[Therapeutic approach in blast injuries of the ear].

UNLABELLED: Otic blast injury is caused by arrhythmic air blast wave. The perforation of the tympanic membrane is the commonet finding associated with lacerations of mucosa in the middle ear. Makki [6] published 34 cases of myringoplasty after war blast injuries. However, healing of such perforations is common; Kerr [7] noted a healing rate of 83% after the blast injury. The aim of the study was to evaluate different therapic procedures of otic blast injuries. There were 74 patients with bilateral otic blast injuries, who underwent otomycroscopic examination. The following parameters were note

Blast injuries from glass bottles containing dry ice.

We present three cases (two incidents) of severe blast injury from the explosion of covered glass bottles containing dry ice. The first patient sustained lacerations of the face, left eye, right wrist and forearm, and abdomen with protrusion of the small bowel. The second sustained a deep laceration to her anterior neck. The third sustained multiple lacerations to her lower extremities and one to the labia majora. All three patients were taken to the operating room for surgical exploration and repair and were discharged without complications.

[Blast injuries of the ear in military artillerymen].

Hearing impairment was examined in a group of 1073 soldiers artillerymen during regular training of shooting from artillery weapons of different calibers. Immediately after shooting the transient increase of hearing threshold (fatigue) of different degrees was found in 907 (84,25%) of the examined. Contusion and rupture of the eardrum were more commonly found in the right ear which confirmed the "phenomenon of screening". At admission it was found that 22,5% of soldiers suffered from different diseases of the ear, nose and throat while at discharge this percentage was increased for 6,4%. A sig

Blast-Related Otologic and Facial Nerve Injuries in Gaza.

Explosive weapons used in densely populated settings often disproportionately injure craniofacial nerves and lead to otologic trauma. During the Israeli military invasion of Gaza, the collapse of referral pathways and surgical capacity constrained otologic and craniofacial care. The authors aimed to characterize otologic trauma and facial nerve palsy, and to identify predictors of hearing loss severity in this context. The authors conducted a retrospective cohort study of civilians with conflict-related otologic trauma presented to the Nasser Medical Complex in Palestine's Khan Yunis, Gaza Str

[Gunshot and Blast Injuries from a Trauma Surgery Perspective].

The severity and extent of gunshot and blast injuries are often misjudged due to a lack of specialist knowledge. The aim of this article is to outline the characteristics of gunshot and blast injuries, including emergency diagnostics and initial surgical treatment.Due to multiple high-energy penetrating injuries, barotrauma, or blunt trauma, affected patients are usually polytraumatized, with multiple organ damage and an average Injury Severity Score (ISS) of > 36. In addition to the complexity of the wounds, they are characteristically severely contaminated. After pre-hospital treatment, life

Bullet and Blast ENT Injuries in Counter-Insurgency Area.

BACKGROUND: Many ENT injuries are not recognized easily, but they have the potential of increasing the morbidity. METHODS: ENT injuries managed in two tertiary care Level-IV hospitals between 2006 and 2007 were studied with a view to formulate strategy in efficient management of these cases. RESULT: Emergency bags did not carry readymade packs to control nasal bleeds. Routinely screening of ears in all blast injury cases in the 'Blast Injury Program' helped in early identification of hearing loss. Lack of sufficient stenting of nasal cavities resulted in severe nasal stenosis which was diffi

Blast injuries--and the pivotal role of trauma surgeons.

An explosion is the sudden release of energy and its radial propagation through air, solid structures and living tissue. Treatment of blast injuries is complex and combines the principles of penetrating and blunt trauma, chemical or thermal burns and disaster and mass casualty management. Primary blast injuries are a direct result of the explosion itself. The sudden release of energy is translated into a shock wave that travels at supersonic speed (5000 metres/second). There is a sudden and short-lived rise in pressure, followed by a prolonged negative pressure, or vacuum, responsible for addi

[Hemipelvectomy due to blast injuries: possibilities of occupational rehabilitation].

A case of traumatic hemipelvectomy due to explosion of an antitank shell in a 21-year-old Croatian Army soldier is described. Explosion was due to inadvertent handling of the shell in the army barracks. Literature data on persons who survived traumatic hemipelvectomy are extremely scarce; the injury is characterized by a very high death rate (60-100%). After prompt surgical treatment the patient developed a postoperative anaerobic clostridial infection. Owing to intensive clinical and physical therapy the patient was enabled for moving around in a special wheel-chair and walking with the help

Urban bomb blast injuries: patterns of injury and treatment.

A review of many series reporting injuries following blasts, data allows certain conclusions to be made: 1. Most patients sustain minor injuries, which may be treated on an outpatient basis. 2. Injuries predominantly affect the head and neck and the periphery, which suggests that clothing plays a major role in protection from secondary injuries. 3. Injuries to the chest and abdomen are relatively uncommon but have a high mortality, also associated with head injury. 4. Primary blast injuries are uncommonly seen in a hospital setting, because they usually result in immediate death.

Patterns of Firework-blast Injuries: A Descriptive Case Series.

Fireworks are commonly used during holidays in the United States. Firework related injuries (FWRIs) have been rising throughout the country since 2012. Here, we describe injury trends and patterns in patients with FWRIs. The trauma registry at Desert Regional Medical Center's Level I Trauma Center was queried from July 2016 to August 2021. Injury patterns and procedures performed were reviewed. The data were reviewed, and a description of the most common injury types and the association between FWRIs and holiday celebrations, alcohol use, and toxicology results is presented. Ten patients were

Maxillofacial air-containing cavities, blast implosion injuries, and management.

PURPOSE: Distinctive mechanisms of primary blast effects have produced a transitional era of facial trauma. Implosion mechanism damage is one of these. Implosion mechanism damage results in injury limited to the gas-containing structures of the auditory canal, paranasal sinuses, gastrointestinal tract, and lungs. Worldwide, the victims of explosive detonations have increased and advanced dramatically. The outcome is greater mortality and morbidity and new types of injuries, especially in the maxillofacial region. Thus, the knowledge of, and experience with, their management should be shared gl

Otologic blast injuries due to the Kenya embassy bombing.

Otologic injuries are frequently associated with large blasts. On August 7, 1998, a large truck bomb exploded next to the U.S. Embassy in Nairobi, Kenya. Initial patient findings and care are reviewed. Five months later, an otologic screening and care mission was then sent to comprehensively screen all remaining blast victims on site in Nairobi and to determine degree of persistent injury. Surgical care appropriate for an outpatient environment was provided. Five of 14 tympanic membranes without intervention failed to heal, while 3 of 3 with previous intervention had. Blast injury severity did

The spectrum of pediatric injuries after a bomb blast.

The spectrum of pediatric injuries seen after a bomb blast is poorly documented. The pathophysiology of blast injuries differ significantly from other forms of trauma and typically result in large numbers of distinctly patterned injuries. On April 19, 1995, a truck bomb was detonated directly adjacent to the Alfred P. Murrah Federal Building in Oklahoma City, Oklahoma. A total of 816 adults and children were injured or killed as a direct result of the blast. Twenty infants and children were seated by the window of the second floor day care center at the time of the explosion. The injuries incu

Anorectal injury in pelvic blast.

The signature injury of the Afghanistan campaign has, amongst other things, included an increased incidence of destructive anorectal injury. There is no significant body of evidence about this type of injury on which to base management strategies. This review examines the historical military data, later civilian reports, many of which have challenged the military dogmas of Vietnam, and the spartan contemporaneous military data which does not particularly address pelviperineal blast injury. There is no evidence to support a move away from the doctrine of the four D's (diversion, distal washout,

Otologic considerations of blast injury.

The ear by design is exquisitely sensitive to barotrauma. As a result, it is typically the first organ affected in primary blast injury. The most common symptoms encountered include hearing loss, ringing, and drainage. In severe cases, the highest priority is appropriately directed toward diagnosis and treatment of life-threatening injuries; however, injury to the ear is missed frequently. With simple screening procedures, limited management, and appropriate otolaryngologic referral, acute and long-term morbidity can be averted for both critical and noncritical patients. The article provides a

Pool chemical blast injury.

Swimming pools are one of the most popular forms of recreation in the United States. Pool-related injuries may produce significant morbidity and mortality, and those related to pool chemicals are of particular importance. The majority of injuries associated with pool chemicals are respiratory, with the remainder composed mainly of dermal exposures. There are few case reports about injuries from pool chemicals, and the potential for significant injury from blast force is presented here.
