30 patients submitted to partial gastrectomy (Billroth II) who later suffered digestive complaints (pain, "heartburn", bilious vomiting) showed gastric hyperemia, oedema and some erosions of the gastric stump when examined by endoscopy, the presence of cancer having been excluded by biopsy. Case report on a successful treatment of a cimetidine-resistant gastric ulcer with carbenoxolone-Na. Administration of 1.0g cimetidine/d over a period of 8 weeks failed to reduce gastric ulcer size. Subsequent therapy with carbenoxolon-Na caused a complete healing of the gastric ulcer within 4 weeks. This case is discussed with respect to the possible advantages of a combined regimen with drugs of different mechanisms of action.

Excessive olivo-cerebellar synchrony is implicated in essential tremor. Because synchrony in some networks is mediated by gap junctions, we examined whether the gap junction blockers heptanol, octanol, carbenoxolone, and mefloquine suppress tremor in the mouse harmaline model, and performed an open-treatment clinical study of mefloquine for essential tremor. Digitized motion was used to quantify tremor in mice administered harmaline, 20 mg/kg s.c. In mice the broad-spectrum gap junction blockers heptanol, octanol (350 mg/kg i.p. each), and carbenoxolone (20 mg/kg) suppressed harmaline tremor.

Type I pseudohypoaldosteronism (PHA) is a hereditary syndrome of salt wasting resulting from unresponsiveness to mineralocorticoids. Technetium-99m pertechnetate is known to concentrate in the stomach following intravenous injection, but there is controversy on the cellular site of this uptake, with evidence for both the mucous and parietal cells being primarily responsible. Eighty-six patients with duodenal ulcer were treated in a multicentre controlled trial with carbenoxolone capsules (Duogastrone, 50 mg four times a day for six weeks) or a placebo. Sixty-nine patients accepted endoscopy at the beginning and end of the treatment. BACKGROUND: In the kidney carbenoxolone impairs inactivation of glucocorticoids and facilitates their access to mineralocorticoid receptors by inhibiting 11 beta-hydroxysteroid dehydrogenase (11 beta-OHSD). 11 beta-OHSD is also expressed in vascular smooth muscle, and, in humans, carbenoxolone potentiates vasoconstrictor sensitivity to cortisol and noradrenaline.

57 patients with a chronic gastric ulcer were treated as outpatients in a double blind comparison of carbenozolone 100 mg three times daily with the same dose of geranyl farnesyl acetate (Gefarnate). Healing rates for carbenoxolone were better than for gefarnate, but not significantly so. Nearly half the carbenoxolone-treated patients developed hypokalaemia or oedema, whereas no similar side-effects were found with gefarnate. Gefarnate, while not as effective as carbenoxolone, does appear to promote gastric ulcer healing. Its virtual absence of side-effects makes it a safe and useful drug.
