Newer medications to aid treatment of asthma.

The selective beta-2 adrenergic bronchodilator preparations should be considered the medications for the first line of defense in the treatment of the average asthmatic patient. Theophylline and derivatives of theophylline are the second line of defense. Cromolyn sodium, atropine and derivatives, corticosteroids and other selected agents follow, depending on the individual case and the circumstances involved.

Muscarinic acetylcholine receptor X-ray structures: potential implications for drug development.

Muscarinic acetylcholine receptor antagonists are widely used as bronchodilating drugs in pulmonary medicine. The therapeutic efficacy of these agents depends on the blockade of M3 muscarinic receptors expressed on airway smooth muscle cells. All muscarinic antagonists currently used as bronchodilating agents show high affinity for all five muscarinic receptor subtypes, thus increasing the likelihood of unwanted side effects. Recent X-ray crystallographic studies have provided detailed structural information about the nature of the orthosteric muscarinic binding site (the conventional acetylch

Pulmonary medications.

Many of the patients seen by physical therapists have primary or secondary diagnoses for which cardiovascular- or pulmonary-active medications may be prescribed. There is a need, therefore, for physical therapists to understand the pharmacologic treatment of such patients. This article discusses medications commonly used in the treatment of pulmonary disorders. These medications are typically divided into the following categories: bronchodilators, anti-inflammatory agents, decongestants, antihistamines, antitussives, mucokinetics, respiratory stimulants and depressants, and paralyzing and anti

National guidelines for the management of asthma in adults.

Asthma is now considered primarily an inflammatory disease in which bronchospasm occurs secondary to airway inflammation. Management strategies include the use of inhaled anti-inflammatory agents, notably inhaled corticosteroids and cromolyn. Mild intermittent asthma may be treated with inhaled bronchodilators. Moderate asthma should be treated with an inhaled anti-inflammatory agent in addition to an inhaled beta agonist. If symptoms persist, an oral bronchodilator (either a beta-adrenergic agonist or theophylline) should be added. Therapy for severe asthma includes combinations of the forego

Bronchodilators for the prevention and treatment of chronic lung disease in preterm infants.

BACKGROUND: Chronic lung disease (CLD) occurs frequently in preterm infants (< 37 weeks gestational age) and has a multifactorial etiology. Bronchodilators have the potential effect of dilating small airways with muscle hypertrophy. Increase in compliance and tidal volume and decrease in pulmonary resistance have been documented with use of bronchodilators in short term studies of pulmonary mechanics in infants with CLD. Therefore it is possible that bronchodilators might have a role in the prevention and treatment of CLD. OBJECTIVES: To evaluate the effect of bronchodilators, given prophylac

Anticholinergic agents in chronic bronchitis and emphysema.

Inhaled anticholinergic agents produce as much absolute increase in airflow in chronic bronchitis as in asthma, namely about 0.2-0.3 litres increase in the FEV1 and a doubling of specific airways conductance. Their major advantage in bronchitis is due to the fact that adrenergic agents are relatively poor bronchodilators in this disorder. In bronchitis and emphysema a large number of studies suggest that anticholinergic agents not only are more potent than adrenergic agents but, when given in optimal dosage, achieve all of the available bronchodilatation in these patients. Possible reasons for

Prophylactic drugs in the management of childhood asthma.

Recurrences of explosive, severe asthma in children can often be prevented by immunotherapy and constant surveillance of environmental and dietary control. In addition, judicious and preserving use of pharmaceuticals may be employed for prophylaxis in childhood asthma. These include cromolyn sodium by inhalation, beclomethasone dipropionate aerosols, round-the-clock methylxanthines and beta 2 adrenergic bronchodilator agents. Their application is discussed.

Stepwise Management of Status Asthmaticus Refractory to Initial Therapy: A Case Report.

Asthma is one of the most prevalent chronic respiratory illnesses, significantly impacting patients through shortness of breath and even death. Acute exacerbations are usually controlled with a short-acting beta agonist, such as an albuterol inhaler, as well as long-acting agents to prevent the occurrence of exacerbations and status asthmaticus. Status asthmaticus is an emergent episode of asthma that is refractory to standard treatment. This disease presents as tachycardia, tachypnea, and dyspnea. The forced expiratory volume measures the severity of asthma in one second and the serial peak e

Inhaled bronchodilators increase maximum oxygen consumption in chronic left ventricular failure.

Bronchoconstriction is seen at rest in patients with chronic heart failure, and may contribute towards exercise limitation. To investigate the effect of bronchodilator agents on exercise capacity, 10 patients (mean age 60 years, range 39-72) in New York Heart Association class II and III heart failure, underwent symptom-limited maximal exercise testing after inhalation of nebulized salbutamol (5 mg), ipratropium bromide (500 micrograms) or placebo delivered on separate days in a randomized, double-blinded study. There was an increase in forced expiratory volume in one second from pre-treatment

Equine respiratory pharmacology.

Differentiation of diseases of the equine respiratory tract is based on history, clinical signs, auscultation, endoscopy, imaging, and sampling of airway exudate. Upper respiratory therapies include surgical correction of airway obstructions; flushing of localized abscesses (strangles), guttural pouch disease, or sinusitis; and oral or parenteral antibiotic and anti-inflammatory therapy if deemed necessary. Pneumonia usually is treated with antimicrobials, anti-inflammatories, and bronchodilators. Pleural drainage is indicated if significant pleural effusion is present. The most commonly used

Pharmacological strategies to reduce exacerbation risk in COPD: a narrative review.

Identifying patients at risk of exacerbations and managing them appropriately to reduce this risk represents an important clinical challenge. Numerous treatments have been assessed for the prevention of exacerbations and their efficacy may differ by patient phenotype. Given their centrality in the treatment of COPD, there is strong rationale for maximizing bronchodilation as an initial strategy to reduce exacerbation risk irrespective of patient phenotype. Therefore, in patients assessed as frequent exacerbators (>1 exacerbation/year) we propose initial bronchodilator treatment with a long-act

Combination therapy in pediatric bronchitis: Examining mucolytic, bronchodilator and anti-inflammatory drug effects on secretion management and recovery time.

This study investigated the effectiveness of combining mucolytic agents (ambroxol hydrochloride), bronchodilators (terbutaline sulfate), and anti-inflammatory drugs (budesonide) in treating pediatric bronchitis. From September 2023 to February 2024, 410 pediatric patients were divided into two groups: A control group treated with bronchodilators and anti-inflammatory drugs, and a study group receiving the combined therapy. The results indicated that the study group exhibited significantly better clinical efficacy, with faster resolution of symptoms such as dyspnea, cough, and fever compared to

Criteria for the clinical evaluation of prophylactic agents in bronchial asthma.

The clinical investigation of prophylactic agents for bronchial asthma is at once simple and laborious: simple because provocation tests are available for measuring the bronchial reactivity of the asthma patient to numerous stimuli and his response to treatment; but laborious because some of these methods are poorly standardised and above all because bronchial spasticity varies greatly from patient to patient and in the same patient at different times. The fluctuating pattern of asthma, is, moreover, an obstacle to clinical investigation. It has to be taken into account in devising trial proto

Responses to inhaled bronchodilators in infancy are not linked with asthma in later childhood.

Many doctors consider wheezing infants and young children who respond to beta-adrenergic agents to be asthmatics, or at least at risk for later asthma. The aim of this study was to compare responses to inhaled albuterol and racemic epinephrine in infancy between children with and without asthma later in childhood. In a crossover study setting, 100 acutely wheezing infants aged less than 24 months were randomized to receive inhalations of either racemic epinephrine and placebo, or albuterol and placebo. Clinical evaluation consisted of measurements of respiratory rate, heart rate, and oxygen sa

Indacaterol: a new long-acting β2-agonist in the management of chronic obstructive pulmonary disease.

INTRODUCTION: Bronchodilators represent the mainstay of symptomatic treatment for chronic obstructive pulmonary disease (COPD). The principal bronchodilator agents are β(2)-agonists, anticholinergics and methylxanthines, used singly or in combination. Indacaterol is a novel long-acting β(2)-agonist for maintenance bronchodilator treatment of airflow obstruction in patients with COPD, approved in December 2009 by the European Medicines Association, and recently by the US Food and Drug Administration. It is administered once daily and is delivered by means of a single-dose dry powder inhaler (SD

Anticholinergic bronchodilators in combination.

Chronic obstructive pulmonary disease (COPD), which affects approximately 14 million Americans, is the fourth leading cause of death in the United States and is responsible for an estimated US$6.5 billion in direct and indirect costs per year [1,2]. Its usual course is a slow deterioration of lung function and progressive breathlessness with activities. The age-adjusted death rate for COPD rose 71% from 1967 to 1987, and the 10 year mortality rate is about 50%. Bronchodilators form one of the mainstays of therapy in COPD patients. The judicious use of these agents increases airflow and reduces

Practice of nebulization in intensive care unit patients receiving invasive ventilation - A nationwide survey.

BACKGROUND: Nebulization with mucolytic and bronchodilator agents is frequently applied in Intensive Care Units (ICUs) for invasively ventilated patients, despite limited evidence for clinical benefit and potential risks associated with routine use. Understanding current practice and rationale behind nebulization is essential to targeted de-implementation strategies. METHODS: We conducted a nationwide, cross-sectional telephone survey among ICU healthcare professionals in the Netherlands. The survey addressed nebulization practices for mucolytics and bronchodilators, including agents used, ad

Inhaled adrenergics and anticholinergics in obstructive lung disease: do they enhance mucociliary clearance?

Pulmonary mucociliary clearance is an essential defense mechanism against bacteria and particulate matter. Mucociliary dysfunction is an important feature of obstructive lung diseases such as chronic obstructive pulmonary disease, asthma, cystic fibrosis, and bronchiectasis. This dysfunction in airway clearance is associated with accelerated loss of lung function in patients with obstructive lung disease. The involvement of the cholinergic and adrenergic neural pathways in the pathophysiology of mucus hypersecretion suggests the potential therapeutic role of bronchodilators as mucoactive agent

Isolated airways in equine respiratory pharmacology: They never lie.

Pre-clinical studies on human isolated bronchi have relevant translational value in human in vivo, conversely no investigation has been performed to assess whether data resulting from equine isolated airways can have any translational application in asthmatic horses. Thus, a meta-regression analysis via random-effect method was carried out to correlate the pharmacological characteristics of bronchodilators resulting from experiments performed in equine isolated bronchi with their impact on the lung function outcomes in asthmatic horses. Data on the potency of different bronchodilators were ext
