Delayed stent fracture has been observed in many different arteries and may represent a risk factor for restenosis. We describe the case of a delayed rupture of an endovascular brachiocephalic trunk stent. The complete fracture allowed a fragment to migrate distally and tilt, resulting in a hemodynamic pattern similar to that of a prevertebral stenosis with complete inversion of the homolateral vertebral blood flow. The induced vertebral steal syndrome as well as the risk of cerebral embolism was corrected by an aortobrachiocephalic bypass and resection of the ruptured stent.

Although patent ductus arteriosus (PDA) is the most common type of extracardiac shunt, aberrant PDA associated with other cardiac malformations as double brachiocephalic trunk and interrupted left subclavian artery is extremely rare. To the best of our knowledge, the literature contains no other report of a patient who has PDA originated from left subclavian artery associated with ventricular septal defect (VSD), double brachiocephalic trunk and interrupted left subclavian artery.

Aortic arch anomalies occur due to disorders of the development of primitive double aortic arch system. In this study a patent ductus arteriosus, which had the appearance of a muscular type artery, was observed in addition to an enlarged right pulmonary artery with a diameter of 2.4 cm, causing thickening to right ventricular wall. A left common carotid artery arising from the brachiocephalic trunk was also observed. Such a complicated variation is extremely rare.

The authors report the immediate and mid term results of percutaneous transluminal angioplasty of stenosis of the subclavian (16 cases), brachiocephalic (1 case) and axillary arteries (1 case) in a series of 18 patients. These lesions were responsible for isolated ischaemia of the arm in 6 cases, vertebrobasilar insufficiency in 6 cases and an association of the two presentations in 2 cases; in 4 asymptomatic patients, the procedure was undertaken to maintain cerebral perfusion. A technical success was obtained in all cases, with one femoral haematoma and one episode of transient visual distur

The surgical treatment of ascending aortic and arch aneurysms, even though technically complex and requiring care to avoid neurological sequelae, is well established. Nevertheless, the presence of a significant innominate artery trunk aneurysm implies an adaptation of the surgical options available. We report a case of an aortic aneurysm that involved the ascending aorta, the proximal transverse arch and the brachiocephalic trunk with cranial displacement of the right subclavian and common carotid arteries. This pattern, meant that it was mandatory to change the usual surgical approach. We res

Importance is placed on aberrant arteries in the radiological and surgical literature. A normal left brachiocephalic trunk is characteristic for the right aortic arch. However, an aberrant left brachiocephalic trunk arising as the last branch of the aortic arch on the left side has not yet been described in the literature. Described here is a new variant of the retro-oesophageal aberrant left brachiocephalic trunk, occasionally observed in a patient during diagnostic investigation or surgical treatment for steno-obstructive involvement of the carotid district. The triple anomaly of the left ao

Following the unsuccessful puncture of the right subclavian vein during central catheterism, a 80-year-old women developed a pseudoaneurysm on the external face of the brachiocephalic artery. Her symptomatology and haemodynamic status having remained steady, the patient was closely observed. The pseudoaneurysm itself thrombosed spontaneously and the foreseen endovascular procedure doesn't have been achieved. A review of the literature has been done concerning the venous catheterism complications and the pseudoaneurysm treatment.

OBJECTIVE: We sought to evaluate the feasibility and results of intraoperative balloon angioplasty and additional stent placement of isolated stenosis of the brachiocephalic trunk.
PATIENTS AND METHODS: Between May 1993 and October 1996, we treated eight patients with local stenosis of the innominate artery. Seven lesions were situated in the proximal and one in the middle third of the brachiocephalic trunk. Five patients were men and three were women, with ages ranging from 55 to 72 years (mean 59.5 years). All stenoses provoked severe blood flow reduction and caused clinical symptoms. Proce

The authors report a case of combined surgical and endovascular treatment of a traumatic pseudo-aneurysm of the innominate artery in which the left common carotid artery originated from the brachiocephalic trunk. After a conventional surgical intervention with the implantation of the left common artery on the left subclavian artery, to correct the anatomic anomaly, a safe and effective endovascular stent-graft placement excluded the aneurysm. This new technique proposes a good chance for polytraumatized patients to receive a better prognosis and a much faster rehabilitation.

Occlusions and severe stenoses of the innominate artery (brachiocephalic trunk) are rare and present with a wide variety of clinical manifestations, with hemispheric, vertebrobasilar and right upper limb ischemic symptoms. The most common cause is atherosclerosis. Duplex scanning may show right vertebral artery flow reversal, diminished subclavian flow, and several patterns of right carotid flow disturbance, including slow flow, partial flow reversal during the cardiac cycle and even complete reversal of flow in the internal carotid artery, which is a very uncommon finding. Herein, the authors

Preoperative haemodynamic studies were performed in 60 out of 64 patients operated upon for occlusive disease of the subclavian artery. The pressure differences over the occlusive lesion were recorded in 53 patients. Blood flow was studied in 56 patients with the aid of electromagnetic flowmetry. The average mean pressure difference was 30 mmHg in 6 patients with occlusion of the brachiocephalic trunk and 20 mmHg in 20 patients with left subclavian artery occlusion. In 3 patients with right subclavian occlusion it was 17 mmHg. The highest individual mean pressure differences were found in pati

BACKGROUND: Central venous catheterization, crucial for device insertion, monitoring, medication, and fluid resuscitation, commonly uses the subclavian, internal jugular, and femoral veins. Despite its general safety, complications like arterial puncture can be life-threatening, requiring rapid diagnosis and treatment.
CASE PRESENTATION: A 74-year-old woman in the recovery phase of cerebral infarction underwent right subclavian vein catheterization. The catheter was mistakenly placed in the brachiocephalic trunk, with its tip in the ascending aorta, as confirmed by computed tomography (CT) an

The right and left vertebral arteries are the first branches of the ipsilateral subclavian arteries. However, in the presence of anatomical variation due to complex embryogenesis, the vertebral artery can arise directly from the aortic arch or any of its major branches. The atypical origin of the vertebral artery is commonly associated with the left vertebral artery. Anatomical variation in the origin of the right vertebral artery is rare. Most available reports are case reports from international studies. We report on a case of right vertebral artery arising from the ipsilateral common caroti

Hemodynamically relevant lesions of the innominate artery can be treated either by extraanatomic bypass or by intrathoracic repair. 32 patients were retrospectively evaluated after ascending aorta to innominate artery interposition graft. In all except 6 cases additional bypass-procedures to extracranial vessels were necessary for complete revascularisation. The patency rate was 100% after a mean follow up period of 871 days. Only one patient suffered from a perioperative stroke with upper extremity paresis which resolved after 2 weeks. There were 3 wound complications which required reoperati

INTRODUCTION AND IMPORTANCE: Central venous catheter (CVC) placement is a common procedure, particularly in critically ill patients. However, it carries the risk of vascular complications, including rare and severe injuries to the brachiocephalic trunk (BCT). We present a case series of two patients who developed BCT injuries following tunneled CVC placements, highlighting the critical importance of early diagnosis to prevent fatal outcomes.
CASE PRESENTATION: An 89-year-old female with chronic renal failure on hemodialysis developed hypotension, tachycardia, and dyspnea after left subclavian

Central venous catheter (CVC) insertion with ultrasound guidance is routine clinical practice in the critically ill patient. Arterial malposition is serious and may lead to severe complications such as hemorrhage, stroke, or death. We describe a bail-out technique for removal of right-sided CVC that was mispositioned into the brachiocephalic trunk (BCT) at the origin of the right common carotid artery (CCA). Covered stenting of the BCT extending into the CCA in combination with plug embolization of the right subclavian artery was utilized.
