The Brain-Intestinal Mucosa-Appendix- Microbiome-Brain Loop.

The brain and the gut are connected from early fetal life. The mother's exposure to microbial molecules is thought to exert in utero developmental effects on the fetus. These effects could importantly underpin the groundwork for subsequent pathophysiological mechanisms for achieving immunological tolerance and metabolic equilibrium post birth, events that continue through to 3-4 years of age. Furthermore, it is understood that the microbiome promotes cues that instruct the neonate's mucosal tissues and skin in the language of molecular and cellular biology. Post birth mucosal lymphoid tissue f

Brain shaving: adaptive detection for brain PET data.

The intricacy of brain biology is such that the variation of imaging end-points in health and disease exhibits an unpredictable range of spatial distributions from the extremely localized to the very diffuse. This represents a challenge for the two standard approaches to analysis, the mass univariate and the multivariate that exhibit either strong specificity but not as good sensitivity (the former) or poor specificity and comparatively better sensitivity (the latter). In this work, we develop an analytical methodology for positron emission tomography that operates an extraction ('shaving') of

Asymptomatic brain tumor detected at brain check-up.

Brain check-up was performed in 4000 healthy subjects who underwent medical and radiological examinations for possible brain diseases in our hospital from April 1996 to March 2000. Magnetic resonance imaging revealed 11 brain tumors which consisted of six meningiomas, three pituitary adenomas, one astrocytoma, and one epidermoid cyst. The detection rate of incidental brain tumor in our hospital was 0.3%. Nine patients underwent surgery, with one case of morbidity due to postoperative transient oculomotor nerve paresis. The widespread use of brain check-up may increasingly detect asymptomatic b

Type 2 immunity in the brain and brain borders.

Recent research in neuroimmunology has revolutionized our understanding of the intricate interactions between the immune system and the central nervous system (CNS). The CNS, an "immune-privileged organ", is now known to be intimately connected to the immune system through different cell types and cytokines. While type 2 immune responses have traditionally been associated with allergy and parasitic infections, emerging evidence suggests that these responses also play a crucial role in CNS homeostasis and disease pathogenesis. Type 2 immunity encompasses a delicate interplay among stroma, Th2 c

[Alcohol and the brain--morphological and functional brain changes].

It has been established long ago through autopsy studies that chronic alcohol consumption may lead to severe organic brain damage in about 10% of the patients. The recent rapid development of novel tools for structural and functional brain imaging has enabled researchers to expand upon this early knowledge. Some of these newer studies have demonstrated that chronic alcohol consumption causes cortical volume loss, which is partly reversible with abstinence. Less known but of utmost clinical relevance are non-organic neurocognitive dysfunctions, which have only been described recently. Functiona

Phylogenetic development of brain and brain sulci in primates.

The purpose of this study was to investigate the phylogenetic development of the cerebrum and cerebral sulcus in primates. The species selected were Macacus, Hylobates, Pan and Homo sapiens. These samples are classified as old world monkeys (Cercopithecidae), anthropoid apes (Pongidae), and Man (Hominidae). Although these four species divided up and went their separate ways from about the Oligocene era, the pattern of the cerebral sulci is similar. Of various cerebral sulci, the cingulate and calcarine sulci were selected, because they run on the medial surface of the cerebrum. The length of t

Treatment of brain swelling and brain edema in man.

Despite our relative lack of understanding of how many agents work to improve brain edema states, the ability to treat patients empirically has improved dramatically over the last 10 years. The armamentarium of the treating clinician has been aided by the availability of the nonosmotic diuretics, by the more rational use of mannitol therapy, by the availability of higher doses of dexamethasone, and perhaps by the availability of barbiturates. What is required and what is now under way is the testing of various combinations of drugs and therapeutic regimens in laboratory models, which then can

Abnormal brain morphology and morphological brain network in adults with acute mild traumatic brain injury.

BACKGROUND: Mild traumatic brain injury (mTBI) commonly has long-term cognitive and functional consequences; however, it is not clear whether these adverse outcomes begin in the acute phase of mTBI and are associated with changes in brain morphology and function. METHODS: The current study used T1-weighted MRI to determine whether cortical thickness, gray matter volume (GMV), and morphological brain networks were altered in patients with mTBI within 7 days of injury, and to examine whether these changes were associated with postacute cognitive and emotional abnormalities. Adults aged 18-56 ye

Attenuation of brain nitrostative and oxidative damage by brain cooling during experimental traumatic brain injury.

The aim of the present study was to ascertain whether brain cooling causes attenuation of traumatic brain injury by reducing brain nitrostative and oxidative damage. Brain cooling was accomplished by infusion of 5 mL of 4°C saline over 5 minutes via the external jugular vein. Immediately after the onset of traumatic brain injury, rats were randomized into two groups and given 37°C or 4°C normal saline. Another group of rats were used as sham operated controls. Behavioral and biochemical assessments were conducted on 72 hours after brain injury or sham operation. As compared to those of the sha

Peritumoral brain edema associated with pediatric brain tumors: characteristics of peritumoral edema in developing brain.

The incidence and clinical significance of peritumoral brain edema in pediatric patients is not well understood. The purpose of this study is to clarify the clinical significance of peritumoral brain edema in pediatric patients. Seventy seven pediatric patients (under 15 year old) with brain tumor were studied by MRI. The volume of peritumoral edema and brain tumor were measured by integration of the cross-sectional area on serial MRI. The severity of brain edema was expressed by the ratio of edema volume to tumor volume. The results were compared with that of 408 adult patients with brain tum

Pathways in gut-brain communication: evidence for distinct gut-to-brain and brain-to-gut syndromes.

OBJECTIVE: The rich interconnectedness between gut and brain is increasingly being identified. This article reviews the evidence for brain-gut and gut-brain syndromes, particularly recent epidemiological evidence, and animal studies demonstrating bi-directionality at the formative stage of development. METHOD: Narrative literature review with selection for relevance and quality. RESULTS: Population surveys show a strong correlation between anxiety, depression, and functional gastrointestinal disorders, contradicting early suspicions that the high prevalence of anxiety and depression in the c

Effect of brain structure, brain function, and brain connectivity on relapse in alcohol-dependent patients.

CONTEXT: In alcohol-dependent patients, brain atrophy and functional brain activation elicited by alcohol-associated stimuli may predict relapse. However, to date, the interaction between both factors has not been studied. OBJECTIVE: To determine whether results from structural and functional magnetic resonance imaging are associated with relapse in detoxified alcohol-dependent patients. DESIGN: A cue-reactivity functional magnetic resonance experiment with alcohol-associated and neutral stimuli. After a follow-up period of 3 months, the group of 46 detoxified alcohol-dependent patients was

Brain Connectomics.

A central tenet of modern neuroscience is the conceptualization of the brain as a collection of complex networks or circuits with a shift away from traditional "localizationist" theories. Connectomics seeks to unravel these brain networks and their role in the pathophysiology of neurologic diseases. This article discusses the science of connectomics with the examples of its potential role in clinical medicine and neuromodulation in multiple disorders, such as essential tremor, Parkinson's disease, obsessive-compulsive disorder, and epilepsy.

[Brain Calcification].

Brain calcification can be either physiological or pathological. Pathological calcification occurs due to a wide spectrum of causes, including congenital disorders, infections, endocrine/metabolic diseases, cerebrovascular diseases, and neoplasms. The patient's age, localization of the calcification, and association with other imaging findings are useful for the correct diagnosis. Dural arteriovenous fistulas with cortical venous reflux should be included in the differential diagnosis of subcortical calcification via CT. MRA should be conducted subsequently. We recently reported the clinical a

Brain arteriolosclerosis.

Brain arteriolosclerosis (B-ASC), characterized by pathologic arteriolar wall thickening, is a common finding at autopsy in aged persons and is associated with cognitive impairment. Hypertension and diabetes are widely recognized as risk factors for B-ASC. Recent research indicates other and more complex risk factors and pathogenetic mechanisms. Here, we describe aspects of the unique architecture of brain arterioles, histomorphologic features of B-ASC, relevant neuroimaging findings, epidemiology and association with aging, established genetic risk factors, and the co-occurrence of B-ASC with

Brain Death.

Death determined by neurologic criteria, commonly referred to as "brain death," occurs when function of the entire brain ceases, including the brain stem. Diagnostic criteria for brain death are explicit but controversy exists regarding nuances of the evaluation and potential confounders of the examination. Hospitals and ICU teams should carefully consider which clinicians will perform brain death testing and should use standard processes, including checklists to prevent diagnostic errors. Proper diagnosis is essential because misdiagnosis can be catastrophic. Timely, accurate brain death dete

Brain abscess.

Brain abscess is a serious and life-threatening disease among children despite advances in diagnosis and management. Changes in the epidemiology of predisposing conditions for brain abscess are associated with changes in the patient population and causative organisms. Though still a potentially fatal infection, there have been recent improvements in diagnosis, treatment, and outcome. Although mortality appears to be decreasing, a significant percentage of children continue to have residual neurological deficits, including epilepsy, permanent motor or sensory dysfunction, visual field defects,

[Gambling brain].

Pathological gambling (PG) is a chronic mental disorder, and patients cannot stop gambling despite severe negative consequences, such as huge debts, job loss, family break-up, and so on. It is said that PG is more prevalent in Japan than in Western countries. However, PG has not received much attention and has even been thought of as a lack of will to stop gambling rather than a mental disorder. PG has been classified under "Impulse-Control Disorders Not Elsewhere Classified," along with compulsive stealing (kleptomania), starting fires (pyromania) and hair-pulling (trichotillomania), but accu
