Issues Concerning the Mechanisms of Bone Conduction.

Air and bone conduction thresholds are used to differentiate between conductive and sensori-neural hearing losses because bone conduction is thought to bypass the conductive apparatus, directly activating the inner ear. However, the suggested bone conduction mechanisms involve the outer and middle ears. Also, normal bone conduction thresholds have been reported in cases of lesions to the conduction pathway. Therefore, further investigation of bone conduction mechanisms is required.

Bone conduction hearing in congenital aural atresia.

Previous researches focusing on BC hearing mechanisms proved that the two routes, (1) EAC sound radiation and (2) inertial of ossicular chain, partially contribute to normal BC hearing. Therefore, the BC hearing for those patients with congenital aural atresia should partially decrease theoretically due to their abnormal anatomy. However, there are not many studies which mention these patients' BC hearing up till now. The objective of this study is to investigate congenital aural atresia patient's BC hearing by analysis of pre-surgical audiogram and to study their potential BC hearing mechanis

[Bone conduction hearing aids with fluid medium].

In patients with chronic otorrhoea or middle ear pathology, traditional hearing aids which depend on air conduction are of little use. In these situations a bone conduction hearing aid is preferable. Such devices by-pass the middle ear cleft and ossicular chain by conducting sound waves through bone to the cochlea. To date, bone conduction hearing aids have transmitted sound via a vibrating transducer applied either to skin (transcutaneous) or to bone (percutaneous). Unfortunately, these hearing aids possess a number of disadvantages, which include: cost, aesthetic appeal, a general anaestheti

Sound localization with bilateral bone conduction devices.

PURPOSE: To investigate sound localization in patients bilaterally fitted with bone conduction devices (BCDs). Additionally, clinically applicable methods to improve localization accuracy were explored. METHODS: Fifteen adults with bilaterally fitted percutaneous BCDs were included. At baseline, sound localization, (un)aided pure-tone thresholds, device use, speech, spatial and qualities of hearing scale (SSQ) and York hearing-related quality of life (YHRQL) questionnaire were measured. Settings to optimize sound localizing were added to the BCDs. At 1 month, sound localization was assessed a

[Bone Conduction and Active Middle Ear Implants].

The majority of patients with moderate to severe hearing loss can be supplied with conventional hearing aids depending on severity and cause for hearing loss in a satisfying way. However, some patients either do not benefit enough from conventional hearing aids or cannot wear them due to inflammatory reactions and chronic infections of the external auditory canal or due to anatomical reasons. For these patients there are fully- and semi-implantable middle ear and bone conduction implants available. These devices either directly stimulate the skull (bone conduction devices), middle ear structur

Otosclerosis: Shift in bone conduction after stapedotomy.

OBJECTIVES: To analyze changes in bone conduction (BC) thresholds before and after stapedotomy in patients with clinical otosclerosis with a focus on the Carhart notch, which is defined as a significant loss of BC at a certain audiometric frequency. METHODOLOGY: Retrospective case review of pure tone audiometry (PTA) in 213 patients who underwent a stapedotomy at a tertiary referral center between 2004 and 2011. The patients were grouped by age and the frequency of the Carhart notch. The non-operated ear was also evaluated. RESULTS: The Carhart notch was present on pre-operative audiometry i

[Semi-implantable transcutaneous bone conduction hearing devices].

BACKGROUND: Patients with air-bone gaps or combined hearing loss that cannot be corrected by tympanoplasty can be treated with bone conduction hearing aids. The disadvantages of conventional and percutaneous systems are, on one hand, the obvious external fixation components and on the other hand, the biological and psychosocial problems of open implants. This project was therefore set up to develop a semi-implantable, magnetically anchored transcutaneous bone conduction device, introduce it into clinical application and follow-up the results. PATIENTS AND METHODS: The principle of this bone c

The future of bone conduction hearing devices.

The bone-anchored hearing aid (Baha) is today an important rehabilitation alternative for patients with mixed and conductive hearing loss and where air conduction devices should not or cannot be used. Some patients with single-sided deafness are also successfully treated with a Baha. Despite successful treatment of these patient groups, there is always a need for future improvements. First, it is well known that Baha are associated with some drawbacks related to skin infections, accidental or spontaneous loss of the bone implant, and patient refusal for treatment due to stigma. Therefore, in t

Bone conduction auditory brainstem responses in infants.

The contribution of air conduction auditory brainstem response (AC-ABR) testing in the paediatric population is widely accepted in clinical audiology. However, this does not allow for differentiation between conductive and sensorineural hearing loss. The purpose of this paper is to review the role of bone conduction auditory brainstem responses (BC-ABR). It is argued that despite such technical difficulties as a narrow dynamic range, masking dilemmas, stimulus artifact and low frequency underestimation of hearing loss, considerable evidence exists to suggest that BC-ABR testing provides an imp

Bone conduction impairment in chronic ear disease.

The preoperative and postoperative bone conduction (BC) thresholds were prospectively investigated in 24 patients with chronic ear disease in the form of cholesteatoma, chronic suppurative otitis media, or adhesive otitis media. All underwent tympanoplasty with mastoidectomy. Ossicular reconstruction was performed in 14, and the remaining 10 were still awaiting second-stage ossicular reconstruction at the time of this investigation. In each group, the postoperative results were compared with the preoperative results by the paired-samples t-test. In the ossicular reconstruction group, the resul

Improvement in bone conduction threshold after tympanoplasty.

OBJECTIVE: To investigate the causes of bone conduction threshold impairment associated with middle ear pathoses and the factors influencing improvement in bone conduction threshold after tympanoplasty. STUDY DESIGN AND SETTING: The records of 98 consecutive patients with unilateral chronic otitis media who underwent tympanoplasty were reviewed. Pre-operatively, 15 dB or more depression of bone conduction threshold at least in 2 frequencies between 500 and 6000 Hz was considered to be significant. Similarly in the postoperative period, 15 dB or more improvement of bone conduction threshold at

Carhart notch and electric bone-conduction audiometry.

The Carhart notch is a depression in the bone-conduction audiogram of patients with clinical otosclerosis. The middle frequencies from 0.5 to 2 kHz, which correspond to the resonance frequency of the middle ear, can be substantially improved following successful stapes surgery. Twenty-nine consecutive patients with clinical otosclerosis were tested 2 weeks before and 7 months after stapes surgery with conventional air-conduction (AC) and bone-conduction (BC) audiometry and electric bone-conduction (EBC) audiometry using an Audimax 500 audiometer. The BC thresholds improved at 0.5-2.0 kHz corre

Bone conduction thresholds in patients with otosclerosis.

PURPOSE: Sensorineural hearing loss in patients with otosclerosis is commonly encountered. This study was conducted to determine if surgery on the otosclerotic ear had an effect on the sensorineural hearing. METHODS: A cohort of 262 patients subjected to operation in 311 ears were evaluated. All patients had a minimum of 5 years follow-up and patients over 60 years of age were excluded. Audiograms obtained 1 day before surgery were compared with those obtained 1 year postoperatively and at the last follow-up examination in the study. Results were evaluated using the Student's t test for stati

Implantable bone-conduction hearing device: practical considerations.

An implantable bone conduction hearing device can be of significant benefit to carefully selected patients with noncorrectable conductive hearing losses. However, for some patients the device has significant limitations. This paper presents several practical issues that need to be considered before a decision is made regarding implant surgery. It is recommended that, whenever possible, air conduction hearing aids remain the first option considered when a patient's conductive hearing loss cannot be resolved through traditional medical management.

Pre-operative bone conduction curves in stapedectomy.

Significant post-stapedectomy cochlear deficit is associated with the following factors in an uncomplicated operation: (1) The age of the patient. (2) A pre-operative bone conduction loss, averaged over 0.25, 0.5, 1.0 and 2.0 KHz, of 20 db or more. (3) A left-to-right sloping bone conduction curve (descending audiometric curve), especially where there is a similar curve in the other ear.
