESIS: Outcomes after blade-plate fixation are similar to those after condylar screw-plate, distal femoral nail, or locking condylar plate fixation.
MATERIAL AND METHODS: We reviewed outcomes after 62 patients managed with blade-plate fixation and includ

The problems experienced by most patients with trochanteric fractures are caused by their advanced age, particularly multimorbidity and osteoporosis, and due to this early postoperative functional treatment is only possible with full weight-bearing ability. To avoid perforation of the femoral head by overstress and collapse of the weak cancellous bone, any stabilizing device should be designed with an H-beamed blade, providing a large load-bearing surface. In our experience the 130 degrees-Double-T-blade-Plate is an effective implant for the treatment of pertrochanteric fractures, offering hig

BACKGROUND: Treatment of proximal humerus fractures with locking plates is associated with complications. We aimed to compare the biomechanical effects of removing screws and blade of a fixed angle locking plate and hybrid blade plate, on a two-part fracture model.
METHODS: Forty-five synthetic humeri were divided into nine groups where four were implanted with a hybrid blade plate and the remaining with locking plate, to treat a two-part surgical neck fracture. Plates' head screws and blades were divided into zones based on their distance from fracture site. Two groups acted as a control for

INTRODUCTION: Osteosynthesis of pertrochanteric fractures is one of the most frequently performed procedures in orthopaedic trauma care. Auto-dynamization during fracture healing can lead to lateralization of the screw or blade. This can cause debilitating complaints, which sometimes necessitate the removal of the screw or blade. After removal of the implant, we observed spontaneous femoral neck fractures in five patients. This article presents an evaluation of these cases with reference to literature.
MATERIALS AND METHODS: Five cases of spontaneous femoral neck fracture (SFNF) after cephalo

Electron microscopic observations were made from tissues apposing titanium and ceramic root form and blade implants. The tissue was serially sectioned from the most coronal epithelium, through the gingival connective tissue, to the osseous support tissues, and directly to the most apical tissue support. Of the thousands of sections analyzed for each implant, 500 micrographs were routinely viewed for each of the implants analyzed by this study. Of the 120 total implants placed in 30 adult dogs, 60 were used for electron microscopy. Osseointegrated implants were often apposed by a mineralized ma

The mechanical complications of osteosyntheses after hip fractures are previously investigated by mostly static or dynamic uniaxial loading test systems. However, the physiologic loading of the hip joint during a normal gait is a multiplanar, dynamic movement. Therefore, we constructed a system to test osteosyntheses for hip fractures under physiologic multiplanar loading representative of normal gait. To evaluate the testing system, 12 femora pairs were tested under 25,000 cycles with two standard osteosyntheses (Proximal Femoral Nail Antirotation/Gamma3 Nail). For angular movement, the varus

INTRODUCTION: The decision to retain or remove blade plate implants after proximal femoral osteotomy (PFO) in paediatric patients remains contentious. While retention provides ongoing support, it increases stress shielding, potentially hindering bone remodelling and causing long-term complications. Conversely, early removal may restore normal mechanical loading and promote bone recovery. This study compares the effects of blade plate retention versus removal on bone density changes and implant risk of yield (RoY) over 36 months in a paediatric femur.

Initial stability and development of long-term fixation for cementless tibial components at the knee both depend on the accuracy of fit between implanted components and prepared bone surfaces. Tibial surfaces prepared for total knee replacement with conventional saw-blades and guides were shown by Toksvig-Larsen to vary by over 2 mm, from a flat surface at the point of maximum variation, and all points varied with a standard deviation of up to 0.4 mm. Surface cutting errors are caused by flexion of the saw-blade and blade angulation from the ideal alignment, due to poor guidance or control by

The aim of the present study was to analyze patients treated with 95 degrees Condylar Blade Plate and to describe the technique that can prevent complications. 69 patients with closed intertrochanteric femoral fractures were treated by one surgeon using the 95 degrees Condylar Blade Plate, and it was found that 56 of them met the minimum 10 months follow up requirement. 53 patients (94%) healed after the procedure. There were 4 patients (7%) requiring surgical intervention. One patient required bipolar arthroplasty because of the cutting through of the implant, two patients needed repeated ope

INTRODUCTION: The specific aim of this retrospective study was to determine whether bone quality has any effect on the complication rates or overall survivorship between helical blades and lag screws in cephalomedullary nails used for intertrochanteric hip fractures.
METHODS: The authors reviewed clinical charts and radiographic studies of patients between January 2012 and August 2019. We reviewed radiographic images (pre-, intra-, and post-operative) to evaluate fracture fixation type, fracture reduction grade, and post-operative complications. We collected dual energy x-ray absorptiometry s

OBJECTIVE: To compare the biomechanical performance of a cephalomedullary nail (CMN), a proximal femoral locking plate, and a 95° angled blade plate in a comminuted subtrochanteric fracture model.
METHODS: A comminuted subtrochanteric femoral fracture model was created with a 2-cm gap below the lesser trochanter in 15 pairs of human cadaveric femora confirmed to be nonosteoporotic. The femora were randomized to treatment with one of the previously mentioned 3 devices. Each was tested under incrementally increasing cyclic load up to 90,000 cycles from 50% to 250% of body weight to simulate pro
