The treatment of external carotid artery stenosis has been described with a variety of operative interventions. We present a patient who presented with amaurosis fugax and a critical left external carotid artery stenosis with known left internal carotid artery occlusion. We treated this stenosis with angioplasty and stenting rather than endarterectomy. Our patient did well and had no complications from the procedure. Endovascular repair of symptomatic external carotid artery stenosis provides an alternative treatment method to conventional endarterectomy for patients with high surgical risk.

Two patients with previous brain stem infarction and current symptoms of vascular insufficiency in the basilar circulation had evidence of vertebral artery occlusive disease. Angiography demonstrated ostial stenosis of the dominant right vertebral artery and retrograde flow down the left vertebral artery to the level of the transverse process of C2 in the first case and to the level of a severely stenosed origin in the second case. Carotid circulation was patent in both cases. The blood flow in the posterior circulation was patent in both cases. The blood flow in the posterior circulation was

PURPOSE: In the presence of carotid occlusion, the external carotid artery (ECA) becomes an important source of cerebral blood flow, especially if the circle of Willis is incomplete. The contribution of the ECA to hemispheric blood flow in patients with severe ipsilateral carotid stenosis has never been previously investigated.
METHODS: One hundred eight patients were monitored during sequential cross-clamping of the external (ECA) and then ipsilateral internal carotid artery (ICA) during carotid endarterectomy using transcranial Doppler sonography (TCD) (Neuroguard CDS, Los Angeles, Calif),

Tumors of the carotid body are rare neoplasms that arise from neuroectodermic tissues. They are located in the fork of the primitive carotid artery. They are benign, slow-growing, and asymptomatic, and predominate in women. We present a case of bilateral carotid chemodectoma on the external carotids. These tumors were diagnosed by CT, MRI and superselective angiography. The treatment of choice is surgical after arteriographic embolization. Exeresis is curative in most cases, but some cases require radiotherapy.

BACKGROUND AND PURPOSE: Occlusion of the common carotid artery (CCA) is generally associated with an occlusion of the ipsilateral internal carotid artery (ICA) or of the external carotid artery (ECA) or of both. Sometimes, collateral circulation to the ECA may preserve patency of the ICA via retrograde perfusion through the bulb. We herewith report a case of reversed flow in the ECA in the presence of a patent but stenosed CCA as diagnosed with color-coded duplex flow imaging (CDFI).
CASE REPORT: This 85-year-old right-handed man was admitted for rapidly regressive acute dysarthria associated

A case of visual disturbance by embolization through the external carotid artery (ECA) after internal carotid artery (ICA) occlusion is reported. A 65-year-old female presented left ICA occlusion 14 years ago. She was admitted with acute deterioration of left visual acuity. Ophthalmologically, the retinal central artery was occluded. Cerebral angiography disclosed an irregular wall at the left ICA stump and stenosis of the maxillary artery. Carotid endarterectomy of the common carotid artery, ECA and residual ICA was performed and atheromatous plaque with ulceration was removed. The postoperat

The utility of angiography and embolisation of selected branches of the external carotid artery is occasionally helpful in the management of recurrent epistaxis, pre-operative devascularisation of tumours such as angiofibromas, and other head and neck conditions. The use of embolisation for recalcitrant post-tonsillectomy bleeding due to the formation of an aneurysm or pseudoaneurysm of branches of the external carotid artery has been described [P. Simoni, J. Bello, B. Kent, Pseudoaneurysm of the lingual artery secondary to tonsillectomy treated with selective embolization, Int. J. Pediatr. Ot

Surgical treatment of vertebrobasilar insufficiency has long lagged behind that of carotid disease. However, as more aggressive microsurgical techniques have been developed, surgical intervention in vertebrobasilar occlusive disease is seen with increasing frequency. The authors describe three cases of external carotid vertebral artery anastomosis, a rarely reported operative procedure, in which blood flow was successfully restored to the diseased vertebral artery. Awareness of this new technique will be useful to the radiologist servicing an active neurosurgery practice. The clinical, radiolo

Congenital arteriovenous fistulas of the external carotid artery are extremely rare as compared with acquired lesions. Early diagnosis permits surgical excision before irreversible cardiac damage occurs.

Therapeutic transcatheter embolization with spongostan in the region of the external carotid artery has been employed in 17 patients, in some of them on several occasions. The technique is presented and different means of avoiding complications are discussed in detail. Embolization has been used preoperatively and as the single mode of treatment of ENT tumours or meningeomas, and also as the only treatment in arteriovenous malformations. The technique has proven particularly useful in preoperative de-arterialization of ENT tumours when surgery follows in 1 to 2 days. In cases with arteriovenou

Anomalous origin of extracranial and intracranial carotid system is subject to permutations of embryonal regression of the developing primitive embryonal vasculature. We present a case of segmental agenesis of left external carotid artery from the cervical carotid circulation. The entire left external carotid artery was supplied by the vertebral artery through muscular branches via the occipital artery. This anomaly of proximal external carotid agenesis with the entire vascular tree dependent on the posterior circulation via occipital artery muscular collaterals is unique and has not been desc

The authors describe a patient with bilateral internal carotid artery occlusion, bilateral external carotid artery (ECA) stenosis, and suboptimal collateral circulation from the right ECA to the right cerebral hemisphere. The patient manifested clinical and radiographic signs of hyperperfusion syndrome following stent placement in the right ECA. This represents a rare case of a stent placed in the ECA of a patient in addition to the development of hyperperfusion syndrome after the procedure.

BACKGROUND: The role of external carotid artery (ECA) preservation during carotid graft interposition remains unclear. This study evaluated its impact on perioperative and long-term outcomes.
METHODS: A retrospective study included 161 patients undergoing carotid graft interposition. Patients were divided into ECA-preserved (n = 36) and ECA-ligated (n = 125) groups. Outcomes included perioperative events and long-term outcomes, including in-graft stenosis, reintervention, and stroke-free survival.
RESULTS: The mean follow-up was 30.28 ± 19.24 months (range, 1-66 months). In-graft stenosis oc

OBJECTIVES: to assess the application of external carotid artery (ECA) shunting in cerebral protection during carotid endarterectomy (CEA).
DESIGN: prospective study.
MATERIALS AND METHODS: the study comprised 137 consecutive patients who underwent CEA under locoregional anaesthesia. Transcranial Doppler was used to monitor the mean velocity of the middle cerebral artery (mv-MCA): (1) before carotid clamping; (2) after clamping both the common and external carotid arteries; (3) after clamping the internal carotid artery alone ("ECA test"). The decision to shunt was based on the occurrence of

BACKGROUND: Central retinal artery occlusion (CRAO) is a serious complication caused by carotid artery plaque dispersal into the ophthalmic artery (OA) during carotid artery stenting (CAS). The purpose of this study is to determine the cause of CRAO despite CAS with both external carotid artery (ECA) and internal carotid artery (ICA) protection.
CASE DESCRIPTION: An 85-year-old man was hospitalized two years ago for cerebral infarction and was diagnosed with left carotid artery stenosis. Carotid ultrasound showed low-intensity plaque, and peak systolic velocity had worsened to 466 cm/s, and m

OBJECTIVE: Although carotid endarterectomy (CEA) is the standard treatment for carotid stenosis, the incidence, predictors, and clinical significance of external carotid artery (ECA) occlusion after CEA remain poorly characterized. We assessed these factors and whether exoscope introduction influenced the ECA occlusion rate.
METHODS: We retrospectively analyzed 75 consecutive CEA cases with a patent preoperative ECA performed between January 2020 and December 2025 (mean age 73.4 years; 60 male). Preoperative ECA diameter was measured on MRA. Intraoperative indocyanine green (ICG) angiography

The carotid duct (CD) is a transient embryological structure connecting the 3rd and 4th aortic arches. We found a persisting CD in an adult female case, by studying the computed tomography angiogram. On the left side, the proximal external carotid artery (ECA) agenesis was noted. The CD was inserted into the left subclavian artery and continued upwards to reach the level of the atlas, and then it descended to connect to a normally configured segment of that ECA. It could be speculated that the CD-to-ECA connection was possible via unregressed 1st and/or 2nd aortic arches. The segmental ECA age

Chronic complete occlusion of the internal carotid artery (ICA) is a common and important cause of ischemic cerebrovascular disease, which can be treated in many ways: antithrombotic and anticoagulant agents, direct or indirect procedure for revascularization. However, there is still no evidence to prove which method is the most effective. Here we reported a case showed the successful endarterectomy of the external carotid artery (ECA) for a chronic complete occlusion of ICA patient. A 50-year-old male patient presented with progressively aggravated left-sided limb weakness and limb cold feeli

The contribution of the external carotid artery to cerebral blood flow in the presence of an internal carotid occlusion or severe stenosis is well documented. This study was undertaken in order to try and exploit the external carotid artery as a collateral pathway to avoid cerebral ischaemia during carotid surgery. The main problem is to ascertain when the external carotid artery is relevant to cerebral perfusion, and to assess if the insertion of a shunt from the common to the external carotid artery is a useful way of ensuring adequate cerebral perfusion in patients with cerebral ischaemia d
