Migraine is a neurovascular disorder characterized by autonomic nervous system dysfunction and severe headache attacks. Studies have shown that changes in the intracranial vessels during migraine have an important role in the pathophysiology. Many studies have been conducted on the increased risk of stroke in patients with migraine, but insufficient data are available on the mechanism underlying the increase. This study aimed to evaluate basal cerebral blood flow velocity and vasomotor reactivity in patients with chronic migraine. We evaluated 38 patients with chronic migraine. Three of them w

INTRODUCTION: The aim of this study was to investigate the long-term prognosis for children with severe breath-holding spells (BHS).
MATERIAL AND METHODS: The study was a retrospective cohort study. Data from the medical records of all patients with severe BHS admitted during a ten-year period were recorded and a questionnaire was sent to the families. A matched group of adolescents with febrile convulsions served as controls (n = 289).
RESULTS: A total of 85 out of 115 families (73.9%) responded. The mean age of the included patients was 20.5 years. Among first-degree relatives 21.5% had BH

OBJECTIVE: Because breath holding causes arterial pCO2 to increase, we used it to test the hypothesis that in panic disorder (PD) a biological suffocation monitor is pathologically sensitive.
METHOD: Nineteen patients with PD, 17 with generalized anxiety disorder (GAD), and 22 normal controls took deep breaths on signal and held them until a release signal was given 30 seconds later. This was repeated 12 times separated by 60-second normal breathing periods.
RESULTS: PD patients reported having had in the past more symptoms of shortness of breath when anxious, and more frequent frightening s

The two phases of breath holding, the voluntary inactive and involuntary active phases, were identified by non-invasive methods using the induction plethysmograph. Eight trained divers and 7 non-diving control subjects familiar with respiratory apparatus were studied. During breath holding from normocapnia and total lung capacity it was not possible to distinguish between the two groups in respect of the pattern or duration of breath holding or alveolar gas tensions at the breakpoint. Divers could, however, hold their breath much longer after hyperventilation (165 +/- 40.0 and 121 +/- 31.4 sec

In 5 healthy young men, the maximal voluntary breath holding--was studied under conditions of different initial values of PACO2--after oxygen inhalation at ordinary minute volume of respiration or at hyperventilation, or after breathing with oxygen mixtures with 2, 4, 6, 8 or 10% CO2. PACO2 was recorded at the moment of resuming of the intercostal muscles electric activity and at the point of breaking up the apnoea. A mathematical model was constructed. The hypothesis of participation of two independent factors (chemo- and mechanoreceptive) in genesis of the imperative stimulus for breathing w

Bodily disturbances, like dyspnea, elicit responses to regain homeostasis and ensure survival. However, this life-saving function can become hyperreactive, which may lead to the emergence of psychopathology. This study investigated whether maximal voluntary breath-holding time (mvBHT), a biobehavioral marker that characterizes sensitivity to respiratory stimulation, predicts defensive mobilization to cues signaling the proximity of a mild electric shock vs. a respiratory threat (shortness of breath elicited by forced breath-holding) and the opportunity to avoid threat delivery in 60 healthy pa

OBJECTIVE: Breath-holding spells (BHSs) are a non-epileptic paroxysmal phenomenon characterized by frequent apnea episodes, loss of consciousness, and changes in skin tone and postural tone triggered by negative stimuli of childhood. The pathophysiology of the disease remains unclear; autonomic dysregulation caused by delayed myelination is believed to play a role. In this study, we aimed to evaluate the brainstems of children with BHS using diffusion tensor imaging (DTI) and investigate the etiology of this phenomenon.
METHODS: The study group consisted of 16 children with a history of sever

Thirty-two children with severe breath-holding spells and 42 control children were studied by using the Child Behavior Checklist. No single behavioral profile could identify children with breath-holding spells, and there were no group differences with respect to individual behavior categories, broad-band profiles, or total scores. No correlation was found between frequency of breath-holding spells and profile scores.

This study attempted to determine noninvasively whether a dysregulation of autonomic nervous system reflexes exists in children with severe cyanotic breath-holding spells (BHS). This was a cross-sectional study performed in the neurophysiology laboratory at a tertiary care hospital. Patients, 18 months of age and older taken from a referral population of children with severe cyanotic BHS and normal controls, were studied. Fourteen children with cyanotic BHS and 12 controls were evaluated. Several noninvasive measures of parasympathetic and sympathetic reflex functions were assessed and statist

From clinical history 58 children were diagnosed as having reflex anoxic seizures secondary to provoked cardioinhibition (also known as white breath-holding attacks). Before referral, these seizures were commonly misdiagnosed as epileptic either because the provocation was ignored, not recognised, or was a febrile illness, or because there was no crying, no obvious breath-holding, little cyanosis, and often no pallor to suggest syncope and cerebral ischaemia. The duration of cardiac asystole after ocular compression was measured in these children and in 60 additional children with other paroxy

Breath holding spells are common in childhood. They peak about 2 years of age and abate by 5 years of age; they are rare before 6 months of age. We report a case of cyanotic breath holding spells starting at the age of 3 days. There was a family history of breath holding spells including a three-year-old brother, patient's father, his sister, and the paternal great grandfather. Pharmacological intervention with iron replacement therapy and piracetam was not successful. We want to draw attention to the fact that breath holding spells should be included in the differential diagnosis of neonatal

Breath-holding spells are common in childhood and can be associated with bradycardia and pulselessness. This report details severe breath-holding spells complicating postoperative management after atrial septal defect closure. The patient required cardiopulmonary resuscitation despite the use of a temporary pacemaker to prevent bradycardia. After multiple episodes of chest compressions, the decision was made to not intervene immediately to pulselessness and the patient was able to recover without further intervention.

BACKGROUND: The medical treatment for severe pallid breath-holding spells accompanied with severe bradycardia or transient cardiac arrest is controversial. Although various medications have been reported to be effective, patients treated with pacemaker insertion are not always evaluated for pharmacological therapy beforehand.
CASE REPORT: A 9-month-old boy developed pallid breath-holding spells. At 15 months of age, a Holter electrocardiogram revealed 12 s of asystole during a breath-holding spell. Treatment with low-dose theophylline sustained-release dry syrup (5.3 mg/kg/day) led to complet

AIM: Iron deficiency anaemia (IDA), which is reported very commonly among patients with breath holding spells (BHS), is the most common presentation of coeliac disease (CD). In that aspect, IDA may be a common pathway linking these two diseases. The aim of this study was to evaluate the frequency of CD in patients with BHS.
METHODS: We studied 348 children with BHS, and 470 age- and sex-matched controls with no known disease. Serological screening for CD was performed in all patients by searching for serum tissue transglutaminase immunoglobulin A.
RESULTS: The first group consisted of 348 ch

OBJECTIVE: While there is recent interest in using repeated deep inspiratory breath-holds, or prolonged single breath-holds, to improve radiotherapy delivery, breath-holding has risks. There are no published guidelines for monitoring patient safety, and there is little clinical awareness of the pronounced blood pressure rise and the potential for gradual asphyxia that occur during breath-holding. We describe the blood pressure rise during deep inspiratory breath-holding with air and test whether it can be abolished simply by pre-oxygenation and hypocapnia.
METHODS: We measured blood pressure,

INTRODUCTION: We sought to characterize the patterns of active pressure development of the inspiratory and expiratory rib cage muscles (P(rcm,i) and P(rcm,e)), the diaphragm (P(di,i)), and the expiratory abdominal muscles (P(abm,e)) during maximal "dry" breath holding in trained apnea divers (n = 8).
METHODS: Respiratory contractions were assessed via esophageal and gastric manometry. It was expected that inspiratory/expiratory pressures would progressively increase in both magnitude and frequency during the struggle phase, and that inspiratory rib cage muscle pressures would rise at a rate e

PURPOSE: The study investigated the changes in cardiovascular and cerebral hemodynamics elicited by the diving response during static (SBH) and dynamic (DYNBH) breath-holding (BH) in moderately trained recreational breath-hold divers (BHDs).
METHODS: Nineteen BHDs (42.9 ± 7.8 years, 5.7 ± 2.5 years of breath-hold practice) participated in the study. Cardiovascular and cerebral hemodynamics, along with muscle and pre-frontal cortex oxygenation, were continuously tracked throughout a single SBH and DYNBH by means of arterial volume clamp, transcranial Doppler ultrasound, and near-infrared spect

The purpose of this study was to determine the physiological effects of breath-holding during high-intensity exercise. Twenty participants (age: 23 ± 4 years, 10 females) performed 20 s sprints of simultaneous arm and leg ergometry exercise, under free-breathing (FB) and breath-holding (BH) conditions. Identical power outputs were sustained for both breathing conditions. Heart rate was significantly higher under the breath-holding condition during exercise (FB = 118 ± 12 bpm versus BH = 131 ± 14 bpm; p < 0.001) but lower for 5 min post-exercise (FB = 110 ± 16 bpm, BH = 102 ± 15 bpm, p = 0.003)

BACKGROUND AND OBJECTIVE: Spontaneous intracranial hypotension (SIH) from CSF leak commonly produces headache. It also may produce sagging brain syndrome (SBS), often with neurocognitive symptoms indistinguishable from behavioral-variant frontotemporal dementia (bvFTD). The authors describe a new clinical sign that appears to be pathognomonic of SBS.
METHODS: We reviewed medical records and brain imaging in patients seen at our 2 centers who presented with SIH, SBS, and bvFTD symptoms.
RESULTS: There were 51 patients (12 women, 39 men) with mean age 55.5 years (range, 26-70 years). MRI showe
