Neonatal primary hyperparathyroidism in familial hypocalciuric hypercalcemia.

A Japanese female neonate exhibiting severe respiratory distress was found to have primary hyperparathyroidism. Features included demineralization of the bones, hypercalcemia, hypophosphatemia, and elevated levels of parathyroid hormone, together with marked, generalized aminoaciduria. Four enlarged parathyroid glands were surgically removed, and remineralization of the bones was noticed after the operation. The characteristic pathologic change of the glands was chief cell hyperplasia. A survey of the family identified three other hypercalcemic but asymptomatic relatives. The mode of inheritan

Murine Osteoblast and Osteoclast Co-culture on Demineralized Bone Paper for Bone Remodeling.

Continuous and balanced bone remodeling is essential for maintaining mechanical integrity, mineral homeostasis, and hematopoiesis. Dysregulated bone metabolism develops pathological conditions, such as osteoporosis and bone metastasis. Functional and analytical recapitulation of bone remodeling in vitro is critical for advancing our understanding of bone mineral metabolism, disease mechanisms, and drug development. However, conventional models fail to replicate the essential complexity of the bone extracellular matrix (ECM) and the dynamic interplay between bone-forming osteoblasts and bone-re

A study to evaluate the cause of bone demineralization in gynecological cancer survivors.

BACKGROUND: An association between treatment for gynecological cancers and risk of osteoporosis has never been formally evaluated. Women treated for these cancers are now living longer than ever before, and prevention of treatment-induced morbidities is important. We aimed to distinguish, in gynecological cancer survivors, whether cancer therapy has additional detrimental effects on bone health above those attributable to hormone withdrawal. METHODS: We performed a retrospective cross-sectional analysis of dual energy x-ray absorptiometry (DEXA) scan results from 105 women; 64 had undergone b

Pathologic fracture in systemic mastocytosis. Radiographic spectrum and review of the literature.

Systemic mastocytosis is a multiorgan disease that most commonly affects the skin and skeletal system. Radiographically, the skeletal changes in the majority of patients consist of either a wide-spread mixture of bone lysis and osteosclerosis or generalized osteoporosis. The osteoporotic form is less well known but may lead to severe generalized demineralization and pathologic fractures. Mast cells secrete a number of substances, two of which (heparin and prostaglandins) are believed to have a role in the induction of osteoporosis. Sclerotic lesions are induced by another mast cell by-product,

Hyperechoic metaphyses in hypophosphatasia: what does it mean?

We report a case of hypophosphatasia diagnosed using US and CT at 29 weeks' gestation and confirmed by molecular analysis. Prenatal US revealed very short fetal limbs and severe demineralization of the skull. The diaphyses were normal, but the metaphyses of the long bones appeared hyperechoic with no posterior shadowing. No fractures or long-bone deformations were observed. Three-dimensional helical CT performed at 29 weeks' gestation provided additional details of the abnormal bones, i.e. irregular and cupped metaphyses that were very similar to the radiological findings of hypophosphatasia d

[Peri-tumor mandibular demineralization].

This study describes hitherto unpublished observations suggesting mechanism by which a tumor can cause massive resorption of the mandible. Microradiographic analysis of the cancerous areas has shown a mobilization of minerals from compact and spongious bone tissue accompanied by incomplete degradation of collagen fibres. The minerals are deposited in the lumen of the Haversian canals, the medullar cavity and the perimandibular soft tissue, areas which are not normally mineralized. Electronic microprobe and X-ray diffraction analysis revealed that in the mineralized zones undergoing demineraliz

Biochemical, bone and renal patterns in hyperparathyroidism associated with multiple endocrine neoplasia type 1.

Primary hyperparathyroidism associated with multiple endocrine neoplasia type I (hyperparathyroidism/multiple endocrine neoplasia type 1) differs in many aspects from sporadic hyperparathyroidism, which is the most frequently occurring form of hyperparathyroidism. Bone mineral density has frequently been studied in sporadic hyperparathyroidism but it has very rarely been examined in cases of hyperparathyroidism/multiple endocrine neoplasia type 1. Cortical bone mineral density in hyperparathyroidism/multiple endocrine neoplasia type 1 cases has only recently been examined, and early, severe an

Anterior lumbar interbody fusion with titanium mesh cages, coralline hydroxyapatite, and demineralized bone matrix as part of a circumferential fusion.

BACKGROUND CONTEXT: Anterior lumbar interbody fusion (ALIF) has become one of the primary choices for eliminating motion between vertebral segments in patients with severe discogenic pain and other lumbar pathologies. Autograft is the gold standard for spinal fusion. However, multiple authors have reported complication and morbidity rates associated with iliac crest harvesting to be as high as 25%. Drawbacks to the use of allograft in the anterior column include slower incorporation rates, the possibility for disease transmission, increasing cost resulting from stringent processing and unavail

Use of demineralized bone matrix and plate for sternal stabilization after traumatic dislocation.

Traumatic sternal segments dislocation is a rare pathology that is on the increase because of road accidents. Ideal treatment is controversial as it is a benign non-life-threatening lesion. The few cases reported in the literature do not shed light on any preferred procedure. In the present study, three patients were treated by surgical steel monofilament and eight by titanium screws and plates and demineralized bone matrix. The reduced hospitalization, rapid functional recovery, and excellent aesthetic results of the titanium patients make use of the surgical approach inevitable.

Perspective on the consequences of short- and long-duration space flight on human physiology.

During the past three decades, humans have made significant progress in accomplishing their aspirations for exploring the Moon and the planets. It is now appreciated that humans undergo a remarkable number of physiologic adaptations in microgravity that affect most physiologic systems. Space motion sickness was one of the first adaptations that humans experienced in microgravity. However, it is self-limiting and, most of the time, is effectively treated pharmacologically. Of particular concern is that, in microgravity, there is marked wasting of the skeletal musculature and skeleton that appea

Long-term bone loss in kidney transplant recipients: a cross-sectional and longitudinal study.

Organ transplantation is associated with an early bone loss that subsequently increases the risk of osteopenia and bone fractures. It is not known whether bone loss continues in long-term survivors, but persistent bone demineralization could further jeopardize an already diminished bone mass. To better define long-term bone status of kidney transplant recipients (KTR), we conducted cross-sectional and longitudinal evaluations of bone mineral density (BMD) in 70 KTR with a mean posttransplantation time of 8.1 years. BMD was determined by dual-energy X-ray absorptiometry and was repeated in 55 o

Bone mineralization in infants.

The major physiologic control of bone mineralization in infancy involves calcium and phosphorus. Ca and P metabolism in turn is affected by endogenous stores at birth and the ability to deliver and absorb exogenous sources of these minerals. Calciotropic hormones (parathyroid hormone, calcitonin and 1,25 dihydroxyvitamin D) modulate the response of major end organs - intestine, kidney and bone - to balance the need to maintain a relatively stable extracellular biochemical environment with the need for adequate mineralization of the bone. Many other factors such as glucocorticoids under patholo

Hyperparathyroidism and Bone Health.

Bone pain, proximal muscle weakness, skeletal deformities, and pathological fractures are features of osteitis fibrosa cystica which occur in severe primary hyperparathyroidism (PHPT). In this condition, bone mineral density is usually extremely low, but may be reversible after parathyroidectomy. On X-ray, bone abnormalities are described as having a salt-and-pepper appearance in the skull, with bone erosions and resorption of the phalanges, brown tumors and cysts, as well as diffuse demineralization, along with pathological fractures, particularly in the long bones of the extremities. A marke

The Treatment of Large Bone Defects with Atrophic Nonunion by Bioactive Scaffolds Remodeling the Regeneration Microenvironment.

Atrophic nonunion, a particularly challenging complication of diaphyseal bone fractures, arises from a deteriorated regenerative microenvironment characterized by insufficient vascularization and pathological accumulation of reactive oxygen species (ROS). To address this clinical challenge, a dual-function bioactive scaffolds is developed that simultaneously blocks disease progression and promotes tissue regeneration. The scaffolds design incorporates three key components: First, MnO2-Cu2+ (MC) nanoparticles are synthesized to combat the pathological microenvironment through dual mechanisms of

[Physiopathology of osteoporosis (author's transl)].

Osteoporosis is the result of the progressive loss of bone substance which occurs during a lifetime. Its histomorphological characteristics have been well-defined and it appears to be a single entity more particularly related to the general problem of cellular senescence. The rate at which bone is lost can be influenced by many events or general factors, known or unknown, which are capable of accelerating or slowing down its progression. The period when the fracture threshold is passed appears to depend on a bone mass which is weak at the end of the growth period, followed by genetic, mechanic
