A Novel Case of Paraglottic Burkholderia Cepacia Complex Infection Mimicking a Laryngeal Malignancy.

The Burkholderia cepacia complex (BCC) is a rare pathogen typically causing respiratory illnesses in immunocompromised individuals. We present a novel case of BCC manifesting as a laryngeal abscess, mimicking a laryngeal malignancy in an immunocompetent patient. A 74-year-old male presented to the emergency department with acute respiratory failure and was emergently intubated, revealing abnormal supraglottic tissue. ENT consultation post-extubation and flexible laryngoscopy suggested malignancy in the right supraglottis. A subsequent CT neck showed an expansive paraglottic mass, again consist…

Liver Cirrhosis Complicated by Spontaneous Bacterial Peritonitis Caused by the Burkholderia cepacia Complex.

A 79-year-old man with underlying alcoholic liver cirrhosis presented with complaints of a fever, abdominal pain, and difficulty walking. A diagnostic work-up revealed liver atrophy and chylous ascites, and spontaneous bacterial peritonitis (SBP) was diagnosed based on the cell and neutrophil counts. The Burkholderia cepacia complex (Bcc) was detected on blood and ascitic fluid cultures. Although broad-spectrum antibiotic therapy was initiated, the infection was difficult to control, and the patient died of multiple organ failure. Bcc is often multidrug-resistant and difficult to treat. SBP ca…

In vitro activity of temocillin against planktonic and sessile Burkholderia cepacia complex bacteria.

Burkholderia cepacia complex (Bcc) bacteria are opportunistic respiratory pathogens which are particularly difficult to eradicate from the lungs of cystic fibrosis (CF) patients because of their innate resistance to antimicrobials and their capacity to form biofilms. The goal of the present study was to evaluate the bacteriostatic and bactericidal activities of temocillin against planktonic and sessile Bcc bacteria. 37 strains belonging to 17 Bcc species were tested. 75.7% of the strains were susceptible when grown planktonically (minimal inhibitory concentration <16 μg/ml). The minimal bacter…

Challenges and mitigation strategies associated with Burkholderia cepacia complex contamination in pharmaceutical manufacturing.

Burkholderia cepacia complex (BCC) is a Gram-negative, non-spore-forming bacterium with more than 20 opportunistic pathogenic species, most commonly found in soil and water. Due to their rapid mutation rates, these organisms are adaptable and possess high genomic plasticity. BCC can cause life-threatening infections in immunocompromised individuals, such as those with cystic fibrosis, chronic granulomatous disease, and neonates. BCC contamination is a significant concern in pharmaceutical manufacturing, frequently causing non-sterile product recalls. BCC has been found in purified water, cosme…

A Phylogeny-Informed Proteomics Approach for Species Identification within the Burkholderia cepacia Complex.

Ancestral genetic exchange between members of many important bacterial pathogen groups has resulted in phylogenetic relationships better described as networks than as bifurcating trees. In certain cases, these reticulated phylogenies have resulted in phenotypic and molecular overlap that challenges the construction of practical approaches for species identification in the clinical microbiology laboratory. Burkholderia cepacia complex (Bcc), a betaproteobacteria species group responsible for significant morbidity in persons with cystic fibrosis and chronic granulomatous disease, represents one …

Occurrence and antibiotic susceptibility profiles of Burkholderia cepacia complex in coastal marine environment.

During an environmental study of bacterial resistance to antibiotics in coastal waters of the Kaštela Bay, Adriatic Sea, Croatia, 47 Burkholderia cepacia complex (Bcc) isolates were recovered from seawater and mussel (Mytilus galloprovincialis) samples. All isolates showed multiple antibiotic resistance. Among the isolates, two Burkholderia cenocepacia isolates produced chromosomally encoded TEM-116 extended-spectrum β-lactamase (ESBL). Analysis of outer membrane proteins revealed that decreased expression of a 36-kDa protein could be associated with a high level of β-lactam resistance in both…

Evaluation of the antimicrobial and antibiofilm activities of aztreonam-avibactam against the Burkholderia cepacia complex.

Beta-lactamase production by Burkholderia cepacia complex (Bcc) leads to beta-lactam resistance and worsens patient prognosis. This study describes the activity of aztreonam, a monobactam, in combination with avibactam, a beta-lactamase inhibitor, against Bcc isolates. Broth microdilution and time-kill assays were conducted for planktonic cells. Additionally, broth microdilution was repeated using media supplemented with 5% and 10% bovine serum albumin to evaluate its impact on MICs. The effects of aztreonam/avibactam on biofilm formation and the metabolic activity of mature biofilms were dete…

A new selective culture medium for isolation of Burkholderia cepacia complex in pharmaceutical industry.

OBJECTIVES: Burkholderia cepacia complex (Bcc) are typical objectionable microorganisms of concern for water-based pharmaceuticals. In order to achieve quick and effective detection of Bcc to prevent final product contamination, a new selective medium, Burkholderia cepacia complex selective agar (BCCSA), for the detection of Bcc in water-based pharmaceutical products was reported. METHODS: The formulation of BCCSA was optimized based on the carbon sources utilization of 60 Bcc strains from multiple origins. BCCSA and Burkholderia cepacia selective agar (BCSA), which was adopted in the examina…

The Intricate Nonribosomal Assembly of a Potent Antifungal Lipopeptide from the Burkholderia cepacia Complex.

The Burkholderia cepacia complex (BCC) is a group of Gram-negative bacteria known for their pathogenicity to patients suffering from cystic fibrosis (CF). The BCC-belonging strain B. pyrrocinia BC11 (formerly B. cepacia BC11) produces AFC-BC11, a compound with strong activity against phytopathogenic fungi. In this contribution, we report on the unprecedented N-acyl-tetrapeptide structure and antifungal potency of this natural product. We further provide insights into central steps of its biosynthesis mediated by a nonclassical nonribosomal peptide synthesis machinery lacking condensation domai…

Bacteria of the Burkholderia cepacia complex are cyanogenic under biofilm and colonial growth conditions.

BACKGROUND: The Burkholderia cepacia complex (Bcc) is a collection of nine genotypically distinct but phenotypically similar species. They show wide ecological diversity and include species that are used for promoting plant growth and bio-control as well species that are opportunistic pathogens of vulnerable patients. Over recent years the Bcc have emerged as problematic pathogens of the CF lung. Pseudomonas aeruginosa is another important CF pathogen. It is able to synthesise hydrogen cyanide (HCN), a potent inhibitor of cellular respiration. We have recently shown that HCN production by P. a…

A CRISPR-Cas-associated transposon system for genome editing in Burkholderia cepacia complex species.

UNLABELLED: Genome editing in non-model bacteria is important to understand gene-to-function links that may differ from those of model microorganisms. Although species of the Burkholderia cepacia complex (Bcc) have great biotechnological capacities, the limited genetic tools available to understand and mitigate their pathogenic potential hamper their utilization in industrial applications. To broaden the genetic tools available for Bcc species, we developed RhaCAST, a targeted DNA insertion platform based on a CRISPR-associated transposase driven by a rhamnose-inducible promoter. We demonstrat…

Phage therapy to treat cystic fibrosis Burkholderia cepacia complex lung infections: perspectives and challenges.

Burkholderia cepacia complex is a cause of serious lung infections in people with cystic fibrosis, exhibiting extremely high levels of antimicrobial resistance. These infections are difficult to treat and are associated with high morbidity and mortality. With a notable lack of new antibiotic classes currently in development, exploring alternative antimicrobial strategies for Burkholderia cepacia complex is crucial. One potential alternative seeing renewed interest is the use of bacteriophage (phage) therapy. This review summarises what is currently known about Burkholderia cepacia complex in c…

Burkholderia cepacia complex infection in a case of sinonasal polyposis recurrence without cystic fibrosis.

Burkholderia cepacia complex (BCC) has been rarely isolated in infections of the ear, nose and throat region in immunocompetent patients without cystic fibrosis. There is only one previous study in literature that reported BCC isolation in immunocompetent patients with sinonasal polyposis. We describe herein a rare case of multiresistant sinonasal infection by BCC in an immunocompetent patient with recurrent sinonasal polyposis. BCC seems to be a potentially emerging bacterial pathogen of sinonasal structures in patients with polyposis, also after FESS.
