OBJECTIVE: To establish the incidence of cranial nerve injuries caused by carotid artery surgery.
METHODS: During the years 1982 to 1992, 689 carotid artery operations were performed at Malmö General Hospital. Of these 656 were pre- and postoperatively examined at the Department of Phoniatrics concerning the function of the cranial nerves IX, X, and XII.
RESULTS: There was injury to one or more of the examined nerves after 75 operations (11.4%). In 70 (10.7%) operations the hypoglossal nerve was injured, in eight (1.2%) the recurrent laryngeal nerve, in two (0.3%) the glossopharyngeal nerve

OBJECTIVES: Restenosis or neointimal hyperplasia remains an important complication after carotid artery stenting (CAS) for carotid artery stenosis. The purpose of this study was to examine if an anti-hypertensive drug, angiotensin receptor blocker (ARB), prevents post-CAS neointimal hyperplasia during the first 1-year period after CAS, and to clarify the possible mechanisms.
METHODS: Hypertension had been treated with a calcium channel blocker (CCB) and/or an ARB, valsartan, by the preference of the neurosurgeon in charge in our department. At admission to perform CAS, patients were assigned

OBJECTIVE: to evaluate the possibility, efficiency, and safety of using the proximal protection systems in patients with concomitant injury to the ipsilateral external carotid artery (ECA) during internal carotid artery (ICA) stenting.
MATERIAL AND METHODS: The results of ICA stenting were analyzed in 39 patients. Their mean age was 66.4 +/- 7.8 years; 29 (74.4%) patients were male. All the patients were symptomatic. The time of ICA blood flow arrest averaged 5.4 +/- 2.1 min. Intolerance to blood flow arrest was noted in 3 (7.7%) patients. The intervention time was 27.6 +/- 8.3 min. The resul

The detection of blunt carotid artery injures has improved because of more aggressive screening protocols. Initial treatment depends on multiple factors; however, controversy exists with regard to the treatment of pseudoaneurysmal degeneration, especially in this age of endovascular treatment options. Current options include anticoagulation, open surgical repair, and endovascular repair. We report a rare case of bilateral carotid artery pseudoaneurysm degeneration after bilateral carotid artery dissection caused by blunt trauma.

BACKGROUND: Cervical carotid artery injuries entail high morbidity and mortality and are technically challenging to repair. This retrospective study describes the management and outcomes of cervical carotid injuries sustained during the recent wars in Iraq and Afghanistan.
METHODS: The Department of Defense Trauma Registry was queried to identify US military personnel who sustained battle-related cervical carotid injury between January 2002 and December 2015. Retrospective chart reviews of the military Electronic Health Record were performed on patients identified. Demographics, injury charac…

BACKGROUND: Internal carotid artery (ICA) injury represents one of the most challenging management scenarios for the endoscopic skull base surgeon. Techniques developed through use of an animal model of carotid injury have shown direct vessel closure techniques to be effective in gaining hemostasis and preventing subsequent complications. The aim of this study was to investigate the effectiveness of the novel AnastoClip vessel closure system in ICA injury.
METHODS: Nine sheep underwent ICA dissection/isolation followed by the artery placement within a modified "sinus model otorhino neuro trai

BACKGROUND: Blunt carotid injuries are rare, and present late with devastating strokes. A sizeable single-institution descriptive report could help characterize the injury and its diagnosis and treatment.
METHOD: We performed a retrospective review of blunt carotid artery injuries from May 1988 to December 1997 at a level I trauma center. Chart review consisted of demographics, mechanism of injury, associated injuries, diagnostic modalities, initial neurologic status, treatment, and outcome. Discharge outcome was classified as "good" (normal-mild deficit), "fair" (needing daily assistance), "…

An eighty-year-old man slipped in the bathroom and received a deep slash wound in his left neck caused by a broken fragment of the bathroom door. A fragment of the glass stuck into his left neck. He was carried to our clinic after 20 minutes in a state of shock and showed right hemiparesis and aphasia. Following immediate orotracheal intubation and emergent therapy for shock, he was transferred to the operation theater for massive arterial bleeding from the wound. The left common carotid artery and internal jugular vein were exposed by extending the skin incision from the cervical wound along

In a series of 109 carotid arterial reconstructions cranial nerve injury was observed in 14 of 102 patients. Ipsilateral peripheral hypoglossal nerve injury occurred in nine patients with carotid occlusive disease. The marginal mandibular nerve was injured in three patients and recurrent laryngeal nerve dysfunction was noted in four patients. Two cranial nerves were injured in two patients. Full recovery of hypoglossal dysfunction was seen within 2-52 weeks (average 20 weeks). The nerves are injured by retraction to clear the operative field or by postoperative haematoma. Risk factors include

A 37 year old woman who had bred horses for more than 20 years was leading two horses in a horse corral. One horse bolted and the lead rope wrapped around her neck. Four hours after the accident, she developed sudden aphasia, right facial palsy, and right arm weakness. Conventional and magnetic resonance angiography showed complete traumatic occlusion of the left common carotid artery due to thrombosis at the C6/7 level. There was good collateral flow from the other side. The patient was maintained on heparin followed by warfarin and recovered completely by the third day. This case shows the i

The relative infrequency of blunt carotid artery trauma prompted a multicenter review to determine the spectrum of injuries, treatment strategies, and neurologic outcome. During a six-year period, 60 carotid artery injuries from blunt mechanisms in 49 patients were treated at 11 institutions. There were 11 bilateral injuries. Injury mechanisms were diverse but involved motor vehicles in 35 (72%) patients. In 14 (29%) patients, significant neurologic deficits developed more than 12 hours after a normal admission neurologic examination. The diagnosis was confirmed by angiography in 42 (86%). Dup…

Fifty-six patients with carotid injuries were reviewed (35 penetrating and 21 blunt). Shock correlated with a profound neurologic deficit on admission (p less than 0.03) in those with penetrating wounds. Thirty-one percent had primary repair, 25% had interposition grafting, 17% were ligated, and 17% were anticoagulated. Two graft failures resulted in death. Three blunt common carotid injuries followed direct cervical soft-tissue trauma; 18 internal carotid (ICA) dissections followed apparent extreme neck extension or flexion. Seven had bilateral ICA dissections (39%); none of these died. All d

Six patients with trauma to the intracranial internal carotid artery are reported. One patient died and two are permanently disabled due to ischemic sequelae. The incidence of this complication of trauma is unknown because of the infrequent use of angiography in head-injured patients. The pathology, clinical course, and management of this condition are discussed with reference to the 25 previously reported cases in addition to the six in this series.

Three patients with extracranial carotid artery injury are presented. The trauma to the neck was blunt and nonpenetrating. Two patients showed the progressive neurologic signs of an intracranial lesion three and seven days respectively after head injuries. Injury to the neck did not appear to be a factor in one and, at most, a minor factor in the other. The provisional diagnostic impression in each instance was subdural hematoma. The third case was found in a patient with brain contusion, fractured mandible, and severe neck contusion, who was investigated by contrast study when he failed to re
