Fully Threaded Versus Partially Threaded Screws: Determining Shear in Cancellous Bone Fixation.

Many researchers have studied and compared various forms of intraosseous fixation. No studies have examined the effects of shear through stiffness and failure strength of a fully threaded versus a partially threaded screw. Our hypothesis was that the fully threaded lag screw technique would provide greater shear strength and resistance. Thirty-six synthetic sawbone blocks were used to test screw fixation. In group 1 (n = 9), 2 blocks were fixed together using a fully threaded 4.0-mm stainless steel cancellous bone screw and the lag technique. In group 2 (n = 8), 2 blocks were fixed together us

Continuous decompression of unicameral bone cyst with cannulated screws: a comparative study.

We determined the role of mechanical decompression in the resolution of unicameral bone cyst. A total of 69 children with unicameral bone cysts were treated either by (i) open curettage and bone grafting, (ii) steroid injection or (iii) cannulated screw insertion. During a mean follow-up of 69 months (range, 12-58), the cysts were evaluated by radiological criteria. The healing rates in the three groups were 25, 12 and 29% after the first treatment, and a further 50, 19 and 65% after the second. The study has demonstrated the advantages of the decompression technique for unicameral bone cysts

Fixation pull-out response measurement of bone screws and porous-surfaced posts.

Screws and posts are used in various implant designs to contribute to the short- and long-term fixation stability of artificial joints. This study was undertaken to measure the detailed pull-out load-displacement response of bone screws, beaded porous coated posts, and smooth-surfaced posts in both proximal tibial cancellous bone and polyurethane material under monotonic static and repetitive cyclic loadings. The effect of a number of parameters such as insertion site on the proximal tibia, rate of displacement, insertion depth, outside diameter, drill size, repetitive loading and boundary con

Bone tunnel widening with autogenous bone plugs versus bioabsorbable interference screws for secondary fixation in ACL reconstruction.

BACKGROUND: Use of an autogenous bone plug for the tibial tunnel in anterior cruciate ligament (ACL) reconstruction has been advocated to achieve tendon-to-bone healing. Our hypothesis was that use of an autogenous bone plug, instead of a bioabsorbable interference screw, for secondary fixation of tendon allograft to the proximal part of the tibia would reduce the complication rate and tibial tunnel widening. METHODS: We prospectively reviewed the cases of eighty-one patients (average age, 32.0 years) who had undergone ACL reconstruction with Achilles tendon allograft between 2000 and 2006. A

Screw angulation affects bone-screw stresses and bone graft load sharing in anterior cervical corpectomy fusion with a rigid screw-plate construct: a finite element model study.

BACKGROUND CONTEXT: Anterior corpectomy and reconstruction with bone graft and a rigid screw-plate construct is an established procedure for treatment of cervical neural compression. Despite its reliability in relieving symptoms, there is a high rate of construct failure, especially in multilevel cases. PURPOSE: There has been no study evaluating the biomechanical effects of screw angulation on construct stability; this study investigates the C4-C7 construct stability and load-sharing properties among varying screw angulations in a rigid plate-screw construct. STUDY DESIGN: A finite element

Superior biomechanical stability of pedicle screws compared to lateral mass screws: recommendations for bicortical positioning and enhancing bone contact in geriatric C1 vertebrae.

BACKGROUND: In atlantoaxial instabilities, posterior C1/C2 fusion using lateral mass screws (LMS) or pedicle screws (PS) in a mono- or bicortical position in the atlas is a typical treatment. The bone microstructure and positioning of the screw trajectories appear to be of significant relevance for stability. PURPOSE: The aim of this study was a comparative analysis of the mechanical durability of screw fixation concerning microstructural characteristics of the trajectories of LMS and PS in mono- and bicortical position. METHODS: Human C1 from geriatric body donors (n = 28; 50% female, age 8

Bioactive Surface Modifications on Bioresorbable Bone Screws: A Step Forward in Orthopedic Surgery.

Despite metals currently being widely used in orthopedic surgery, their mechanical properties significantly differ from the surrounding tissues and organs, causing low biocompatibility. Biodegradable, non-toxic, and non-immunogenic materials seem to be more convenient for clinical implementation. Our research was aimed at the construction of a polylactide screw covered with collagen, nanohydroxyapatite, and polylactide, with a variant including silver nanowires for antibacterial properties, as well as the analysis of their physico-chemical and biological properties. Adherent human osteosarcoma

Percutaneous screw fixation for fractures of the sesamoid bones of the hallux.

Over a period of one year we treated nine fractures ofhe sesamoid bones of the hallux, five of which were in the medial sesamoid. All patients had symptoms on exercise, but only one had a recent history of injury. The mean age of the patients was 27 years (17 to 45) and there were six men. The mean duration of symptoms was nine months (1.5 to 48). The diagnosis was based on clinical and radiological investigations. We describe a new surgical technique for percutaneous screw fixation for these fractures using a Barouk screw. All the patients were assessed before and after surgery using the Amer

Scaphoid osteosynthesis: early experience with the Herbert bone screw.

The results of our first fourteen patients with fractured scaphoids treated with the Herbert bone screw have been disappointing. Six have failed to unite. The operation is technically demanding and there is a high incidence of malpositioning of the screw which correlates with failure to achieve union. This is predictable from initial radiographs and peroperative radiology is advised. The osteosynthesis should be protected until union has occurred radiologically, especially in manual workers.

Management of the fractured scaphoid using a new bone screw.

A new and simple operative technique has been developed to provide rigid internal fixation for all types of fractures of the scaphoid. This involves the use of a double-threaded bone screw which provides such good fixation that, after operation, a plaster cast is rarely required and most patients are able to return to work within a few weeks. A classification of scaphoid fractures is proposed. The indications for operation included not only acute unstable fractures, but also fractures with delayed healing and those with established non-union; screw fixation was combined with bone grafting to t
