Neonatal screening for sickle cell diseases in Camberwell: results and recommendations of a two year pilot study.

The sickle cell diseases are a major health problem for Afro-Caribbean peoples. Neonatal detection and prophylactic management can reduce mortality and morbidity in childhood. A study was therefore conducted analysing the results of the first two years of cord blood screening in the Camberwell health area. Thirteen cases of sickle cell disease and two of haemoglobin (Hb)C disease were identified among 2202 non-white infants screened. The carrier state, sickle cell trait (HbAS), was present in 11.9% and HbC trait (HbAC) in 4.1% of Afro-Caribbean infants. The incidence of disease and of carrier

Cultural expectations and perceptions of risk communication among Afro-Caribbean mothers and daughters in the United States.

INTRODUCTION: Mothers are key influencers in daughters' decision making about risk behaviors. Much research on parent-child relationships and communication has been conducted among predominantly White, nonimmigrant families. However, parent-child relationships and communications about risk behaviors may significantly differ for Black immigrant families. In particular, transnational behaviors that serve to maintain multiple social identities with attributes from both the receiving and sending nations may shape communication about risky behaviors. The current study evaluated the association betw

Multiple Lenses to Unearth Hidden Voices: Living with Diabetic Foot Ulceration in an Afro-Caribbean Community.

(1) Background: This study was conducted in the small island developing state of Barbados, which has dubiously earned the title of "amputation capital of the world", to understand perspectives of persons living with diabetic foot ulceration. (2) Methods: An exploratory multi-lens approach was used (focus groups; dyads; and triads) to gather indigenous Afro-Caribbean perspectives of living with diabetic foot ulceration that may be obscured by using a single method. (3) Results: Findings in this group highlighted the necessity of creating culturally sensitive education tools, as well as understa

HEAL-D Online: Exploring the potential for the spread and adoption of a virtual culturally tailored diabetes self-management programme for adults of African and Caribbean heritage.

BACKGROUND: People of African and Caribbean heritage in the UK have a higher prevalence of Type 2 Diabetes (T2D) and poorer health outcomes than white Europeans. Healthy Eating and Active Lifestyles for Diabetes Online (HEAL-D Online) is a co-designed, culturally tailored T2D self-management programme for black African and Caribbean adults, which, due to online delivery, is well positioned for spread. This qualitative evaluation uses the Exploration-Preparation-Implementation-Sustainment (EPIS) framework to explore factors affecting scale-up from delivery and commissioning perspectives. METHO

Psychometric properties of the Center for Epidemiologic Studies Depression Scale in African American and Black Caribbean US adults.

A 12-item version of the 20-item Center for Epidemiological Studies-Depression (CES-D) Scale was not validated in Black US adults but demonstrated strong psychometrics in other populations. Using data from the National Survey of American Life (n = 4,815), the psychometric properties of the scale were tested in African American and Black Caribbean adults. When compared with the DSM-IV-TR criteria for Major Depressive Disorder (MDD) and Dysthymia, most items in the CES-D Scale focus on depressed mood, providing evidence for content validity. Construct validity was questionable in African America

Cardiovascular Disease and Risk Factors in Women in Latin America and the Caribbean.

BACKGROUND: Cardiovascular disease (CVD) remains the leading cause of death among women in Latin America and the Caribbean (LAC), with ischemic heart disease as the primary contributor. Despite this burden, women's health programs in the region have traditionally prioritized cancer prevention, often neglecting CVD. OBJECTIVE: To analyze the burden of CVD and the distribution of cardiovascular risk factors among women in LAC, highlighting sex-related differences and regional disparities. METHODS: This study analyzed epidemiological data on CVD mortality and cardiovascular risk factors across

The role of sex and ethnicity in the variability of estimated glomerular filtration rate (eGFR) in people with type 1 diabetes.

AIMS: To examine sex-based differences in estimated glomerular filtration rate (eGFR) trajectories for people with type 1 diabetes (T1D) over 10 years and to test the hypothesis that sex and ethnicity interaction predicts eGFR decline. METHODS: A cohort of 1495 individuals, 48% men (81% White, 12% African-Caribbean and 7% Other) and baseline eGFR ≥45 mL/min/1.73 m2 was analysed. Group-based trajectory modelling (GBTM) was used to identify eGFR sex-specific trajectories, and linear mixed models were used to evaluate the role of sex and ethnicity interaction in predicting eGFR heterogeneity. Analysis of the Caribbean Neurosurgery Workforce: Scope of Practice, Challenges, and Ways Forward.

BACKGROUND: The neurosurgical workforce in the Caribbean and surrounding countries is largely unknown due to the diversity in cultural, linguistic, political, financial disparities, and colonial history between the countries. About 45 neurosurgeons serve 16 million people in the Caribbean Community and Common Market, a trade alliance including most Caribbean nations. We aimed to understand the current scope of neurosurgical workforce in this region while highlighting any system challenges and potential solutions for upscaling the workforce. METHODS: We surveyed neurosurgeons within Caribbean

Ethnicity and registration as a kidney donor: the significance of identity and belonging.

Studies of attitudes towards cadaveric organ donation have failed to adequately explain the disproportionately low rates of cadaveric organ donation among ethnic minorities. This may reflect an unhelpfully static and narrow definition of 'culture' entailing a collection of predefined cognitive 'beliefs' and 'attitudes.' This paper takes a more integrative approach and considers how ethnicity shapes perceptions of identity and belonging that underpin organ donation discourse among a section of Black British Caribbean people, one of largest migrant groups in the UK. The study is based on 14 in-d

Attitudes towards homosexuals in seven Caribbean countries: implications for an effective HIV response.

Between 2000 and 2015, the number of people newly infected with HIV in the Caribbean decreased by 76% and HIV-related deaths by 42%. The number of people living with HIV (PLHIV) on anti-retroviral therapy (ART) increased from near zero to 50% (44% to 57%) in 2015. In many Caribbean countries communities of men-who-have-sex-with-other-men (MSM) have higher incidence and prevalence of HIV. They are often stigmatized and subjected to both social and institutional discrimination. This study compared attitudes of the general public obtained through public opinion polls 2013-2014 towards homosexuals

Race: a risk factor for dengue hemorrhagic fever.

Cuban DHF/DSS outbreaks have provided evidence of a reduced risk of people of Negroid race for DHF/DSS compared to those of Caucasoid race. These observations from Cuban dengue outbreaks have significant epidemiological interest, as the differences in susceptibility to DHF/DSS among racial groups in Cuba coincide with that reported in African and Black Caribbean populations. In this article, we review the literature on race as a risk factor for DHF/DSS and discuss recent results from ongoing studies. Taking into consideration the origins of contemporary Cuban inhabitants, we propose that the C

Case Report: Flagellate eruption of adult-onset still disease in an Afro-Caribbean woman: highlighting atypical cutaneous disease in richly pigmented skin.

Adult-onset Still disease (AOSD) is an uncommon autoinflammatory disorder characterized by high spiking fevers, arthralgia, and, classically a transient, salmon-colored rash. Atypical cutaneous variants, such as persistent erythematous papules and plaques often arranged in a flagellate pattern, are increasingly reported; however, few cases illustrate this morphology in individuals with richly pigmented skin. We report a 70-year-old Afro-Caribbean woman (Fitzpatrick skin type VI) with persistent high-grade fevers, polyarthritis and diarrhea of unknown origin which persisted despite multiple cou

Barriers and facilitators to STI and HIV testing among African and Caribbean heritage communities in high-income countries mapped to the behaviour change wheel: a scoping review.

BACKGROUND: African and Caribbean communities in high-income countries face disproportionate sexually transmitted infection (STI) risks. In the US, the gonorrhoea rate among non-Hispanic Blacks is 7.7 times greater than that among non-Hispanic Whites, and the chlamydia rate is 5.6 times greater. In the UK, black Caribbeans have the highest gonorrhoea and chlamydia rates among all ethnic minority groups. Identifying barriers to and facilitators of HIV/STI testing is crucial for developing effective interventions. This scoping review maps current evidence on multilevel factors influencing STI te

Latin America and the Caribbean: Challenges and Opportunities for HIV Treatment and Prevention.

INTRODUCTION: In Latin America, there are rising numbers of new HIV acquisitions, while the Caribbean has achieved decreases in both new acquisitions and AIDS-related deaths. The Latin American and Caribbean (LAC) region has worked towards widely available treatment for people with HIV (PWHIV) within public health systems and, in Latin America, has relied principally on domestic financing to do so. DISCUSSION: While new methods of prevention are in the process of being incorporated into regional HIV programmes, the HIV cascade of care remains below the UNAIDS goals. Only 17 of the 32 LAC coun

Sign language ideologies in deaf education in Trinidad and Tobago.

This paper, representing a multilingual Global South-Global North deaf research collaboration, reports and critically analyzes findings from interviews with 5 deaf adult and 5 sign language interpreter participants regarding their educational and training experiences in the postcolonial Caribbean environment of Trinidad and Tobago. Caribbean sign languages are frequently said to be varieties of American Sign Language (ASL) and/or to have been displaced by ASL in schools with deaf learners. In the anglophone Caribbean context, deaf children have not had access to early childhood care and educat
