Limb Salvage Versus Amputation for the Management of Primary Bone Tumors of the Upper Extremity: A Systematic Review.

Primary bone tumors are rare yet present a significant burden to patients, with surgery being the mainstay of treatment. We reviewed outcomes of limb salvage (LS) and amputation for management of upper extremity tumors. We followed Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines for systematic reviews. A total of 2,886 studies were screened, and 5 studies were included (n = 187 patients; LS = 101, Amputation = 86). Commonly reported outcomes included local recurrence, metastasis at follow-up, and survival. Metastasis and local recurrence rates were in favor of LS

Upper Extremity Diaphyseal Osseous Gap Reconstruction with Free Vascularized Bone Flaps: A Scoping Review.

BACKGROUND: Reconstruction of upper extremity osseous diaphyseal defects often requires complex reconstructions. In this study, we characterized and summarized the available literature on free vascularized bone flap (VBF) reconstruction for upper extremity diaphyseal defects. METHODS: A scoping review using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis extension for Scoping Reviews was conducted. A literature search of major electronic databases was conducted to identify journal articles relating to the management of VBF reconstruction of upper limb long bone defects.

Mixed sclerosing bone dystrophy presenting with upper extremity deformities. A case report and review of the literature.

We present a detailed study of a 59-year-old white woman with mixed sclerosing bone dystrophy: the rare occurrence of two or more sclerosing bone dysplasias in a single subject. She exhibited features of osteopoikilosis, osteopathia striata and melorheostosis. The symptoms were primarily the result of the melorheostosis lesions which were distributed within the C6 sclerotome. This is an unusual case of mixed sclerosing bone dystrophy in that the upper extremity was the main site of involvement, instead of the usual symmetrical involvement of all limbs. The patient developed a recalcitrant late

Microsurgical reconstructive surgery following wide resection of bone and soft tissue sarcomas in the upper extremities.

A preliminary report is presented of six cases of microsurgical tissue transfer following wide tumor resection, utilizing free vascularized fibula graft (five cases) and free vascularized muscle graft (one case). In five cases, the grafted tissue survived completely, and the attempted functional reconstruction was successful. Although the follow-up period has been relatively short, there has been no local recurrence or distant metastasis in any of the cases.

Muscle strength is a determinant of bone mineral content in the hemiparetic upper extremity: implications for stroke rehabilitation.

Individuals with stroke have a high incidence of bone fractures and approximately 30% of these fractures occur in the upper extremity. The high risk of falls and the decline in bone and muscle health make the chronic stroke population particularly prone to upper extremity fractures. This was the first study to investigate the bone mineral content (BMC), bone mineral density (BMD), and soft tissue composition of the upper extremities and their relationship to stroke-related impairments in ambulatory individuals with chronic stroke (onset >1 year). Dual-energy X-ray absorptiometry (DXA) was used

Successful treatment of necrotizing fasciitis in an upper extremity caused by Clostridium perfringens after bone marrow transplantation.

We report a 47-year-old man with acute leukemia who survived a severe case of necrotizing fasciitis caused by Clostridium perfringens involving his right upper extremity. On day 5 after stem cell transplantation, progressive local tissue necrosis led to septicemia and disseminated intravascular coagulation. Early diagnosis and prompt initiation of appropriate therapy, including surgical debridement and broad-spectrum antibiotics, were crucial. A recombinant thrombomodulin might have not only resolved the coagulation problem but also prevented multiple organ failure associated with the systemic

Intra-arterial autologous bone marrow cell transplantation in a patient with upper-extremity critical limb ischemia.

Induction of therapeutic angiogenesis by autologous bone marrow mononuclear cell transplantation has been identified as a potential new option in patients with advanced lower-limb ischemia. There is little evidence of the benefit of intra-arterial cell application in upper-limb critical ischemia. We describe a patient with upper-extremity critical limb ischemia with digital gangrene resulting from hypothenar hammer syndrome successfully treated by intra-arterial autologous bone marrow mononuclear cell transplantation.

[Borderline cases in implantology. Extensive bone augmentation and implants in extremely atrophied upper jaw: two case reports].

The placement of implants in the vertically and horizontally resorbed maxilla for a fixed prosthodontic reconstruction still presents a challenge to the therapist. To enable acceptable implant placement and positioning, optimizing the residual bone is necessary. A new method for the reconstruction of the extremely atrophied maxilla is presented in two edentulous patients. This extensive osseous augmentation (GBR) was performed using bilateral sinus grafts, simultaneous ridge splitting and guided bone regeneration with bone harvested from intraoral sites and bone replacement material. In both p

Management of the complications of traditional bone setting for upper extremity fractures: the experiences of a French Forward Surgical Team in Chad.

The practice of traditional bone setting (TBS) in sub-Saharan Africa often leads to severe complications after upper extremity fracture. The purpose of this study was to evaluate the management of these complications by a French Forward Surgical Team deployed in Chad. An observational, prospective study was conducted over a six-month period between 2010 and 2011. During this period 28 patients were included. There were 20 males and 8 females with a mean age of 30.6 years (range 5-65 years). Thirteen patients (47%) had mal-union of their fracture, nine had non-union (32%), three children (10.5%

One-bone forearm reconstruction procedure as salvage operation after severe upper extremity trauma: a case report.

An industrial worker in his early 20s sustained a severe injury to the right dominant upper extremity: fracture, inversion, and complete devascularization of the ulna; transection of the median nerve, the radial artery, and almost all flexor tendons of the hand and fingers; loss of all extensor muscles; and transection of the biceps and brachialis muscles at the elbow. Treatment consisted of conversion to one-bone forearm, immediate reconstruction of the biceps and brachialis muscles and of all flexor tendons of the hand, repair of the radial artery and median nerve and late tendon transfer fo

Calcium phosphate bone cement and metaphyseal -corrective osteotomies in the upper extremity: long-term follow-up of 10 children.

BACKGROUND AND PURPOSE: The evaluation of metaphyseal angular deformities in children includes indication and timing for corrective osteotomy, and possible need for several operations during growth. Gap-fillers are usually autologous bone grafts, which might cause donor site problems. Calcium phosphate (CaP) bone cement may be a possible alternative. PATIENTS AND METHODS: We performed 15 corrective osteotomies from 2007 to 2013 in 10 children, ages 5 to 18, with Norian SRS bone cement as a gap-filler, in the distal radius (12), proximal radius (1), and proximal humerus (2). Due to growth arre

Clinical Observations of the Effectiveness of the Masquelet Induced Membrane Technique in the Treatment of Critical Long-Bone Defects of the Lower and Upper Extremities.

Background and Objectives: Successful treatment of severe trauma and fractures of the long bones with successful healing and bone union is still a significant challenge for surgeons. Unfortunately, up to 10% of long-bone fractures develop bone healing disorders. The aim of this study was to evaluate the results of treating bone defects with different etiologies in the upper and lower extremities using the induced membrane technique. Materials and Methods: We prospectively evaluated the radiological and clinical outcomes of 45 patients with severe bone defects treated with the induced membrane
