Effect of particles and interface conditions on fibrous tissue interposition between bone and implant. A particle challenge model in rabbit.

Interposed fibrous tissue at bone-implant interfaces was quantitatively measured in the presence or absence of polyethylene (PE) or alumina particles. Three different conditions of the interface were designed by implanting a pre-polymerized polymethylmethacrylate (PMMA) plug (plug group), a doughy PMMA (injection group) and a hydroxyapatite (HA) plug (HA group) in the hole drilled at the intercondylar notch of rabbit knees. PE (170+/-18 microm) or alumina particles (88+/-26 microm) were repeatedly administered into the knee joints at one month intervals (six times). All animals were sacrificed

A 3-Dimensional finite element analysis of patient-specific implant (with strut abutments) interface on stress concentration on the implant and surrounding bone in bilateral maxillary deformities.

PURPOSE: To determine the efficacy and longevity of patient-specific implants (PSIs) with strut abutment design to rehabilitate bilateral maxillectomy defect. MATERIALS AND METHODS: Finite Element Analysis was performed on a PSI with strut abutments to repair a patient's bilateral maxillectomy defect due to COVID associated mucormycosis. RESULTS: The von Mises stress recorded was maximum in the zygomaticomaxillary buttress region, and displacement values were noted to be highest in the posterior-most strut, although both parameters were within acceptable limits, which is favorable. CONCLUSI

Composite morphology of the bone and associated support-tissue interfaces to osseointegrated dental implants: TEM and HVEM analyses.

Correlated transmission electron and high-voltage electron microscopic analyses examined the undecalcified bone and associated support tissues of 60 endosseous titanium blade and titanium and ceramic root-form implants in dogs. The implants supported fixed partial dentures for up to 2 years. Data obtained from this investigation suggest that a range of tissues, both mineralized and unmineralized, support osseointegrated dental implants. This study examined the tissues apposing not just isolated aspects of the implant surface, but the entire length of the implant, and found that mineralized and

Effects of fixation technique on displacement incompatibilities at the bone-implant interface in cementless total knee replacement in a canine model.

Bone-implant displacements can be caused by rigid body motion and by differences in material properties of the implant and bone. In the present study of the tibial component in total knee replacement, we tested a series of tibial component fixation designs to determine how certain design features influenced the magnitude of the tangential displacement between the component and supporting bone in a canine model. The transverse expansion of the proximal tibia under static axial loading was measured in the intact tibia and then in the same bone following implantation of tibial components with dif

Thermodynamic Effects of 3 Different Diode Lasers on an Implant-Bone Interface: An Ex-Vivo Study With Review of the Literature.

The aim of this study is to assess the increase of temperature following laser irradiation with 810 nm, 980 nm, and 1064 nm diode laser wavelengths, of an implant under conditions that more closely replicate those of the human body. A 4 × 14 mm machined surface implant was placed in a porcine rib to replicate the conductivity of heat given by the bone. A peri-implant vertical defect was made that was 2 mm wide and 2 mm deep to simulate bone resorption. Two thermocouples were positioned crestally and apically on the implant surface. The tip of the laser was kept 3 mm away from the surface and c

Effects of magnesium coating on bone-implant interfaces with and without polyether-ether-ketone particle interference: A rabbit model based on porous Ti6Al4V implants.

We investigated the effects of magnesium (Mg) on osteogenesis and bone resorption at a porous structure interface. A three-dimensional (3D)-printed porous Ti6Al4V implant coated with Mg was introduced, and polyether-ether-ketone wear particles were added to generate an animal model of implant loosening. We also examined the effects of Mg leach liquor on osteoblast/osteoclast gene expression, alkaline phosphatase activity, collagen secretion, tartrate-resistant acid phosphatase activity, and bone resorption in vitro. Mg inhibited the early stage of osteoclast differentiation and inhibited bone

Three-dimensional ultrastructural analysis of the interface between an implanted demineralised dentin matrix and the surrounding newly formed bone.

Previous investigators have reported that transplanted demineralised dentin matrix (DDM) influences bone formation in vivo. However, the specific mechanism of how dentinal tubules contribute to bone formation has not been determined with regard to DDM transplantation therapy. In this study, we ultrastructurally investigated how DDM contacted the surrounding newly formed bone using a scanning electron microscopy (SEM) three-dimensional reconstruction method that is based on focused ion beam slicing and SEM (FIB/SEM). A pulverised and processed DDM derived from human teeth was implanted into rat

The strain at bone-implant interface determines the effect of spinopelvic reconstruction following total sacrectomy: a strain gauge analysis in various spinopelvic constructs.

PURPOSE: There is still some controversy regarding the optimal biomechanical concept for spinopelvic stabilization following total sacrectomy for malignancy. Strains at specific anatomical sites at pelvis/sacrum and implants interfaces have been poorly investigated. Herein, we compared and analyzed the strains applied at key points at the bone-implant interface in four different spinopelvic constructs following total sacrectomy; consequently, we defined a balanced architecture for spinopelvic fusion in that situation. METHODS: Six human cadaveric specimens, from second lumbar vertebra to prox

Simulated Coating Interface of (Au-n-HAP-TiO2) Ti-Metal Bone Implants: Improved Coating Thickness and Long-Term Corrosion Resistance by the Electrophoretic Deposition Method.

Recent investigations into novel biomaterials have revealed a necessary trade-off between mechanical and biological properties. This understanding has driven the exploration of various dopant elements to optimize the material performance for biomedical applications. The prime objective of this work is to develop gold-decorated nanohydroxyapatite coatings (Au-n-HAP) on titanium (Ti) via electrophoretic deposition (EPD) owing to their ability to enhance the corrosion resistance and biocompatibility of Ti. The crystalline nature of Au-n-HAP was evidenced by XRD, and the presence of phosphate grou

Stress transfer properties of different commercial dental implants: a finite element study.

Dental implantology has high success rates, and a suitable estimation of how stresses are transferred to the surrounding bone sheds insight into the correct design of implant features. In this study, we estimate stress transfer properties of four commercial implants (GMI, Lifecore, Intri and Avinent) that differ significantly in macroscopic geometry. Detailed three-dimensional finite element models were adopted to analyse the behaviour of the bone-implant system depending on the geometry of the implant (two different diameters) and the bone-implant interface condition. Occlusal static forces w

Where Is the "Weak Link" of Fixation in Contemporary Cemented Total Knee Replacements?

BACKGROUND: Aseptic tibial loosening is a frequent cause of long-term failure following primary cemented total knee replacement. Failure of the tibial implant can occur at the implant-cement interface or at the cement-bone interface. Currently, it is unknown at which interface failure occurs in cases of aseptic tibial loosening. The following study was designed to determine which interface represents the "weak link" for tibial implant fixation. METHODS: We performed a retrospective analysis of 149 patients who were revised secondary to aseptic tibial loosening at our institution from 2005 to

Histological and ultrastructural appearance of the hydroxyapatite-bone interface.

This article describes the histological and ultrastructural appearance of the interface created in the implantation bed, between bone tissue and implants made of dense sintered hydroxyapatite (HA). Biopsies from dog subjects included: a) loaded permucosal dental implants for tooth substitution, b) subperiosteally placed implants for alveolar bone correction, c) endosseously placed dental root implants to retain ridge form following extraction. The light and electron microscopical results show extensive bone apposition on the osseous sides of the implant surfaces. There is an intimate, direct b

Participation of the Periosteum, Endosteum, and Hematogenous Marrow in the Early Osseointegration of a Titanium Implant Inserted in Contact with the Hematogenous Marrow.

Background and Objectives: Implant osseointegration has been widely studied over the past few decades, particularly focusing on surface modifications that aim to improve integration. However, the literature includes few studies regarding the role of the endosteum in early osteointegration. Therefore, the aim of the present work is to approach the technique of implant insertion into bones with marrow, with an emphasis on the use of implants that are long enough to achieve marrow penetration. Materials and Methods: Titanium implants were inserted into the femurs of eight eleven-month-old rabbits

Histological evaluation of the peri-implant bone around plasma-sprayed non-submerged titanium implants retrieved from man: a report of two cases.

Two cases of non-submerged implants are reported: one was removed, unloaded, after 1 year, for psychiatric reasons, and the other, after a loading period of 4 years, for a fracture of the implant. The implants and surrounding tissues were processed to obtain thin ground sections. In the first case compact lamellar bone was observed in close contact with the titanium all around the implant perimeter. No connective tissue or empty spaces were found at the bone-implant interface. Histochemical staining with von Kossa demonstrated that all the bone around the implant was highly mineralized. No are

Clinical and histologic features of a nonaxial load on the osseointegration of a posterior mandibular implant: report of a case.

The authors report on the clinical and histologic features of a single implant replacing a mandibular molar, which fractured after a 1-year loading period. Because of anatomic factors, the implant had been placed in an angulated position. The histologic examination showed a very high percentage of bone-implant contact. The bone located at the interface with the implant was mature, compact, and had few marrow spaces. No resorption areas were present, and no connective tissue was seen at the interface. Most likely, the lateral nonaxial forces exerted on the implant created very high bending mome

Is cement mantle thickness a primary cause of aseptic tibial loosening following primary total knee arthroplasty?

BACKGROUND: Aseptic tibial loosening following primary total knee replacement is one of the leading causes of long-term failure. Cement mantle thickness has been implicated as a source of aseptic tibial loosening. Therefore, the following study was designed to determine (1) what is the cement mantle thickness in patients that develop aseptic tibial loosening, and (2) is there a difference in cement mantle thickness based on the interface of failure? METHOD: This retrospective cohort included 216 patients revised for aseptic tibial loosening. Patient demographics, operative data, and clinical

Emerging ideas: Instability-induced periprosthetic osteolysis is not dependent on the fibrous tissue interface.

BACKGROUND: Stable initial fixation of a total joint arthroplasty implant is critical to avoid the risk of aseptic loosening and premature clinical failure. With implant motion, a fibrous tissue layer forms at the bone-implant interface, leading to implant migration and periprosthetic osteolysis. At the time of implant revision surgery, proresorptive signaling cytokines are expressed in the periimplant fibrous membrane. However, the exact role of this fibrous tissue in causing periprosthetic osteolysis attributable to instability remains unknown. QUESTIONS/HYPOTHESES: We propose an alternativ
