Neuropathies Following an Ultrasound-Guided Axillary Brachial Plexus Block.

PURPOSE: Ultrasound-guided brachial plexus block (UGBPB) has interscalene, supraclavicular, infraclavicular, and axillary approaches. The axillary block is considered to be the safest and with fewer adverse events compared to the interscalene (eg, phrenic nerve block, spinal cord or vertebral artery puncture) and supraclavicular (eg, pneumothorax). However, with regard to postoperative neurological symptoms (PONS), it is controversial whether its incidence after an axillary block was higher than that after non-axillary approaches". In this study, we investigated whether the incidence of a neur

[Bilateral brachial plexus block. Case report and systematic review].

BACKGROUND: The bilateral brachial plexus block is considered a contraindication, due to the possible development of complications, such as: toxicity from local anaesthetics or bilateral diaphragmatic paralysis. However, with the real time visualisation provided by the ultrasound scan, these complications have decreased and it is a safer procedure. CLINICAL CASES: Four cases are presented where the bilateral block was performed using guided ultrasound, as the patients were unable to receive general anaesthesia due to a history of adverse effects or the use of opioids in the post-operative or

Convulsions after ropivacaine 300 mg for brachial plexus block.

A healthy 18-yr-old male (weight 60 kg, height 167 cm), with a history of febrile convulsions in childhood, developed a grand mal convulsion 10 min after the second of two injections of ropivacaine 150 mg, both given incrementally 15 min apart (total 300 mg), for combined axillary/interscalene brachial plexus block. Treatment was with oxygen, lung ventilation, and i.v. midazolam, and the patient made a complete recovery. Arterial plasma ropivacaine concentration 2 min after the onset of convulsions was only 2.13 mg litre(-1), suggesting that this patient was particularly susceptible to local a

Airway difficulty after a brachial plexus subclavian perivascular block.

We report a case of upper airway obstruction after subclavian perivascular block in a patient with a preexisting unrecognized paralyzed vocal cord on the opposite side. We discuss the incidence of vocal cord paralysis after subclavian perivascular block and the risk of airway compromise if contralateral vocal cord paralysis is known or suspected.

Should supraclavicular brachial plexus block be avoided in pregnancy?

Unilateral phrenic nerve block is common after supraclavicular brachial plexus block techniques, although it is rarely symptomatic in patients without respiratory disease. A 24-weeks-pregnant woman was scheduled for a carpal tunnel release because of intractable pain. After a perivascular subclavian brachial plexus block with 30 ml of 0.33% plain bupivacaine was performed, the patient developed a right phrenic nerve block manifested by acute dyspnea and cough. No deleterious consequences followed, but surgery was canceled. Respiratory changes produced by pregnancy might compromise ventilatory

Supraclavicular brachial plexus blocks: review and current practice.

This article reviews the possible revival of the supraclavicular brachial plexus blockade due to the use of ultrasound guidance. The brachial plexus is a complex network of nerves, extending from the neck to the axilla, which supplies motor and sensory fibers to the upper extremity. Understanding the complexities of the formation and structure of the brachial plexus remains a cornerstone for effective regional anaesthesia. On the level of the supraclavicular fossa, the plexus is most compactly arranged. The supraclavicular approach of the brachial plexus has a high success rate including block

Ultrasound-guided block of the brachial plexus at the humeral canal.

PURPOSE: Conduction block of the brachial plexus block at the humeral canal, as described by Dupre, has certain clinical indications. The aim of this preliminary study was to assess the feasibility of this technique under ultrasound guidance. METHODS: After ultrasound evaluation of the brachial plexus at the humeral canal in 61 adult volunteers, we performed ultrasound-guided blocks in another 20 adult patients. A linear 38 mm probe, 13-6 MHz, and a 50-mm insulated block needle were used to guide injection of lidocaine 1.5% with epinephrine. RESULTS: Ulnar and median nerves are superficial a

Brachial plexus blocks for upper extremity orthopaedic surgery.

Regional anesthesia of the upper extremity has several clinical applications and is reported to have several advantages over general anesthesia for orthopaedic surgery. These advantages, such as improved postoperative pain, decreased postoperative opioid administration, and reduced recovery time, have led to widespread acceptance of a variety of regional nerve blocks. Interscalene block is the most commonly used block for shoulder surgery. Other brachial plexus nerve blocks used for orthopaedic surgery of the upper extremity are supraclavicular, infraclavicular, and axillary. Several practical

Verapamil as an Adjunct to Local Anaesthetic for Brachial Plexus Blocks.

BACKGROUND: Calcium channel blockers potentiate the effects of local anaesthetics. We examined the effect of adding verapamil to local anaesthetic solution on anaesthetic duration in patients undergoing surgery under brachial plexus block. METHODS: This study was a prospective, randomized, controlled, double blind study. Sixty patients undergoing elective upper limb surgery were divided into two groups of 30 each. Group A received 40 ml of 1% lignocaine with 0.25% bupivacaine, while Group B patients had 2.5 mg verapamil added. RESULT: Onset of sensory blockade time was marginally faster in G

[Topographical and anatomic substantiation of sheath block of the brachial plexus].

The authors have studied the anatomic peculiarities of the fascial case of the plexus in an experiment on 60 preparations of the brachial plexus as well as the ways of diffusion of the solution along the case making use of various methods of block. It has been shown that the fascial case forms a closed space determining the diffusion of the solution on a considerable way along the plexus. The main way of diffusion of the anaesthetic solution are the paravasal loose connective tissue structures. On the basis of the clinical application of this method in 85 patients the authors believe that the

Continuous brachial plexus block for replantation in the upper extremity.

For replantation surgery, continuous brachial plexus block was employed using supraclavicular or axillary approach. Of a total of fifty blocks, forty-seven (94%) gave adequate surgical analgesia throughout the operations. No serious complication appeared in this series. Pneumothorax was detected in one supraclavicular block, but no surgical drainage was required. One patient had paraesthesia in the blocked arm, but it subsided within a month. No toxic reaction was recognised in spite of high dose of anaesthetic agents. Thus continuous brachial plexus block has been ascertained to be safe, reli

Exploring phantom phenomena following brachial plexus block in intact limbs.

BACKGROUND AND OBJECTIVE: Phantom limb experiences, including phantom limb sensations (PLS) and phantom limb pain (PLP), are common after limb amputation or deafferentation, with PLP significantly impacting quality of life. However, the mechanisms underlying PLP remain unclear, complicating treatment development. Investigating phantom phenomena has been proposed to gain insights into the mechanisms behind the insurgence of PLS and PLP, potentially informing new therapeutic approaches. However, small, heterogeneous samples and a lack of objective pain metrics often limit research on individuals

Risk factors for insufficient ultrasound-guided supraclavicular brachial plexus block.

PURPOSE: Ultrasound-guided supraclavicular brachial plexus block (SCBPB) is performed by surgeons for upper limb anesthesia; however, certain patients need additional local anesthesia. This study aimed to identify risk factors for additional local anesthetic injection requirements. METHODS: In total, 269 patients receiving ultrasound-guided SCBPB were enrolled. Patient age, sex, body mass index, anesthetic drug dose, surgeon expertise (hand surgeon or resident), tourniquet time, comorbidities (diabetes mellitus and mental disorders), and preoperative blood pressure representing anxiety were c

Continuous interscalene brachial plexus nerve block prolongs unilateral diaphragmatic dysfunction.

BACKGROUND AND OBJECTIVES: Single interscalene blocks (ISB) impair pulmonary function (<24h). We hypothesized that continuous ISBs would prolong pulmonary dysfunction until h48 compared with a single ISB. We compared the time course of spirometric and diaphragmatic dysfunction following single or continuous ISBs. METHODS: We prospectively included consecutive adult patients scheduled to undergo shoulder surgery under standard general anaesthesia with single (n=30) or continuous (n=31) ISB. Before ISB (baseline), spirometric tests were recorded and the diaphragm course was evaluated with a B-m
