Aggregatibacter actinomycetemcomitans, an etiological agent associated with periodontitis, endocarditis, and other infections, has rarely been implicated in spondylitis. A 70-year-old man with aortic valve replacement presented with a 4-month history of lower back pain and was diagnosed with spondylitis. Prolonged incubation of blood cultures and a biopsy yielded A. actinomycetemcomitans. Concurrent infective endocarditis (IE) was probable considering the infectious organism and the patients' prosthetic valve. The patient was treated with ceftriaxone and recovered well. Pyogenic spondylitis wi

We report the case of using a long-term combination of meropenem and amikacin to treat infective endocarditis caused by Enterobacter cloacae resistant to third- and fourth-generation cephalosporins. Multi-drug resistant Gram-negative bacilli, such as the E. cloacae in our study, may become possible pathogens of infective endocarditis. Our experience with this case indicates that long-term use of a combination of β-lactam and aminoglycosides might represent a suitable management option for future infective endocarditis cases due to non-Haemophilus, Actinobacillus, Cardiobacterium, Eikenella, Ki

BACKGROUND: HACEK (Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, Kingella) endocarditis is a rare cause of subacute infective endocarditis typically occurring in patients with pre-existing valvular pathology or structural heart disease.
CASE SUMMARY: A 65-year-old male presented with months of night sweats, fevers, anemia, and weight loss concerning for malignancy. Transthoracic echocardiography was negative for vegetations. Blood cultures grew Aggregatibacter actinomycetemcomitans, and transesophageal echocardiography confirmed vegetations on the mitral and aortic valves. The pat

The bacteria causing infective endocarditis have not changed significantly despite the diversity of potential portals of entry. Streptococci (viridans) cause 35 to 45% of cases of endocarditis. Group D streptococci of gastrointestinal origin cause about 20% of cases of endocarditis: Streptococcus bovis is the most commonly isolated species followed by the enterococcus. The beta-haemolytic streptococci of Groups A, B, C and G are rarely isolated. Seventeen to thirty per cent of endocarditis is caused by staphylococci, above all Staphylococcus aureus. These infections are mainly observed in pati

A 38-year-old patient with a prosthetic aortic valve had weeks of fever and respiratory symptoms. Although endocarditis was suspected early, numerous blood cultures and echocardiograms failed to confirm the diagnosis. Key questions were: What is a broad differential diagnosis for the above symptoms? What is unique about diagnosing prosthetic valve endocarditis compared with native valve endocarditis? What other imaging modalities are useful for diagnosing prosthetic valve endocarditis when initial echocardiographic evaluation is negative? What organisms are classically implicated in "culture n

A 66-year-old woman with a history of tissue aortic valve replacement and chronic back pain presented to the emergency department with a suspected right leg deep vein thrombosis. A recent outpatient MRI had revealed discitis. A ventricular fibrillation cardiac arrest occurred in the emergency department. Cardiac output was restored on the fifth defibrillation. A transthoracic echocardiogram showed large aortic valve vegetations. Clinical impression was of infective endocarditis with cardiac arrest secondary to coronary artery embolisation. Peripheral blood cultures grew Cardiobacterium hominis

Infective endocarditis can be associated with complex clinical presentations, sometimes with a difficult multi-disciplinary management. Actinobacillus actinomycetemcomitans belongs to the Haemophilus species, Actinomycetemcomitans, Cardiobacterium hominis, Eikenella corrodens and Kingella species group, responsible for 5% to 10% of infective endocarditis in native heart valves. These organisms have slow fastidious growth pattern, often associated with negative cultures, and cause systemic embolism with abscess formation. The authors present the case of a 59-year-old man, admitted due to fever

Bacterial endocarditis (BE) is a disease resulting from the association of morphological alterations of the heart and bacteraemia originating from different sources that at times can be indiscernible (infectious endocarditis). It is classified on the basis of the morphological alteration involved, depending on the clinical manifestations and course of illness, which varies according to the causative microorganism and host conditions (for example, it is characteristic in I.V. drug users). The most common microorganisms involved are: Streptococcus viridans (55%), Staphylococcus aureus (30%), Ent

Fourty-two human isolates that had been designated Pasteurella, or pasteurella-like organisms in the bacteriological routine laboratory were phenotypically characterized considering conventional, morphological and physiological features, and respiratory quinones. Thirty-seven of these strains fitted into the large traditional species, P. multocida, the majority of them being associated with alterations of the respiratory tract, and the rest with intestinal diseases, or putrid wound secretions mostly following animal bite or scratch lesions. Two strains isolated from sputum, or sinus maxillaris

Kingella kingae, a rare member of the Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella (HACEK) group, is an uncommon cause of infective endocarditis (IE) in adults. Advances in molecular diagnostics, including polymerase chain reaction (PCR) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), have significantly improved its identification. We present the case of a 78-year-old immunocompromised woman who developed watershed territory cerebral infarctions in the setting of K. kingae endocarditis. The patient initially presented w

Aggregatibacter aphrophilus, a fastidious Gram-negative member of the Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella group, is an exceptionally rare cause of hepatic abscess, particularly in immunocompetent individuals. We describe a case of a healthy, middle-aged man who presented with a 5-day history of cyclical fevers, chills, and night sweats. Computed tomography imaging revealed a solitary hepatic lesion, initially suspicious for malignancy. Magnetic resonance cholangiopancreatography confirmed a hepatic abscess. Culture from a computed tomography-guided aspirate i

It has been recommended that blood cultures be routinely subcultured aerobically on the day after the specimen is received, anaerobically after 48 h, and, finally, after 5 to 7 days if the cultures appear negative (Bartlett et al., Cumitech 1, American Society for Microbiology, Washington, D.C., 1974). To evaluate the necessity for the final routine subculture, 2,780 previously negative blood culture bottles were subcultured after 7 days of incubation. Of four bottles positive by subculture, three yielded the same organism as previously isolated from the companion bottle, and one yielded an or

Infective endocarditis (IE) is most frequently caused by gram-positive organisms such as Staphylococci, Streptococci, and Enterococci. IE due to gram-negative organisms is uncommon and is most often associated with the HACEK group (Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella) of organisms. IE due to other gram-negative organisms such as Escherichia coli (E. coli) is rare and less is known about its natural pathology. Valvular abscesses due to E. coli IE have been described, however these are not usually reported in a native mitral valve. Here we describe the case of

"Classical" Whipple's disease (cWD) is caused by Tropheryma whipplei and is characterized by arthropathy, weight loss, and diarrhea. T. whipplei infectious endocarditis (TWIE) is rarely reported, either in the context of cWD or as isolated TWIE without signs of systemic infection. The frequency of TWIE is unknown, and systematic studies are lacking. Here, we performed an observational cohort study on the incidence of T. whipplei infection in explanted heart valves in two German university centers. Cardiac valves from 1,135 patients were analyzed for bacterial infection using conventional cultu

Despite the availability of numerous beta-lactam antibiotics, benzylpenicillin remains the most important beta-lactam antibiotic in the treatment of bacterial endocarditis. Penicillin alone and in combination with an aminoglycoside is effective in the treatment of endocarditis due to all streptococci, Streptococcus pneumoniae, penicillin-susceptible Staphylococcus aureus, Haemophilus aprophilus, Actinobacillus actinomycetemcomitans, Cardiobacterium hominis, Listeria monocytogenes. Oral phenoxymethylpenicillin in combination with streptomycin is effective in treating endocarditis due to viridan

OBJECTIVE: To investigate the associations of the oral microbiome status with diabetes characteristics in elderly patients with type 2 diabetes mellitus.
METHODS: A questionnaire was used to assess age, sex, smoking status, drinking status, flossing frequency, T2DM duration and complications, and a blood test was used to determine the glycated haemoglobin (HbA1c) level. Sequencing of the V3-V4 region of the 16S rRNA gene from saliva samples was used to analyze the oral microbiome.
RESULTS: Differential analysis revealed that Streptococcus and Weissella were significantly enriched in the late

Chronic rhinosinusitis (CRS) is the chronic inflammation of the sinus cavities of the upper respiratory tract, which can be caused by a disrupted microbiome. However, the role of the oral microbiome in CRS is not well understood. Polymicrobial and anaerobic infections of CRS frequently increased the difficulty of cultured and antibiotic therapy. This study aimed to elucidate the patterns and clinical feasibility of the oral microbiome in CRS diagnosis. Matched saliva and nasal swabs were collected from 18 CRS patients and 37 saliva specimens from normal volunteers were collected for 16S rRNA s

Infective endocarditis due to fastidious microorganisms is commonly encountered in clinical practice. Some organisms such as fungi account for up to 15% of cases of prosthetic valve infective endocarditis, whereas organisms of the HACEK group (Haemophilus parainfluenzae, H. aphrophilus, and H. paraphrophilus, Actinobacillus actinomycetemcomitans, Cardiobacterium hominis, Eikenella corrodens, and Kingella kingae) cause 3% of community-acquired cases of infective endocarditis. Special techniques are necessary to identify these microorganisms. A history of contact with mammals or birds may sugges

AIM: This study aimed to explore the composition of the tooth-surface plaque (subgingival with marginal supragingival) microbiome in dentate older adults residing in long-term care (LTC) facilities, stratified by clinically assessed oral disease burden (ODB). A total of 196 LTC residents aged ≥62 years underwent oral examinations and microbial sampling from each dentate quadrant. Microbial profiling was performed using 16S rRNA gene sequencing.
RESULTS: Participants were more frequently categorized into Moderate (n = 95, 48%) than Low (n = 32, 16%) or High (n = 69, 35%) ODB groups. Those with
